Objective To compare the carriage of dematiaceous fungi in wild toads from both en-demic area and non-epidemic area of chromoblastomycosis.Methods A total of354and45wild toads(Bufo Chinese)were caught from two counties of Shandong Province,Zhangqiu county,an endemic area of chromoblastomycosis,and from Laiyang county,an area where the infection of dematiaceous fungi in humans has not been found,respectively.The toads were killed and the suspension of the internal organs were used to isolate dematiaceous fungi.Results Eleven species including162strains of dematiaceous fungi were iso-lated from151of354(42.7%)toads from the endemic area.The isolated strains consisted of49Phialopho-ra verrucosa,33Exsophiala,19Phialophora(nonpathogenic),19E.jenselmei,12Cladosporium carrionii,3suspected of C.carrionii,7Rhinocladiella sp.,5Fonsecaea pedrosoi,4C.trichoides,3E.spinifera,1C.mi-nourae,4unidentified.Only4nonpathogenic dematiaceous fungi were isolated from4of45(8.9%)toads obtained from non-epidemic area.Conclusions Varieties of pathogenic dematiaceous fungi,especially C.carrionii,are isolated from wild toads in endemic area of chromoblastomycosis,while none from non-endemic area.The results suggest that the colonization of pathogenic dematiaceous fungi in wild toads may be associa-ted with endemicity of chromoblastomycosis.

AIM:To discuss the technique for preparing high-purity solanesol from tobacco. METHODS:The pre-treated tobacco leaves were extracted with mixed solvent of petroleum ether and ethanol,and saponified to obtain solanesol extract; The solanesol extract got through redissolution and dewaxing,then crystallized in the temperatare of -18 ℃; Finally purified by macroporous resin(YPR-Ⅱ,D-1300,HZ-816,HZ-802). RESULTS:The ratio of mixed solvent of petroleum ether and ethanol was 1 ∶ 4,ratio of liquid to materials was 1 ∶ 12,solanesol purity was about 32% after saponification,then crystallization made the purity up to 63. 4% ,finally purified through macroporous resin,the solanesol purity reached above 93. 6%. CONCLUSION:Through the general solvent extraction,saponification,crystallization and macroporous resin adsorption process,the solanesol purity of the product reaches higher than commonly expected values.

Objective: To evaluate the prognostic value of chemotherapy-induced neutropenia (CIN) in metastatic colon cancer undergoing first-line chemotherapy with FOLFOX.Methods: Data were collected from a retrospective survey of 158 consecutive metastatic colon cancer patients who had undergone FOLFOX chemotherapy.The clinicopathological characteristics and chemotherapy features of the patients were analyzed as potential prognostic factors.The patients were stratified by the decreased level of CIN to three groups: large decreased level (the number of neutrophil decreased more than 1.0×109 compared with that before chemotherapy),small decreased level (the number of neutrophil decreased less than 1.0×109 compared with that before chemotherapy) and the absence of neutropenia.Results: According to a multivariate COX model, decreased level of CIN was a independent prognostic factor of colon cancer patients.Hazard ratios of death were 0.687 (95% CI: 0.381-0.812, P=0.016) for patients with large decreased level of CIN and 0.817 (95% CI: 0.527-0.939,P=0.027) for those with small decreased level of CIN compared with those of absent neutropenia patients.Median overall survival was 12.9 months (95% CI: 10.4-15.4) for patients without neutropenia (A) compared with 20.8 months (95% CI: 18.3-23.1) for patients with large-decreased level of CIN (L) and with 17.3 months (95% CI: 16.2-18.8) for those with small-decreased level of CIN (S vs.L, P=0.018;L vs.A, P=0.009;S vs.A, P=0.011).Conclusion: Our results demonstrate that the decreased level of CIN is a predictor of prognosis in patients with metastatic colon cancer undergoing FOLFOX chemotherapy.Patients who have experienced large decreased level of CIN haave longer survival time than small decreased level of CIN or absent patients.To monitor CIN decreased level timely and adjust chemotherapy drug dose may help improve the prognosis.

