Purpose:To evaluate the different dose distribution created by three different plans on a nasopharyngeal carcinoma case with simultaneous integrated boost intensity modulated radiation therapy (SIB-IMRT). Methods:we designed three different SIB-IMRT plans on a nasopharyngeal carcinoma case, one was from anterior direction of the head, 5 non-equally spaced fields, another was from posterior direction of the head, 5 non-equally spaced fields also, third was 5 equally spaced fields around the head. Each plan had its dose distribution to the target and main critical structures.Results:No significant difference of dose distribution to the target when comparing the previous three different IMRT plans. However, dose to the parotids delivered from 5-equally spacd was obviously lower than the other plans, dose to the optic chiasm was much lower created by the plan designed from the posterior direction of the head with 5 non-equally spacea field.Conclusions:For a nasopharyngeal carcinoma case, there is no significant difference of dose distribution to the target compared with three different 5-beam orientation SIB-IMRT plans. To protect parotids, we recommend designing a plan with equal beam direction around the head. When the target is very close to the optic chiasm, plauning from posterior direction of the head should be better.

Objective To analyze the CT signs and the clinical relevant factors of degenerative osteoarthritis of athnto-odontoid joint.Methods The CT signs and the clinical information of 214 patients received CT scans of skull from June 2011 to April 2012 were analyzed retrospectively.Results One hundred and twenty patients were diagnosed with degenerative osteoarthritis of atlanto-odontoid joint,while 67 of them having subaxial degenerative osteoarthritis at the same time.Forty-three patients had a history of chronic cervical and occipital pain.95.83％ (115/120) patients showed osteophytes in CT scans,while 44.17％ (53/1 20) showed calcification around dens and 24.17％ (29/120) showed joint space obliteration.The incidence rate increased with age.Seventy-seven patients [64.17％(77/120)] appeared cervical and occipital pain.Conclusions Degenerative osteoarthritis of atlanto-odontoid joint shows hyperostosis,osteophytes at the edge of joint,joint space obliteration and calcification around dens in the CT scans.The incidence rate is increased with age and correlated with cervical and occipital pain.

Objective To investigate the inactivating effect of heat and ultraviolet(UV) light on HCV JFH-1 strain using the cell culture system. Methods The HCV JFH-1 virus stock, with an initial titer of 2.5 × 104 FFU/ml, was exposed in 56℃ water bath or to UV light for varying durations of time for explo-ring their inactivating effects on the virus. The kinetics of virus titer reduction was determined by an indirect immuno-fluorescence assay (IFA). If the cells infected with the exposed virus stock were IFA negative after three blind passages, the virus stock was considered to be inactivated completely. Results After incubation of the HCV JFH-1 virus stock (2.5 × 104 FFU/ml)in 56℃ water bath for 10 min, 20 min and 30 min, the virus titers were reduced to 1.6 × 103 FFU/ml, 3.1 × 102 FFU/ml and 3.3 × 10 FFU/ml, respectively. The exposure of the virus stock to UV light (wavelength 253.7 nm, intensity ≥60 μW/cm2, 30 cm below the UV lamp) for 15 s, 30 s and 45 s resulted in virus fiter reduction to 1.0 × 103 FFU/ml, 1.1 × 102 FFU/ml and 2.7 × 10 FFU/ml, respectively. After 40 min incubation of the virus stock at 56℃, or 1 min exposure to UV light (wavelength 253.7 nm, intensity ≥60 μW/cm2) the virus infectious titer was reduced below the detection limit of IFA, and the IFA was still negative even after three blind passages, indicating that the virus was inactivated completely. Conclusion HCV is sensitive to heat and UV light treatment. For HCV JFH-1 virus stock containing 2.5 × 104 FFU/ml virus, heat treatment at 56℃ for 40 min, or UV light expo-sure at an intensity of ≥60 μW/cm2 for 1 min, resulting in complete virus inactivation.

