1.Relation of negative emotions to erroneous gambling cognition and gambling addiction in college students

Objective:To explore the relation of negative emotions to erroneous gambling cognition and gambling addiction in college students.Methods:Totally 357 college students in Macau were selected and assessed with the Depression Anxiety Stress Scales (DASS),Gambling Related Cognition Scale (GRCS) and South Oaks Gambling Screen (SOGS).Bootstrap was used to test the mediating effect.Results:The students with higher erroneous gambling cognition got higher scores on negative emotions and addiction gambling than low-score group with erroneous gambling cognition (Ps <0.01).Mediating effect test showed that the direct effect of negative emotions on gambling addiction was 0.14,and the indirect effect was 0.05 (Ps <0.05).The indirect effect accounted for the total effect 26.3%.Conclusion:It suggests that negative emotions not only have direct effect on gambling addiction,but also have indirect effect on gambling addiction through erroneous gambling related cognition.
2.Value of bone marrow biopsy histopathological finding in diagnosing myelodysplastic syndrome

Objective To evaluate the value of plastic-embedded bone marrow (BM ) biopsy sections in diagnosing myelodysplas-tic syndrome(MDS) .Methods HGF and Gomori stained .Glycol methacrylate(GMA) embedded BM biopsy specimens were detec-ted from 125 MDS patients with 25 cases in good health as control .Dyshematopoiesis and abnormal stroma cell responses were se-lected as main observed projects .Results Bone marrow biopsy sections gave an exact assessment of cellularity ,19 cases (15 .2% ) showed a decrease of cellularity as hypocellular MDS .66 cases(52 .8% ) of these cases did show abnormal localization of erythro-blastic clusters ,biopsy of 64(62% ) of low risk group 103 cases did show abnormal localization of immature precursors such as blasts and promyelocytes(ALIP positive) .Numeration of megakaryocytes and mast cells within 1mm2 acre of BM tissue were all much in MDS group than in control group .Gomori′s stain revealed moderate increase in reticulin fibres in 65 cases ,10 cases(8 .0% ) showed degree of myelofibrosis were + + + as hyperfibrotic MDS .Among 125 cases ,33 cases(26 .4% ) of bone marrow biopsies showed different types of erythrocytic and granulocytic immature cells with unusual intrasinusoidal bone marrow infiltration .Con-clusion It is clear that GMA embedded BM biopsy section is helpful in making diagnosis of MDS ,and has important clinical appli-cation value .
3.Stady on the clinical characteristics of 10 patients with idiopathic cytopenia of undetermined significance

Objective To analyze the clinical and pathologic features in patients with ldiopathic cytopenia of undetermined signif-icance (ICUS) ,and provide the diagnostic basis .Methods 10 ICUS patients who did not fulfil minimal diagnostic criteria for MDS but suffered from constant (>6 months) progressive cytopenia ,with no marked dysplasia (<10% in three-major cell lineages) and abnormal karyotype were reported .Results 2 of 10 patients transited to MDS ,bone marrow review showed that ≥2 cells appeared diagnostic pathology development .Karyotype analysis showed that 1 case was still normal ,1 case lost a normal chromosome 20 (20q-) ,the rest 8 cases had been efollowed up for 6 to 25 months and still maintain ICUS .Conclusion Patients with MDS who fail to meet the diagnostic criteria of ICUS ,must carry out regular follow-up .There are some patients can transition to public MDS after a certain incubation period ,in fact it is Pre-MDS or MDS-early before ,which shows that ICUS should cause clinical attention ,so that avoid missed diagnosis .
4.Factors influencing prognosis of 90 polymyositis and dermatomyositis patients

