1.Insertion of gluteus maximus tendo-chilles lengthening with Z-shaped for the treatment of severe gluteal muscle contracture.

OBJECTIVETo investigate clinical curative effects of gluteal muscle contracture release combined with insertion of gluteus maximus tendo-chilles lengthening with Z-shaped in treating severe gluteal muscles contracture.
METHODSFrom 2006 May to 2011 May, 20 patients (35 sides) with severe gluteal muscle contracture were collected, including 12 males and 8 females, aged from 8 to 34 years old with an average of 13 years old; the courses of disease ranged from 3 to 21 years. All patients manifested abnormal gait at different degree, knees close together cannot squat,positive syndrome of Ober, positive test of alice leg. Gluteus contracture fascia release were performed firstly in operation, then insertion of tendo-chilles lengthening with Z-shaped were carried out. Preoperative and postoperative gait, and knee flexion hip extensor squat test, cross leg test, adduction and internal rotary activity of hip joint, stretch strength and motor ability after hip abduction were observed and compared.
RESULTSTwenty patients were followed up for 1 to 5 years. Gluteus maximus were released thoroughly, and snapping hip was disappeared, Ober syndrome were negative. There was significant differences in knee flexion hip extensor squat test, adduction and internal rotary activity of hip joint,stretch before and after operation (P<0.01). Gluteus muscle strength was protected,stretch strength and motor ability of hip joint were recovered well. Among them,31 cases got excellent results and 4 good.
CONCLUSIONFor severe gluteal muscles contracture,insertion of gluteus maximus tendo-chilles lengthening with Z-shaped performed after gluteus contracture fascia release could release gluteal muscle contracture to the greatest extent and obtain postoperative curative effect without resection of normal hip muscle fibers and destroy joint capsule.

2.Effects of thymoquinone on oxidative stress and cytokine expression in brain of type 2 diabetic rats

Objective To investigate the effects of thymoquinone (TQ) on the levels of proteins involved in oxidative stress and cytokine in the brain of rats with type 2 diabetes mellitus.Methods Wistar rats (n=48) were randomly divided into 3 groups:normal control group,model group and TQ treatment group (n =16).The normal control group was fed with clean grade ordinary feed.The model group and TQ treatment group rats were fed with high-glucose and high-fat diet.After 6 weeks' feeding,model group and TQ treatment group rats were administered streptozocin (STZ) (35 mg/kg) to establish type 2 diabetic model by intraperitoneal injection.Then,the model group and TQ treatment group rats were fed with high-glucose and high-fat diet for another 6 weeks.After that,TQ treatment group rats were administered TQ (5 mg/kg) by intraperitoneal injection every other day.The model group rats were injected with an equal dose of ethanol.Six weeks later,all the rats were sacrificed to obtain the hippocampal tissue for the protein extract.The protein levels of nuclear factor erythroid 2-related factor 2 (Nrf2),heme oxygenase-1 (HO-1) and cyclo-oxygenase 2 (COX-2) in the hippocampal tissue were measured by Western blot.Superoxide dismutase (SOD) activity was determined by SOD kit.Blood-glucose meter was used to assess the blood glucose before the rats were sacrificed and after the model was successfully established.All the measurement data was described by the x ± s,measurement data were compared among groups using One Way ANOVA.Results The results by Western blots showed that the levels of the Nrf2 and HO-1 protein were significantly decreased in the model group compared with the normal control group,the levels of the Nrf2 and HO-1 proteins were increased in the TQ treatment group in comparison with the model group (P< 0.05).Meanwhile,compared with the normal control group,the SOD activity of the hippocampus in model group was significantly lower (P< 0.05);by contrast with the model group,the SOD activity in the TQ treatment group was considerably increased (P<0.05).By contrast,the COX-2 level in the model group was substantially higher than that in the normal control (P<0.05),and the COX-2 level in the TQ treatment group was lower than that in the model group (P<0.05).Conclusions TQ might inhibit inflammation and improve oxidative stress of the brain tissue in the rats with type 2 diabetes mellitus.
3.Identifying the Epitope of Monoclonal Antibody with Phage-displayed Random Peptide Library

Objective To obtain the short peptides from phage-displayed random peptide library through screening the epitope of monoclonal antibody against tumor necrosis factor(TNF-?). Methods Anti-TNF-? was used to immunoscreen a phage random peptide library of 12 amino-acidresidues displayed as a fusion to protein Ⅲ of filamentous phage M13. The positive clones were obtained by three rounds of biopanning, and the reactivity of each clone binding to anti-TNF-? was examined by double-antibody sandwich ELISA and Dot-ELISA. Mixed positive phage clones were used to detect the serum from SLE patients and healthy persons by Dot-ELISA. Results The eluted phages were enriched nearly 100 fold through three rounds of biopanning, 7 phage clones from the third round biopanning were randomly selected and 5 clones of them could bind to the anti-TNF-?. The binding rate of mixed clones with SLE patients was significantly higher than that of healthy persons. Conclusion The phage display technique can be applied to study the anti-TNF-? antigenic peptides, and these epitopes provide the potential for developing immunodiagnostic reagents of vaccines.
4.Effect of Bailing Capsules on the Expressing of TGF-β1 in the Peritoneal Dialysis Solution for Peritoneal Di-alysis Patients