BACKGROUND:Studies have shown that tibial plateau posterolateral fractures can be treated by posterolateral knee approach, however, there is no clear anatomical research to deeply evaluate and analyze this approach. OBJECTIVE:To evaluate the efficacy and safety of posterolateral knee inverted“L”shaped pathway in invloving tibial plateau posterolateral fractures. METHODS:Eight fresh frozen adult corpses, 16 knees side, were al dissected using posterolateral inverted“L”shaped pathway. During the dissection, the exposure range was observed and important parameters of anatomical structure were measured. RESULTS AND CONCLUSION:The pathway may be ful y exposed to the posterolateral aspect of tibial plateau and posterior cruciate ligament tibial insertions. The operations completed by out team did not create any obvious interference to superior tibiofibular joint, fibular head and posterolateral corner structure. The exposed mean length of common peroneal nerve in incision was 56.48 mm, with a mean angle of 14.7° tilt towards the axis of the fibula. The mean distance between the neck of the fibular and fibular head tip was 31.26 mm, an average of 42.18 mm to the joint line. The mean distance between the opening of the interosseous membrane and the articular surface was 48.78 mm. The divergence of the fibular artery from the posterior tibial artery was an average of 76.46 mm from articular surface. These results confirm that posterolateral inverted“L”shaped pathway met the requirements of anatomical reduction and buttress fixation for posterolateral tibial plateau fracture. Exposure of the common peroneal nerve can be minimized or even avoided by modifying the skin incision. Because the popliteal artery branches anterior tibial artery passed through interosseous membrane hole and peroneal artery and then separated from the posterior tibial artery, pathways dissection to distal deep area should be carried out careful y. Placement of a posterior buttressing plate carries a high vascular risk if the plate is implanted beneath these vessels.

Objective To explore the biological effects of the luteinizing hormone receptor (LHR) and vascular endothelial growth factor (VEGF) on the ovary of mice during peri-implantation. Methods The immunohistochemistry SABC method and image analysis were used to study the distribution and changes of the LHR and VEGF in Kunming mouse( n =28) ovary during estrous,pregnancy of day 1, day 4 and day 6 stage. Results The expression of LHR-immunoreactive substance and that of VEGF-immunoreactive substance had the same distribution and changes. Compared with other groups,the level of LHR-immunoreactive substance and that of VEGF-immunoreactive substance increased highly on the stroma cells around largergrowing follicles in estrous group ( P <0.05). Along with the pregnancy, the positive immunostaining for LHR and VEGF increased gradually on the granulosa lutein cells, and reached the highest level on day 6 of pregnancy. Positive immunostaining for LHR or VEGF on some endothelia and blood cells were observed in day 1 of pregnancy or estrous group respectively. Form day 1 of pregnancy, the theca cells had positive immunostaining for LHR. Conclusion The expression of LHR and VEGF is closely related with the process of follicle growing, ovulation and corpus luteum formation.

Objective To observe the clinical effects of eyehd reconstruction by regional flap combined with xenogeneic acellular dermal matrix transplantation for eyelid defect after malignant tumor excision.Methods Thirty-five cases (35 eyes) in our hospital were selected as the objects.Among them,basal cell carcinoma was 21 cases,meibomian gland carcinoma was 13 cases,squamous cell carcinoma was 1 cases;12 cases of upper eyelid and 23 case of lower eyelid were involved.All patients were subjected to intraoperative frozen,and the incision margin was determined according to the frozen results.After resection of the tumor,the eyelid had full-thickness defects in different degrees.The xenogenic acellular dermal matrix was used to replace the conjunctival tarsal tissue,and the adjacent flap or transposition flap was used to repair the eyelid defect according to the size of skin defect.The healing of flap,oral repair film,eyelid closure and adhesion were observed.Results After half a year follow-up,the acellular dermal matrix were completely dissolved by crawling the conjunctival epithelium covering,the flap healed with no flap necrosis.28 patients recovered well after operation without hypophasis and entropion,ectropion.4 patients had mild hypophasis,and there was no case of exposure keratitis.3 patients were with mild symblepharon.Conclusion The acellular dermal matrix can replace tarsal conjunctival tissue,which combined with regional flap has good clinical curative effect for eyelid defect after malignant tumor excision.This treatment can reduce the pain of patients who take oral mucosa and avoid the reoperation of eyelid reconstruction.

