1.The study on levofloxacin resistance and gyrA gene mutation of Helicobacter pylori in Jinhua Zhejiang province

Objective To analyze levofloxacin resistance in Helicobacter pylori (Hp) and the sequence difference of gyrA gene in levofloxacin resistance and sensitive Hp strains. To explore the function of gyrA gene mutation in the development of levofloxacin resistance Hp strain.Methods From July 2007 to December 2008 in Department of Gastroenterology, Jinhua People hospital of Zhejiang Province, the gastric mucosa from gastroscopy biopsy of chronic gastritis and peptic ulcer patients were cultured in Hp selective medium under microaerobic condition at 37 degrees for three to five days. The Hp strains were isolated and identified by oxidase test, catalase test,urease test and UreA gene detecting. The levofloxacin susceptibility was determined by E-test and then resistance and sensitive strains were screened. The genomic DNA of Hp strains was isolated. The gyrA gene was amplified by PCR and the sequences were analyzed. Results 38 clinical isolated Hp strains were passed the levofloxacin susceptibility E-test, among those the minimum inhibitory concentration (MIC) of 12 strains was over 1.0 μg/ml, the percentage of resistant strain was 31.58%, while the sensitive stains was 68.42%. The gyrA gene sequence result indicated 10 resistant strain with 261, 271 and 272 10 site mutation, 2 strains with C261A mutation, one strain with C261G mutation, two strains with G271A mutation, 2 strains with A272G mutation, 2 strains with C261A,G271T andA272G mutation, 1 strain with C261G and A272G mutation. However, no mutation sites were found in 26 sensitive strains. Conclusion The rate of levofloxacir-resistance in isolated clinical Hp strain was high. The drug-resistance was associated with 261,271 and 272. site mutation of gyrA gene.

Allergic rhinitis and rhinosinusitis often complicate with asthma,and their relationship has long been investigated.From the view of epidemiology,all of these three diseases have higher prevalence,complicate with each other,are risk factors and prognostic factors for each other.Besides,they share common in anatomy and pathophysiology.In this paper,the interrelationship among allergic rhinitis,rhinosinusitis and asthma in pathogenesis is discussed.
3.Research on standard of Magnoliae Officinalis Cortex commercial specification and grade.

The market and literature were studied to understand the existing situation of Magnoliae Officinalis Cortex goods, and the collected samples were analyzed, combined with the actual production, a new standard of Magnoliae Officinalis Cortex commercial specification and grade was drafted. Magnoliae Officinalis Cortex goods was divided into two categories according to the source in the old standard. Then each category was divided into four kinds of specifications according to the site. Each kind of specification was divided into several grades according to the length and weight. To judge the quality of Magnoliae Officinalis Cortex goods was mainly based on the appearance quality. In the new standard, the classification of commercial specification and grade is based on the thickness, magnolol and honokiol content. The goods of Magnoliae Officinalis Cortex can be divided into three specifications: Tongpu, Genpu and Doupu. Tongpu is divided into three grades, the remaining two are not graded.

4.Comparison of Cellular Immunity Raised by a Novel Adjuvant DC-Chol with Different HBsAg Preparations

Objective: To screen candidate antigen for therapeutic HBV vaccine with a novel adjuvant DC-Chol. Methods: BALB/c mice were injected with DC-Chol liposome and HBsAg prepared from CHO and Yeast respectively. One week later, IL-4, IL-2, IFN-?were measured by ELISA or ELISPOT. Results: The levels of IL-2, IFN-?of HBsAg from Yeast with DC-Chol liposome were 20 and 119 times higher respectively than those of HBsAg from CHO with DC-Chol liposome. ELISPOT assay showed that the counts of spot-forming cells of IL-4 and IFN-?of HBsAg from Yeast with DC-Chol liposome were 2.8 and 46.3 times higher respectively than those of HBsAg from Yeast with Al(OH)3. Conclusion: HBsAg prepared from Yeast together with DC-Chol liposome may be an appropriate candidate for therapeutic HBV vaccine .
6.Study of the effect of leeching on plasma endothelin and soluble interleukin-2 receptor in patients with systemic lupus erythematosus.

