Objective To compare the effects of different blood purification methods on the removal of large and medium molecular toxins in patients with maintenance hemodialysis(MHD), and to provide the clinical basis for selecting suitable dialysis modalities to prevent or delay the long term complications of uremic patients.Methods60 MHD patients with long term and stable hemodialysis were enrolled in this study. All of them were selected from the patients with insufficient hemodialysis.According to the digital table,60 patients were randomly divided into high flux hemodialysis (HFHD)group and hemodiafiltration (HDF) group,30 cases in each group. And before,3 months and 6 months after dialysis, the serum β2-MG,PTH and Cys C after dialysis were tested. Results The clearance of PTH in the HFHD group[(286.34±127.33)pg/mL] was significantly higher than that in the HDF group[(376.04±141.74)pg/mL],the difference was statistically significant(t=2.45,P<0.05).The clearance of β2-MG in the HDF group[(11.34±1.96)mg/L]was significantly higher than that in the HFHD group[(15.41±3.02)mg/L],the difference was significant(t=5.88,P<0.05).The clearance of Cys C in the HFHD group[(263.67±98.72)μg/dL]was significantly higher than that in the HDF group[(345.63±105.00)μg/dL],the difference was statistically significant(t=2.96,P<0.05).Conclusion Both two dialysis methods are very effective in the removal of large and medium molecular uremic toxins in patients. In the removal of patients with increased PTH,HFHD is more effective. In the removal of patients with increased β2-MG,HDF is more effective. In the removal of Cys C,HFHD is better.

Objective To investigate the gastric empty and electrogastrogram(EGG) in patients with renal insufficiency before and after hemodialysis, and to study the enteric nervous system (ENS) in experimental acute renal failure rats. Methods The comparisons of gastric emptying and EGG before and after hemodialysis were carried out in 15 patients with uremia who have gastrointerstinal symptoms. Acute renal failure models were also established. The neuroplexus of ENS and levels of some gut hormones were investigated. Results The EGG and gastric emptying were abnormal in patients with uremia before hemodialysis, but improved after dialysis. The neuroplexus of ENS with positive nitric oxide synthase(NOS) staining in rats with acute renal failure was significantly decreased. The levels of gastrin, vasoactive intestinal peptide and motilin in rats with acute renal failure were (159.1?55.6)?g/L, (121.6?48.9)pg/L, and (6.32?3.55)pmol/L, respectively, which were significantly different from those of the control group(P

Objective To evaluate the effect of transitional care on the quality of life in patients with maintenance hemodialysis (MHD).Methods 101 MHD patients with symptoms of kidney < 80 points were assigned into the interventional group (47 of 51 cases completed the study) and the control group (45 of 50 cases completed the study) by random number table method. The patients in the interventional group received transitional care for 6 months, while control group received routine care. The effects on function index, satisfaction index and clinical index were evaluated at the 3rd month and 6th month after intervention. Results 6 months after intervention, the scores of physical composite, mental composite, burden of kidney disease,symptoms/problems list, effects of kidney disease in the interventional group were 51.78±5.91,55.32±5.36,29.79±17.64,79.39±6.73,68.02±11.38,while the scores of the control group were 38.22±9.46,42.03±8.32,19.58±17.25,69.49±10.10,52.09±16.78,and the difference between the two groups was statistically significant (t=5.308-9.063,Z=-3.07,P < 0.01).The scores of self-management behavior, health locus of control in the interventional group were 71.30-±5.42,25.74±2.42,while the scores of the control group were 65.0±8.74,22.47±3.97 (t=4.132,4.759,P < 0.01).The nursing satisfaction in the interventional group scored 91.06±7.29,which was significantly higher than the control group scored 86.22±10.72 (Z=-2.45,P< 0.05).The level of serum phosphorus (PO4),Ca × PO4 products, parathormone (PTH) in the interventional group were (1.76±0.35) mmol/L,(51.02±10.04) mg2/dl2,464.56 ng/L,which were significanlty lower than (2.01±0.54) mmol/L,(57.41±17.38) mg2/dl2,625.78 ng/L in the control group (t=-2.691,-2.167,-2.000 in tum,P < 0.01 or 0.05).Conclusions Transitional care achieves good effectiveness on the quality of life in patients with MHD.

Objective To investigate the effects of antisense p21 oligodeoxynucleotide (p21 ASODN) on the expression of p21 protein and extracellular matrix in cultured human glomerular mesangial cells (HGMC) under high glucose medium. Methods HGMCs were transfected with 50 nmol/L or 100 nmol/L p21 ASODN or scrambled control oligodeoxynucleotide (SCODN) using lipofectamine 2000. After incubation under normal (5.5 mmol/L) or high (30 mmol/L) glucose media and different times, HGMCs lysates were collected and the expression of p21, fibronectin and laminin was examined by Western blot. The secretion of fibronectin and laminin by HGMCs in supernatants of culture media was also detected with EOSA. Results High glucose media significantly stimulated the expression of p21, increased the syntheses and secretion of fibronectin and laminin in cultured HGMCs in a time-dependent manner. Transfection of HGMCs with p21 ASODN not only decreased p21 protein level caused by high glucose media, but also attenuated the expression and secretion of fibronectin and laminin in supematants of HGMCs lysates and culture media. Meanwhile, SCODN had no significant effects on the expression of p21, fibronectin and laminin. Conclusions High glucose promotes the expression of p21 , fibronectin and laminin in cultured HGMCs. Transfection of HGMCs with p21 ASODN can decrease p21 protein level caused by high glucose media, and attenuate the expression of fibronectin and laminin in supematants of HGMCs lysates and culture media. Modulation of p21 expression in HGMCs may work as an effective way to mitigate the progression of diabetic nephropathy.

Objective To study the effects of simvastatin on proliferation and apoptosis in rat mesangial cells in vitro, and to investigate the signal pathways involved in apoptosis induced by simvastatin. Methods Cultured mesangial cells were treated with simvastatin. Proliferation of mesangial cells was examined by MTT assay. Simvastatin-treated apoptotic mesangial cells were observed by electron microscopy. Propidium iodide (PI) staining and flow cytometry were employed for quantitative measurement of apoptosis. Caspase-3 activation was determined by CaspGLOW Green Caspase-3 Staining Kit. Results (1)Simvastatin significantly inhibited proliferation of mesangial cells compared with control (P

AIM:To observe the influence of the different bore diameter ceramic membranes on the processing parameters of Jinlida Granules(Radix et Rhizoma Salviae miltiorrhizae,Radix Sophorae flavescentis,Rhizoma Polygonati, etc.) and optimize the process of membrane filtration. METHODS: Three different membranes were tested to observe the changes in membrane flux and the content of effective components by Jinlida Granules extract. RESULTS: The 50 nm membrane had the great flux, the transfer rates of matrine and salvianolic acid were the highest; The optimum conditions: the operation differential pressure was 0.08~0.12MPa;the operation temperature was 50~60℃;the membrane surface flow rate was 3.0 m/s.When the volume of filtration solution was condensed to one tenth, adding the same volume aeionized water, and the effective components transfer rate reached 80%.Using the strong acid and strong alkali to wash it in turn, the flux can revivify above 90%. CONCLUSION: Applying the ceramic membrane to ultrafiltrate Jinlida Granule is good and useful to herb purification.
