1.Preparation of Quality Control of Chloramphenicol and Ephedrine Nasal Gel

OBJECTIVE:To prepare chloramphenicol and ephedrine nasal gel and establish its quality control method.METHODS:Gel was prepared with chloramphenicol and ephedrine hydrochloride as principal agents and with carbomer-940 as base.The contents of the principal agents were determined by HPLC and the stability of the gel was investigated as well.RESULTS:The preparation was transparent colloid.The linear ranges of chloramphenicol and ephedrine hydrochloride were 62.5~1 000 and 31.25~500 ?g?L-1(r=0.999 9),respectively,with average recovery rates at 99.64%(RSD=0.5%)and 99.57%(RSD=0.7%),respectively.Storing under room temperature,no obvious change was noted for the property of the sample at 12 months.CONCLUSION:The preparation technique of the gel is simple and feasible,and its quality is stable and controllable.
2.Effect of Stellate Ganglion Block on Patients with Cerebral Infarction:Report of 36 Cases

Objective To observe the effect of stellate ganglion block on patients with cerebral infarction.Methods36 patients with cerebral infarction were randomly divided into the control group and research group.Both two groups were treated with traditional treatment.The research group was added with stellate ganglion block.ResultsAfter 20 days treatment,the Neuro-function Deficit(NFD) scores of both two groups were significantly lower than that before treatment(P<0.01);but the NFD scores of the research group were significantly lower than that of the control group(P<0.01).The scores of Fugl-Meyer Assessment(FMA) and Barthel Index(BI) of both two groups were significantly higher after treatment(P<0.01);but the scores of FMA and BI of the research group were significantly higher than that of the control group(P<0.01).ConclusionStellate ganglion block can improve the nerve function of patients with cerebral infarction.
3.Establishment and assessment of orthotropic and heterotrophic valve implantation models in goat with minimally invasive methods

Objective To establish and evaluate 3 kinds of minimally-invasive valve implantation model in vivo.Methods A novel tissue engineered heart valve(TEHV) manufactured by branched polyethylene glycol cross-linked acellular porcine valve and a minimally-invasive valve implantation system according to the design of Corevalve revalving system were adopted.After anesthesia,18 adult male goats were randomly divided into 3 groups: the ulrasound-directed orthotropic group (group A,n =6),angiography-directed orthotropic group (group B,n =6) and direct-released heterotopic group (group C,n =6),and all received minimally-invasive valve implantation orthotropically or heterotopically.4 weeks later,the valvular function was evaluated by CTA and/or echocardiography.Results All 3 kinds of caprine model were successfully constructed.The operation success rate of each group was A: 66.7%,B: 50.0% and C: 100.0%,respectively(multiple x2 analysis,group A and B P >0.05; group A and C,group B and C,P <0.05).The operation-time of each group was A: (79 ± 18) min,B:(61 ±23) min,C: (45 ± 15) min(one-way ANOVA,P <0.05).The survival rate at4 weeks was A: 100%,B: 100% and C: 83.3% (multiple x2 analysis,P > 0.05).Echocardiography and CTA proved the short-term function of implanted TEHV was satisfactory.Conclusion All 3 kinds of caprine valve implantation model can be established without cardiopulmonary bypass and blood transfusion.The devices and equipments required in group A is relatively simple,but the procedure cost longer time for it is hard to determine the right position by ultrasound.The application of angiography made the positioning much easier in group B while the procedure had to be performed in specific operation room with angiographic apparatus.Group C did rely on neither special equipments nor complex operation,but the valve leaflets cannot work normally,so this model was only suitable for testing in vivo characteristics such as biocompatiblities.
4.Application of decellularized scaffold combined with loaded nanoparticles for heart valve tissue engineering in vitro.

