2.Progress of clinical research of neoadjuvant chemotherapy for breast cancer

Breast cancer is a systemic disease, only through multidisciplinary treatment including surgery, radiotherapy, chemotherapy, endocrine, can we enhance its effectiveness. In recent years, neoadjuvant chemotherapy for breast cancer has aroused great interest in tumor academia. We hope that this therapy can not only improve breast conservation rate, but also extend the overall survival through a series of clinical studies. This review presents the research progress of neoadjuvant chemotherapy for breast cancer.
3.Application of intravenous anesthesia in combination with oropharyngeal airway ventilation in hysteroscopy

Objective To observe the safety and efficiency of two different ways of intravenous anesthesia combined with oropharyngeal airway ventilation in hysteroscope operation.Methods 40 ASA Ⅰ ~ Ⅱ patients undergoing hysteroscopy were randomly assigned into two groups,20 cases in each group.Patients in the group A,B,were administered respectively with sufentanil 0.1 μg/kg or dexmedetomidine 0.5μg/kg by micro-pump in at least 10 minutes before operation.The perioperative hemodynamic index was recorded.The different outcomes including total dose of propofol,hemodynamic changes,recovery time,postoperative uterine contraction pain and episodes of adverse actions were observed and compared.Results Compared with group A,heart rate (HR) and systolic blood pressure (SBP) were higher in group B (P < 0.05).The recovery time in group A was longer than that in group B (P <0.05).But there were no statistical differences of the total dosage of propofol,the incidence of respiratory depression,and the incidence rate of drowsiness after operation betwween group A and group B (P > 0.05).Conclusion There was safe and effective of two different ways of intravenous anesthesia combined with oropharyngeal airway ventilation in hysteroscope operation,but the dynamic index was more stable,and the recovery time was shorter in patients with lowdose dexmedetomidine.
5.Build of focal cerebral ischemia model in different varieties of mice with modification monofilament.

OBJECTIVETo establish a general method of focal cerebral ischemia model in different varieties of mice.
METHODEach group of healthy adult KM and C57BL/6 mice were randomly divided into control group (n = 10) and MCAO group (n = 10). The mice in MCAO group were applied in the preparation of the MCAO model by intraluminal occlusion using monofilament. Twenty-four hours after operation,the neurologic function was evaluated,middle cerebral artery blood flow was monitored and the infarction volume was calculated by TTC staining, to evaluate the reliability of the model.
RESULTIn the MCAO group, the base value of the cerebral blood flow down of KM and C57BL/6 mice respectively was (81.65 ± 4.59)%, (83.68 ± 6.25)%. The neurological deficit score respectively was (2.30 ± 0.82), (2.50 ± 0.80). TTC staining can clearly show the infarction area, and relatively stable, 24 hours of the survival rate of KM and C57BL/6 mice were 100% and 80% respectively.
CONCLUSIONThe key link is the optimization and improvement of monofilament, temperature, anesthesia and so on. The modified intraluminal occlusion of MCAO using monofilament is a kind of reliable and simple method to establish experimental cerebral ischemia model in mice.

7.Influence of 256-slice Spiral CT Perfusion Imaging on the Cerebral Blood Flow before and after Cranioplasty

Objective To evaluate changes in cerebral blood flow before and after cranioplasty by 256-slice Spiral CT perfusion imaging,and evaluate the effect of cranioplasty on the cerebral blood flow in patients with skull defect.Methods 256-slice spiral CT scan was performed in 20 cases with early cranioplasty surgery,CTP check time points were 1 to 2 days before and 10 to 14 days after cranioplasty surgery.We recorded the the CBF and CBV of the cortex,basal ganglia,and thalamus and other parts,MTT on rCBV,parameter values rCBF,MTT and 1TrP etc.and analyzed and compared.(RCBF,rCBV,MTT and TTP) Results The CBF of cortex after cranioplasty at injured side had statistically significant increase (P<0.05).The CBF of cortex,basic nuclei,thalamus on contrateral had no statistically significant difference.The cerebral blood flow on both sides of the basal ganglia and the thalamus was increased after surgery,but there was no significant difference between before and after surgery (P>0.05) Conclusion Cranioplasty can significantly improve the ipsilateral cortex cerebral blood flow,and CT brain perfusion can accurately assess changes in brain tissue blood flow before and after cranioplasty.
8.Study on analgesia of oxymatrine and its relation to calcium channels.

