1. The progress of virus-targeted new therapy against Epstein-Barr virus-associated malignancies
Tumor 2014;34(2):180-186
The Epstein-Barr virus (EBV) has been demonstrated to be associated with several human tumors, including nasopharyngeal carcinoma (NPC), Burkitt's lymphoma (BL), post-transplant lymphoproliferative disease (PTLD), Hodgkin's lymphoma (HL), non-Hodgkin's lymphomas (NHL) and gastric cancer (GC). In most EBV-associated malignancies, the tumor cells contain the viral genome. This provides a unique opportunity to develop therapeutic strategies utilizing the viral genome of EBV as a potential drug target. This review focuses on different therapeutic strategies against EBV-associated malignancies, further discusses the possible therapeutic approach using drug-induced virus activation and its certain pathways. Objective: To detect the relationship between variant of rsl255372 in transcription factor 7-Like 2(TCF7L2) gene and the type 2 diabetes mellitus (T2DM) in Tianjin Han populations. Methods: Three hundred and fifty-two T2DM patients and 176 healthy controls were randomly selected to extract the genome DNA. Denaturing high performance liquid chromatography was used to detect the PCR amplified fragment. The different peaks were chosen for sequencing. The differences of different genotype frequency between the groups were analysed. The Logistic regression was used to evaluate the risk factors of T2DM. Results: The G/T genotype frequencies of rsl2255372 were 18.5% and 2.8% in T2DM group and healthy control group respectively (P < 0.05). The G/T genotype, glycosylated hemoglobin, urea nitrogen and the systolic blood pressure were the independent risk factors of T2DM, and the high density lipoprotein was the protective factor. There was no significant difference of the G/T genotype frequency in T2DM patients with different complications. Conclusion: This study indicates that G/T genotype in the TCF7L2 gene significantly contributes to T2DM susceptibility in Tianjin Han populations. The G/T genotype is one of the risk factors of the T2DM.

BACKGROUND : After femoral neck fracture, incidences of ischemic necrosis and bone non-healing are closely related to biomechanical characteristics of internal fixation devices. Compression stress can improve fracture healing, tension force can delay fracture healing, and shear stress can inhibit fracture healing. How to relieve shear stress and expand compression stress is of significance for theories and clinical applications.OBJECTIVE: To compare the biomechanical characteristics of the two geometric configurations of three cannulated compression screws in the fixation for femoral neck fracture and to provide a theoretical foundation in the treatment of femoral neck fracture.DESIGN : Observational contrast study.SETTING: Department of Orthopaedics, the 251 Hospital of Chinese PLA; Department of Orthopaedics, the Third Hospital, Hebei Medical University.PARTICIPANTS: The experiment was carried out in the Hebei Orthopaedic Institute from November 2002 to March 2003. Five cadavers including 4 males and 1 female with similar bone mineral density (BMD) were provided by Department of Anatomy, Hebei Medical University. X-ray photographs proved that all the cadavers did not have rheumatism, tuberculosis, tumor, fracture or deformity.METHODS: Ten femurs were randomly divided into inverted and upright isosceles triangle with 5 in each group. Femur samples with femoral neck fracture were fixed with three cannulated screws and measured with the biomechanical machine (CSS-44020, made in Changchun Experimental Researching Institute, provided by Hebei Orthopaedics Institute). The compression strength, torsibility and the maximal vertical loading were compared between the two configurations of screws.MAIN OUTCOME MEASURES:①Displacement of femoral head at 600 N and 750 N during torsibility test; ②torque-moment at 2°and 4°during torsibility test;③load during maximal vertical loading test.CONCLUSION: The effect of three cannulated compression screws configured as an inverted isosceles triangle on the treatment of femoral neck fracture is superior to that of three cannulated compression screws as an upright isosceles triangle.

Objective To investigate the expression of slit diaphragm proteins of glomerular podocyte, such as NEPH1 and Nephrin in type Ⅴ lupus nephritis (V-LN). Methods Twenty-five patients with V-LN and 18 patients with idiopathic membranous nephritis (IMN) were enrolled into the study, and 5 normal renal samples were the normal control group. Twenty-four hours urine protein excretion, serum albumin, creatinine, triglyceride, total cholesterol, serum C3, C4, urine C3 and NAG were tested respectively.Glomerular lesions were measured by light microscopy. The expressions of NEPH1 and Nephrin were determined by indirect immunofluorescent staining. The statistical treatment was used t-test. Results Compared to the IMN group, the 24 hours urine protein excretion and the concentrations of serum albumin, creatinine, urine C3 were not significantly different while the triglyceride, total cholestorel, serum C3, C4 were significantly decrease in the V-LN group (P<0.05). Urine NAG was increased in the V-LN group (P<0.01). By indirect immuno-fluorescent histochemitry examination, the glomerular expressions of NEPH1 and Nephrin were significantly decreased in both V-LN and IMN. Compared with the IMN group, the decrease of NEPH1 and Nephrin expression was more remarkable in the V-LN group. Conclusion The expression changes of NEPH1 and Nephrin may play an important pathogenic role in proteinuria of Ⅴ lupus nephritis. Renal tubular epithelial cell damage may play a role in proteinuria of V-LN.
