2.Monitoring on Irrational Use of Drugs in the Inpatients in Our Hospital by MTP Intervention

OBJECTIVE: To verify the monitoring effects on irrational physicians'orders by MTP intervention in our hospital.METHODS: The irrational physicians' orders in our hospital between 2005 and 2007 were monitored by PASS using MTP(Monitoring-Training-Plan) intervention model.RESULTS: As compared with before intervention,the black-lamp-precaution-medication's revision rate increased significantly after intervention,which greatly reduced the incidence of potential adverse drug events.CONCLUSION: MTP intervention can arouse physicians' attention on the monitoring results with PASS,increase the black-lamp-precaution-medication revision rate and prevent the occurrence of medication errors.
3.Long-term efficacy of calcitonin and bisphosphonates on renal osteopathy in maintenance hemodialysis patients

Objective To observe the efficacy of ealcitonin and bisphosphonates on renal osteopathy of maintenance hemodialysis (MHD)patients. Methods Forty-three MHD patients were raindomly divided into two groups: A group and B group. All the patients were routinely received oral calcium carbonate 1.0 g tid and calcitriol 0.25 μg qd. Calcitonin (20U) hypodermic injection was given three times a week additionally during hemodialysis in A group. Patients in B group received bisphosphonates 70 mg once a week based on the therapy of A group. Serum levels of intact parathyroid hormone (iPTH), calcium, phosphorus, alkaline phosphatase (AKP), bone mass density (BMD) of lumbar spine and femoral neck, and the degree of bone ache (visual analogue scale, VAS) were assessed before the therapy and 3, 6 and 12 months after treatment. The adverse reactions were recorded during treatment. Results The levels of AKP and iPTH in both two groups decreased significantly after treatment. The above values of pre-treatment and 12 months after treatment were as follows: AKP(U/L)of A group 244.05±41.99 and 148.35±27.71,of B group 245.60±40.86 and 143.40±28.03;PTH(ng/L) of A group 697.5±119.7 and 267.4±45.9,of B group 708.2±120.3 and 277.6±41.9 (all P<0.05). While the levels of calcium and phosphorus did not change obviously during treatment (P>0.05). BMD was not improved at 3, 6 mouths and became better at 12 mouths after treatment. As compared to pre-treatment, BMD of lumbar spine(g/cm2) in A group was 1.062±0.223 vs 1.202±0.251 ,in B group 1.033±0.152 vs 1.189±0.225; BMD of femoral neck (g/cm2)in A group was 0.993±0.108 vs 1.067±0.095,in B group 0.947±0.083 vs 1.018 ±0.217 (all P<0.05). The scores of VAS also decreased significantly at 3, 6, 12 months after treatment(P<0.05). No severe adverse reaction was found during the treatment. Conclusions Utilization of calcitonin and combination with bisphosphonates during bemodialysis can effectively preserve the BMD and prevent bone loss in MHD patients and is well tolerated. No significant difference of therapeutic effect is observed between using ealcitonin or combination with bisphosphonates.

5.THE CLINICAL SIGNIFICANCE OF ECHOCARDIOGRAPHIC DIAGNOSIS OF AORTA-SEPTAL ANGLE VARIANCE:AN ANALYSIS OF 307 CASES

An statistical analysis of aorta-septal angle(AS angle) between anterior wall of aorta and ventricular septum was taken,AS angle was measured fromthe right ventricle with long-axis, two-dimensional echocardiongram of the left ventricle in 227 consecutive cases of patients with cardiovascular diseases and 80 healthy adults since 1985. It was found that AS angle for normal adults at the end of systoie(ES) and the end of diastole(ED) being 118.50?? 7.70? and 110.60??8.23?; the difference between ES and ED was 7.89??4.51?. In patients with overburdening of left ventricular volume and resistance of coronary artery disease, car-diomycpathy and hypertension , AS angles were amaller,and in patients with overburdening of right ventricular pressure of rheumatic mitral stenosis, AS angles were larger than healthy adults. The difference between ES and ED was smaller than normal for all patients. It was also found that in patients with heartdisease AS angle tends to be smaller or larger before the appearance of abnormality of the left ventricular cavity. Our data suggested that AS angle variance might afford some clinical significance of diagnosis

] AIM: To investigate the expression of the urotensin Ⅱ (UⅡ) receptor GPR14 in the aorta of apoE knockout mouse. METHODS: The expression of GPR14 in the aorta of apoE knockout C57BL/6J mice at various ages (18 weeks, 28 weeks, and 38 weeks old, respectively) was determined with competitive RT-PCR. A binding assay of [ 125 I]-UⅡ on the aortic tissue was also performed in 28 weeks group. RESULTS: We found significant upregulation of GPR14 mRNA at all three ages. 7.Effect of traditional Chinese medicine foot bath on postoperative gastrointestinal function in patients with digestive system tumor

