Effective treatments of acute ischemic stroke include the specialist wards (stroke units) with multidisciplinary treatment team receiving treatment and intravenous thrombolytic therapy. However, only a few patients can receive these treatments.Telestroke can improve the thrombolytic rate of acute ischemic stroke on site in hospitals for lacking of specialists and enable stroke care maintained at a high level in rural areas. This article reviews telestroke in the treatment of acute ischemic stroke and the roles of nurses in telestroke.

Objective:To explore the relationship among professional psychological help-seeking intention, mental symptoms, and self-efficacy in adults. Methods:Totally 618 adult samples[286 males and 332 females,304 early adulthood (18-35 years) and 314 middle adulthood (36-60 years)] were selected on the internet. They were assessed with the Intention of Seeking Counseling Inventory (ISCI),Symptom Checklist 90 (SCL-90) and General Self-Efficacy Scale (GSES) to evaluate professional psychological help-seeking intention, mental symptom and self-efficacy respectively. Results:Females and middle adulthood subjects got higher ISCI scores than males and early adulthood subjects [(82.7 ± 6.7) vs.(77.8 ± 7.8),(83.9 ± 6.2) vs.(76.8 ± 7.3);Ps < 0.001].Step linear regression analysis showed that age, scores of self-efficiency,horror and obsessive-compulsive symptoms had positive prediction effect on help-seeking intention (3 =0.34,0.29,0.10,0.11).Mental symptoms, hostility and paranoia had negative prediction effect on help-seeking intention (β =-0.17,-0.15,-0.11),and the 7 variables could explain 45.6% of the variation. Conclusion:It suggests that the adults' professional psychological help-seeking intention may be related to subject age, mental symptom and self-efficiency.

Objective To explore motivation alteration influencing factors of internet addiction disorder (IAD) undergraduates. Methods The study sampled randomly 793 undergraduates from China University.According to CIAS-R and Young's internet addiction diagnostic criteria,a total of 37 internet addicts were selected.Internet addiction survey was used to study the motivation alteration influencing factors of IAD undergraduates. Results (1) The negative effect of internet using on academy, personal relationship etc.(damage from internet using) had a significant positive correlation with motivation alteration (r =0.672,0.699,0.658,P < 0.01 ; r =0.653,0.673,0.628,P < 0.01 ; r =0.604,0.681,0.703,P < 0.01).(2) Benefit from psychological satisfactory level of internet using had no significant difference with motivation alteration.(3) Self-efficiency of behavioral control on using internet had a significant negative correlation with motivation alteration (r =-0.397,-0.370,P <0.05).Conclusion The motivation alteration of IAD undergraduates is influenced by the factors of damage from internet using, sense of achievement,self-efficiency of behavioral control on using internet and so on.

Objective To study the clinicopathological and imaging features, key points of diagnosis and prognosis of endolymphatic sac tumor. Methods Seven cases of endolymphatic sac tumor were studied with routine histopathological, immunohisochemical staining and CT,MRI scanning.Patients' prognosis and life quality were aesessed by follow-up.Results The clinical symptoms of patients were progressive sensorineural deafness, tinnitus, vertigo and affection of cranial nerves.CT revealed reticular of moth-eaten bony destruction of the petrosal region, and MRI revealed a region of high-signal intensity void of vascular flowing signals. The histopathologic examination of the tumor presented a papillary-cystic pattern. The tumor cells were round or cubaid in shapen, with nuclei in similar shape and size. There were cyst-like structares filled mith pinkish gelatinous.Tumor cells were roundor cuboid in shape, and some of them contained clear cytofrlasm. There was materials, which were stained strongly with PAS.Hemorrhage, cholesterol crystal and foreign body giant cells were found in the tumor stroma with abundant small blood vessels and extravasation of blood, cells with intracellular hematin, and giant cells collagen. There was also proliferation of fibrous connective tissues.Necrosis and bone destruction were also found in some cases.Immunohistochemical results showed that CK and NSE were positive in all the tumors, and CD56 and S-100 were positive in part of cases,GFAP,EMA,CgA,TG and Syn were negative in all the specimens. One patient had received radiotherapy after partial resection.Relapse occurred only in one patient two years after the first surgery, and no metastasis or relapse occurred in the rest of patients. Conclusions Endolymphatic sac tumor is a rare neoplasm. The diagnosis relies mainly on histopathological and immunohisochemical findings. The patients often have good prognosis.

Objective To study the MR imaging technology and 3D reconstruction of the articular cartilage of knee.MethodsConventional,2D,3D and multiparameter MR imaging scans of knee were performed in ninety patients with trauma(n=34)andosteoarthritis(n=56) of knee and 10 healthy volunteers. The articular cartilages on 3D images were reconstructed using maximum intensity projection(MIP).The mean thickness of articular cartilage was measured and analysed statistically. Results Articular cartilages of knee were clearly depicted by MR imaging technique and 3D.On 2D FSE sequence, about 50% of articular cartilage appeared as three laminar apearance. On 3D imaging, the three-dimensional morphology of articular cartilage was showed clearly. Conclusion MR imaging and 3D reconstruction of the articular cartilages of knee is helpful in early diagnosis and treatment of disordered cartilages.

Chinese Journal of Nephrology 2012;28(9):692-697
Objective To investigate haemodynamic change during hemodialysis and analyze effects of cardiac index (CI) on hemodynamic parameters and associated influencing factors in maintenance hemodialysis (MHD) patients. Methods Seventy-five patients bearing an arteriovenous fistula (AVF) entered the study.Cardiac output (CO),cardiac index (CI),central blood volume (CBV) and peripheral vascular resistance (PR) were determined by ultrasound dilution technique at the end of 1 hour,2 hours and 3 hours of dialysis.AVF blood flow (Qa) was also measured with the same device before haemodynamic parameters investigation. Results Mean age of patients was (55.84 ±12.39) years old (range 21-81 years) and 43 patients (57.3%) were female.Systolic arterial pressure,SV,CO,CI and CBV were significantly declined and no significant change for diastolic arterial pressure and heart rates at the end of 2 hour and 3 hour hemodialysis, whereas PR was increased gradually during hemodialysis.Patients were divided into there groups with CI less than 2.5 L·min 1·(m2)-1,2.5-4.2 L·min-1·(m2)-1 and more than 4.2 L·min-1 ·(m2)-1 at the end of 1 hour dialysis.Statistically significant decreasing SV,CO and increasing PR were detected in patients with CI<2.5 L·min-1· (m2)-1 and 2.5-4.2 L· min-1· (m2)-1,compared with CI>4.2 L·min-1· (m2)-1 group (P<0.01).The hemodynamic change was the most obvious in the group of CI less than 2.5 L· min-1· (m2) 1,and no significant changes happened in CI>4.2 L·min-1 ·(m2)-1 group. Some factors were found to be associated to CI values.Qa and systolic arterial pressure had positive relationship with CI,while age and diabetes had negative relationship with CI.Conclusions Systolic arterial pressure,CO,CI and CBV decrease and PR increases during hemodialysis.Obvious change occurs when CI is less than 2.5 L·min-1· (m2)-1.CI is associated with Qa,systolic arterial pressure, age and diabetes.
