Objective To promote themedical quality and ensure patient safety by information technology . Methods The computer monitoring system was designed based on the data from electronic medical records , LIS, RIS, PACS and so on, taking time as the main lineand electronic medical record system as platform .Results Thanks to computer monitoring system ,the management of medical quality and patient safety transferred from passive management at end of the process to process active management .Conclusion The computer process control system can significantly improve the level of hospital management , contribute to improve the medical qualityand ensure pa-tient safety.
2.Effects and mechanisms of hyperhomocysteinemia on intimal hyperplasia in balloon-injuried rats carotid arteries

Objective To study the effects and mechanisms of diet-induced hyperhomocysteinemia on intimal hyperplasia of balloon-injured carotid arteries in rats. Methods Wistar rats were bred with 1 g/(kg?d) L-methionine. After 4 weeks, all animals underwent left common carotid arteries balloon denudation and the left common carotid arteries was extracted at day 14 and 28. The areas of carotid arteries intima and media, the proliferative index of smooth muscle cell and the percentage of reendothelization were measured and calculated. Results Intimal hyperplasia developed in all balloon-injuried arteries. The neointimal hyperplasia in the HHCY group was more obvious than that of the control group. The intimal thickness, neointimal area, Intima/Media area ratio were increased by 36%, 41%, 36% at 14 day and 33%, 30%, 21% at 28 day, respectively. Luminal area was decreased markedly in HHCY group. The percentage of intimal and medial PCNA positive cells increased by 1.7-fold and 2.3-fold at 14day. The percentage of endothelial covering area was decreased by 52% at 14day and 31% at 28day in HHCY group. Conclusion Diet induced hyperhomocysteinemia can stimulate SMC proliferation and inhibit endothelial regeneration, which leads to exacerbated intimal hyperplasia. This study implicates that homocysteine may be a significant risk factor for restenosis following vascular intervention.
3.The value of HP combined with serum CEA, CA72-4 and CA24-2 in early gastric cancer screening

Objective To investigate the value of Helicobacter pylori (HP),blood carcinoembryonic antigen (CEA),carbohydrate antigens 72-4 (CA72-4) and CA24-2 in the screening of gastric cancer.Methods The contents of HP,blood CEA,CA72-4 and CA24-2 in 50 healthy controls,45 patients with benign gastric lesions mnd 70 patients with gastric cancer were detected.According to the number of positive cases,the positive rate of single detection and the positive rate of combined detection were calculated.Sensitivity,specificity,and accuracy of serum tumor markers were calculated according to HP single and combined detection.Results The positive rate of each individual detection and 4 joint detection in gastric cancer group was significantly different from that in the benign lesion group and the normal control group (P <0.01).The positive rate of 4 joint detection in gastric cancer group was 89.25%,and the difference was statistically significant (P <0.01).There was no significant difference in the positive rate of the single test and the 4 joint detection in benign stomach group and normal control group (P > 0.05).The sensitivity,specificity and accuracy of HP,blood CEA,CA72-4,CA24-2 joint detection were significantly higher than single detection (P < 0.05).Conclusions Joint detection was significandy better than single detection,and can significantly improve the detection rate of gastric cancer.
4.Clinical and pathological features of colon polyps and risk factors for colon polyps

Objective To investigate the clinical and pathological features of colonic polyp canceration and related risk factors of colon polyps,and explore clinicopathologic features and major risk factors of the canceration of polyps of colon polyps.Methods Totally 80 patients with colon polyps including 6 cases of cancerations and 20 patients with colon cancers were selected from Xuzhou Tumor Hospital Admissions.Those patients were divided into 74 cases of non cancer group and 26 cases of cancer group.The clinical data,the detection index and the statistical analysis were performed.Results The clinical features of patients included 51 cases of multiple polyps,30 cases of fatty liver,13 cases of fecal occult blood positive,and 30 patients with colonoscopy.There were 218 polyps,191 data intact,and 6 cases showed cancer (3.14%).Inflammatory polyps mainly occurred in rectum,cecum,and significant risk other than the colon.Tubular adenoma occurred mainly in the transverse colon.Sigmoid colon cancer colon polyps of the highest risk showed OR =2.30 (P < 0.05).The cancer risk from high to low was from tubular villous adenoma to villous adenoma hair (P <0.01),and no difference in tubular adenoma cancer risk (P >0.05).There was no difference between the indexes of the canceration group and the non cancerous group (P > 0.05).The single factor analysis showed pathological type,and the lesion were the main risk factors of malignant colonic polyps (P < 0.05).Multi-factor regression analysis showed WBC,pathological type,and the lesion were the main risk factors of malignant colonic polyps (P < 0.05).Conclusions The colonic polyps in male have higher incidence with fatty liver,hypertension and other pathological features,and are mainly tubular adenomas,inflammatory and hyperplastic polyps,with different lesions,pathological types occur and different cancer risk.

6.Nasal submicron emulsion of Scutellariae Radix extract preparation technology research based on phase transfer of solute technology.

Based on the demand of nasal drug delivery high drug loadings, using the unique phase transfer of solute, integrating the phospholipid complex preparation and submicron emulsion molding process of Scutellariae Radix extract, the study obtained the preparation of the high drug loadings submicron emulsion of Scutellariae Radix extract. In the study of drug solution dispersion method, the uniformity of drug dispersed as the evaluation index, the traditional mixing method, grinding, homogenate and solute phase transfer technology were investigated, and the solute phase transfer technology was adopted in the last. With the adoption of new technology, the drug loading capacity reached 1.33% (phospholipid complex was 4%). The drug loading capacity was improved significantly. The transfer of solute method and timing were studied as follows,join the oil phase when the volume of phospholipid complex anhydrous ethanol solution remaining 30%, the solute phase transfer was completed with the continued recycling of anhydrous ethanol. After drug dissolved away to oil phase, the preparation technology of colostrum was determined with the evaluation index of emulsion droplet form. The particle size of submicron emulsion, PDI and stability parameters were used as evaluation index, orthogonal methodology were adopted to optimize the submicron emulsion ingredient and main influential factors of high pressure homogenization technology. The optimized preparation technology of Scutellariae Radix extract nasal submicron emulsion is practical and stable.