OBJECTIVEto explore the mechanism of leeching in treating systemic lupus erythematosus (SLE).
METHODSForty-four patients with SLE were randomly divided into conventional corticosteroid treated group (control group, n = 20) and conventional treatment group with leeching intervention added (leeching group, n = 24). Before and after treatment the concentration of plasma endothelin (ET) and soluble interleukin-2 receptor (sIL-2R) were determined.
RESULTSBefore treatment the level of plasma ET and sIL-2R in the SLE patients were all higher than those in the normal healthy group, (P < 0.01). But after treatment the level of these in both groups were significantly improved than those of before treatment (P < 0.05), and comparison between these two treated groups showed that the difference between them was significant (P < 0.05).
CONCLUSIONLeeching added to conventional treatment of SLE could be more effective in improving the level of plasma ET and sIL-2R, and ameliorating the impairment of renal tissues.

7.Estimation of expanded criteria donor usability for islet isolation

Objective Islet transplantation has been an effective method for diabetes mellitus. The quality of donor pancreas is important for successful islet isolation. In this study, we evaluated expanded criteria donor usability based on the warm ischemic time, fatty pancreas and perfusion injury. Methods The marginal pancreases include those from cardiac death donor, fatty pancreas and edema pancreas from perfusion injury. Islets were isolated and purified using a modified University of Minnesota method. Islet yield and purity was determined by Dithizone (DTZ) staining and microscopic examination. Islet viability was assessed by AO/EB staining, and islet function was assessed by static glucose stimulation test. Results In the cardiac death donor group, the islet quality, viability, and in vitro function were similar when the warm ischemic time within 15 min. The quality and viability was decreased when the warm ischemic time beyond 30 min, but the function remained well. With 45 min warm ischemic time, insulin release index was decreased significantly. The islet quality, viability, and in vitro function from severe obesity group and severe edema group were decreased obviously. Conclusion Donor factors play a vital role in pancreas transplant outcomes. We concluded that pancreas severe obesity, severe edema and pancreas from cardiac donors (warm ischemic time >30 min) are unsuitable for islet isolation.

9.Study of biomechanical properties of mucosa scars after cleft palate surgery.

OBJECTIVETo explore biomechanical properties and stress-strain of mucosa scars after cleft palate surgery.
METHODSAfter the model of mucosa scars was made, the mucosa scars and normal mucosa were excised and examined immediately by tensionometry.
RESULTSThe mucosa scars after cleft palate surgery were compared with normal mucosa. The Poisson's ratio of mucosa scars and normal mucosa was 0.5 and 0.49, respectively, showing no significant difference between the two groups. The ultimate Young's modulus of mucosa scars was about 24.22 MPa, however, it declined to 3.32 Mpa in normal mucosa.
CONCLUSIONSThe mucosa scars after cleft palate surgery are biomechanically weaker than normal mucosa. It can be used for further research, such as maxillary orthognathic surgery, distraction osteogenesis, and orthodontic treatment.

10.Anatomical study and clinical application for the vascular axis fasciocutaneous flap with sural nerve preserved

Objective To explore the feasibility, method and indications of distally-based vascular fasciocutaneons island flap with sural nerve preserved for improving the sensory absence of lateral foot after traditional surgical procedures. Methods Sixteen lower limbs of 8 cadavers, 8 fresh limbs were infuse dthe red and blue latexes via femoral artery and vein. After 48 hours, anatomical study of the sural nerves and lesser saphenons veins with their axial arteries was performed. Clinically, 12 cases of the skin defects of foot and anterior tibias were repaired by vascular fascioeutanoons island flap, in which the aural nerves were dissociated and preserved from the flaps. Results There was a respective axial artery accompanying the sural nerve and the lesser saphonous vein. The distances between axial artery and nerve and vein were uncertain, range from 1-8 mm. Furthermore, the 2 axial arteries are communicated each other. The furthest perforators of sural artery were 12-32 nun from the top of lateral ankle, the average was 22 mm. The diameters of the perforator were 1.0-1.5 mm, the average were 1.2 mm. The sural nerves of all 12 cases were preserved in surgery. The skin sensation of the innervation of sural nerve was S3+- S4 in 4 cases, and S2+ - S3 in 4 cases. The sensation of lateral ankle area was So in 2 cases, and the lateral foot area was S1 in another 2 cases. After 6-24 months (mean 14 months) follow-up, 4 cases of S2-S3 recovered to normal in 5-30 d after surgery. Conclusion There is a positive value of preserving sural nerve for the foot sensation. The repective axial arteries accompanying the sural nerve and the lesser saphenons vein are the anatomic bases of preserving nerve. Be cantion of injuring the accompanying artery while surgery.
