1.Multi-slice spiral CT angiography of the posterior nutcracker syndrome

Objective To assess the value of multi-slice spiral CT angiography (MSCTA) in diagnosis of posterior nutcracker syndrome (NCS). Methods MSCTA of 15 patients clinically diagnosed as posterior NCS (patient group) and 50 subjects with normal renal vessels (control group) were retrospectively analyzed. The anatomy, course and relationship to the adjacent structure of left renal vein (LRV) and its branches were observed. The anteroposterior diameter (d1) and cross areas (s1) of the angle of control group and posterior abdominal artery (AA), the anteroposterior diameter (d2) and cross areas (s2) at the largest lumen near the renal hilar were measured and the ratio of s2/s1 and d2/ d1 were calculated.Results All LRVs posterior AA of patient group were compressed and narrowing with dilating apparently near the renal hilar. The difference of d2 was not significant (P>0.05), but of other values were significant (P<0.05).Conclusion MSCTA can show anatomy and three-dimentional trend clearly of LRV for posterior NCS, and evaluate the extent of narrow and dilating lumen accurately with cross areas measuring.

Immune thrombocytopenia is the most common diagnosis
of isolated thrombocytopenia. The dilemma encountered by
paediatricians is missing diagnosis of acute leukaemia in
children with isolated thrombocytopenia. We demonstrated
childhood ITP could be diagnosed using a four point clinical
criteria without missing a diagnosis of acute leukaemia.
Hence, bone marrow examination is not necessary in
children with typical features compatible with ITP prior to
steroid therapy. This can encourage paediatricians to
choose steroid therapy, which is cheaper and non-blood
product, as first line platelet elevating therapy in children
with significant haemorrhage.

3.Antisense intergrin αV and integrin β3 suppress the growth of implanted hepatocellular carcinoma in nude mice

Objective To investigate whether antisense integrin αV and β3 gene therapy has antitumor activity for hepatocellular carcinoma.Methods In this study,120 male nude mice at the age of 4 weeks old were devided into 4 groups randomly.The antisense integrin αV and β3 expression vectors were iniected into HepG2 hepatomas established in nude mice to monitor tumor growth after 6 weeks.Tumot inhibit rate was calculated.Tunor microvessel density(MVD)was determined by immunohistochemical staining,and tumor apoptosis by TUNEL. Results The tumor weight of the control group,αV group,β3 group and αVβ3 group was(1.38±0.92)g、(1.28±0.27)g、(1.30±0.34)g、(1.08±0.16)g respectively.The difference between each two groups was significant(q12=4.76,q13=3.73,q14=14.28,P<0.05);The MVD was(17.53±1.88)、(16.06±1.92)、(15.83±2.00)、(14.86±1.69)respectively,(q12=3.91,q13=4.55,q14=7.13,P<0.01);The AI of the control group,αV group,β3 group and αVβ3 group was 10.53%±3.29%,19.80%±4.06%,21.93%±3.26%,24.03%±4.45%respectively,with the difference being significant among groups(q12=29.41,q13=33.52,q14=39.7,P<0.01).Conclusions Antisense gene therapy targeting αV and β3 integrins exerts dramatic inhibition on the growth of the tumor.

The fine structure of enoxaparin sodium samples with different degree of 1,6-anhydro derivatives were analyzed with polyacrylamide gel electrophoresis, high performance liquid chromatography, ultraviolet spectroscopy, infrared spectroscopy and nuclear magnetic resonance spectroscopy. A further study of anticoagulation activity of enoxaparins was performed, including those on their inhibition activities of coagulation factor Xa (FXa) and thrombin (FIIa). The results showed that the anti-FXa and -FIIa activities of enoxaparins with different degree of 1,6-anhydro derivatives (20.0%-39.7%) with similar structure characteristics, had decreasing tendency when the degree of 1,6-anhydro derivatives increased. Especially, the anti-FXa activity was sensitive to the change of the degree of 1,6-anhydro derivatives.

7.Bioactive sterols from red alga Acanthophora spicifera boergesen.

OBJECTIVETo study the chemical constituents of the red alga Acanthophora spicifera boergesen aiming at searching for bioactive leading compounds.
METHODCompounds were isolated by various chromatographic techniques including column chromatography over normal phase silica gel and Sephadex LH-20 gel and reverse phase HPLC as well as recrystallization. Their structures were determined by spectroscopic methods including IR, MS, 1D and 2D NMR techniques. MTT method was used for testing cytotoxicity of compounds against human cancer cell lines HCT-8, Bel-7402, BGC-823, A549 and HELA. Their inhibition against proliferation of dog vascular smooth muscle cells was also screened by MTT assay.
RESULTSix sterols were isolated from the ethanolic extract of the red alga Acanthophora spicifera. Their structures were identified as 6-hydroxycholest-4-ene-3-one (1), cholest-4-ene-3, 6-dione (2), cholest-5-ene-3 beta-ol (3), 5 alpha-cholestane-3, 6-dione (4), beta-sitosterol (5) and saringosterol (6).
CONCLUSIONCompounds 1-3 and 5 were obtained from this genus for the first time. Compounds 1, 2 and 4 showed moderate cytotoxic activity against human cancer cell lines.

