Chinese Journal of Gastrointestinal Surgery 2011;14(11):830-833
In recent years, preoperative therapy has become the standard procedure to improve radical resection rate and local control for advanced rectal cancer. Tumor responses to chemoradiotherapy, however, vary considerably, thus increasing the demand for both functional and morphologic radiologic evaluation of response to chemoradiotherapy to distinguish responders from nonresponders. MR imaging is considered the most accurate tool for the primary staging of tumor extent, and can be used to evaluate the efficacy of chemoradiotherapy. Functional imaging modalities including DW-MRI and PET-CT have shown promising prospect in the early evaluation of the response of rectal cancer to preoperative chemoradiotherapy. However, wide clinical application will take some time.

ObjectiveTo compare the clinical effects of laparoscopically assisted vaginal hysterectomy (LAVH) and transabdominal hysterectomy (TAH).MethodsOne hundred and twenty-six cases operated with hysterectomy were classified into LAVH group and TAH group by random digits table with 63 cases each. The operation time, intraoperative bleeding amount, exhaust time, hospitalization time and postoperative complications were observed and compared.ResultsThe operation time, intraoperative bleeding amount, exhaust time and hospitalization time was ( 115.6 ± 13.8 ) min,(92.5 ± 11.3 ) ml,(21.8 ±4.8) d and (6.1 ± 1.7) d in LAVH group,( 82.5 ± 8.7) min,( 112.3 ± 17.8) ml,(28.9 ± 6.2) d and (9.6 ±2.2) d in TAH group, there were significant differences between two groups (P<0.05).The incidence of incision pain, delayed wound healing, vaginal stump infection, thrombophlebitis, and gastrointestinal disorders was 9.5%(6/63),0,1.6%(1/63),0 and 7.9%(5/63) in LAVH group, which was significantly lower than that in TAH group [52.4%(33/63),7.9%(5/63),11.1%(7/63),7.9%(5/63) and 22.2% ( 14/63 ) ] (P < 0.05).ConclusionCompared with TAH,LAVH can shorten exhaust time, hospitalization time, reduce intraoperative bleeding amount, decrease complication rate, which can be further applied in clinic.

Objective To summarize the operative experience of transumbilical single-port laparoscopic cholecystectomy. Method The clinical data of 15 patients underwent transumbilical singleport laparoscopic cholecystectomy between May to October in 2010 was reviewed. Results The operative time was 30-85(45±16) min, hospitalization time was (2.5 ± 1.1 ) d. No surgical complications such as bile leak or uncontrolled hemorrhage occurred postoperatively. All the incision were healed well and the surgical scar was virtually concealed whithin the umbilicus. Conclusions Transumbilical single-port laparoscopic cholecystectomy is feasible and safe when patients are well selected. Comparing with conventional laparoscopic cholecystectomy, it's more manipulatively difficult.

Objective: To study the roles of N methyl D aspartate acid (NMDA) receptors of glutamate in mediating the arterial baroreceptor reflex (ABR)in rostral ventrolateral medulla (RVLM) in rats. Methods: The blood pressure, heart rates and ABR were observed after the bilateral microinjection of 0.1 ?l 50 mmol/L ketamine into RVLM of ureth anesthetized rats. Results: Bilateral microinjection of ketamine into RVLM induced decrease of the blood pressure and heart rate ( P

Objective To observe the effect of connect meridians inducing Yin from Yang on muscle-tension after cerebral infarction. Methods 82 patients with cerebral infarction in Second Hospital of Harbin from 2010 to 2014 were divided into observation group (n=41) and control group (n=41) according to hospitalization order. The observation group accepted acupuncture of connecting meridians inducing Yin from Yang, and the control group accepted the traditional acupuncture method for 2 weeks. The situation of spasticity, the motor limb function of limbs, and activities of daily living were evaluated with modified Ashworth Scale (MAS), Clinic Spasticity Index (CSI), Fugl-Meyer Assessment (FMA) and Barthel index (BI), respectively. Results Compared with the control group, the score of MAS decreased (t=2.212, P=0.030), the score of CSI decreased (t=2.514, P=0.014), the score of FMA increased (t=4.288, P<0.001), and the score of BI in-creased (t=2.970, P=0.004) in the observation group after treatment. Conclusion The acupuncture of connecting meridians inducing Yin from Yang can reduce the muscle-tension of limbs, improve the motor function and increase the activities of daily living.

Objective To explore the effect of high-fat diet(HFD) on redox states of duodenum and calcium absorption in mice, and to analyze the relation between them.Method Fifty C57BL/6 male mice were randomly assigned to five groups. The control group consumed an ordinary diet(0.6% Ca,w/w),and other four groups were fed with HFD(19.64%lard,0.6% Ca),HFD plus 0.1% lipoid acid(LA),HFD with calcium supplement(1.6%Ca) and HFD with 1.6% Ca and 0.1% LA supplement.Calcium apparent absorption was measured by mineral balance study after feeding for 8 w.Plasma and duodenum levels of ROS,SOD,CAT,MDA,GSH/GSSG,and T-AOC were measured to evaluate the antioxidant status. Results HFD induced oxidative stress of duodenum and decrease of intestinal calcium absorption in mice. There were positive correlations between calcium apparent absorption with GSH/GSSG(r=0.801,P

Acta Nutrimenta Sinica 1956;0(03):-
Objective To investigate the changes in femoral gene expression profiles in C57BL/6 mice fed high-fat diet (HFD) via clustering analysis of DNA microarray.Method Sixteen male C57BL/6 mice (4w old) were randomly assigned to two groups,8 in each, after 4-d ordinary diet for adaptation. The control group was fed with an ordinary diet, and the HFD group with HFD(19.5% lard). All mice were sacrificed at the end of 12w and the femoral gene expression was detected by oligonucleotide microarray analysis with Affymetrix Gene Chip Mouse U430A.DAVID,an online tool, was used for clustering analysis on femoral gene expression. Results Longtime administration of HFD caused femoral gene expressed differences related to cation ion channel, transcription regulation and signal transduction, bone mineralization, phosphate metabolic process regulation, and collagen synthesis. Conclusion Longtime intake of HFD will change the expression of numerous bone metabolism-related genes in bone of mice, and then inhibit bone formation.
