1.Clinical application of preoperative TACE in treating squamous cell carcinoma of tongue

Objective To discuss the clinical application of preoperative transcatheter arterial chemoembolization (TACE) in treating squamous cell carcinoma of tongue. Methods A total of 26 patients with pathologically proved squamous cell carcinoma of tongue were enrolled in this study. Using Seldinger technique, lingual artery angiography and TACE were carried out in all patients. Radical resection of tongue cancer was performed in 2-3 weeks after TACE. Results Super-selective catheterization and TACE was successfully accomplished in all patients. After TACE the tumor size was significantly decreased in all patients. During the operation, clear lesion's border was observed, and the amount of blood loss was less than usual. Conclusion For the treatment of squamous cell carcinoma of tongue, preoperative superselective TACE is safe and effective;it can increase the opportunity of surgical resection, reduce the surgical damage, and improve the quality of life as well as the survival rate.
2.Dosimetric study of adaptive radiotherapy for nasopharyngeal carcinoma

Seven patients with nasopharyngeal carcinoma treated with ART were recruited . All patients were treated with SIB-IMRT regimen( named IMRT) . All patients had a second computed tomography scan after 10 fractions and a new plan based on this was initiated from fraction 11 ( ART1 ) . And a third CT scan after 21 fractions and a new plan based on this was initiated from fraction 22(ART2). When the treatment was finished,a fourth CT scan was made. Results revealed that the volumes of primary tumors and parotid glands regressed significantly after the teat-ment. The position of the parotid glands shifted medially an average of 4.5(1.6~9.2)mm during the plans. ART1 reduced mean dose to left parotid by 0. 42 Gy(1. 68%)(P=0.019) and right parotid by 0. 79 Gy (3.04%) (P=0. 013) over the IMRT alone. ART2 further reduced the mean left parotid dose by 0. 86 Gy or 3.5% ( P =0.001 ) and right parotid by 1 . 04 Gy or 4 . 13% ( P=0.035 ) .
4.Application of evidence-based nursing in nursing of patients with spinal fracture

Objective To discuss the nursing countermeasures of common problems in patients with spinal fracture. Methods All patients received evidence- based nursing in on:ler to tackle problems such as pain, paralysis, pulmonary infection, pressure ulcer, urinary tract infection and deep vein thrombosis. Results No one complicated with pulmonary infection, pressure ulcer and deep vein thrombosis, 2 with lower limb swelling and 3 with urinary tract infection. Conclusions Evidence- based nursing is the base of clinic nursing. It can not only improve the quality of nursing and benefit patients,but also can expand the nurses' knowledge.
5.Comparison of the effect of ropivacaine combined with fentanyl or sufentanil in labor analgesia

Objective To compare the effect oropivacaine combined with fentanyl or sufentanil in labor anal-gesia.Methods 116 cases of maternal birth in obstetrics and gynecology were selected.According to the random number table method,116 cases of parturients were randomly divided into observation group (58 cases,received ropiv-acaine and sufentanil analgesia)and control group (received ropivacaine and fentanyl).In two groups of maternal anesthesia before induction (T0 ),used the drug after 30min(T1 ),60min(T2 ),180min(T3 )detection of the mean arterial pressure (MAP)and heart rate (HR),and digital evaluation table Rating Scale (NRS).The complications of maternal and infant in the two groups were recorded,the change of the mode of delivery and the Apgar score of neo-nates in the two groups were analyzed.Results There were no significant differences in the levels of MAP and HR between the two groups in T0-T3 (tMAP =0.123,1.078,0.212,0.047;P =0.903,0.283,0.832,0.963;tHR =0.033, 0.289,0.288,0.036;P =0.974,0.773,0.774,0.972).The NRS scores in T1 -T3 of the two groups were significantly lower than those in the T0;the T1 -T3 scores of the observation group were (2.21 ±0.36 )points,(1.78 ± 0.24)points,(1.83 ±0.25)points,which were significantly lower than (3.61 ±0.75)points,(3.22 ±0.12)points, (2.84 ±0.26)points of the control group(t =12.816,40.871,21.325,P =0.000,0.000,0.000).In the observation group,there was 0 case (0.00%)had cardiac arrhythmia,the control group had 4 cases (6.90%),and the differ-ence was statistically significant(χ2 =4.143,P =0.042).Two groups of labor analgesia,the way of delivery and birth of the newborn after birth,1min,5min of Apgar score were compared,the differences were not statistically significant (χ2 =0.703,t =0.631,1.149,P =0.402,0.530,0.253).Conclusion Ropivacaine combined with sufentanil or fentanyl in analgesia have no effect in childbirth and maternal vital signs and neonatal,and have good analgesic effect, but ropivacaine combined with sufentanil has stronger analgesic effect,higher safety.
6.Measure of Quadriceps Femoris with Ultrasonography: A Preliminary Study