Objective To study the prognosis of polymyositis(PM) and dermatomyositis(DM) patients and its influence factors.Methods Sixty DM and 30 PM patients diagnosed according to the diagnostic criteria of Bohan and Peter in our hospital during 2000-2008 were used as study subjects.The patients were followed up till their death or to August 2009.Gender,age of disease onset,disease course,serum creatine kinase,interstitial lung disease,heart involvement,connective tissue disease(CTD),malignancies,and treatment with corticosteroids,immunoglobulin and immunosuppressive agents were assessed as predictive factors for the prognosis of patients.Characteristics of muscular biopsy from 20 cases were analyzed.Results The median age of the 90 patients(29 males and 61 females) was 51 years(range 6-74 years).The male and female ratio was 1∶2.The most commonly involved muscles were the proximal muscles of limbs(83.3%),followed by neck muscles(25.6%),laryngea pharyngeal muscles(12.2%) and masticatory muscles(2.2%).Among the 42 patients(46.7%)with lung disease,interstitial lung disease and hear involvement were found in 29(32.2%)and 13(14.4%)patients,respectively.Of the 13 patients complicated by connective tissue disease,DM and PM accompanying connective tissue disease were diagnosed in 9 and 4,respectively,and DM and PM accompanying malignancies were observed in 2 patients.Muscular or skin biopsy was performed for 23 patients,which showed typical inflammatory infiltration in 13,dermatomyositis in 3,and no significant lesion in 2 patients,respectively.Of the 18 patients who died during the follow-up,5(16.67%) and 13(21.67%) died of PM and DM,respectively.Seventy-two patients survived.Their 1-,5-,and 9-year survival rate was 90%,84.4%,and 80%,respectively.The complete and partial remission rate was 22.2% and 36.7%,respectively,with a relapse rate of 20%.Advanced age of disease onset(P=0.003 8),interstitial lung disease(P=0.011 3) and malignancies(P=0.004 9) were main causes of death.Malignancy(RR=6.34,P=0.001 2)was the factor for poor prognosis and long-term treatment with hormones and immunosuppressive agents is the protective factor for PM and DM.Conclusion Complete and partial remission can be achieved in 58.9% patients with DM and PM.Advanced age of disease onset,interstitial lung disease and malignancy are the factors for poor prognosis of such patients.Long-term treatment with corticosteroids and immunosuppressive agents are the protective factors.
5.Expression of myositis specific autoantibodies in polymyositis/dematomyositis and other neuromuscular diseases

Objective To investigate sensitivity and specificity of myositis specific autoantibodies (MSAs) in polymyositis/dematomyositis(PM/DM ) and other neuromuscular disease. Methods Taking serum from 63 patients with PM/DM (PM/DM group)and 60 definite neuromusclar disease(non-myositis) patients(control group). All the sera were detected for two kind autoantibodies:anti-Jo-1,anti-SRP by immunoblotting method. Calculating sensitivity,specificity of anti-Jo-1 and anti-SRP autoantibodies for the diagnosis PM/DM. Results Positive ratio for anti-Jo-1 and anti-SRP autoantidodies were 17% and 5% respectively in the PM/DM group, while none of the sera from control group detected positive.The specificity of both autoantibodies diagnosis for PM/DM were 100% and (95% CI:94%~100%).The sensitivity was 22%( 95%CI:13%~34%). Conclusion Anti-Jo-1 and anti-SRP autoantibodies are highly specific to PM/DM diagnosis.
6.Effects of large dose gamma globulin on cardiac function and immune regulation in patients with dilated cardiomyopathy and heart failure

