Carvedilol is a non-selective beta-blocker with additional blockade of alpha_(1)-adrenoceptors.It has been applied in the treatment of hypertention、stable angina pectoris、chronic heart failure.In addition,Carvedilol exerts antioxidant and anti-apoptosis effects,it can inhibits vascular smooth muscle cell proliferation、migration and neointimal formation following vascular injury.The article makes a review on the molecular cytologic mechanism of Carvedilol in these aspects.
2.Inhibitory effect of electro-acunpunture at Zusanli point on flammatory factors of postoperative intra-abdominal adhesions

Objective To observe the inhibitive effect of electro-acupuncture (EA)at Zusanli points (ST36)on inflammatory mediators of postoperative intra-abdominal adhesions and study the relationship between EA and cholinergic anti-inflammatory pathway.Methods Forty-eight male Wistar rats were divided into 6 groups (each =8):Group A (control),Group B(abdominal adhesions model),Group C (abdominal adhesions plus EA),Group D(sham acu-point control),Group E (abdominal adhesions plus α-bungarotoxin )and Group F (abdominal adhesions plus EA after α-bungarotoxin).Animal models of abdominal adhesion were produced by Chiang’s path.Bilateral Zusanli points (ST36) and shame acupoints were electro-acupunctured at a constant voltage for 1 hour while rats were awake.The ɑ-BGT(1 μg/kg)was injected into the abdominal cavity after surgery.All the rats were sacrificed on the 3rd day,and the levels of inflammatory mediators (TNF-ɑ,NO and NOS)in tissues were evaluated.Results Three days after surgery,the damaged cecum of abdominal adhesion groups developed obvious edema that did not adhere with other tissues.Compared with sham control,the abdominal adhesion resulted in significant elevation of inflammatory mediators (TNF-ɑ,NO and NOS).EA at Zusanli points obviously lowered the elevated levels of inflammatory mediators (P <0.01 and P <0.05).EA at Zusanli points following the injection of ɑ-BGT showed less anti-inflammatory effect(P <0.01).Conclusion EA at Zusanli points significantly lowers the elevated levels of inflammatory mediators after abdominal adhesion challenge.The activation of cholinergic anti-inflammatory pathway might be one of the mechanisms by which Zusanli points exert anti-inflammatory effects.
3.Antisense oligonucleotide mediated inhibition on telomerase activity in gallbladder carcinoma cell

Objective:To study the antisense oligonucleotide mediated inhibition on telomerase activity and cell proliferation of GBC-SD cell.Methods:We design the antisense,sense,and random oligonucleotide with phosphoric acid modification for the hTR(Human Telomerase RNA)template sequence.MTT and PCR methods were used to observe the inhibition on telomerase activity and cell proliferation of GBC-SD cell ,and fibroblast cells were used as control group.Results:PS-ODN can lead to the reduction of cell survival rate of GBC-SD cell,wich dosage dependence.Tne experimental group cell detected by scanning electron appeared apoptotic feature.Conclusion:PS-ODN can inhibit telomerase activity of GBC-SD cell effectively and induce the cell apoptosis.
4.Clinical and electroencephalographic study of idiopathic nocturnal frontal lobe epllepsy

Objective To analyze the characteristics,electroencephalographic(EEG)features,neuroimaging findings,therapeutic effects and prognoses of 114 cases of idiopathic nocturnal frontal lobe epilepsy (NFLE). Methods The clinical data of 114 patients with idiopathic NHLE admitted in the epilepsy outpatient department from June,1999 to January,2007 were collected and analyzed. Results Idiopathic NFLE was clinically characterized by nocturnal clustered,postural or dystonic seizures and complex motor activities.The interictal routine EEG in 22.9% and dynamic EEG in 28% of the cases in wakefuIness and the interictal dynamic EEG in 38% of tbe cases in sleep showed frontal lobe epileptiform discharge,which was found in the EEG during seizure attacks in66.7% of the cases.Drug therapy was effeclive in 79.8% of the cases.with 29.7% of the cases showing complete seizure control. Conclusions Idiopathic NFLE has distinct clinical features wim relatively low rate of EEG abnormality during seizure attack or the interictal period.Frontal lobe epilepsy is more likely to occur in sleep,and sleep EEG is therefore of much importance in the diagnosis of idiopathic NFLE.
5.Targeted inhibition of vascular endothelial growth factor (VEGF) expression in human osteosarcoma cell line by antisense VEGF165 cDNA promoted by hypoxia reaction element.

