1. Chemical constituents from Spiraea salicifolia

Objective: To investigate the chemical constituents from the ethanol extract of Spiraea salicifolia. Methods: The compounds were isolated and purified by chromatography on silica gel, ODS, and preparative HPLC methods. Their structures were elucidated on the basis of chemical and spectral data. Results: Twelve compounds were isolated and identified as 7S,8R-3,5- dimethoxy-4',7-epoxy-8,5'-neolignan-3',4,9,9'-tetraol (1, salicifoneoliganol), 3β-acetylursolic acid methyl ester (2), 3β-acetyloleanolic acid methyl ester (3), lupeol (4), β-amyrin (5), (7R,8S)-5-methoxydiyhdrodehydroconiferyl alcohol (6), 8-hydroxy-7'-epipinoresinol (7), 8-hydroxypinoresinol (8), fraxiresinol (9), (+)-africannal (10), (+)-lyoniresinol (11), and 5-methoxy-(+)-isolariciresinol (12). Conclusion: Compound 1 is a new lignan named salicifoneoliganol. Compounds 6 and 7 are isolated from the plants of Spiraea L. for the first time, and compounds 4, 5, and 8-12 are firstly obtained from this plant. Compounds 2 and 3 are artifacts of isolation from ursolic acid and loleanolic acid.
2.Comparison of the quantitative evaluation in high-resolution CT and pulmonary function tests in chronic obstructive pulmonary disease

Objective To analyze the correlation between quantitative evaluation of high-resolution CT (HRCT) and pulmonary function tests (PFT) in chronic obstructive pulmonary disease (COPD), and evaluate the characteristics of PFT in different CT phenotypes of COPD. Methods 65 patients with COPD underwent CT chest scan and PFT. The dimensions of subsubsegmental apical bronchi of right upper lobes were measured , which included the following parameters:airway wall thickness (T), total airway diameter (D), and pulmonary artery (PA), ratio of airway wall thickness and pulmonary artery (T/PA), thickness diameter ratio (TDR), percentage of wall area (WA%) and percentage of airway luminal area (Ai%). The percentage of total lung area occupied by low attenuation area (LAA%) was measured by lung density software. The relationship between the measurements and PFT parameters were analyzed. Patients were classified into three phenotypes based on the presence of emphysema or bronchial wall thickening evaluated by HRCT. Phenotype A: without or with little emphysema (LAA% < 25%), with or without bronchial wall thickening; Phenotype E: emphysema without bronchial wall thickening (LAA%≥25%, T/PA < 30%); Phenotype M: emphysema with bronchial wall thickening (LAA%≥25%, T/PA≥30%). The characteristics of different CT phenotypes were analyzed. Results LAA% had a good correlation with PFT parameters. PA, T/PA, WA% and Ai% also had correlation with PFT parameters. There was 53 patients of Phenotype A, 12 patients of Phenotype M. PA, T/PA, LAA%and PFT were different in two groups. Conclusions LAA%and airway quantitative measurements in HRCT have certain correlation with PFT parameters. Patients with different CT phenotypes differs in PFT parameters , which can provide reference for clinical diagnosis and treatment.
3.Preliminary analysis on the treatment of infection caused by pandrug-resistant Acinetobacter baumannii

Objective To analyze the clinical features of pandrug-resistant Acinetobacter baumannii (PDR-Ab) in a hospital and compare the efficacy of different antibiotic treatments on patients with pneumonia caused by PDR-Ab.Methods Data were ret- rospectively collected from all isolated PDR-Ab strains in our hospital from February 2004 to March 2005.The clinical features and outcomes were reviewed.Results A total of 77 strains of PDR-Ab were collected, 45 of which were pathogens causing clini- cal infections (35 strains from lower respiratory tract, 6 from bloodstream, 3 from drainage fluid, and 1 from wounds).Lower respiratory tract was the most common source of PDR-Ab.More than 90% of the isolated PDR-Ab strains produced OXA-23 type?-lactamase.Cefoperazone-sulbactam plus minocyeline showed good efficacy for patients with PDR-Ab pneumonia.The total clinical cure rate was 68.4%.Bacterial eradication rate was 42.1%.The factors influencing bacterial clearance were pro- longed mechanical ventilation prior to positive culture (17.5 d vs 5.5 d).mixed infection (100% vs 12.5%) and lower GCS score (9.1?0.7 vs 13.2?2.1).Concomitant septic shock (OR=13.8) and APACHEⅡscore (OR=2.1) were independent factors of clinical outcome.Conclusions Nosocomial infections caused by PDR-Ab are not untreatable.Our analysis suggests that cefoperazone-sulbactam plus minocycline may be an effective treatment for lower respiratory tract infections caused by PDR-Ab in our hospital.