Objective:To investigate the effects of Bailing capsules on the expression of TGF-β1 in the peritoneal dialysis solution for peritoneal dialysis patients. Methods: Totally 40 patients treated with peritoneal dialysis ( PD) were randomly divided into two groups (20 cases in the control group and 20 cases in the experimental group). All the patients were treated with PD by 1. 5% perito-neal dialysis effluent (6 000 ml everyday), and the patients in the experimental group were additionally treated with Bailing capsules (5 capsules each time, three times a day after meals) for 6 months. The adequacy of PD (including Kt/V and Ccr) of the two groups was examined after the one-month treatment. The renal function and the level of TGF-β1 in the effluent in 1, 3 and 6 month were com-pared between the two groups. Results:The adequacy of PD ( Kt/V and Ccr ) had no significant difference between the two groups (P<0. 05). The level of serum creatine was decreased significantly in both groups after the one-month treatment (P<0. 05), and no significant change was shown in 3 and 6 month (P>0. 05). In the control group, the expression of TGF-β1 in the effluent in 1, 3 and 6 month was increased gradually with significant difference (P<0. 05). In the experimental group, the expression of TGF-β1 in the effluent had no significant difference in 1, 3 and 6 month (P>0. 05), which was lower than that in the control group (P<0. 05). Conclusion:Bailing capsules can decrease the expression of TGF -β1 in the effluent for the patients treated with PD and inhibit the peritoneal fibrosis.
5.Clinicopathologic features of 9 cases of adult rhabdomyosarcoma

Objective·To investigate the clinicopathologic characteristics and prognosis of adult rhabdomyosarcoma (RMS).Methods·The clinical and pathological data,immunohistochemical test results of 9 RMS were analyzed retrospectively.Results·The information of 6 males and 3 females were collected in this research,whose average age was (46.89+20.09) years old.The tumors had a wide range distribution.The subtypes of histology included 6 cases of embryonal RMS,1 case of alveolar RMS,1 case of spindle cell RMS and 1 case of pleomorphic RMS.Immunohistochemically study showed that 6 cases were myogenin positive,4 cases were MyoDl positive and 8 cases were desmin positive.During the follow-up period of the 7 cases,only 2 patients survived free of tumors,2 patients presented with tumor recurrence and metastasis after the operations,and 3 patients died of tumor.Conclusion·In this study,embryonaI RMS is the most common type,and immunohistochemistry test is helpful to diagnosis of RMS.
6.Modified Reconstruction of Old Rupture of Finger Flexor Tendon

Objective To explore the effect of reconstruction on old rupture of finger flexor tendon. Methods There were 30 patients (54 fingers) with old rupture of finger flexor tendon. The silica gel stick was implanted at the first stage. The palmaris longus tendon wrapped in the bioabsorbable membrane replaced the silica gel stick and was implanted at the second stage. The effectiveness was assessed by total active movement (TAM). Results There were 30 fingers as excellent, 20 fingers as good among 54 fingers of 30 patients. The percent of excellent and good was 92.6%. Conclusion Modified reconstruction is effective to prevent from tendon adhesion.
7.Researches on the Processing Methods and Raman Spectroscopy of Matrii Sultas Exsiccatus

Objective:To find out the transformation reasons of the processing methods for Matrii sultas Exsiccatus through the bencaological study and the modern experimental researches.Methods:The different processing methods for Matrii sultas Exsiccatus in different periods were collected through the researches on the ancient agrostology and the modern processing standards.Furthermore,different processed products were prepared and analyzed by Raman spectroscopy.Results:There were significant differences in the processing methods for Matrii sultas Exsiccatus in the ancient herbal documents while tending to be the same in the modern processing standards.No difference was shown in the Raman spectrogram between Matrii sultas Exsiccatus and Weathered mirabilite produced by the processing methods in the ancient agrostology.The Raman characteristic peaks of the two processed products were as follows:1 153,1 132,1 102,992,648,632,621,466,450 cm-1,which were in line with the Raman spectrogram of the products produced by the modern processing methods.Conclusion:The change of processing methods for Matrii sultas Exsiccatus has certain reasons,which is worthy of further studies.
8.Nursing intervention and evaluation of postoperative pain in preschool children with cleft lip and palate.

