1.Perioperative nursing of modified maze procedure using bipolar radiofrequency ablation accompanied with valve replacement for surgical treatment of permanent atrial fibrillation

Objective To summarize the perioperative nursing of modified maze procedure using bipolar radiofrequency ablation accompanied with valve replacement for the surgical treatment of heart valve diseases complicated with permanent atrial fibrillation (AF).Methods A total of 55 patients with permanent AF and heart valves diseases were undergone surgical treatment from June 2010 to February 2013 in the Second Affiliated Hospital of Anhui Medical University.Preoperative psychological care and health education were given to patients.The Medtronic Cardioblate 68000 flush bipolar radiofrequency ablation system was applied to all patients.After valve replacement surgery in addition to routine care,rhythm and heart rate monitoring were paid attention to,maintenance of cardiac function,observation of drainage,maintenance of electrolyte balance,observation of pacemaker,observation of amiodarone medication and nursing,as well as health guidance and psychological care were implemented.Results The surgery of 55 patients was successfully completed.The day after surgery,atrial fibrillation in 48 cases turned into sinus rhythm.In all patients,2 of them had suffered Ⅲ degree atrioventricular block after surgery and permanent pacemakers were applicated.There were no cardiac perforation and non-hospital mortality.The average length of stay was(12.5±2.1) d.The mean follow-up was(8.6±3.0) months.87.3% of patients maintained sinus rhythm,9.1% in AF rhythm,3.6% in paced rhythm.Conclusions Modified maze procedure using bipolar radiofrequency ablation is a simple,safe and effective surgical procedure for the treatment of permanent atrial fibrillation.High quality of perioperative nursing is to improve the success rate of surgery and the key to reduce mortality.
2.Study on mechanisms of gastrointestinal symptoms in renal insufficiency

Objective To investigate the gastric empty and electrogastrogram(EGG) in patients with renal insufficiency before and after hemodialysis, and to study the enteric nervous system (ENS) in experimental acute renal failure rats.Methods The comparisons of gastric emptying and EGG before and after hemodialysis were carried out in 15 patients with uremia who have gastrointerstinal symptoms. Acute renal failure models were also established. The neuroplexus of ENS and levels of some gut hormones were investigated. Results The EGG and gastric emptying were abnormal in patients with uremia before hemodialysis, but improved after dialysis. The neuroplexus of ENS with positive nitric oxide synthase(NOS) staining in rats with acute renal failure was significantly decreased. The levels of gastrin, vasoactive intestinal peptide and motilin in rats with acute renal failure were (159.1?55.6)?g/L, (121.6?48.9)pg/L, and (6.32?3.55)pmol/L, respectively, which were significantly different from those of the control group(P
3.Effect of working memory training task on visual contrast sensitivity for college students

Objective To explore the influence on college students' contrast sensitivity how dual N-back working memory training task based on Gabor,and to provide new training method for improving the contrast sensitivity.Methods 42 college students were randomly divided into three groups:a control group (17 students) ; a working memory training group (13 students) ; and a Gabor orientation training group (12 students).The control group received no training,while the working memory training group and the Gabor orientation training group received N-back working memory training and Gabor training,respectively,over three weeks (5 days a week,approx.half an hour per day).The above-mentioned two training methods performed with the same visual stimulus,i.e.,the Gabor patch with a fixed spatial frequency and contrast.The contrast sensitivity of the 42 students was measured and recorded using OPTEC6500 before and after training.Results In the working memory training group,a salient gain in contrast sensitivity was observed after the training (before:(1.48 ± 0.26) log,after:(1.87 ± 0.13)log),with a statistically significant difference (t =-6.20,P < 0.05),while both the Gabor orientation training group (before:(1.75 ± 0.19) log,after:(1.68 ± 0.30) log) and the control group (before:(1.67 ±0.25) log,after:(1.61 ± 0.26) log)) showed no statistically significant difference (P > 0.05) in terms of contrast sensitivity.Conclusion N-back working memory training shows a positive effect on improving contrast sensitivity in Gabor identification training,and can be used as a means to improve contrast sensitivity.
4.Correlation between Gross Motor Function and Activities of Daily Living in Children with Cerebral Palsy

Objective To analyze the correlation between gross motor function and activities of daily living(ADL)in children with cerebral palsy.Methods 42 children with CP(26 males,16 females,average age 49.5±21.5 months,11 children with spastic quadriplegia,22 with spastic diplegia,5 with spastic hemiplegia,2 with athetosis and 2 with dystonia)were assessed with Gross Motor Function Classification System(GMFCS)and Gross Motor Function Measure(GMFM)for the assessment and classification of gross motor function.Pediatric Evaluation of Disability Inventory(PEDI)was used to assess the ADL.The ADL of children with different levels of GMFCS and the correlation between gross motor function and ADL were analyzed.Results The more severe of GMFCS,the less standard score of PEDI.There was the highest correlation between the GMFM66 scores and scaled scores of PEDI in mobility and social function(r=0.84),whereas a weaker correlation appeared between GMFM66 scores and scaled scores of PEDI in social function(r=0.72).Conclusion Since the gross motor function of children with CP greatly affects the ADL,the gross motor function training should be the main item for CP children,the training goal of ADL should be made according to the levels of GMFCS.
5.Endoscopic ultrasonOgraphy and CT in preoperative TN staging of esophageal cancer

objective To retrospectively evaluate the accuracy of endoscopic ultrasonography (EUS)and CT in preoperative tumor,and nodal metastasis(TN)staging of esophageal carcinoma.Methods TN stages of 87 cases diagnosed with preoperative EUS and CT were compared with postoperative pathological results.No patient underwent radiotherapy or chemotheraphy.The radial echoendoscope was used,and balloon dilation was required in 5 cases with stricture.Results The total accuracy of T staging with EUS was 85.1%.CT could not differentiate Tl from T2.The sensitivity of EUS for N staging was 85.0%,higher than that of CT(60.8%).However,some lymph nodes which were not detected by EUS could be revealed by CT.Accuracy of EUS plus CT in T staging is 85.1%.and that in N staging is 90.8%.Conclusion EUS is the most accurate measure in assessing the depth of tumor invasion,whereas the combination of EUS and CT is capable of an overall evaluation for TNM staging.
6.Protection of neurons in vitro and improvement of learning and memory in mice by 2-phenoxy-indan-1-one derivatives