OBJECTIVEUtilizing the hypoxia inducible factor 1/hypoxia reaction element (HIF-1/ HRE) gene regulation system to construct antisense vascular endothelial growth factor (VEGF165) cDNA eukaryotic expression vector promoted by HRE, and investigate its targeted inhibiting VEGF expression of osteosarcoma cells in hypoxia environment.
METHODSEukaryotic expression plasmid with HRE promoter was constructed containing luciferase reporter gene and antisense VEGF165 cDNA by using PCR and recombinant DNA techniques. The recombinant vectors were transfected into osteosarcoma cells with lipofectin method. Hypoxia-inducible reporter gene expression was determined by liquid scintillation analyzer and the expression of VEGF protein was detected by ELISA method.
RESULTSThe eukaryotic expression plasmid containing antisense VEGF165 and luciferase promoted by HRE was constructed successfully. After being transferred into MG63 cells, luciferase expression was increased 3.5 x 10(2) times and VEGF protein expression decreased 45% under hypoxia condition.
CONCLUSIONAntisense VEGF165 cDNA expression, efficiently realized by HRE promoter under hypoxia condition, provides an experimental basis for targeted antiangiogenesis of tumors.

6.Clinical Characteristics and Obstetric Outcomes of Pregnant Women Who Underwent Surgery for Adnexal Torsion

ObjectiveTo explore the clinical characteristics and obstetric outcomes of pregnant women who underwent surgery for adnexal torsion at different gestational weeks. MethodsA retrospective study was done on 39 women who underwent surgery for adnexal torsion during pregnancy in the First Affiliated Hospital, Sun Yat-sen University between March 2013 and March 2023, with 18 cases in 1st trimester (<14 weeks), 11 in 2nd trimester (14-27+6 weeks) and 10 in 3rd trimester (≥28 weeks). The clinical characteristics, treatment and obstetric outcomes were compared among the three groups. ResultsThe 1st trimester group had higher proportion of assisted reproductive technology (ART) use than the 2nd and 3rd trimester groups (P=0.026). There was no significant difference in the clinical manifestations, including abdominal pain, nausea, vomiting and fever among the three groups, while elevated white blood cells (WBC) counts was more commonly seen in the 2nd and 3rd trimester groups. Adnexal masses <5 cm in diameter occurred in 0, 18.2%, and 10.0% of cases in 1st, 2nd and 3rd trimester groups respectively (P=0.014). No statistical significance was found in the location of twisted adnexa, number of circles or pathological nature. The 1st trimester group had a higher sensitivity of ultrasound in the diagnosis of adnexal torsion compared with the 2nd and 3rd trimester groups (77.8%, 36.4%, 20.0%; P=0.008). More laparoscopic surgery were performed in the 1st trimester group than the other two groups (55.6% , 27.3%, 0.0%; P=0.008). There was no significant difference in gestational week of delivery, delivery mode, newborn gender, neonatal birth weight and follow-up of newborns among the three groups. The 3rd trimester group showed a higher risk of preterm delivery (P=0.050). ConclusionsDuring the 1st trimester of pregnancy, adnexal torsion is more common in patients using ART and ultrasound plays a crucial role in the diagnosis. During the 2nd and 3rd trimester, adnexal torsion should be suspected in patients with abdominal pain and elevated WBC but no aspetic inflammation. Laparoscopic surgery is safe for adnexal torsion during pregnancy and can achieve a favorable maternal and neonatal outcome.
7.The effect of pre-operative chemotherapy on calpain in cells of gastrointestinal malignant tumors.

OBJECTIVEPrimary to search the rule of pre-operative chemotherapy and suitable duration for it by investigating the changes of calpain content and activity after 5'-deoxy-5-fluorouridine (5'-DFUR) per oral administered pre-operatively in different time. Further to investigate the mechanism of chemotherapy.
METHODSSeventy-three patients with gastrointestinal malignant tumors were divided into 4 groups by the time of 5'-DFUR (600 approximately 1200 mg/d) by oral administration before operation, group A, 3 days, 27 cases; group B, 1 week, 22 cases; group C, 2 weeks, 15 cases; group D, 2 months, 9 cases. And group E, control group, had 24 patients with gastrointestinal malignant tumors at the same term. The patients above all had not received the other chemotherapy and radiotherapy. Western blot and immunoelectron microscopy were employed to detect the expressing levels and activities of calpain in tumor tissues of different groups.
RESULTSElectronic microscopic examination showed gold granula mainly on the membranes of mitochondria of tumor cells to groups after chemotherapy. And the tumor cells of group A were mildly damaged. Besides that, serious injury for tumor cells of group B could be seen, and the phenomena were common in group C. But the damages to tumor cells of group D were mainly about mildness. The results of immunoelectron microscopy revealed that the contents of calpain increased following the time of chemotherapy prolonging, and peaked in group C. Still more, there was no significant difference for the results between group C and group D. The changes of calpain activities observed by western blot had the same tendency as the results from immunoelectron microscopy (r = 0.86, P < 0.0001).
CONCLUSIONS5'-DFUR via oral administered pre-operation could have anti-cancer effect through calpain. And the effect might be strongest in 2 weeks also after chemotherapy.