ObjectiveTo evaluate the usefulness of an ultrasonography-based method to examine quadriceps femoris contracting. MethodsThe thickness of bilateral rectus femoris (RF) and vastus intermedius (VI) and the cross-sectional area (CSA) of RF was measured in 10 healthy subjects (n=20) in a relaxed position and 3 isometric contraction states using ultrasonography by two examiners. The date was compared with paired t test. Half of these subjects (n=10) were also measured with MRI at rest. The date was also compared with paired t test. ResultsThere was no significant difference between MRI and static compound ultrasonography, nor between two examiners. There was a significant difference (P<0.05) in thickness of RF and VI between relaxed and isometric contraction conditions. There was no significant difference among isometric contraction conditions. There was no significant difference among measurements of RF-CSA. Conclusionultrasonography shows good validity and reliability in measuring quadriceps shape.
7.Effect of berberine on D-dimer, fibrinogen and interleukin-6 in peripheral blood of ovarian cancer patients

Objective To analyse the effect of berberine on D-dimer, fibrinogen (FIB) and interleukin-6(IL-6) in peripheral blood of ovarian cancer patients.Methods 130 cases selected from the hospital were diagnosed as unilateral ovarian cancer, treated by surgery and chemotherapy.The patients were divided into control group (25 cases) and experimental group (105 cases), and the experimental group were randomly divided into A, B, C three groups, 35 cases in each group.The experimental group received berberine hydrochloride tablets, orally, three times a day, on the basis of conventional chemotherapy.The experimental groups were divided into three subgroups according to the dosage: A group: 0.3 g, B group: 0.2 g, C group:0.1 g, 35 cases in each group, continuously treated for 2 weeks.The levels of D-dimer, FIB and IL-6 and quality of life score were detected before and after treatment.Results After treatment, D-dimer level in each subgroup of experimental group was lower than that in control group, respectively, and A group
8.Role of 5-hydroxy trptamine 2A receptor in biliary fibrosis after liver transplantation in rats

Objective To evaluate the role of 5-hydroxy trptarine 2A receptor (5-HT2A) in the pathogenesis of biliary fibrosis after liver transplantation in rats.Method Rats were randomly divided into control group Ⅰ and control group Ⅱ[(supplied livers were preserved for 1 or 12 h),ketanserin group (recipients of control group Ⅱ were intraperitoneally injected with ketanserin 24 h postoperatively at the dosage of 5 mg · kg-1 · day-1),and sham group (rats were subjected to transverse laparotomy and closure without manipulation of the liver).During 4-week observation period,serum biliary enzymes,5-HT content in the liver,the expression of fibrosis-related genes,cholangiocytes proliferation and biliary fibrosis were evaluated.Result Compared with the sham group,the serum ALP,GGT,TBil and 5-HT contents in the liver homogenate were increased on the postoperative day 1 (POD1) and then restored to the normal level.There was slight proliferation of bile duct epithelial cells on POD3 in the control group Ⅰ,with fewer collagen fibers and α-sooth muscle actin (α-SMA)-positive myofibroblasts in the portal area.The expression of matrix metalloproteinase-2 (MMP-2) and procollagen α1-mRNA in graft livers was not significantly increased in the control group Ⅰ.To the contrast,the control group Ⅱ demonstrated high levels of serotonin in the liver homogenate and enhanced serum biliary enzymes.Active cholangiocytes proliferation was triggered on POD3 and remained higher than in the control group Ⅰ and the sham group.The control group Ⅱ showed a large number of α-SMA-positive myofibroblasts and collegan fibers at the postoperative week 4.In parallel,the major profibrogenic transcripts MMP2 and procollagen α1 were significantly increased at 2nd,and 4th week postoperation in the control group Ⅱ.Importantly,we also found that ketanserin relieved the signs of biliary fibrosis at 4th week postoperation in 5-HT2A group by the demonstration of reduced collagen fibers and a-SMA-positive myofibroblast in the portal area,as well as the decrease in the fibrosis-related gene expression.In addition to the lower cholangiocytes proliferation,serum levels of biliary enzymes including GGT,ALP and TBil in 5-HT2A group were significantly decreased at 4th week postoperation as compared with the control group Ⅱ.Conclusion Selective 5-HT2A receptor antagonist,Ketanserin retards biliary fibrosis progression posttransplantation,suggesting that 5-HT2A receptor is a potential therapeutic target for ischemia-related biliary fibrosis after DCD liver transplantation.
9.The protective effect of hydrogen-rich saline on lung injury associated with severe acute pancreatitis and its impact on P38MAPK and NF-κB expression