Objective:To explore the effects of omeprazole combined with somatostatin on intra-abdominal pressure and intestinal mucosal barrier function of elderly patients with severe acute pancreatitis.Methods:Selected 82 cases of elderly patients with severe acute pancreatitis from our hospital.They were divided into research group (48 cases) and control group (34 cases).The control group was treated with basic therapy,and the research group was treated with omeprazole combined with somatostatin on the basis of the control group.Analyzed and compared the clinical effects,intestinal mucosal barrier function and symptom indexes of patients in the two groups.Results:①The total effective rate of the research group was significantly better than that of the control group,and the difference was statistically significant (P=0.005).②After treatment,the D-lactose and diamine oxidase (DAO) levels of both groups were significantly lower than before treatment,and the difference was statistically significant (both P＜0.001).In addition,after treatment,the D-lactose and DAO levels were significantly lower in the research group than in the control group,and the differences were statistically significant (mean P ＜ 0.001).③After treatment,patients of the research group showed better results in abdominal pain and abdominal distension relief time,bowel recovery time,hospital stay and blood amylase index than those in the control group,and the difference was statistically significant (mean P ＜ 0.001).④After treatment,the intra-abdominal hypertension of two groups was significantly lower than before treatment,with statistical difference (P ＜ 0.001).Yet it was significantly lower in the research group than in the control group,with statistical difference (P ＜ 0.001).Conclusion:The combined treatment of omeprazole and somatostatin can effectively improve the clinical efficacy of severe acute pancreatitis in elderly patients,correct intra-abdominal hypertension and improve the intestinal mucosal barrier function of patients,so it is worthy of reference and promotion.

OBJECTIVETo identify the Cynomorii Herba and its analogues species using DNA barcoding technique.

METHODTotal genomic DNA extracted from all materials using the DNA extraction kit. The internal transcribed spacer 2 (ITS2) regions were amplified using polymerase chain reaction (PCR), and purified PCR products were sequenced bi-directionally. Sequence assembly and consensus sequence generation were performed using the CodonCode Aligner 3.7.1. The Kimura 2-Parameter (K2P) distances and GC content were computed using MEGA 5. 0. Species identification analyses were conducted through the species identification system for traditional Chinese medicine and neighbor-joining (NJ) trees.

RESULTThe ITS2 sequence lengths of Cynomorii Herba were 229 bp. The average intra-specific genetic distances of Cynomorii Herba were 0.003. The average inter-specific genetic distances between Cynomorii Herba and its adulterants species were 0.760. The results showed that the minimum inter-specific divergence is larger than the maximum intra-specific divergence. The species identification system for traditional Chinese medicine and NJ trees results indicated that Cynomorii Herba and its adulterants species can be easily identification.

CONCLUSIONThe ITS2 region is an efficient barcode for identification of Cynomorii Herba, which provide a new technique to ensure clinical safety in utilization of traditional Chinese medicine.

Objective To investigate the value of whole-liver MR perfusion imaging(MRPI)for early detection of coagulative necrosis after percutaneous ethanol injection(PEI)in rabbit liver VX_2 tumors. Methods VX_2 tumor cell suspension was inoculated into rabbit liver and liver VX_2 tumors[diameter of (2.6?0.6)cm]were induced in 10 male rabbits.MR T_1 WI and T_2 WI were performed to monitor the development of the liver tumor on the 2~(nd)and 3~(rd)week after inoculation.Whole-liver MRPI was performed in the 10 rabbits with liver VX_2 tumors before and 6 days after PEI therapy(1.0 ml ethanol was injected into the most enhanced tumor region under CT guiding).Signal intensity(SI)values of untreated tumor parts and treated areas 6 days after PEI were recorded respectively.The steepest slope(SS)and bolus arrival time (TO)of SI-time curves were measured.The t-Student test was used in statistical analysis of the data.Results There was significant difference in MRPI data between untreated tumor parts[TO:(16.0?1.2)s and SS: 38.9?2.2]and treated areas[TO:(50.8?5.9)s and SS:6.0?1.2]6 days after PEI(t was 15.8 and -39.6 respectively,P

In order to construct an integrated DNA barcoding database for identifying Chinese animal medicine, the authors and their cooperators have completed a lot of researches for identifying Chinese animal medicines using DNA barcoding technology. Sequences from GenBank have been analyzed simultaneously. Three different methods, BLAST, barcoding gap and Tree building, have been used to confirm the reliabilities of barcode records in the database. The integrated DNA barcoding database for identifying Chinese animal medicine has been constructed using three different parts: specimen, sequence and literature information. This database contained about 800 animal medicines and the adulterants and closely related species. Unknown specimens can be identified by pasting their sequence record into the window on the ID page of species identification system for traditional Chinese medicine (www. tcmbarcode. cn). The integrated DNA barcoding database for identifying Chinese animal medicine is significantly important for animal species identification, rare and endangered species conservation and sustainable utilization of animal resources.