8.Therapeutic effect of vertebral artery stenting for treating vertebral artery stenosis

Objective:To summarize experience of vertebral artery stenting in nine cases with vertebral artery stenosis for providing reference for clinical treatment .Methods :Success rate of operation ,vertebral artery stenotic degree before and after operation were evaluated in nine cases undergoing vertebral artery stenting .Results:A total of nine stents were implanted in nine cases and the success rate of operation was 100% ,after stents were implanted blood flow was unobstructed ,stent expansion was good and intracranial blood supply was good .Compared with before op‐eration ,stenosis rate significantly reduced after operation [(84.0 ± 5.7)% vs .(10.0 ± 2.0)% ] (P<0.01) .No vas‐cular restenosis occurred within three‐year follow -up .Conclusion:Vertebral artery angioplasty and stenting is a new emerging ,minimally invasive ,effective and highly safe therapeutic method for vertebral artery stenosis ,the therapeutic effect is sure and application prospect is broad .
9.Antimicrobial resistance and mechanisms of ceftriaxone resistance in clini-cally isolated nontyphoidal Salmonella

Objective To investigate antimicrobial resistance and ceftriaxone resistance mechanisms in clinically isolated nontyphoidal Salmonella(NTS),and provide evidence for the prevention and control of NTS infection and rational use of antimicrobial agents.Methods 108 NTS isolates were isolated from stool specimens of outpatients with acute diarrhea in the Second Hospital of Tianjin Medical University and Tianjin Medical University General Hospital from May to October of 2014,NTS were performed antimicrobial susceptibility testing;non-ceftriaxone-susceptible isolates were typed by serological,multilocus sequence (MLST),and pulsed-field gel electrophoresis (PFGE)methods,extended-spectrumβ-lactamase (ESBL)detection and AmpC genes were detection.Results Among 108 NTS isolates,mono-drug resistance rate to 11 antimicrobial agents was 49.07% (n= 53),multidrug resistance rate was17.59% . Susceptibility rates to nalidixic acid,levofloxacin,ciprofloxacin,ceftriaxone,and ertapenem were 61.11% ,66.67% ,68.52% ,97.22% ,and 100.00% respectively. Three non-ceftriaxone-susceptible NTS isolates were detected,2 were ST11 Salmonellaenterica serotype (Sa8709,Sa8771),1 was ST34 Salmonellatyphimurium serotype(Sa8763). Cluster analysis of PFGE revealed that Sa8709 was highly similar to Sa8771 strains(91 .70% ), but the similarity to Sa8763 was low(55.80% );Sa8709 strain carried CTX-M gene,Sa8771 strain carried CTX-M and TEM genes,Sa8763 strain carried OXA gene. Conclusion Clinically isolated NTS in this area are low resistant to fluoroquinolones,multidrug resistant strains carrying ESBLs have emerged.
10.Evaluation criteria and maintenance experience of cardiac death organ donor heart from Chinese first category

Objective To summarize the evaluation standard of cardiac death organ donation from Chinese first category donor heart application and experience of donor heart function maintenance.Method From Jan.2013 to Jan.2015,donor hearts for heart transplantation were obtained in 18 cases of Chinese first category cardiac death organ donors from 109 cases of organ donation donors through rigorous assessment and effective donor heart function maintenance.The diagnosis of brain death was based on the diagnosis of brain death criteria (adult) by the neurological department of internal medicine,department of neurosurgery,and intensive care unit with brain death qualification.Organ donation work followed the basic principles of voluntary,free,fair,equitable and technical access.Result Eighteen case of heart transplantations were all operated with double lumen venous anastornosis.The cold ischemia time was (125.5+ 18.7) min (61-60 min),and cardiopulmonary bypass time was (130.4+ 12.5) min (99-193 min).In 18 heart transplantations,16 cases survived,and 2 eases died of acute right heart failure.During the follow-u period,the quality of life in the survival recipients was satisfactory.Conclusion Using cardiac death organ donation from Chinese first category donor heart transplantation has achieved good results.The accurate assessment of donor hearts and effective donor heart function maintenance are the key factors to guarantee the success of heart transplantation.
