1.Clinical evaluation and ultrasonic diagnosis of mild ventriculomegaly in fetuses

Objective To evaluate the importance of the prenatal diagnosis to fetus with mild ventriculomegaly ( MVM) in pregnancy outcome and prognosis. Methods The ultrosonographic results of 51 pregnant women who were admitted to our hospital were collected in this study. In each case,a thorough evaluation of fetal anatomy was performed,karyotyping or MRI was done if needed,and follow-up was done un-til afterbirth. Results In 51 cases of lateral ventriculomegaly fetal:ultrasonography indicated there were 26 fetuses with mild ventriculomega-ly. The incidence of MVM complicated with other abnormities was 46. 0%,and that complicated with chromosome disorder was 11. 5%. Con-clusion Ultrasonography plays an important role in the detection of MVM. Chromosome analysis should be carried out for elder pregnant women when their fetuses are diagnosed as MVM and other complicating abnormities.
2.Investigation into the current status of the drug resistance of enterococci

Objective To analyze the current status of the drug resistance of enterococci and monitor the prevalence of drug-resistant strains of enterococci in hospital infection. Methods 318 strains of enterococci separated from infectious samples in nine general hospitals in Hubei were assayed and tested with regard to drug sensitivity and the software of WHONET4 was used to analyze the data. Results The separation ratio of ARE, HLARE, VRE and ARE complicated with HLARE was respectively 16.98% , 37.76% , 0 and 5.66% . Conclusion The drug resistance of enterococci shows the tendency of increasing and consequent hospital infection shows the tendency of scattered prevalence. Laboratories of clinical microorganisms ought to closely watch the detection and spread of VRE.
3.Sampling investigation into the problems related to mental health in 1 498 adolescents in some cities and rural areas in Hubei province

BACKGROUND: The incidence of mental problems in adolescents is increasingly higher and has gradually become an important factor affecting their health and growth.OBJECTIVE: To probe into the mental health status and its influencing factors in adolescents in some cities and rural areas in Hubei Province.DESIGN: Clustering stratified random sampling, cross-sectional survey.SETTING: Department of Maternal, Child and Adolescent Health, Tongji Medical College, Huazhong University of Science and Technology.PARTICIPANTS: According to the selected investigation spots of National Surveillance of Constitution and Health of Students, we selected representative urban areas (Wuhan City and Huangshi City) and rural areas (Yun County, Xiantao City, and Xiaogan City) in Hubei Province as sample spots. Totally 1 498 adolescents, aged 16-18 years, were surveyed by clustering stratified random sampling. There were 605 adolescents in urban group and 893 in rural group.METHODS: The self-made survey scale was used to investigate general conditions, such as age, parents' occupation and educational level, family type, social atmosphere, school type and atmosphere, and interpersonal relationship. Symptom Checklist-90 (SCL-90) was used to determine the adolescents' mentalhealth status; 9 subscales (somatization, obsessivecompulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychosis) were set as the observation indexes. SCL-90 is an inventory which is rated on a 5-point scale. Factor score of SCL-90 ≥ 3 or total score ≥ 160 was considered mental disorders. The family educational pattern was investigated through Egma Minnen av Bardndosnaupporstran(EMBU) by questionnaires. It contained positive rearing pattern and negative rearing pattern. Positive rearing pattern (namely, affectional warmth and understanding) contributes to mental health, while negative rearing pattern (e.g. punishment, firm control, excessive interference, partialism or rejection, denial and excessive protection) is adverse to mental health. The family environment was investigated with Family Environment Scale(FES). Four subscales (intimacy, conflict, intellectual-cultural orientation, and positive-recreational orientation) with high reliability and validity were set as indexes. All participants were given unified instructions and required to fill in anonymous questionnaires. The questionnaires were then collected by the teacher in charge of the class.Some indexes of adolescents in the urban and rural areas were observed including the score of SCL-90 and its relation with general conditions,parental rearing pattern and family environment quality. The total scores of symptom checklist 90 were set as dependent variables for stepwise regression analysis of related factors.MAIN OUTCOME MEASURES:①The incidence of mental health ,gender and regional difference of adolescents in some cities and rural areas in Hubei province; ②The score of SCL-90 and its comparison with national norm;③Correlation analysis of the total scores of SCL-90 with general condition and parents' educational type.RESULTS: ① The general incidence of mental disorders was 18.13 % in the 1 498 adolescents in Hubei province. It was 18.20% in boys and 18.04% in girls with no significant difference (P > 0.05). Regional differences: The incidence in rural areas [23.74% (212/893)] was significantly higher than that in urban areas [16.69% (101/605), P < 0.05]. Among all kinds of mental disorders, compulsion, interpersonal sensitivity, hostility,paranoid ideation, and depression were more common. ② Comparison with the national norm: The factor scores of SCL-90 and family environmentalquality were significantly lower that those of the national norm.③ Single factor analysis: The total score of SCL-90 was obviously correlated with age, gender, parents' educational level and expectation, and school atmosphere (P > 0.01). It was negatively correlated with positive rearing pattern (affectional warm, understanding) (P > 0.01), but positively correlated with negative rearing pattern (punishment, firm control, excessive interference,partialism or rejection, and denial). It had significant positive correlation with conflict and significant negative correlation with intimacy, intellectualcultural orientation and positive-recreational orientation(P > 0.01).④Multiple factor analysis: The mental health status of the students was closely related to age, gender, parents' educational level and expectation, interpersonal sensitivity as well as school atmosphere, parental rearing pattern,and quality of family environment.CONCLUSION: The incidence of mental problems in adolescents in some urban and rural areas in Hubei Province is relatively high. Score factors of SCL-90 are all higher than those of the national norm and the mental health level is poor. This has obvious correlation with family factors including age, gender, parents' educational level and expectation, and school atmosphere. Moreover, the quality of family environment and parental rearing pattern are the major factors affecting their mental health.
4.The role of color doppler ultrasonography, thyroid function and auto antibody for the screening of Graves' disease in pregnancy