Objective To discuss the therapeutic effect of ulinastatin combined with dexamethasone on blast-induced acute lung injury (ALI) in rabbits.Methods Thirty-eight New Zealand white rabbits with blast-induced ALI were divided into four groups according to the random number table:injury group (n =8),treatment group Ⅰ (dexamethasone alone,n =10),treatment group Ⅱ (ulinastatin alone,n =10),combination therapy group (dexamethasone + ulinastatin,n =10).In addition,8 uninjured rabbits were randomly selected as control group.Treatment groups were given relevant medication by intramuscular injection at 0.5,12,24,36 and 48 hours after successful modeling.Injury group and control group were given equal volume of isotonic saline at the same time.Parameters of respiratory rate,oxygenation index (PaO2/FiO2),TNF-α,IL-6,IL-8 and neutrophil elastase (NE)were detected before and afterfirst treatment at 6,24,48 and 72 hours.All rabbits were sacrificed after 72 hoars to detect TNF-α,IL-6,IL-8 and NE contents in bronchoalveolar lavage fluid (BALF) and to measure wet to dry lung weight ratio.HE staining was performed to observe pathological changes of lung tissues Results In contrast,respiratory rate was high in injury group at each time point,which increased to the peak (75.0 ± 7.4) times/min at 6 hours far higher than (33.0 ± 4.5) times/min in control group (P ＜0.01).Wet to dry lung weight ratio and TNF-α,IL-6 as well as IL-8 contents of the peripheral blood and BAI.F in injury group were significantly higher than those in control group (P ＜0.01).PaO2/ FiO2 in injury group was decreased to the lowest level of (180.5 ± 12.6)mmHg at 72 hours far lower than (403.7-8.0)mmHg in control group (P ＜0.01).While improvements were observed in treatment groups with respect to these indicators,and significantly much better results were detected in combination therapy group rather than in treatment groups Ⅰ and Ⅱ (P ＜ 0.01).Conclusion Ulinastatin combined with dexamethasone is an effective therapy on blast-induced ALI in rabbits,and the effect is significantly better than that with single use of uliinastatin or dexamethasone.

Background Researches documented that retinal nerve fiber layer thickness (RNFLT) in unaffected carriers of Leber hereditary optic neuropathy (LHON) becomes thickened in different quadrants to different degrees.But the change of their macular thickness is still unclear.Objective This study was to clarify RNFLT and macular thickness by optical coherence tomography (OCT) in unaffected female carriers of LHON families.Methods Five female LHON patients (5 eyes) from 5 LHON families,eighteen unaffected female carriers (18eyes) from 18 LHON families and twenty-five age-matched healthy female controls (25 eyes) were included in this study.The patients and genetic carriers were diagnosed in PLA General Hospital from 2011 September to 2012 October.Regular ocular examination were performed followed by OCT measurement of retinas.The Optic Disc Cube 200×200 and Macular Cube 200×200 protocols were used during the OCT measurement.Average (360°) RNFLT,RNFLT at four quadrantic sections,cube average macular thickness and macular thickness of nine Early Treatment Diabetic Retinopathy Study (ETDRS) sub-areas were compared among the LHON genetic carriers,LHON patients and normal controls.Results Compared to the normal control group,significant reduced values were seen in temporal,superior,nasal and inferior side of sub-area macular thickness in the LHON female carriers (P=0.022,0.046,0.024,0.008).In addition,but no significant differences were found in cube average thickness,central subarea macular thickness,temporal,superior,nasal and inferior side of lateral sub-area macular thickness,average RNFLT,and temporal,superior,nasal and inferior quadrant RNFLT between the LHON female carriers and normal controls (P=0.102,0.051,0.238,0.663,0.1 10,0.104,0.419,0.371,0.158,0.063,0.563).Compared to the unaffected female carrier group,female patients showed significant reductions in cube average macular thickness,temporal,superior,nasal and inferior side of sub-area macular thickness,temporal,superior,nasal and inferior side of lateral sub-area mac ular thickness,average R NFLT and temporal,superior,and inferior quadrant RNFLT (P =0.000,0.000,0.000,0.007,0.002,0.002,0.000,0.000,0.040,0.000,0.016,0.000,0.000) except for the central subarea macular thickness and nasal quadrant RNFLT (P=0.388,0.580).Conclusions Unaffected LHON female carriers show a normal peripapillary RNFLT,but the macular thickness at medial sub-area is thinner.This first report offers an information of macular structure change in unaffected LHON female carriers,which suggest that macular damage appears prior to RNFLT change.

Objective To explore and identify the role of scientific research management departmentin the construction of biobank,discuss problems encountered and possible strategies.Methods Our hospital scientific research management department actively involved,and played an important role in the coordination work for hardware and software construction during the setting up of Infectious disease biorepository.A series of institutional policies and procedures were developed,such as organizational structure of the biobank,sample collection rules at the clinic and research achievementtransformation guidelines.Results A total of 500-case samples were collected,involving different kinds of infectious diseases,like HBV,HCV and HIV.The biobank undertook the Science and Technology Resources Platform Construction Projectof Tianjin health and Family Planning Commission,also established collaboration relationships with domestic and foreign scientific research institutions andhospitals like Memorial University of Newfoundland.Conclusions Scientific research management department should play an important role in the construction of biobank and lay a solid foundation for the development of the biobank.