8.Evaluation of jaws-only intensity-modulated radiation therapy for nasopharyngeal carcinoma.

OBJECTIVETo investigate the feasibility and efficiency of jaws-only intensity-modulated radiation therapy (JO-IMRT) in treatment of nasopharyngeal carcinoma (NPC) using direct aperture optimization (DAO) technique and the independent jaws of linear accelerator.
METHODSBoth JO-IMRT and MLC-IMRT were planed in 10 NPC cases. The differences in the target coverage and dose uniformity, as well as the total monitor units and delivery times of the two IMRT plans were compared.
RESULTSAll the tested plans met the clinical requirement of the designed simplified IMRT (sIMRT). The conformal index (CI) of JO-IMRT and MLC-IMRT were 0.941±0.015 and 0.981±0.013, respectively (P<0.001), showing a minor superiority of MLC-IMRT. While controlling the total segment numbers to approach the limitation of sIMRT, the two therapies showed a total MU of 474.3 and 419.6 (P<0.05) with delivery times of 8.0 and 7.5 min (P<0.01), respectively. The efficiency of JO-IMRT was slightly lower than that of MLC-IMRT.
CONCLUSIONJO-IMRT can meet the sIMRT requirement in NPC treatment, and is feasible as an alternative treatment modality for the centers not equipped with MLC in their accelerators.

9.The preparation of the polylacticacid nanoparticles of cucurbitacin and their drug loading.

This paper introduced an experimental study of the preparation of polylacticacid (PLA) nanoparticles of cucurbitacin (CuC) using a precipitation method. The residual acetone, ratio of CuC PLA precipitates, and the relationships between the ratios of two precipitates and drug incorporation rates were measured. It appeared that the nanoparticles with 60% of PLA incorporated with 5.5% of CuC were formed when acetone was injected into the aqueous phase. As the acetone gradually evaporated, drug incorporation/encapsulation continued, with most of CuC (about 70%) formed new crystalline cores and suspended in the form of microcrystals in the medium, resulting a suspension containing both nanoparticles and microcrystals. We also concluded that this system may not necessarily be suitable for all lipophilic drugs to be prepared to PLA nanoparticles with good incorporation rate. The drug incorporation depended on the interactions among drug, PLA, and organic solvents, in addition to the solubility of the drug.

10.Effects of angiotensin converting enzyme inhibitors and angiotensin II receptor blockers on angiogenesis of gastric cancer in a nude mouse model.

OBJECTIVETo observe the effects of angiotensin converting enzyme inhibitors (ACEI) and angiotensin II receptor blocker (ARB) on tumor growth and angiogenesis in implanted gastric cancer mouse model, and to explore the probable mechanism of ACEI and ARB anticancer effect.
METHODSNude mouse model with human gastric cancer was established by subcutaneously inoculating human gastric cancer cell line SGC-7901. One week later, 60 mice were randomly divided into 5 groups: control group, perindopril group, captopril group, losartan group, and valsartan group. These groups respectively received the normal saline, perindopril (2 mg/kg), captopril (5 mg/kg), losartan (50 mg/kg), valsartan (40 mg/kg) by gavage once a day. Twenty-one days after treatment the tumors were removed and the tissues were stained by immunohistochemistry method to observe the expression of VEGF, MMP-7 and microvessel density (MVD).
RESULTSIn all the ACEI and ARB groups, tumor volumes were significantly inhibited and MVD also decreased significantly as compared with control group (all P<0.01). In captopril and perindopril groups, the expression of VEGF and MMP-7 decreased significantly as compared with control group(all P<0.05). In losartan and valsartan group, the expressions of VEGF were significantly decreased as compared with control group (all P<0.05). The expressions of MMP-7 between ARB groups and control group were not significantly different.
CONCLUSIONACEI and ARB can inhibit the tumor growth in gastric cancer model and suppress the angiogenesis of the tumor.