The purpose of this study was to fabricate decelluarized valve scaffold modified with polyethylene glycol nanoparticles loaded with transforming growth factor-β1 (TGF-β1), by which to improve the extracellular matrix microenvironment for heart valve tissue engineering in vitro. Polyethylene glycol nanoparticles were obtained by an emulsion-crosslinking method, and their morphology was observed under a scanning electron microscope. Decelluarized valve scaffolds, prepared by using trypsinase and TritonX-100, were modified with nanoparticles by carbodiimide, and then TGF-β1 was loaded into them by adsorption. The TGF-β1 delivery of the fabricated scaffold was measured by asing enzyme-linked immunosorbent assay. Whether unseeded or reseeded with myofibroblast from rats, the morphologic, biochemical and biomechanical characteristics of hybrid scaffolds were tested and compared with decelluarized scaffolds under the same conditions. The enzyme-linked immunosorbent assay revealed a typical delivery of nanoparticles. The morphologic observations and biological data analysis indicated that fabricated scaffolds possessed advantageous biocompatibility and biomechanical property beyond decelluarized scaffolds. Altogether this study proved that it was feasible to fabricate the hybrid scaffold and effective to improve extracellular matrix microenvironment, which is beneficial for an application in heart valve tissue engineering.
6.The relationship between angle of puncture and distribution of bone cement of unilateral percutaneous kyphoplasty for the treatment of thoracolumbar compression fractures.

OBJECTIVETo explore the relationship of bone cement distribution and the puncture angle in the treatment of thoracolumbar compression fractures with unilateral percutaneous kyphoplasty (PKP).
METHODSThe clinical data of 37 patients with thoracolumbar osteoporotic compression fractures underwent PKP between January 2013 to March 2014 were retrospectively analyzed, all punctures were performed unilaterally. There were 6 males, aged from 65 to 78 years old with an average of (71.83 ± 6.15) years; and 31 females, aged from 57 to 89 years old with an average of (71.06 ± 7.89) years. Imaging data were analyzed and puncture angle and puncture point were measured before operation. According to the measured data, the puncture were performeds during the operation. Distribution area of bone cement were calculated by X-rays data after operation. The effect of bone cement distribution on suitable puncture angle was analyzed; VAS score was used to evaluate the clinical effects.
RESULTSThe puncture angle of thoracic vertebrae in T8-T12 was from 28° to 33° with an average 30.4°; and the puncture angle of lumbar vertebrae in L1-L5 was from 28° to 35° with an average of 31.3°. Postoperative X-rays showed the area ratios of bilateral bone cement was 0.97 ± 0.15. Bilateral diffuse area were basic equal. Postoperative VAS score decreased significantly (1.89 ± 1.29 vs 7.03 ± 1.42).
CONCLUSIONThrough measure imaging data before operation with PKP,the puncture point and entry point can be confirmed. According the measured data to puncture during operation, unilateral puncture can reach the distribution effect of the bilateral puncture in the treatment of thoracolumbar compression fractures.

7.NRP1 expression is associated with chemosensitivity and poor prognosis in advanced non-small cell lung cancer patients treated with ifrst-line platinum-based chemotherapy

Background and purpose:Neuropilin-1 (NRP1), a vascular endothelial growth factor (VEGF) receptor, plays an important role in tumor angiogenesis and tumor cell migration. The purpose of this study was to de-termine the correlation between NRP1 expression and sensitivity to ifrst-line platinum-based chemotherapy in patients with advanced non-small cell lung cancer (NSCLC), and between NRP1 expression and survival.Methods:NRP1 ex-pression in tumor tissues of 104 advanced NSCLC patients treated with ifrst-line platinum-based regimen was detected by immunohistochemisty.A chi-square test and logistic regression model were used to analyze the relationship between NRP1 expression and the chemotherapy response rate. Kaplan-Meier and Cox proportional hazard regression models were used to analyze the effect of NRP1 expression on patient survival.Results:Among the 104 patients, 56 (53.8%) had high expression of NRP1. High expression of NRP1 was not related to age, gender, histological type, degree of differentiation, performance status, and chemotherapy regimen. The chemotherapy response rate was significantly higher in patients with low NRP1 expression than in patients with high expression (43.8% vs23.2%,P=0.026). The low NRP1 expression was signiifcantly associated with longer progression-free survival (4.6 monthsvs3.0 months, P=0.001 for log-rank test,χ2=11.273) and overall survival (11.5 monthsvs9.2 months,P=0.000 for log-rank test,χ2=14.392) as compared with high NRP1 expression. Multivariate analysis showed that high expression of NRP1 was an independent predictor for the chemotherapy response rate and overall survival in patients with advanced NSCLC.Conclusion:NRP1 expression is associated with response rate and survival in advanced NSCLC patients treated with ifrst-line plati-num-based chemotherapy. NRP1 expression may be a potential biomarker for predicting chemosensitivity and prognosis in patients with advanced NSCLC.
8.The changes of body composition in patients with acromegaly after pituitary adenoma surgery