Objective To determine whether color doppler ultrasonography (CDU),thyroid function or thyroid autoimmune antibodies could identify Graves' disease in pregnancy(GDP) in pregnant patients with newly diagnosed thyrotoxicosis.Methods It is an observational study.Sixty-eight pregnant patients with newly diagnosed thyrotoxicosis including gestational hyperthyroidism (GHT) subjects (GHT group,n =33) and GDP subjects (GDP group,n =35),and 62 age-and sex-matched healthy subjects (C1 group:pregnant,n =32,C2 group:non-pregnant,n =30) were recruited.Thyroid function,human chorionic gonadotropin(HCG),thyroid autoimmune antibodies were detected.Peak systolic velocity of the superior thyroid artery (STA-PSV) and diastole inner diameter(STA-D) of the superior thyroid artery were measured by CDU.A ROC curve was used to evaluate STA-PSV,STA-D,thyroid function and thyroid autoimmune antibodies for identification of GDP.Results The area under the ROC curve of STA-PSV,STA-D and thyroid stimulating hormone (TSH),free T4 (FT4) for GDP were 0.905,0.887,0.803 and 0.786,respectively.The optimal cut-off points of STA-PSV,STA-D,TSH and FT4 for GDP were 40 cm/s,2.0mm,0.03 mIU/L and 30 pmol/L with the sensitivity of 82.9%,72.1%,81.8%,76.2% and specificity of 81.8%,87.9%,75.2%,80.3%,respectively.Conclusions Detection of STA-PSV and STA-D by CDU,as well as thyroid function,is useful in screening GDP in pregnant patients with thyrotoxicosis.
5.Large Calcium-Activated Potassium Channels Contribute to Neuronal Abnormal Firing in the Medial Vestibular Nucleus Following Hypoxia

Objective To investigate the effects of BKCa channel on electrophysiology excitatory regulation in MVN neuron following hypoxia and to reveal its molecular mechanism.Methods C57BL/6 mices were performed MVNs hypoxia mice model,and randomly allowed to normal oxgen group and hypoxia group.The hypoxia group, according to the application of NS1 6 1 9 ,was further divided into the no NS1 6 1 9 pretreatment group and NS1 6 1 9 pre-treatment group.Using the patch clamp experiment technology,we recorded the effects of the MVN abnormal neu-ronal firing and the change of the BKCa currents.Using immunohistochemical technique,the changes of BKCa in the hypoxic MVNs detected were.Results Acute hypoxia increased neuronal activities.NS1619 pretreatment de-creased hypoxia-induced firing rate,and increased and postponed the maximum increase by hypoxia(P<0.05),al-so alleviated 10-min-hypoxia-induced depolarization(P<0.05).Perfusion with hypoxic significantly reduced the BKCa positive neurons(P<0.05).Conclusion These findings suggest that acute hypoxia increases neuronal activi-ties.The decreased MVN BKCa channels contribute to hypoxia-induced abnormal neuronal activities.
6.Equol reverses the inhibition of cyclosporin A on the proliferation and osteoblastic differentiation of bone marrow mesenchymal stem cells through estrogen receptor/nitric oxide/cyclic guanosine monophosphate signal pathway