OBJECTIVEThis study aimed to observe the postoperative pain rate and degree of pain in preschool children with cleft lip and palate, and investigate the effect of nursing intervention on pain relief.
METHODSA total of 120 hospitalized cases of three- to seven-year-old preschool children with cleft lip and palate were selected from May to October 2011. The subjects were randomly divided into the control group and experimental groups 1, 2, and 3. The control group used conventional nursing methods, experimental group 1 used analgesic drug treatment, experimental group 2 used psychological nursing interventions, and experimental group 3 used both psychological nursing intervention and analgesic drug treatment. After 6, 12, 24, and 48 h, pain self-assessment, pain parent-assessment, and pain nurse-assessment were calculated for the four groups using the pain assessment forms, and their ratings were compared.
RESULTSThe postoperative pain rates of the four groups ranged from 50.0% to 73.3%. The difference among the four groups was statistically significant (P < 0.001). The differences among the control group and experimental groups 1 and 2 were not statistically significant (P = 0.871), whereas the differences among experimental group 3 and the other groups were statistically significant (P < 0.001).
CONCLUSIONPostoperative pain in preschool children with cleft lip and palate is common. Psychological nursing intervention with analgesic treatment is effective in relieving postoperative pain.

9.Mechanisms and molecular typing of carbapenem-resistant Proteus mirabilis strains deficient in swarming motility

Objective To investigate the mechanism and epidemiological characteristics of carbap-enem-resistant Proteus mirabilis ( PM) strains deficient in swarming motility. Methods PM strains were isolated from Hangzhou General Hospital of CAPF ( Chinese People′s Armed Police Forces) during January 2013 to December 2014. Bacterial motility and flagella of the PM strains were observed through semi-solid agar culture and flagella staining. Pulsed-field gel electrophoresis ( PFGE) was performed for homology anal-ysis. Antimicrobial susceptibility test and phenotypic confirmatory test were also carried out. PCR analysis and DNA sequencing were performed to confirm the genotype of resistant genes. Plasmid electroporation and S1-PFGE in combination with Southern blot hybridization were used to determine the location of the carbap-enem-resistant genes. Genetic structure of the blaKPC-2 gene was obtained by PCR mapping. Results A total of 42 PM isolates deficient in swarming motility were screened out and the resistance rates to imipenem and meropenem were 57. 1% and 52. 4%, respectively. PCR analysis and DNA sequencing confirmed that 24 carbapenem-resistant PM isolates deficient in swarming motility carried blaKPC-2 gene and belonged to three clones as indicated by the results of PFGE. Southern blot hybridization indicated that the blaKPC-2 gene was located on plasmids varying in size (26 kb, 55 kb and 139 kb). In addition, some of the strains harbored several resistant genes, such as blaTEM-1 , blaCTX-M-65 and rmtB. The genetic structures of strains carrying blaKPC-2 gene were ISKpn8, blaKPC-2 and ISKpn6-like from upstream to downstream. Conclusion Compared with the PM strains with swarming motility, the carbapenem-resistance rate was significantly higher in these PM strains deficient in swarming motility. Carbapenemases KPC-2 played an important role in the carbapen-em-resistant PM strains deficient in swarming motility. There was a cloning spread trend for carbapenem-re-sistant PM strains in our hospital. Clinicians should pay more attention to the risk of spreading.
10.Effect of Angelica Sinensis Volatile Oil on miRNA differential expression profiling in myocardium tissue in spontaneously hypertensive rats

Objective:To explore the effect of Angelica Sinensis Volatile Oil on the mi RNA expression profiling in myocardium tissue of the spontaneously hypertensive rats (SHRs),and to clarify the possible mechanism of Angelica Sinensis Volatile Oil.Methods:All the SHRs were divided into Angelica group,model group and captopril group,and other Wistar rats with the same age were selected as control group.The non-invasive systolic blood pressure was detected;after 4 weeks of administration,the changes of miRNA expression profiling in myocardium tissue were measured by Affymetrix miRNA 4.0 Array.KEGG analysis was used to identify the target genes. Results:Compared with control group,the systolic blood pressure of the rats in model group was significantly decreased (P < 0.05)before treatment.4 weeks after administration,compared with model group,the systolic blood pressure of the rats in Angelica group was significantly decreased (P <0.05).Compared with model group, 29 differential miRNAs of rats in Angelica group were found (P < 0.05 ), with 13 up-regulated miRNAs and 16 down-regulated miRNAs.The KEGG analysis results showed that miR-19a,let-7i,and miR-181c were related to insulin signaling pathways; let-7i, miR-181a, and miR-455 were related to VEGF signaling pathways;miR-122,miR-181a, miR-200b, miR-181c, let-7i, and miR-19a were related to apoptosis (P < 0.05 ). Conclusion:Angelica Sinensis Volatile Oil can decrease the blood pressure in SHRs and it can regulate the blood pressure by regulating the miRNA related to insulin signaling pathways and VEGF signaling pathways.