Aim To study the effect of novel AChE inhibitors, 2-phenoxy-indan-1-one derivatives (YKY-1~7), against glutamatic acid-induced neurotoxicity in PC12 cells and on learning & memory impairment in dementia model mice induced by A?25~35 icv Methods The PC12 cells were preincubated with different concentrations of YKY-1~7 for 24 h and subsequently treated by glutamatic acid, at the high concentration of 2 mmol?L-1 for 15 min to induce cytotoxicity. The cell viability was assessed with MTT method.. Dementia model mice were made by intracerebroventricular injection (icv) of aggregated A?25~35. From the next day, the model mice were administered YKY-7 (2.5, 5, 10 mg?kg-1, ig) for 10 consecutive days and sham control mice or A? model control mice received daily ig saline. After the final treatment, the passive avoidance learning was tested, regional cerebral blood flow at cerebral cortex was assessed, and the activity of AChE in the cerebral cortex, hippocampus and blood serum were determined. Results Six out of the seven YKY compounds appeared to be effective against glutamatic acid-induced neurotoxicity in PC12 cells, with YKY-7 demonstrating the most activity. YKY-7 significantly ameliorated the learning and memory ability in dementia model mice induced by A?25-35 icv, slightly and selectively inhibited the cortical and hippocampal AChE, and gently increased the blood flow at cerebral cortex. Conclusion Some of 2-phenoxy-indan-1-one derivatives reported here have protective effects against glutamatic acid induced neurotoxicity in PC12 cells, and improve the learning and memory impairment induced by A?25-35, which may be partly attributable to its selective inhibition of AChE activity in the cerebral cortex and hippocampus.
7.Electroencephalogram and CT Analysis of 224 Children with Cerebral Palsy

Objective To study the application value of electroencephalogram and CT in children with cerebral palsy.Methods Electroencephalogram and CT examinations were carried out in 224 children with cerebral palsy.Results Abnormal prevalence of electroencephalogram was 47.77%, 58.88% of which were found as epileptic discharge. Abnormal prevalence of CT was 79.46%. The abnormal rate of CT was significantly higher than that of electroencephalogram ( P<0.001). The abnormal rates of electroencephalogram and CT among different types of cerebral palsy were not significantly different ( P>0.05).Conclusion Combination of CT and electroencephalogram will be helpful for the diagnosis and treatment in children with cerebral palsy.
8.Mechanism of "treating heart and brain with same methods" based on data science.

The traditional Chinese medicine (TCM) theory of "treating heart and brain diseases with same methods (Nao Xin Tong Zhi: NXTZ)" has great significance to the treatment of cardiovascular and cerebrovascular diseases. It has been proven effective by a great deal of clinical researches. However, the underlying mechanism for this theory is still unclear. To provide insights into the potential mechanism of "NXTZ", this study attempts to deeply investigate the mechanism from two representative cardiovascular and cerebrovascular diseases, coronary heart disease (CHD) and cerebral apoplexy. First, various data resources were integrated to obtain different types of biomedical entities including drugs, targets, pathways and diseases. Then, three different approaches including text mining, biological network and enrichment analysis were utilized to recognize the potential common features between CHD and cerebral apoplexy, and the corresponding functions of drugs which could treat both diseases, thus unveiling the mechanism of NXTZ.

10.The relationship between inhibition of hepatitis B virus replication induced by tumor-necrosis factor α and the up-regulated proprotein convertases

Objective To investigate the effects of tumor-necrosis factor α (TNF-α) on the expressions of proprotein convertases (PC) and its relationship with the inhibition of hepatitis B virus (HBV) replication.Methods HepG2.2.15 cells cultured routinely were exposed to 20 μg/L recombinant TNF-α and/or 20 μmol/L PC inhibitor (DEC) for 18 h.Then Followed cells werecollected and cell total RNA and HBV DNA were extracted.PC mRNA and core-associated HBV DNA were measured using real-time polymerase chain reaction (PCR) techniques.Measurement data was compared using t-test.Results When PC mRNA expressions in the blank group was as to 1,the expressions of PC1/3、PC2、furin、PC4 、PC5/6 、PACE4 and PC7/8 mRNA in HepG2.2.15 cells treated with 20μg/L TNF-α treatment for 18 h were all up-regulated,which were 3.3±0.7、79.3±3.3、77.5±1.3、19.2±3.1、1.3±0.1、1.4± 0.2、274.8± 7.1,respectively (all P<0.05).Treatment of 20 μg/L recombinant TNF-α for 18 h significantly reduced core-associated HBV DNA compared with blank gourp (0.21∶1,t =8.79,P =0.002),while 20 μmol/L DEC significantly up-regulated core-associated HBV DNA (3.84∶ 1,t=7.67,P=0.004).Moreover,core-associated HBV DNA in group of DEC and TNF-α treatment was significantly higher than group of TNF-α treatment (0.31∶0.21,t=10.49,P=0.007).Conclusion Up-regulated PC mRNA expression induced by TNF-α is significantly associated with the inhibition of HBV replication.
