3.Analysis of viral pathogen in children with acute lower respiratory tract infections

Objective To elucidate the etiology feature of viral infection in hospitalized children with acute lower respiratory infection. Methods A total of 5 480 children with acute lower respiratory tract infection, hospitalized from September 2007 to September 2009, were studied. Nasopharyngeal aspirates were screened for 8 types of viruses by direct immunofluorescence (DIF) assay. Results At least one type of viral pathogen was detected in 2 710 out of 5 480 patients and the overall positive rate was 49.5%. The most common virus was RSV (51.1%), followed by hMPV (18.9%), PIVⅢ (12.5%), ADV (7.1%), IFA (4.7%), IFB (2.9%), PIV Ⅰ (1.5%) and PIV Ⅱ (1.2%). The positive rate was highest in children under 6 months (43.5%). The seasonal change of RSV, hMPV was more obvious. The peak of RSV, hMPV appeared in the winter and the spring. The prevalence of viral infection in children with pneumonia, bronchitis, asthmatic bronchitis, non asthmatic bronchitis and asthma were 47.4%、63.6%、 50.5%、 30.1% and 43.5% respectively. Conclusions Viruses are the main cause of lower respiratory tract infections in children, especially in infants and young children. RSV and hMPV were the most common viruses in these years.
4.Neuroprotective effects of Erigeron Breviscapus (vant) Hand-Mazz on NMDA-induced retinal neuron injury in the rats

·AIM: To investigate whether Erigeron Breviscapus (vant) Hand-Mazz (EBHM) EBHM has neuroprotective effect against N-methyl-D-aspartate (NMDA)-induced neuron death in retinal ganglion cell layer (RGCL).· METHODS: 60 healthy SD rats were randomly divided into four groups. 6 animals were normal control group (group A). The others were divided as group B (EBHM group), group C (normal saline+NMDA group) and group D (EBHM + NMDA group). Each group had 18 rats.10nmol NMDA was intravitreally injected to induce partial damage of the neurons in RGCL in the right eyes of Groups C and D. Same volume PBS was intravitreally injected into the left eyes as self-control. Groups B and D were pre-treated intraperitoneally with 6g/L EBHM solution at a dose of 150mg/kg body weight/day seven days before and after NMDA treatment. Group C were administrated intraperitoneally with 9g/L normal saline at the same time of EBHM injection. Rats were sacrificed at 4,7,14d after NMDA treatment. Flat whole retinas were stained with 5g/L cresyl violet and neuron counting in RGCL from both eyes were observed. Each subgroup had 6 rats.· RESULTS: There was no significant difference of neuron counting in RGCL between the right eye and the left eye in group A (P=0.200). There was no significant difference between normal control group and EBHM group either in the right eyes or in the left eyes at 4, 7 and 14 d respectively after intravitreal injection of 10nmol NMDA in group C and group D. (P=0.636, P=0.193). Neuron counting of RGCL in group C and D was significantly decreased in the NMDA-treated eyes at 4, 7 and 14d after intravitreal injection (P<0.001). There was no significant difference between self-control eyes group and normai control group(P>0.05). However, neuron counting was significantly higher in the EBHM+NMDA group than normal saline +NMDA group at 14days after intravitreal injection (P=0.044), but was lowered than normal control group (P<0.05).· CONCLUSION: EBHM has no effect on neuron counting of RGCL when administered alone in normal rats.The results indicates that EBHM plays a partial protective role in NMDA-induced neuron loss in RGCL in the rats.
5.Sensitivity in typeⅠ hypersensitivity compared between BN rats and Wistar rats