Objective In order to lay the foundation for establishment of a clean grade Meriones meridianus popu-lation, and provide reference for the formulation of Meriones meridianus parasite detection standard.Methods A survey of parasite infection in Meriones meridianus was carried out, and three kinds of antiparasitic drugs, tetramethrin/per-methrin, praziquantel, and tinidazole were used in high-, medium-and low-doses for eradication of the target parasites. Results Three types of parasites were detected among the animals.The infection rate of surface parasite Myobia musculi was 82.1%(23/28), that of intestinal parasite Hymenolepis nana was 78.1% (25/32) and Tritrichomonas muris was 90.6%(29/32).Significant results of parasite eradication were achieved with the antiparasitic drugs.Medicated bath u-sing 1%tetramethrin/permethrin for 30 s every 4 days for 5 times and 1.5%tetramethrin/permethrin for 30 s every 4 days for 3 times completely eradicated the surface Myobia musculi infection.Praziquantel in a dose of 31 mg/kg by daily gastric gavage for consecutive 7 days was the optimal dosage to completely eradicate the intestinal Hymenolepis nana.Tinidazole in a dose of 42 mg/kg administered by gastric gavage daily for consecutive 7 days was the optimal dosage for eradication of in-testinal Tritrichomonas muris infection.Conclusions Three types of parasites are detected in Meriones meridianus in our laboratory, i.e.surface infection with Myobia musculi, and intestinal infection with Tritrichomonas muris and Hymenolepis nana.Tetramethrin/permethrin, praziquantel and tinidazole can be used to effectively eradicate these three types of para-sites, respectively.

Objective To study the effect of ulinastatin (UTI) on blood gas analysis in rabbits with acute lung injury (ALI) after an explosion.Methods A total of 24 healthy New Zealand white rabbits were made to be ALI models by an explosion at a distance of 11 cm.The rabbits met the criteria of ALI,and were randomly (random number) divided into A and B groups.In group B,rabbits received UTI injected at 0.5 h,6 h,12 h,24 h after modeling.In group A,an equal volume of saline was given to rabbits instead at the same intervals after modeling.Arterial blood pH,carbon dioxide partial pressure (PaCO2) and partial pressure of blood oxygen (PaO2) were determined by a portable blood gas analyzer before modeling and at given intervals after modeling.All rabbits were sacrificed after 48 h later.The pathological changes of lung tissue were observed with naked eye and extent of lung injury with HE staining.Results Compared with group A,pulmonary edema and infiltration of inflammatory cells in the lungs of rabbits in group B were less significant.The blood gas analysis showed there were significant difference in pH at 24 h after injury between group A (7.24 ± 0.07) and group B (7.35 ± 0.06),P ＜ 0.05.There was significant difference in PaO2 at 24 h after injury between group A (50.5 ± 9.9) mmHg and group B (78.1 ± 8.4) mmHg,P ＜ 0.05.And there was significant difference in PaCO2 at 24 h after injury between group A (52.4 ± 5.5) mmHg and group B (42.8 ± 4.5) mmHg,P ＜ 0.05.Conclusion UTI can significantly reduce acidosis,PaCO2 and increase PaO2 in rabbits with ALI following a blast,improving oxygenation and mitigating lung histopathological changes.

Objective To investigate the consistency of omnidirectional M-mode echocardiography and two-dimensional strain in assessment of left ventricular (LV) radial systolic asynchrony. Methods Thirty patients with heart failure (HF) and 28 healthy volunteers underwent omnidirectional M-mode echocardiography and two-dimensional strain at the same time. The time to peak radial systolic movement (T_m) of LV in 12 segments were measured with omnidirectional M-mode echocardiography and its standard deviation (T_m-12-sd), and the maximum difference (T_m-12-dif) were calculated. The time to peak radial systolic strain (T_(SR)) of LV in 12 segments were measured with two-dimensional strain and its standard deviation (T_(SR)-12-sd), and the maximum difference (T_(SR)-12-dif) were calculated. The T_m-12-sd, T_m-12-dif, T_(SR)-12-sd and T_(SR)-12-dif were used as systolic asynchrony indicators. The value (-x)±2s in the control group was defined as the normal upper limit which represents 97.7% of the control group distribution. Any values above this limit in HF patients were classified as LV asynchrony. The results of two methods were analyzed with Kappa test. Results Compared with the controls, T_m-12-sd, T_m-12-dif, T_(SR)-12-sd and T_(SR)-12-dif were significantly higher in the HF group (P<0.001); there was consistency between T_m-12-sd and T_(SR)-12-sd, T_m-12-dif and T_(SR)-12-dif in detecting LV systolic asynchrony in HF group (Kappa=0.661, 0.733). Conclusion T_m-12-sd and T_m-12-dif of omnidirectional M-mode echocardiography have consistency with T_(SR)-12-sd and T_(SR)-12-dif of two-dimensional strain. These two technologies both have ability to evaluate LV radial systolic synchronicity.
