1.Early diagnosis of primary gallbladder carcinoma

Objective To promote the level of early diagnosis and strengthen the understanding of pathogenesis, pathological staging and prognosis of primary gallbladder carcinoma (PGC). Methods The data of 679 patients with PGC treated in our hospital from 1956 to 1998 were retrospectively analyzed. Results The incidence of PGC has been increasing in recent years while the surgical management for the disease was not satisfactory. Upon diagnosis, most patients were with PGC in advanced stage. PGC was usually found among the aged woman patients. The female/male ratio was 3∶1. There was a close relationship between gallstone and PGC, for the gallstone was found in 60% of the patients with PGC. The diagnostic accordance rate before and after operations was low. Most cases of PGC were found unexpectedly during operation due to gallstone or acute cholecystitis. It was even worse that many patients with PGC missed the opportunity of diagnosis and treatment because doctors noticed the gallstone only. Pathological classification revealed that most cases of PGC were of adenocarcinoma. Development in imaging medicine might help a lot in finding early-stage cases and improving prognosis. Conclusions Strengthening the understanding of pathogenesis, pathological staging and prognosis of the disease and proper use of various examinations are the basic means of obtaining early diagnosis and improving the prognosis.
3.Disinfection of Dental Hand Piece Contaminated with HBV in Practice

OBJECTIVE To investigate the disinfection efficacy of dental high-speed hand-pieces contaminated by HBV with the method of pressure steam sterilizer.METHODS Taking 50 oral clinical patients randomly,and three group-samples which included saliva,hand-pieces contaminated by clinical operation and disinfected by pressure steam sterilizing were collected,then the samples were detected HBsAg and HBV DNA,respectively.RESULTS There were 95% of the saliva samples being HBsAg positive,the positive rate of the high-speed hand-pieces contaminated by clinical operation was lower than the saliva samples,and the positive rate of the hand-piece disinfected by pressure steam sterilizing was the lowest.HBV DNA was undetectable in the sample before or after disinfected hand-pieces used in patients′ saliva which HBsAg s/co value higher than 5.0.CONCLUSIONS Pressure steam sterilizing is effective to reduce the contaminated HBV on hand-pieces,but the biology test should be taken to demonstrate whether the complete sterilizing is achieved.
4.Comparison of different fluid therapies following acute spinal cord injury in rats

Objective:To assess the effects of different fluid therapies on myocardial function,blood-spinal cord barrier and water content of spinal cord after acute high level spinal cord injury(SCI) in rats.Methods: Thirty-two male SD rats, weighing (300?20) g, were anesthetized intraperitoneally with pentobarbital (40 mg/kg). A cervical laminectomy extending from C6-T1 was performed and the experimental acute cervical 7 spinal cord injury model was created by modified Allen’s method,and then the rats were randomly divided into 4 groups(n=8): control group(C group) with no fluid resuscitation, 7.5% hypertonic saline group(HS group), 6% hydroxyethyl starch group(HES group) and banlanced solution group (BS group).Fifteen minutes after injury, rats of each treatment groups were given (iv. at 4 ml/kg) above-mentioned fluids in 4 min,then continuously infused at a rate of 10 ml?kg -1 ?h -1 over for 30 min.MAP,HR,left ventricular systolic pressure (LVSP),and ?dp/dt max were recorded at 5,15 and 30 min.At 30 min, 0.5% Evan's blue(EB) 1 ml was injected iv.Two hours later the animals were sacrificed and the injured segments of spinal cords were removed for determination of water content and EB content.Results:After SCI there were no differences of HR at different time points in each group. 5.Changes of ?_1-adrenergic receptors expression after high-level spinal cord injury in Wistar rats