Objective To investigate the protective effect of hydrogen-rich saline on lung injury associated with severe acute pancreatitis and its impact on P38MAPK and NF-κB expressions.Methods Fifty-four male Wistar rats were randomly (random number) divided into three groups:(1) hydrogen-rich saline treatment group (HRS group,n =18),in which the rats were treated with hydrogen-rich saline (6 mL/kg) administered intravenously via tail vein and HRS (20 mL/kg) administered subcutaneously at 5 min after successful modeling.(2) Severe acute pancreatitis model group (SAP group,n =18),in which rats received equivalent volume of normal saline instead of hydrogen-rich saline both intravenously and subcutaneously as in HRS group.(3) Sham operation group (SO group,n =18),in which rats were treated with sham surgery,and received equivalent volume of normal saline as in SAP group.The model of severe acute pancreatitis (SAP) was made by retrograde injection of 5% sodium taurocholate (1 mL/kg) into cholepancreatic duct.All rats were sacrificed at 3 h,12 h,and 24 h separately after the operation (n =6 at a time).The levels of serum amylase,lipase were measured.The ratio of wet and dry lung tissues was measured.The histopathological changes of lung tissues were observed under optic microscope.The expressions of P38MAPK,p-P38MAPK and NF-κB were measured by using immunohistochemistry method.Results Compared with SAP group,there were no significant differences in levels of serum amylase [12 h (5306.7±909) vs.(5435.0 ±441.2)] and lipase [12 h (1897.8 ±149.4) vs.(1917.9± 106.8)] in HRS group (P >0.05),but there were significant differences in the ratio of wet and dry lung tissues [12 h (3.12 ± 0.58) vs.(1.87 ± 0.25)] and histopathology scores [12 h (2.14 ± 0.38) vs.(3.58 ±0.32)] (P <0.05).There was no significant difference in expression of P38MAPK in lung tissues among three groups at 12 h.Compared with SO group,the expressions of p-P38MAPK and NF-κB were significant increased in SAP group at 12 h,however,they were lower significantly in HRS group than those in SAP group.Conclusions Hydrogen-rich saline has a protective effect on lung injury associated with severe acute pancreatitis,and its mechanism may be likely related to the antioxidant effect and inhibiting the activation of P38MAPK and NF-κB.
10.Effect of Danhong injection on the perfusion function of pancreas in patients with acute pancreatitis

Objective To assess the effects of Danhong injection on the perfusion function of the pancreas in patients with acute pancreatitis (AP). Methods A total of 102 patients with AP were collected, and there were 25 patients with severe acute pancreatitis ( SAP) and 77 patients with mild acute pancreatitis (MAP). They were randomly divided into routine treatment group ( RTG,n =54) and Danhong treatment group (DTG,n =48) , respectively. Another 33 normal individuals were used as controls. Patients in RTG received gastric decompression, acid inhibition, anti-infection and nutritional support. Patients in DTG received Danhong injection 30 ml i. v. b. i. d besides routine treatment. The hemodynamic parameters ( BF, BV, MTT, and PS) of perfusion of the pancreas were measured by 16-slice spiral CT scanner. Results The values of BF of control, MAP, SAP group were time was ( 12.6 ± 2.7) d in DTG, which were significantly shorter than those in routine treatment group [(14.5 ±3.2)d, (18.5 ±5.5)d, P <0.05]. Conclusions The parameters of CT perfusion imaging of the pancreas can be a quantitative criterion to assess the severity of microcirculation dysfunction in patients with AP, and Danhong injection is effective to improve the blood perfusion of the pancreas during the treatment of AP.