OBJECTIVE To compare sensitivity in typeⅠ hypersensitivity between BN and Wistar rats, and to establish a sensitive and reliable determination system for typeⅠ hypersensitivity. METHODS BN and Wistar rats were sc given ovalbumin (OVA) 10, 20 and 40 μg·kg~(-1) every other day for 5 times and normal control group with sc normal saline. The total immunoglobulin E (IgE) levels in serum were determined with ELISA and the specific IgE levels in serum were determined by passive cutaneous anaphylaxis on the 21st day from the 1st injection. The blood pressure, serum histamine and tryptase levels were determined after challenge on the 22nd day. RESULTSTotal IgE, specific IgE, histamine and tryptase levels in serum significantly increased and blood pressure decreased in OVA 10, 20 and 40 μg·kg~(-1) BN rat groups compared with normal control group, while in Wistar rats these symptoms only appeared in OVA 40 μg·kg~(-1) group. CONCLUSION BN rats are more sensitive than Wistar rats in typeⅠ hypersensitivity. The blood pressure, serum total IgE, specific IgE, histamine and tryptase levels can be used as the important indicators in typeⅠ hypersensitivity.
6.POIYMORPHONUCIEAR LEUKOCYTE OXYGEN RADICAI PRODUCTION INCREASED IN PATIENTS WITH ACUTE MYOCARDIAL INFARCTION AND THE SUPPRESSIVE EFFECTS OF VITAMIN C

Objective To evaluate the role of polymorphonuclear (PMN) in patients with acute myocardial infarction (AMI).The preventive effects of Vitamin C-a potent free radical scavenger,on parameters of PMN oxygen free radicals (OFR) production in patients with AMI were also studied.Methods 60 patients with AMI were randomized to receive either conventional treatment only (group Ⅰ,n=30) or conventional treatment supplemented with vitamin C infusion,3.0 g/d,for 7 days (group Ⅱ,n=30).Parameters of PMN OFR production were assayed by a method named as polymorphonuclear chemiluminescence (PMN-CL) on the 1st,3rd,7th and 10th day after been hospitalized.62 healthy controls of similar age,sex were also studied.Results Parameters of PMN-CL increased significantly in patients with AMI compared with that in the healthy controls.Parameters of PMN-CL decreased significantly in group Ⅱ on the 3rd,7th and 10th day compared with that in group Ⅰ(P<0.05,0.01,0.001,respectively).ST segment score was significantly higher in group Ⅱ than that in group Ⅰ on the 3rd,7th and 10th day.Conclusion These results indicate that the PMN act as a potential contributor to extension of tissue injury induced by OFR.Supplementation with vitamin C may suppress PMN OFR production and could be beneficial in preventing myocardial necrosis.
7.The application of a high-performance bloodstain testing reagent in forensic scene

Objective To develop a highly sensitive luminescent reagent for bloodstain testing at forensic crime scenes.Methods Based ontheprincipleof ECL luminescence and the ping-pong conjugate activation principle of chemical electronic chain,this project developed a new type of highly sensitive luminescent reagent for bloodstain testing by usingthe uniform design of experimental methods to optimize the conditions andsynthesize several new compounds.Results The bloodstain testing luminescent reagentdeveloped in this project has high sensitivity andlongluminescence time.In the case of blood samples diluted by 1,000 times,reading the fluorescence withChemiScope 3300 chemiluminescence imaging system,the maximumvalue of gray scale reached 56,and the luminescence time lasted for 10 minutes.Conclusion The project has successfully developed a highly semitivebloodstain testing reagentthat could be applied to crime scene investigation.
8.Transvaginal color Doppler ultrasonography in investigating the influence of different treatments for tubal pregnancy on ovarian reserve function