BACKGROUND: Long term taking cyclosporin A(CsA)can inhibit osteoblastic differentiation and induce osteoporosis.Equol,which has greater binding affinity to estrogen receptors,can stimulate the proliferation and osteoblastic differentiation.OBJECTIVE: To investigate whether equol may protect against the proliferation and osteoblastic differentiation inhibited by CsA in mouse bone marrow mesenchymal stem cells(BMSCs)cultures and analyze signal pathway of protection.METHODS: Primary mouse BMSCs were cultured by using attachment method and assigned to five groups,which respectively treated with equol or/and CsA in the presence or absence of ICI182780,an estrogen receptor antagonist,and Nω-nitro-L-arginine methyl ester.Under an inverted microscope,morphological changes and mineralization ability of BMSCs were observed.The cell proliferation was measured by[3H]-thymidine incorporation.The osteoblastic differentiation and mineralization of extracellular matrix in BMSCs was assessed by measuring alkaline phosphatase activity and calcium deposition,respectively.Nitric oxide production in the conditioned media and cyclic guanosine monophosphata(cGMP)content in BMSCs were determined by using commercial nitric oxide and cGMP kit,respectively.RESULTS AND CONCLUSION: Equol reversed the decreased[3H]thymidine incorporation(P < 0.05),alkaline phosphatase activity(P < 0.05)and calcium deposition(P < 0.01)of CsA,which was accompanied with the changes of nitric oxide production(P < 0.01)and cGMP content(P < 0.01).The group by co-treatment with equol and CsA possessed higher cells growth density and small mineralized nodes than CsA group on day 12 under an inverted microscope.Moreover,the equol-reversed effect was abolished by ICI182780 and Nω-nitro-L-arginine methyl ester.These indicated that equol can reverse the inhibition of CsA on the proliferation and osteoblastic differentiation of mouse BMSCs through estrogen receptor/nitric oxide/cGMP signal pathway.

Objective To explore the efficacy of calmodulin antagonist berbamine(BBM)on multidrug resistance(MDR)reversal and its mechanism. Methods Human breast cancer cell line MCF7 and its adriamycin-resistant counterpart MCF7/ADR were used in the study.The cells were cultured with ADR and different concentration of BBM. MTT assay was used to analyze the effect of BBM on cell growth inhibition.According to the MTT assay,the 50% inhibitory concentration(IC 50 ),the multiples of drug resistance and increased sensitivity of ADR were calculated.The concentration of intracellular ADR and expression level of P-glycoprotein(P-gp)were detected by flowcytometry(FCM).The mRNA expression level of mdr1 gene was detected by semi-quantitative reverse transcriptase polymerase chain reaction(RT-PCR)with ?-actin as internal reference. Results The IC 50 of ADR in MCF7 and MCF7/ADR cells were(0.98?0.06)?mol/L and(101.20?5.72)?mol/L,respectively.The resistant multiple of MCF/ADR cells to ADR was 103 folds higher than that of MCF7 cells.BBM increased the chemo-sensitivity of ADR in MCF7/ADR cells with dose-dependent relationship,i.e.when 5*!?mol/L ,10*!?mol/L and 20*!?mol/L BBM was added into the culture the chemo-sensitivity of ADR was increased to 2.76,5.88,and 28.26 folds(P
9.Laser confocal corneal microscopy: A new diagnostic option for fungal keratitis

Objective: Fungal keratitis is one of the most common diseases leading to blindness.It has a high incidence in China,and its early diagnosis and treatment are of vital importance.We aimed to investigate the value of laser confocal corneal microscopy in the clinical diagnosis of fungal keratitis.Methods: We conducted laser confocal corneal microscopy on 37 patients suspected of fungal keratitis,and compared the results with those obtained from the corneal tissue smears and fungal cultivation.Results: The fungus-infected corneas histologically exhibited defected epithelial cells,damaged subepithelial nerves and disordered stroma with reduced transparence.The hyphae showed bright,disordered,interlaced thread-or short rod-like structures.The positive rates of smear examination,fungus cultivation and laser confocal corneal microscopy were 48.65,62.16 and 78.39% for the 37 eyes,75.00,87.50 and100% for the 16 untreated eyes,and 28.57,42.85 and 61.90% for the other 21 pretreated eyes.Conclusion: Confocal laser corneal microscopy,rapid,effective and noninvasive,can be used as an important new alternative in the diagnosis of fungal keratitis
10.The effect of vitrectomy,endolaser photocoagulation and silicone oil injection for acute retinal necrosis syndrome

Objective:Acute retinal necrosis syndrome(ARNS) is a rare disease with high blinding rate.The surgery of vitrectomy,endolaser photocoagulation and silicone oil injection is considered an effective treatment of ARNS.The study investigated the effect of vitrectomy,endolaser photocoagulation and silicone oil injection treating ARNS.Methods:A consecutive 16 eyes of 16 patients with ARNS were treated with vitrectomy,endolaser photocoagulation and silicone oil injection.The 5 eyes without retinal detachment were photocoagulated from the posterior border of necrosis to the ora serrata.The 11 eyes with retinal detachment were treated with subtotal retinal photocoagulation,air/fluid exchange and silicone oil injection.The follow-up time were 6 months at least,16 months in average.Results:The final best-correcting visual acuity was counting fingers in 3 eyes(18.8%),0.02-0.3 in 7 eyes(43.8%),0.4-0.6 in 5 eyes(31.3%),0.7 in 1 eye(6.3%).The silicone oil was uneventfully removed in 14 patients(87.5%) from 3 to 12 months.No one had a recurrent retinal detachment.Conclusion:There is a favorable effect in ARNS with the treatment of vitrectomy,endolaser photocoagulation and silicone oil injection.