Objective:To observe the changes of ? 1 -adrenergic receptors expression in a high-level spinal cord injury (SCI) rat model,providing information for studying the mechanisms of severe hypertension of autonomic dysreflexia.Methods: Adult male Wistar rats were divided into 2 groups: sham-lesion group(n=6) and SCI group(n=36). Severe spinal crush injury was induced at T 4 using modified Allens device in SCI group, while sham-lesion group received laminectomy at T 4 only.The expression of ? 1 -adrenergic receptors mRNA in different segments of spinal cord:injured segment(I), above(A) and below(B) the injured site, were measured by reverse-transcription polymerase chain reaction (RT-PCR) 1 d, 3 d, 7 d, 2 weeks, 3 weeks and 4 weeks after SCI. Results: Compared with sham-lesion group,the express of ? 1 -adrenergic receptors mRNA in group A decreased 3 d after SCI and dropped to the least 1 week later,and restored to the normal level 2 weeks later.The impairment of ? 1 -adrenergic receptors mRNA expression in group I occured immediately after SCI and hit the lowest value 3 d later,and did not recover to the nomal 4 weeks later.Decrease of ? 1 -adrenergic receptors mRNA expression in group B were detected 1 d after SCI,but began to increase on 3 d and exceeded that of group C 4 weeks later.Conclusion: In chronic SCI rat models ? 1 -adrenergic receptors expression increases below the level of SCI,which might be a contributing factor to the severe hypertension of autonomic dysreflexia.
6.Clinical significance and histological origin of glomerular epithelial proliferative lesion in patients with focal segmental glomerulosclerosis

ObjectiveTo investigate the clinical significance and histological origin of glomerular epithelial proliferative lesion in patients with focal segmental glomerulosclerosis (FSGS). MethodsSeventy-four patients with idiopathic FSGS hospitalized in Peking University First Hospital from Jan. 2000 to Dec.2005 were enrolled in this study. Patients were classified into two groups according to with or without glomerular epithelial proliferative lesion. Estimation of active and chronic pathological scores was carried out using a semi-quantitative grade system by two pathologists. Clinical and pathological characteristics were compared between two groups. Immunohistochemical studies were performed to analyze the histological origin of glomerular epithelial proliferative lesion. ResultsThirty-one patients with glomerular epithelial proliferative lesion showed shorter interval from presentation to biopsy (P<0.05), higher percentage of nephrotic syndrome (NS) (P<0.05), higher frequency of segmental glomerulosclerosis(P<0.05), higher pathological active scores (P<0.05) and lower pathological chronic scores (P<0.05)as compared to 43 patients without glomerular epithelial proliferative lesion. Twenty-nine patients were followed up and renal survival rate in patients with glomerular epithelial proliferative lesion (39.7%) was significantly lower than that in patients without glomerular epithelial proliferative lesion (83.3%) (P=0.049). The frequency of glomerular epithelial proliferative lesion and the serum creatinine (Scr) level at biopsy were independent predictors of ESRD (OR value was 1.204, 1.008 respectively ). Glomerular epithelial proliferative lesion did not express mature podocyte markers including WT-1 and pedocalyxin, but stained positive for PCNA, PAX-2 and CK-8. ConclusionsGlomerular epithelial proliferative lesion represents the pathological change of acute stage and active lesion of FSGS, and also may be the pathological marker of severe clinical presentation and worse renal survival. Glomerular epithelial proliferative lesion may be derived from proliferation of parietal epithelial proliferation or de-differentiated podocytes.
7.L-carnitine inhibits eryptosis induced by uremic serum

[ ABSTRACT] AIM: To investigate whether L-carnitine ( LC) inhibits the eryptosis effect of uremic serum on erythrocytes.METHODS:Erythrocyte suspension (2%) was cultured and divided into 3 groups in vitro: control group ( C group) , uremic serum group ( U group, 30%uremic serum) , and uremic serum+LC group ( L group, 30%uremic serum+200 μmol/L LC) .Erythrocytes were collected at 24 h and 48 h.Eryptosis ( phosphatidylserine expression repre-sents eryptosis) was estimated by flow cytometry with Annexin V staining.The content of reactive oxygen species ( ROS) was also detected.Glutathione ( GSH) was measured by ELISA.RESULTS:Eryptosis in C group was increased as the in-cubating time extended.Eryptosis in U group was higher than that in C group, while that in L group was lower than that in U group.Meanwhile, ROS content was higher and GSH was lower in U group than those in C group.ROS content was low-er and GSH was higher in L group than those in C group.CONCLUSION:LC inhibits uremic serum-induced eryptosis by decreasing ROS and increasing GSH, thus attenuating oxidative stress.
8.Clinical value of synangio-excision-restruction in pancreatic cancer radical operation