Objective: To explore effects of large dose gamma globulin on cardiac function and immune regulation in patients with dilated cardiomyopathy (DCM) and heart failure (HF).Methods: According to random number table, a total of 90 DCM + HF patients were divided into routine treatment group (n=45) and gamma globulin group (n=45, received gamma globulin based on routine treatment, 400 mg kg-1 d-1 intravenous drip for 5d).After three months, therapeutic effect, cardiac function, levels of cardiac troponin T (cTnT), cTnI, N terminal pro brain natriuretic peptide (NT-proBNP) and anti-cardiac myosin heavy chain auto-antibodies (AMHCA) IgG titer before and after treatment, incidence of adverse reactions were observed and compared between two groups.Results: After three-month treatment, total effective rate of gamma globulin group was significantly higher than that of routine treatment group (80.00% vs.57.50%, P=0.02);compared with routine treatment group after treatment, there were significant reductions in levels of cTnT [(0.54±0.18) μg/L vs.(0.31±0.12) μg/L], cTnI [(1.35±0.18) μg/L vs.(0.72±0.13) μg/L], NT-proBNP [(758.34±64.28) pg/ml vs.(708.34±60.22) pg/ml], and significant rise in left ventricular ejection fraction [LVEF, (39.35±6.23)% vs.(47.32±6.10)%] in gamma globulin group, P<0.05 all;compared with before treatment and routine treatment group after treatment, there was significant reduction in AMHCA IgG titer [(0.099±0.047), (0.092±0.045) vs.(0.069±0.032), P<0.05 both] in gamma globulin group;incidence rate of adverse reaction in gamma globulin group was significantly higher than that of routine treatment group (22.22% vs.6.67%, P=0.04), but the symptoms were mild, and they recovered after symptomatic treatment.Conclusion: Large dose gamma globulin can significantly improve cardiac function and possesses good immune regulation effect in patients with dilated cardiomyopathy and heart failure.
8.Super-thin coverage of MEDPOR ear framework:an animal study

Objective It would make a reconstructed ear more naturally when deep temporal fascia, which is much thinner, cover the MEDPOR ear framework instead of superficial temporal fascia. However, without an axial vascular supply, should skingraft be taken on the islanded deep temporal fascia flap? Perhaps this fascia flap could get a new blood supply from the underlying superficial temporal fascia through the porous framework after a period of time. This study was to test its possibility. Methods A piece of 30?15?2mm MEDPOR material was inserted between superficial and deep fascia on the back of a rabbit. In control group, skin graft on the deep surface of deep fascia was operated at once; in experiment group, the incision was sutured and the skin graft was operated 3 weeks later in the same incision. Results Skin grafts were taken in all the 6 rabbits in the experiment group. 80%~100% of necrosis occurred in all the 6 rabbits of control group. Conclusion Without an axial vascular supply, skin graft can not survive on the islanded deep fascia flap. This fascia flap can get a new blood supply from the underlying superficial fascia through the porous MEDPOR material, through which small vessels grow between the two fascia after 3 weeks.

Sphincter of Oddi dysfunction(SOD) is grouped under the functional gastrointestinal diseases.Due to lack of typical symptoms and signs,the diagnosis of SOD is difficult.The proposal to classify SOD significantly improves diagnosis and has important clinical significance in selecting treatment.This paper reviews the researches on classification,recent progress in diagnosis and treatment of SOD.
10.The study and application of the three dimensional anatomical atlas of temporal bone

Objective Celloidin sections of temporal bone were used to develop a serial stereo anatomical atlas of temporal bone by computer aided three dimensional(3-D) reconstruction and stereology. Methods Fifty sets of serial celloidin sections of temporal bone with reference points were prepared and 3-D morphology of structures in temporal bone was restored systemically by the technique of computer aided 3-D reconstruction. A system of stereo anatomical atlas of temporal bone with stereoscope was established. Results Totally 48 cases of reconstruction were accomplished for bony and membranous labyrinth, ossicles, tendons, facial nerve and its canal, round window membrane and niche, posterior ampullary nerve, endolymphatic sac, cochlear aqueduct, VII and VIIIth cranial nerve. The stereo picture pairs of these structures with the best representation were selected as the stereo anatomical atlas of temporal bone which showed the fine spatial morphology and relationship of the above structures. The stereo anatomical atlas was applied to guide ear surgery and to assist anatomic training of temporal bone. Conclusion The stereo anatomic atlas of temporal bone is an innovative powerful anatomic tool and has tremendous application futures in otology, or even in the whole medical science.
