5.Clinic analysis on colonic polyps of elderly people

Objective This study was designed to understand the characteristics of colonic polyps in aged people, and the relationship between the colonic polyps and the malignant change. Methods A total of 158 patients with colonic polyps diagnosed by endoscopy were retrospectively analyzed during the recent years. Among them, 120 patients had been followed up by endoscopy for 1 to 6 years(average 4 5 years) and were compared with 437 young patients. Results The detective rate and the malignant change rate of colonic polyps in the aged people were 30% and 23 4%, respectively, being significantly higher than that of the young (10 2%) and the middle aged people(6 9%, P

By using the software CiteSpace Ⅱ,the paper makes a quantitative analysis and interpretation of research literatures on telemedicine in SCIE of Web of Science from three perspectives,namely the research hotspots,knowledge base and research fronts.The research hotspots of telemedicine mainly include tele-diagnosis,tele-monitoring and evaluation of the application effects of telemedicine.It sets forth key literatures representing the knowledge base one by one and reveals that most of them focus on clinical applications.The analysis of burst terms indicates that more attention has been paid to tele-monitoring and distance education of chronic diseases in recent years and mobile technology is one of the greatest driving forces for the development of telemedicine at the present stage.
7.Kinetics model of spherical immobilized cellulase.

A kinetics model was developed for predicting and simulating immobilized cellulase performance, which follows Michaelis-Menten kinetics with competitive product inhibition. Taking into account the effects of competitive product inhibition, inner diffusional limitation, substrate concentration and carrier size, the substrate distribution and the product distribution in carriers were investigated, and the effectiveness factors were also calculated over a wide range of parameters. The effects of competitive product inhibition are shown to increase the substrate concentration in the carrier, and, additionally, to increase the effectiveness factors slightly. With the increase of inner diffusion coefficient, both the effectiveness factors and the substrate concentration in the carrier increase. As the carrier size increases, on the other hand, these values decrease. The effectiveness factors and the substrate concentration in the carrier are found to increase when substrate concentration in the reaction system increases.

8.Efficacy of laryngeal mask airway Classic, Streamlined Liner of Pharyngeal Airway and laryngeal mask airway ProSeal in patients undergoing laparoscopic surgery

Objective To compare the efficacy of the laryngeal mask airway Classic (CLMA), Streamlined Liner of Pharyngeal Airway (SLIPA) and laryngeal mask airway ProSeal (PLMA) in patients undergoing laparoscopic surgery. Methods One hundred and twenty-three ASA Ⅰ or Ⅱ patients aged 20-64 yr undergoing elective laparoscopic surgery were randomly divided into 3 groups: CLMA group (group C, n =41 ), SLIPA group (group S, n =42) and PLMA group (group P, n =40). Laryngeal mask was inserted after induction of anesthesia with sufentanil 0.3 μg/kg, atracurium 0.5 mg/kg and propofol 2 mg/kg. Mll the patients were ventilated with intermittent positive-pressure ventilation. Anesthesia was maintained with contiunous infusion of propofol 6-8 mg· kg-1· h-1 and with supplementary administration of sufentsnil 0.1-0.2 μg/kg and atracurium 0.2 mg/kg as required. The degree of difficulty in inserting the laryngeal mask was evaluated. The rate of successful placement at first attempt, device placement time, recovery time and complications were recorded . Peak inspiratory pressure (PIP) and the number of patients with an airway sealing pressure less than PIP were recorded before pneumoperitoneum and when intra-abdominal pressure achieved 12 mm Hg. Results The placement was easier and the placement time and recovery time were significantly shorter in group S, and the placement time was significantly longer,the airway sealing pressure was significantly higher and the number of patients with an airway sealing pressure less than PIP was lower in group P than in group C ( P < 0.05). The placement time and recovery time were significantly longer, the placement was harder, the airway sealing pressure was significantly higher, and the number of patients with an airway sealing pressure less than PIP was lower in group P than in group S ( P < 0.05). PIP was significantiy higher when intra-abdominal pressure achieved 12 mm Hg than that before pneumoperitoneum in all the three groups ( P < 0.05). There was no significant difference in the incidence of complications and rate of successful placement at first attempt among the three groups ( P > 0.05). Conclusion All the three airway devices can provide adequate ventilation during operation, with fewer complications. SLIPA placement is easiest. PLMA can assure good airway sealing and is most suitable for patients undergoing laparoscopic surgery.

10.Detection of activated cytotoxic cells in lymphoma by tissue microarray

Objective To detect the expression and distribution of activated cytotoxic cells in types of lymphoma with tissue microarray, and provide evidences for clinical treatment and prognosis. Methods Immunohistochemical staining by S-P technique was used to detect the expression and distribution of perforin and granzyme B in the lymphoma tissue microarray, composed of 60 cases of lymphoma tissue. Ten cases of NK/T-cell lymphoma routine sections were used for relative research, and ten cases of reactive hyperplasia were used for comparison. Results In the tissue microarray, originated from intranode and extranode were 48 cases and 12 cases, respectively; consisting of 42 cases of B-cell lymphoma, 16 cases of T-cell lymphoma, two cases of Hodgkin's disease. 42 cases of B-cell lymphoma cells were negative in perforin and granzyme B. In Ten cases of peripheral T-cell lymphoma, perforin and granzyme B positive were eight cases and nine cases, respectively, but the positive cells were no tumor cells. In 12 cases of NK/T-cell lymphoma (two in the tissue microarray, ten routine sections), both perforin and granzyme B were strongly positive. B-cell lymphoma, T-cell lymphoma and NK/T-cell lymphoma differed significantly (P<0.01). Conclusion Perforin and granzyme B were immunity markers for the identification of activated cytotoxic cells, which also could be used as diagnostic markers of NK/T-cell lymphoma. Their expression in B-cell lymphoma reflected the anti-tumor immunologic reaction of host. Tissue microarray technology has the behavior of high-throughput, convenient, effective, small experiment error, good replication and so on, and can be used as a useful tool for research of lymphoma.