Objective To investigate the influence of synangio-excision-restruction in pancreatic cancer radical operation on the complications,living quality and survival status in the patients with pancreatic cancer.Methods A total of 255 patients with pancreatic cancer in our hospitals from January 2010 to October 2015 were selected and divided into 3 groups according to different operation modes:41 cases in the Synangio-excision-restruction group(A),113 cases in the non-synangio-excision-restruction group (B) and 101 cases in the palliative by-pass operation group(C).The clinical data in 3 groups were analyzed.The influence of Synangio-excision-restruction on operative complications,living quality and survival status was investigated.Results The incidence rate of complications in the group A was 56.10%,which was obviously higer than 34.51% in the group 1β and 20.79% in the group C,and the difference was statistically significant(P<0.05).In the group A,the incidence rates of belly ache and body weight gain were 36.59% and 51.22% respectively,which comparing with 91.09% and 9.09% in the group C showed statistically significant difference(P<0.05).The median survival time(MST) in the group A was 11.83 months,which in the group B and C were 15.43 months and 7.50 months,the difference between the group A and C was statistically significant(x2 =4.27,P<0.05);while the difference between the group A and B was not statistically significant(x2=3.67,P>0.05).Conclusion For the pancreatic cancer patients with affected portal vein and inferior mesenteric vein,the synangio-excision-restruction radical operation can obviously prolong the patients' survival time and improves their living quality.
9.Phylogenetic analysis for Fritillaria hupehensis: evidence from ITS, rpl16 and matK sequences.

The systematic position of Fritillaria hupehensis has been in dispute. Phylogentic analyses were conducted on sequences of ITS, rpl16, matK sequences for species of F. hupehensis and allies. Lilium davidii was designed as outgroup. The analyses were performed using MP and ML methods. Conclusions could be achieved as follow. The topologies of MP and ML are consistent. The samples of F. hepehensis from different places form a supported clade with a strong bootstrap. And then form a strongly supported clade with F. anhuiensis, F. monantha. The results suggests that although F. hupehensis has a closet relation with the two ones, it exists some difference.

10.Improved islet isolation by three-cannula method for collagenase infusion

Objective Pancreas perfusion is an essential step in human islet isolation.To develop the new methods for introductal canulation,collagenase infusion and to observe their effects on islets isolation.Methods A total of 17 pancreases were digested from March 2005 to April 2010.The pancreases were distended by three methods:the standard method (n =3),the one-cannula method (n =11) and three-cannula method (n =3).In the standard group,the pancreases were completely cut into half at the mid-body.Two catheters were inserted into the main duct:one directed toward the tail and the other to the head.In the one-cannula method group,a long tube was inserted into the duct at the head,advancing to the tail In the three-cannula method group,pancreatic parenchyma was then minimally cut at the mid-body and three catheters were inserted into the main pancreatic duct:one at the head (the first catheter) and two at the mid-body,one toward the tail (the second catheter) and the other toward the head (the third catheter).The pancreases were digested by improved Ricordi technique.Ficoll continuous density/grads centrifuge method was performed to purify the islets.DTZ staining was adopted to identify islets and count islet equivalent (IEQ). AO/EB fluorescence examination was used to count active islet percentage.Static glucose stimulating test (SGS) in vitro was designed to estimate islet function and calculate SI.Results The distension volume of the threecannula method group was 1.24 rnl/g pancreas,and higher than the other groups (for the standard group:0.71 ml/g pancreas; for one-cannula method group:0.96 ml/g pancreas,P<0.05).The yield of islet in the three-cannula method group and the one-cannula method group was 2514 and 2270 IEQ/g,which was significantly more than that in the standard group (1914 IEQ/g pancreas,P<0.05).The purity and viability of the islets were 74 %/79.3 %,75.6 %/79.4 % and 78.3 %/84.0 % respectively in the three groups with the difference being not significant among the groups.SI in the one cannula method group (4.74) and the three-cannula method group (5.27) was significantly higher than that in the standard group (3.46).ConclusionThe three-cannula method improved collagenase infusion and the islet yields.