5.Effects of Xiongshao capsule on blood vessel collagenase gene expression in experimental rabbits with arterial restenosis.

OBJECTIVETo observe the effects of Xiongshao Capsule (XSC) on blood vessel collagenase gene expression in experimental rabbits with arterial restenosis, and to probe its mechanisms for preventing restenosis.
METHODSRestenosis rabbit model was established by injuring endothelium of abdominal aorta by balloon dilation and feeding with high fatty diet for 6 weeks. Eighty rabbits were randomly allocated into 8 groups, Group A, normal rabbit for control; Group B, rabbit with simple injured arterial endothelium; Group C, model rabbits at different times after modeling (3 days for Group C1, 2 weeks for Group C2, and 6 weeks for Group C3); Group D, model rabbit treated with Probucol for 6 weeks; Group E and F, model rabbit treated with small and large dose of XSC respectively. The effect of XSC on collagenase gene expression during the course of restenosis was observed adopting RT-PCR method and computer image analyzer, and its mechanisms in preventing RS were probed by combined analyzing the change of collagen and patho-morphological examination.
RESULTSCompensatory dilation of lumens appeared at the end of the 2nd week; while 6 weeks after modeling, the diameters of lumens obviously diminished with an apparently increased proliferation index. The cell proliferation inhibiting effect in Group D and F was significant. The total amount of collagen increased and reached the peak at the 2nd week but without conspicuous accumulation on intima, which increased gradually and reached its peak at the 6th week. In Group D-F, especially in Group F, the amount of collagen in vascular wall (intima, media and externa) was lesser than that in Groups C. MMP-1 mRNA showed weak expression in Group A and Group C1-C3; significant difference only existed in comparing Group F with C3 (P < 0.05).
CONCLUSIONXSC could markedly increase the MMP-1 mRNA expression in injured portion of vessels, suggesting that its action in preventing RS might be related with the up-regulating of MMP-1 mRNA expression, increasing collagen degradation and reducing collagen deposition in vascular wall.

6.Management of moderate to severe pediatric concealed penis in children by Devine's technique via incision between the penis and scrotum.

OBJECTIVETo search for a simple and effective surgical approach to the management of moderate to severe pediatric concealed penis in children.
METHODSWe used Devine's technique via incision between the penis and scrotum in the treatment of 68 cases of moderate to severe pediatric concealed penis. The patients were aged 3 -13 (mean 6.5) years, 30 with moderate and 38 with severe pediatric concealed penis.
RESULTSThis strategy achieved good near- and long-term effects and satisfactory appearance of the penis, which was similar to that of circumcision. At 3 months after surgery, the penile length was 3 - 5.2 cm, averaging (2.35 +/- 0.35) cm.
CONCLUSIONDevine's technique via incision between the penis and scrotum is a simple and effective surgical option for moderate to severe pediatric concealed penis in children.

7.Effect of xinnao shutong capsule on cardiac muscle cell apoptosis and protein expressions of Bcl-2 and Bax in hyperlipidemia rats after myocardial infarction.

OBJECTIVETo investigate the effect of Xinnao Shutong Capsule, whose main ingredients gross saponins from Tribulus Terrestris L (GSTT) on cardiac muscle cell (CMC) apoptosis and expressions of Bcl-2 Compound rat model of and Bax in murine model of hyperlipemia after myocardial infarction (MI).
METHODSMI and hyperlipemia was adopted. TUNEL assay was applied to detect CMC apoptosis after 4 weeks' administration of GSTT or simvastatin, and immunohistochemical SP technique was used to detect the expressions of Bcl-2 and Bax protein.
RESULTSGSTT can relieve the damage of CMC and attenuate the ventricular remodeling after MI; high dose of GSTT and simvastatin could decrease CMC apoptosis (P<0.05), and lower Bax protein expression (P < 0.05); and there was no significant difference among the effects in all the treated group (P> 0.05).
CONCLUSIONGSTT can reduce CMC apoptosis through regulating protein expressions of Bcl-2 and Bax, which may be one of the mechanisms of its anti-ventricular-remodeling effects after MI.