OBJECTIVETo study whether the analgesis of oxymatrine (OMT) affects N-type voltage-gated calcium channels (VGCCs).
METHODSTotally 45 mice were randomly divided into the sham-operation group, the model group [established by partial sciatic nerve ligation (PSNL)] , and the OMT treatment group according to random digit table, 15 in each group. The dorsal root ganglions (DRG) were separated in PSNL pain model mice. Intracellular calcium concentration ([Ca2+]i) was determined with Fluo-3 AM immunofluorescent probe in cultured DRG neurons. Different protein expression levels of N-type (Cav2. 2) and L-type ( Cav1. 3) among VGCCs from brain and DRG tissues were detected with Western blot.
RESULTSCompared with the sham-operation group, [Ca2+]i, increased in cultured DRG neurons (P <0. 05) , protein expression levels of Cav2. 2 in the brain tissue increased (P <0. 05), protein expression levels of Cav2. 2 in DRG tissues decreased in the model group (P <0. 01). Compared with the model group, [Ca2+]i, decreased in cultured DRG neurons (P < 0. 05), protein expression levels of Cav2. 2 in the brain tissue decreased (P <0. 01), protein expression levels of Cav2. 2 in DRG tissues increased in the OMT treatment group (P <0. 01). There was no statistical difference in Cav1. 3 expressions in cultured DRG neurons and the brain (P >0. 05).
CONCLUSIONAnalgesic effect of OMT might be related to Cav2. 2 channel mediated calcium ion flux.

9.Inhibitory effect of scopolamine on withdrawal syndromes in morphine dependent rats

Objective: To observe the inhibitory effect of scopolamine(Spm) and chlopromazine (Clo) on withdrawal syndromes in morphine dependent rats. Methods: The intensity of withdrawal syndromes on the model of morphine dependent rats was recorded after single or muiltiple subcutaneous administration(sc) of Spm and Clo at different doses. Results: Withdrawal syndromes were markedly decreased when single Spm 1 mg/kg and Clo 0.5 mg/kg combined with morphine were injected (P<0.05). Spm+Clo(sc) had much stronger effects on inhibiting withdrawal syndromes after intraperitoneal (ip) naloxone in morphine dependent rats (P<0.01). Conclusion: Spm can act on Ach-receptor and relieve morphine withdrawal syndromes. Clo may have a synergistic action with Spm via α2-receptor in the locus coeruleus of the rat brain stem.
10.Clinical analysis in diagnosis and treatment of serious pyogenic infection in deep neck

Objective To explore the clinical features,diagnosis and treatment of serious pyogenic infection in deep neck. MethodsFrom January 2002 to April 2005,the clinical data of 22 patients hospitalized with serious pyogenic infection in deep neck were collected and retrospectively analysed. Results All the patients suffered unilaterally.Besides the symptoms of infection,dyspnea,pain in swallowing,limitation of mouth opening and progressive limitation of cervical activity were presented.Bacterial cultivation were performed in 14 patients and positive results were detected in six.Fourteen of all the 22 patients were cured with antibiotics,and abscess incision drainage was carried out in the other eight.Preoperative cervical PA-LAT X-ray photography and contrast-enhanced CT scanning were performed on one and seven patients,respectively,all of which indicated the formation of abscess.All the patients were cured except one who died of cerebral infarction and multiorgan dysfunction. Conclusion Imaging evaluation plays a significant role in the preoperative diagnosis and systemic treatment of serious pyogenic infection in deep neck.Bacterial cultivation provides reliable evidence for the etiopathogenisis and helps to make the effective treatment.