Objective To identify the changes of body composition with acromegaly before and after pituitary adeno-ma surgery,and to evaluate the relationship between these changes and serum growth hormone(GH),insulin-like growth factor-1(IGF-1).Methods Serum GH with OGTY,IGF-1 levels,BMI,fat percentage,FFM and total body water(TBW) in patients were measured in active and relieved period of the disease.Results The BMI and FFM with active acromegalic males were significantly higher,but fat percentage was significantly lower than that of healthy males.After the disease was relieved by surgery,serum GH and IGF-1 concentrations of patients were sig-nificantly reduced to normal level,but their BMI was still significantly higher,the fat percentage increased and the FFM decreased was correlated with serum nadin GH and IGF-1 levels.The BMI and FFM with active acromegalic females were significantly higher than healthy females,but the fat percentage had not significant change after sur-gery,and FFM decrease was correlated with reduced serum GH level.The fat percentage did not change signifi-cantly.Conclusion There was significant change of body composition with acromagely before and after treatment,and a relationship between these changes and serum GH and IGF-1 levels was identified.
9.Separation and purification of the active molluscicidal component from Reineckea carnea

Objective To get a molluscicide from Reineckea carnea which is effective on Oncomelania hupensis but has low poison on fish. Methods Methyl soaking, column chromatography and HPLC were used for isolation of molluscicidal component from Reineckea carnea. Results The snail mortality with Reineckea carnea powder was 86.0% for 72 h immersion and 100.0% for 120 h at the concentration of 17.5 mg/L, but the fish mortality was 0 at the concentration of 130 mg/L, indicating that Reineckea carnea had different effects on the two kinds of animals . The fraction A was obtained from Reineckea carnea by methyl immersion, column chromatography. The snail mortality with A was 84.4% for 144 h at the concentration of 10 mg/L, indicating that A had high molluscicidal effect on Oncomelania hupensis. After A being isolated by HPLC, 99.98% RC-Ⅰ was obtained from it . RC-Ⅰ was determined by API2000 LC/MS and other spectrum, and it was deduced to be one of steroidal glycosides. Conclusion Reineckea carnea contains a high effective molluscicidal component.
10.PCNA, Bcl-2 and TERT expression in epiretinal membrane of rat traumatic proliferative vitreoretinopathy

· AIM: To explore the dynamic expression and correlation among telomerase catalytic subunit (TERT), proliferating cell nuclear antigen ( PCNA) and antiapoptosis protein Bd-2 which relate to cell proliferation in epiretinal membrane of rat traumatic proliferative vitreoretinopathy(PVR).· METHODS: S-P technique was applied for immunohistochemical staining of epiretinal membrane of traumatic PVR with TERT, PCNA and Bcl-2 antibody. HE staining was also carried out. The staining results were analyzed with image analysis system.· RESULTS: The positive rate and average A of PCNA protein were upregulated at first and then down-regulated, with the peak value in 14d Group, which was significantly different from those in 7d Group and 28d Group.The positive rate and average A of TERT and Bcl-2 were also upregulated at first and then down-regulated, with the peak value in 14d Group and 21d Group, which were significantly different from those in 7d Group. There was significant correlation among PCNA, Bcl-2 and TERT protein expression (P≤0.01).· CONCLUSTON: TERT and Bcl-2 take part in the regulation of proliferative cells in epiretinal membrane of traumatic proliferative PVR, with high correlation with the dynamic changes of cell proliferation.
