Objective To investigate the anatomy of the skull base for minimally invasive keyhole approach to sellar lesions.MethodsSixty-seven samples of adult skull bone(134 sides) were collected and sawed at the levels of the supraorbital arch and anterior skull base. After inspecting the opening of the frontal sinus,we observed the appearance of the orbit roof(cerebral juga),and measured the maximum height of the two cerebral jugas.Afterwards, the operation distances and angles with regard to the anterior and posterior clinoid processes through the trans-upper orbit,trans-lateral front,trans-pterion,and trans-sub-temple keyhole approaches in 15 skull bone samples were measured. And then,we statistically analyzed the variance in these measurements among the four operative approaches.ResultsOf the 67(134 sides) adult skull bone samples,52 sides showed frontal sinus openness(38.8%,52/134).Most of the skull bases were clawed(multicuspidity or oblique ridge multicuspidity). The maximum height of the cerebral juga that was less than 2.50 mm was found in 36 sides(26.9%),and over 2.51 mm in 98 sides(73.1%).Among the four methods, the shortest distances between the keyhole and the anterior and posterior clinoid processes were found in the trans-sub-temple keyhole approach [(4.87?0.47) cm and(4.93?0.45) cm],and the longest were observed in the trans-lateral front keyhole approach[(6.45?0.30) cm and(7.83?0.54) cm],and trans-upper orbit keyhole approach[(6.47?0.28) cm and(8.31?0.34) cm].The angle between the sagittal section and the keyhole in the trans-lateral front keyhole approach [(40.83?1.11) ? and(37.86?1.37)?] was larger than that in the trans-upper orbit one [(10.23?0.90) ? and(12.29?0.86)?],while the angle between the cross section and the keyhole in the trans-sub-temple keyhole approach [(21.21?0.45) ? and(20.10?0.63)?] was larger than that in the trans-pterion one [(5.49?0.30)? and(9.84?0.58)?].ConclusionsThe size of the frontal sinus, shape and height of the cerebral juga, depth of the skull base operation, and angle between the operational and the skull base planes play important roles in the selection of minimally invasive keyhole approach for sellar lesions.

Objective To observe the effects of insulin pump therapy on type 1 diabetes mellitus children with diabetic keto or diabetic ketoa-cidosis(DK/DKA).Methods Forty-three children with DK/DKA were randomly divided into treatment group(n=26)and control group(n=17).The patients in treatment group were treated with insulin pump, control group were received small amount insulin injravenous injection. The changs of blood sugar, urine ketone bodies, incidence of hypoglycemia and length of stay were observed. Results 1.The effect of decreasing sugar was the same between two groups.2.There was no hypoglycemia events in treatment group. But 2 cases in control group had hypoglycemia.3.There was significant difference of length of stay between two groups(P

Objective To assess the feasibility of K value of dynamic contrast enhanced MRI (DCE-MRI) in evaluation of anti-angiogenic effects on nude mice with orthotopic transplantation tumor model of gastric cancer. Methods Nude mice with orthotopic transplantation tumor model of gastric cancer were randomly assigned to two groups:Treatment group (n =15),mice were given apatinib intragastrically for 18 days (100 mg/kg),and control group (n=15),mice were given ddH2O2 in the same manner. After 18 days,DCE-MRI was performed and K value was measured. Then the tumors were dissected from the adjacent tissues in order to detect the microvessel density (MVD) and vascular endothelial growth factor (VEGF) expression levels.MVD and VEGF expression level were compared between treatment group and the control group. Results MVD (x2 =4.89,P<0.05) and VEGF expression level (x2 =8.69,P<0.01) of treatment group were much lower than those of control groups. The Kt value of treatment group was significantly lower than that of control groups ([0.63±0.05]/min vs [1.66±0.23]/min,t=17.05,P<0.01).Conclusion The value of k in DCE-MRI can be utilized to assess the effects of apatinib on nude mice with orthotopic transplantation model of gastric cancer.

Objective To evaluate the impact of multiple lacunar infarction (MLI) on the outcomes of mild cognitive impairment (MCI). Methods MCI patients were selected in outpatients and hospitalized patients using a common international standard MCI screen. Magnetic resonance imaging (MRI) and computer tomography (CT) were used to determine the presence of MLI. MCI patients were followed up for 6~24 months and their cognitive status and the impact of MLI on MCI were evaluated using Kaplan-Meier survival analysis. Results One hundred twenty patients were diagnosed as having MCI based on clinical evaluation and neuropsychological assessment. Among them, 43 (35.8%) patients had MLI and the rest 77 (64.1%) patients did not have. Twenty-seven MCI patients with MLI (62.8%) developed dementia, while 15 MCI patients without MLI (19.4%) developed dementia during 6~24 months follow-up. Kaplan-Meier analysis indicated that the incidence of dementia was significantly higher in patients with MLI than in patients without MLI did (P<0.05). Conclusions MCI patients with MLI are more prone to dementia. These results suggest that early interventions aimed at controlling the modifiable risks are warranted for dementia prevention in MCI patients with MLI.

Objective To explore the relationship between thoracolumbar disc herniation (TLDH) and Scheuermann1's disease (SD),as well as the role of SD in the etiology of TLDH.Methods From June 2006 to June 2010,45 patients with TLDH (T10-11-L2-3) underwent surgery in our department.Forty-five patients with lower lumbar disc herniation (LDH,L3-4-L5S1) acted as controls. The incidence of SD and Scheuermann's signs of these patients were examined by reviewing CT,MRI and Ⅹ-ray films. The thoracolumbar kyphotic angles of the two groups were compared.Furthermore,in TLDH group, the incidence of disk herniation within segments with the Scheuermann's signs was compared to that within segments without Scheuermann's signs. Results All except one patient in TLDH group(97.8%) had been associated SD while the incidence of SD in LDH group was only 26.7%.The incidence of all Scheuennann's signs was higher in TLDH group than that in LDH group. The average thoracolumbar kyphotic angle of TLDH group was 15.8°±6.9° while that of LDH group was 4.8°±4.0°.In TLDH group, the incidence of disc herniation within segments with Scheuermann's signs was all higher than that within segments without Scheuermann's signs. Conclusion There is a close relationship between TLDH and SD,suggesting that TLDH is probably a manifestation of SD.Schmorl's node, irregular end plate,wedge-shaped vertebra and especially, posterior bony edge separation, are associated with disc herniation.