8.Beard reconstruction by single-hair grafting technique

Objective To search for a good solution for aesthetic beard reconstruction and to explore the effect of the single-hair grafting in beard reconstruction. Methods Under the local anesthesia, a scalp strip (length range from 5~7 cm, width range from 1.0~1.5 cm) was harvested from the back of the head and then was divided into a series of single-hair micrografts. After the slits were made, the grafts were implanted into the holes, parallel to the original beard directions. The donor sites were directly closed. With this method, 18 sparse beard patients and 3 patients with upper lip scars were treated. Results The grafted hairs grew nearly in the direction of normal beard, which achieved 90 %~95 % survival in recipient areas. The patients were satisfied with the results, in which 19 cases received only 1 operation and 2 cases received another repeated operation. Conclusion Beard reconstruction by single-hair grafting technique is a good, safe and effective method.
9.Skin adverse reactions to afatinib and their correlation with anti-lung cancer efficacy

Objective:To investigate the type and severity of skin adverse reactions induced by afatinib in the treatment of lung cancer, and to analyze their correlation with anti-lung cancer efficacy.Methods:A case-case-control study was conducted on lung cancer patients treated with afatinib in ZhongDa Hospital, Southeast University from December 2016 to January 2018. The type and severity of skin adverse reactions were evaluated in 76 patients with lung cancer based on the National Cancer Institute-Common Terminology Criteria for Adverse Events (NCI-CTCAE) Version 4.0, and these patients were divided into 3 groups according to the severity of skin lesions, including grade-0, -1, and -2/3 groups. The patients underwent chest computed tomography (CT) examination every 3 months, and the tumor response to afatinib was evaluated according to the Response Evaluation Criteria in Solid Tumors (RECIST) . Anti-lung cancer efficacy of afatinib was compared among the patients with different grades of skin lesions by using the Kruskal-wallis H rank sum test. Results:After treatment with afatinib, 44 of the 76 patients with lung cancer achieved stable condition or partial remission, and 32 experienced disease progression. Skin adverse reactions occurred in 69 patients, and manifested as acneiform lesions in 42 (55.3%) patients, paronychia in 35 (46.1%) , mucosal erosions in 30 (39%) , hair changes in 8 (10.5%) , and hand-foot syndrome in 6 (7.9%) . Improvement was achieved in 3, 7 and 34 cases in the grade-0, -1, and -2/3 groups ( n = 7, 19 and 50 respectively) , respectively. There was a significant difference in the response rate among the 3 groups ( χ2 = 6.117, P = 0.047) , and the response rate was significantly higher in the grade-1 and -2/3 groups than in the grade-0 group (both P < 0.001) , and higher in the grade-2/3 group than in the grade-1 group ( P < 0.001) . Conclusion:The treatment of lung cancer with afatinib can cause various types of skin lesions, such as acneiform lesions, paronychia, mucosal erosions, hair changes and hand-foot syndrome, and the higher the severity of the skin lesions, the more marked the anti-lung cancer efficacy of afatinib.

Objective To investigate the protective effects of Ulinastatin (UTI) and 1, 6 fructose diphosphate (FDP) on cerebral ischemia-reperfusion injury, and to discuss the protective mechanisms of UT1 and FDP on cerebral ischemia-repeffusion injury. Method Rabbit ischemia-repeffusion injury models were prepared by"four arteries occlusion". All rabbits were divided into four groups (n=6): control group, UTI (10 U/kg) group, FDP (200 mg/kg) group, UTI+ FDP group. Salt water, UTI, FDP and UTI + FDP were respectively used immediately after ischemia-reperfusion. Plasmic MDA and GSH-Px were measured at 30 minutes, 1 hour, 3 hours and 6 hours after reperfusion.