7.Study on effect of 17-β estradiol on cultured vaginal wall fibroblasts from patient with pelvic floor dysfunction after stretch

Objective To study the effect of 17-β estradiol on Collagen Ⅰ,lysyl oxidase like 1 (LOXL1),fibrinectin( FN )after stretch in the pelvic function dysfunction(PFD) patients vaginal wall fibroblasts cultured in vitro,and the effect in PFD pathogenesis.Methods Twelve patients with pelvic function dysfunction who under surgery in the second Hospital,Hebei Medical University from October 2009 to September 2010 were collected vaginal wall organization in this study.Use of tissue and collagenase digestion method for primary culture in the vaginal wall fibroblasts.After stretch with different concentrations of 17-β estradiol cultured fibroblasts.Total mRNA was extracted from fibroblasts and gene expression of Col Ⅰ,LOXL1,FN were measured.Results After stretch,the expression of Col Ⅰ ( 1.1872 ± 0.0733 vs 1.5035 ±0.0733,t =-4.815,P <0.01),LOXLI(0.7724 ±0.1873 vs 1.0855 ±0.0805,t =-5.111,P <0.01 ) were significant higher than before( P <0.01 ),the expression of FN (0.4290 ±0.1168 vs 0.4215±0.0830) was not significantly different ( P >0.05).The expression of all factor Col [ (3.0809 ±0.1862),LOXLI ( 1.5863 ±0.3241 ),FN( 1.1418 ± 1.0030) in high drug concentrations of 17-β estradiol were increased significant compared with controlled groups( P < 0.01 ).Conclusions 17-β estradiol increased synthesis of extracellular matrix components from cultured vaginal wall fibroblasts of patient with pelvic floor dysfunction after stretch.Stretch and estrogen may play an important role in PFD pathogenesis.
8.Diagnosis and treatment of isolated superior mesenteric artery dissection

Objective To explore the diagnosis and treatment of isolated dissection of the superior mesenteric artery (SMA).Methods From Feb 2006 to July 2010,15 patients with isolated SMA dissection were treated in our center,there were 13 males,2 females,the mean age was(53 ± 8) years (range 43 -63).Among them,1 was caused by trauma,14 had unknown etiology,and 9 cases had a history of hypertension.Diagnosis was made by contrast-enhanced computed tomography (CT) in all cases.Management strategies inlcuded placement of self-expanding bare stent,medical treatment,and transperitoneal SMA fenestration.Results Endovascular stenting was attempted in 14 cases,with a success in 5 and a failure in 9 cases who were then given medical treatment with antiplatelet agents.One case with critical intestinal ischemia underwent open exploration and SMA fenestration.Blood vessel patency resumed.Follow-up with duplex and CT was accomplished in 13 cases,time ranging from 12 to 60 months (mean 28 ± 14mos).There was no recurrent abdominal pain or chronic intestinal ischemia developed during the follow-up.In medically treated patients,there was no aneurismal enlargement of SMA,while in the endovasculartreatmentgroup,allstentsremainedpatentthroughoutthefollow-up.Conclusions Endovascular treatment of isolated dissection of SMA appears to be feasible and effective,despite its relatively low technical success rate.For asymptomatic patients,medical treatment is the treatment of choice.In case of critical intestinal ischemia and with a suspected intestinal gangrene,emergency surgical exploration and fenestration should be performed.

10.Clinical value of tumour markers and serum-ascites albumin gradient in diagnosis of malignant ascites

Objective To investigate the clinical value of tumour markers and serum-ascites albumin gradient (SAAG) in diagnosis of malignant ascites. Methods One hundred and fourteen patients with ascites, who were admitted to the hospital between Jan. 2005 and Jan. 2008, were retrospectively reviewed. The patients were divided into malignant (n = 39) or benign (12 with tuberculosis and 93 with aseptic liver cirrhosis) ascites groups according to the etiology. The distribution of tumor markers (CEA, CA19-9 and CA125) and SAAG in both groups were analyzed and receiver operating characteristic (ROC) was constructed. Results The tumor markers and SAAG were found both in malignant ascites group and benign ascites group. The concentrations of CEA and CA19-9 in serum and ascites were higher in malignant ascites group than in benign ascites group. The SAAG in malignant ascites group was significantly lower than that in patients with liver cirrhosis (P<0.05), but had no difference in comparison with tuberculosis patients (P>0. 05). There was no difference in level of CA125 in serum or ascites between malignant ascites group and benign ascites group (P>0.05). The area under the curve of ascitic fluid CEA, CA19-9 and SAAG were 0.79, 0.82 and 0.85, respectively. The cutoff values of ascitic fluid CEA, CA19-9 and SAAG were optimally chosen at 1.45 μ/L, 19.50 μ/L and 13. 50 g/L, respectively. The sensitivity and specificity were 66.7% and 78.1% in CEA, 74.4% and 84.8% in CA19-9, as well as 82.9% and 84.6% in SAAG.The combination of ascitic fluid CA19-9 with SAAG could increase the specificity to 97.14%, but decrease the sensitivity to 61.54%. Conclusion It is feasible to achieve optimum combination of biochemical indicators using ROC in differential diagnosis of malignant ascites from benign ascites.