Objective To investigate the influence of different treatments for tubal pregnancy on ovarian reserve function by transvaginal color Doppler ultrasonography.Methods One hundred and twentyfour cases of tubal pregnancy were divided into three groups according to the treatment,group A with 45 cases under the salpingectomy,group B with 36 cases under the laparoscopic conservative surgery for retaining oviduct,group C with 43 cases taken by Methotrexate.On the day of a spontaneous cycle,all the cases' antral follicles,the ovarian volume,peak systolic velocity (PSV),resistance index (RI),pulsatility index(PI) were measured by transvaginal color Doppler ultrasound.Results The antral follicles,varian volume and PSV of diseased side in group A were lower than those of the other side(P <0.05).The antral follicles,varian volume and PSV of diseased side in group A were lower than those of other groups (P <0.001).The PI and RI of diseased side in group A were higher than those of the unaffected side.The PI and RI of two sides in group B and C were not significantly different(P >0.05).Conclusions Salpingectomy affects the ovarian reserve function.Methotrexate and conservative surgery could not reduce the ovarion reserve.
9.Pegylated liposomal doxorubicin in CHOP regimen for untreated elderly patients with advanced diffuse large B-cell lymphoma:re-sults from a prospective phase II study

Objective:To evaluate the efficacy and safety of pegylated liposomal doxorubicin (PLD) in CHOP regimen for un-treated elderly patients with advanced diffuse large B-cell lymphoma (DLBCL). Methods:In a prospective phase II study, we analyzed the feasibility of PLD-modified CHOP regimen in elderly patients with advanced stages of DLBCL. PLD was administered at 30 mg/m2 in combination with cyclophosphamide, vincristine, and prednisone at standard doses every 21 d for six cycles. CD20 positive patients were given option for rituximab treatment. Results:From November 2011 to March 2014, 30 patients with a median age of 70 years (range:63 to 80) were enrolled in this study. Up to 24 cases (80.0%) obtained an International Prognostic Index of≥3. The overall re-sponse rate was 86.7%, and the complete remission rate was 66.7%. With a median follow-up of 20.1 months, the 18-month overall and progression-free survival rates were 82.4%and 70.1%, respectively. The main toxicity was neutropenia, reaching grades 3 to 4 in the 24 cases (80.0%). No significant changes existed in patients' left ventricular ejection fraction and serum troponin-T during the study. Four patients (13.3%) showed asymptomatic abnormal changes in electrocardiogram after PLD infusion. Conclusion:CHOP regimen with PLD is an effective alternative for the treatment of DLBCL in elderly patients, exhibiting an acceptable toxicity.
10.Animal experimental study of safety for a self-made vena cava stent-filter

Objective To evaluate the safety of a self-made vena cava stent-filter(VCSF)for prevention of pulmonary embolism.Methods Fusiform unmbrella-like vena cava filter was made of Nitinol wires and stainless steel metal pole,and then ten mongrel dogs were implanted with these self-made filters and divided into 5 groups according to the different periods(2,3,4,5 and 6 w) of filter placement,with 2 dogs in each group.After the VCSFs were placed in inferior vena cava via the right femoral vein approach,the dogs in each group were bred for 2-6 weeks,respectively.The blood flow of inferior vena cava and the position of the filters were inspected by inferior vena-cavography according to the indwelling periods.Finally the metal pole was retrieved via the femoral vein,leaving the VCSF as permanent venous stent.The feasibility of retrieval and the free state of filter net with the adhering vascular wall were evaluated.Laparotomies were performed to remove the inferior vena cava from the animals for gross and electron microscopic examinations of the inferior vena cava intimal changes of the involved segment.Results All 10 VCSFs were placed at the right positions of the dogs successfully.Angiography showed patent inferior vena cava without filter thrombosis at 2-6 weeks.There were no tilting and migration of the filter and all the metal poles were successfully retrieved.The superior and inferior extremities of filter nets could be set free with all the filters turning into venous stents.Postmortem displayed retroperitoneal hemorrhage and caval thrombosis.The barbs of the filters penetrated over the caval adventitial coat.Under electron microscope,a thin layer of neointima already covered the braiding net of VCSFs at 2 weeks after the deployment.The tunica intima became slightly thick at 3-4 weeks and with moderate proliferation at 5-6 weeks.Conclusions The self-made vena cava stent-filter possesses rather long indwelling period according to the necessity of treatment with simultaneously keeping patent caval flow,stability of the position and easy for displacement and retrieval.
