4.Potency Material Bases of Xuebijing Formula and Its Multi-target Effects on Sepsis.

OBJECTIVETo explore potency material bases of Xuebijing (XBJ) formula, and to analyze its effects at the molecular network level.
METHODSTotally 16 sepsis-related targets were selected and classified into three categories such as inflammation, immune, and coagulation referring to biological roles. Then molecular database of chemical compositions in XBJ formula were constructed to explore mutual actions with inflammation, immune, and coagulation targets.
RESULTSDanshen root and safflower, with more effector molecules with immune and coagulation targets, have extensive anticoagulation and anti-inflammation effects. The former 10 molecules with better mutual actions with sepsis targets were sequenced as tryptophane, danshensu, gallic acid, salvianolic acid D, protocatechuic acid, salvianolic acid A, danshensu C, vanillic acid, rosmarinic acid, phenylalanine. There existed two phenomena in XBJ formula as follows. One component had stronger actions with multi-targets, for example, danshensu had actions with 13 targets. Meanwhile, different components acted on the same target protein, for example, 8 molecules acted with MD-2.
CONCLUSIONXBJ formula had certain potential synergistic effects with sepsis targets, which could provide certain referential roles for findina new type anti-septic drugs.

5.Management of moderate to severe pediatric concealed penis in children by Devine's technique via incision between the penis and scrotum.

OBJECTIVETo search for a simple and effective surgical approach to the management of moderate to severe pediatric concealed penis in children.
METHODSWe used Devine's technique via incision between the penis and scrotum in the treatment of 68 cases of moderate to severe pediatric concealed penis. The patients were aged 3 -13 (mean 6.5) years, 30 with moderate and 38 with severe pediatric concealed penis.
RESULTSThis strategy achieved good near- and long-term effects and satisfactory appearance of the penis, which was similar to that of circumcision. At 3 months after surgery, the penile length was 3 - 5.2 cm, averaging (2.35 +/- 0.35) cm.
CONCLUSIONDevine's technique via incision between the penis and scrotum is a simple and effective surgical option for moderate to severe pediatric concealed penis in children.

7.Study on the needling depth of point Yaotu with CT scanning.

OBJECTIVETo probe the safe depth and angle of needling the point Yaotu for treatment of prolapse of intervertebral disc.
METHODSCT technique was used for scanning investigation of the depth and angle of needling the point Yaotu.
RESULTSWhen the acupuncture needle was inserted vertically to the external space of intervertebral foramen of human body with arrival of qi, the tip of needle could reach to 73 mm deep, which is the best needling depth and angle.
CONCLUSIONThe needle vertically inserting to the external space of intervertebral foramen at the point Yaotu is proper in treatment of prolapse of intervertebral disc.

8. Comparative study of clinical outcomes of robot versus laparoscopic radical surgery for rectal cancer based on propensity score matching

Objective:
To compare the short-term and long-term outcomes of robotic rectectomy and laparoscopic rectectomy for rectal cancer based on propensity score matching.
Methods:
The clinical data of 106 patients who underwent robotic or laparoscopic radical resection of rectal cancer at Department of General Surgery, the First Affiliated Hospital of Nanchang University from January 2015 to December 2015 were retrospectively collected. Propensity score matching method was used to perform 1∶1 matching between robot and laparoscopic rectal cancer radical surgery. Thirty-two patients in robot group and 32 patients in laparoscopic group were successfully matched. There were 15 males and 17 females in the robotic group, aging (56.2±7.5) years, 19 males and 13 females in the laparoscopic group, aged (55.5±7.6) years. Objective To determine the clinical value of 24 h double-probe pH-mctry for the diagnosis and treatment of laryngopharyngeal reflux (LPR).Methods According to the pH-metry resuhs (whether the reflux events record in the upper esophagus is more than 6.9), patients of refractory pharyngolaryngitis were divided into LPR and control groups (each of 17 cases).All patients treated with anti-acid therapy and conventional pharyngo-laryngitis therapy.Correlation between pH-metry and the reflux symptom index (RSI),the reflux symptom index (RFI) were analyzed.Changes of the RSI and RFI in different group were calculated in post-treatment.Results In the LPR group,the median reflux events of the upright time were higher than the supine time ( Z =- 3.62,P < 0.01 ),but the difference was not discovered in the control group(Z =-0.60,P > 0.05 ).There were no statistical difference between RSI,RSI and RFS with pH-metry,and with moderate concordance ( κ were 0.47,0.53,P < 0.01,respectively).Compared to pre-treatment, the RSI and RFI were decreased both in LPR group and contorl group,Amplitude decreased in LPR group significantly higher than the contorl groups,with statistical difference( t were 3.74,3.01,P < 0.01,respectively).Conclusion The 24 h double-probe pH-metry is significant for the anti-acid therapy of LPR.
10.Research of the therapeutic effect of Zhuangjin Yangxue decoction on osteochondral lesion of the talus

Objective To investigate the clinical efficacy of Zhuangjin Yangxue decoction on Hepple stage Ⅰ and Ⅱ of osteochondral lesion of the talus(OLT).Methods From February 2021 to December 2022,a total of 62 cases of OLT were seleted from the Hangzhou Xiaoshan Hospital of Traditional Chinese Medicine.According to random number table method,the patients were divided into observation group and control group,with 31 cases in each group.The control group was treated with Western medicine,and the observation group received an additional treatment of Zhuangjin Yangxue decoction alongside with Western medicine.Serum matrix metalloproteinase 13(MMP-13),tumor necrosis factor-α(TNF-α),ankle visual analogue scale(VAS)score,American Orthopedic Foot and Ankle Society(AOFAS)ankle-hind foot function score,TCM syndrome score and magnetic resonance observation of cartilage repair tissue(MOCART)score before and after treatment were compared between two groups.Results After 5 weeks and 3 months of treatment,MMP-13,TNF-α,VAS score and TCM syndrome score in two groups were significantly lower than before treatment,and AOFAS ankle-hind foot function score and MOCART score were significantly higher than before treatment(P<0.05).MMP-13,TNF-α and TCM syndrome scores in observation group were significantly lower than those in control group,and MOCART score was significantly higher than that in control group(P<0.05).After 5 weeks of treatment,VAS score of observation group was significantly lower than that of control group,and AOFAS ankle-hind foot function score was significantly higher than that of control group(P<0.05).Conclusion Zhuangjin Yangxue decoction shows a good short-term efficacy in treating Hepple stage Ⅰ and Ⅱ of OLT,with anti-inflammatory and analgesic effects,improved joint function,and promotion of cartilage repair.