Objective:To observe the effect of Jieyu Anmian prescription of Chinese medicine foot bath on gastrointestinal function in patients with digestive system tumor during perioperative period.Methods:A total of 130 patients with digestive system tumors were chosen from January 2019 to March 2020 for the first time visits to the first hospital of lanzhou university for surgical treatment, 15 cases lost follow-up, and the rest 115 cases of patients were divided into the experimental group (59 cases) and the control group (56 cases) according to random number table method. The control group using conventional care method, the experimental group was treated with Jieyu Anmian washing formula on the basis of routine nursing care for 10 days. During the foot bath, the recovery of gastrointestinal function, the time of the first exhaust and defecation, the length of stay, the cost of stay and the satisfaction of patients were compared between the two groups.Results:The recovery time of bowel sounds, anal exhaust time and first defecation time of the experimental group were all earlier than those of the control group (16.21±11.81) h, (19.64±12.40) h and (41.98 ±19.16) h, respectively, while those of the control group were (27.34±31.47) h, (35.81±18.26) h and (34.47±16.41) h, respectively. The differences were statistically significant ( t value was -2.41, -5.33, 2.17, P< 0.05). The results of the survey of patient satisfaction showed that the experimental group was superior to the control group, with 97.47 ± 4.37 in the experimental group and 94.19 ± 3.29 in the control group, with statistically significant differences ( t value was 3.89, P<0.01). The satisfaction of doctors in the experimental group was 97.41 ± 6.25 and 94.21±5.91 in the control group, with statistically significant difference ( t value was 3.95, P<0.05). The satisfaction of nurses in the experimental group was 97.58 ± 4.53 and 93.85±5.23 in the control group, and the difference was statistically significant ( t value was 4.12, P<0.05). There was no significant difference between the two groups in terms of length of stay and hospitalization expenses ( P>0.05). Conclusions:Jieyuanmian prescription of Chinese medicine foot bath can promote the recovery of gastrointestinal function, promote the exhaust and defecation, and improve the satisfaction of patients and nurses.
8.Application value of urine modified nucleoside's detection in prognosis of bladder transitional cell carcinoma

Objective To study the application value of modified urine nucleoside's detection in prognosis of patients with bladder cancer. Methods We enrolled 85 patients with bladder transitional cell carcinoma confirmed by pathological examination.The 85 patients fulfilled one-year follow-up visit after TUR-BT and were reviewed every three months.The 85 patients did not relapse in the first third month after operation.At the sixth month after operation,20 cases relapsed.18 cases and 19 cases relapsed at the ninth month and the twelfth month after operation.Patients with recurrence added up to 57 cases as the recurrent group.The remaining 28 cases did not relapse at one year after operation as the no recurrent group.Of the 85 cases,55 cases were in T(is) - T1,while 30 cases were in T2 - T4.Of the 85 cases,27 cases were with G1,40 cases were with G2 and 18 cases were with G3.In T(is) -T1,there were 35 cases in recurrent group,while there were 20 cases in the no recurrent group.In T2 -T4,there were 22 cases in recurrent group,while there were 8 cases in the no recurrent group.There were 50 normal people in the control group.Highperformance liquid chromatography/electrospray ionization-quadrupole-time-of-flight mass spectromerry was used to measure the levels of change of two urine modified nucleosides (M1A,1-MeI) which the patients with bladder cancer had different pathology grades,clinical stages,before or after operation and recurrence or no recurrence. Results The levels at third month after operation in no recurrent group ( M1A:3.24 ± 0.40,1 -MeI:5.73 ± 0.67 ) were significantly lower than that before operation ( M 1A:4.34 ± 0.98,1-MeI:14.22 ± 4.05,P < 0.005 ),and remained in low status at another time points after operation.The levels at the third month after operation in recurrent group (M1A:3.31 ±0.33,1-MeI:5.67 ±0.55) were significantly lower than that before operation ( M1A:4.32 ± 1.19,1-MeI:14.31 ± 4.12,P < 0.005 ),which was on the rise and indicating a high level approaching the condition before operation.According to the time point before the operation,recurrent group and no recurrent group were higher than control group (M1A:2.91 ±0.84,1-MeI:5.56 ± 1.25,P < 0.01 ).The levels at the sixth month,ninth month and twelfth month after operation in recurrent group ( M 1A referring to 4.04 ± 0.48,4.11 ± 0.47,4.09 ± 0.53 ;1-MeI referring to1 1.46 ± 1.34,12.14 ± 1.22,12.33 ± 1.27) were the highest (P < 0.01 ).The levels of change of two urine modified nucleosides between pathology grade and clinical stage had no statistical difference ( P > 0.01 ).The levels in recurrence group in T(is) - T1 ( M1 A:5.92 ± 1.28,1-MeI:20.01 ± 8.53 )were higher than the levels in no recurrent group ( M1A:4.02 ±1.22,1 -MeI:11.21 ± 6.45,P < 0.05 ),which was the same in T2 - T4. Conclusion Urine modified nucleosides detection offer a certain clinical value the prognostic of operated bladder cancer patients.
9.Influence of methylenetetrahydrofolate reductase gene polymorphisms on antidepressant response.

OBJECTIVETo assess the influence of polymorphisms of methylenetetrahydrofolate reductase (MTHFR) gene on response to antidepressant treatment.
METHODSTwo hundred and eight one Chinese Han patients have received single antidepressant drugs for at least 6 weeks, among whom 275 were followed up for 8 weeks. Hamilton depression scale 17 (HAMD-17) was used to evaluate the severity of depressive symptoms and therapeutic effects. Single nucleotide polymorphisms (SNPs) of the MTHFR gene were determined using gene chips. Associations of single loci and haplotypes with response to treatment were analyzed using an Unphased 3.0.13 software.
RESULTSNo significant differences in gender, age, year of education, family history, episode times, and antidepressant agents were found between responders and non-responders (all P U+003E 0.05), while the baseline scores of HAMD-17 was significantly different(t=2.891, P=0.004). There was also no significant difference between age, years of education, family history, baseline scores of HAMD-17 and antidepressant agents between remitters and non-remitters (both P U+003E 0.05), while proportion of male patients was significantly higher in non-remission group than remission group (t=2.381, P=0.018), and episode times in non-remission group was significantly higher (t=-1.983, P=0.049). Single locus association analysis has found no significant association between SNPs rs1801131 and rs1801133 in the MTHFR gene with antidepressant response (P U+003E 0.05). On the other hand, haplotype A-C of MTHFR gene (rs1801131 and rs1801133) was significantly associated with antidepressant response in total group (U+03C7 2=11.39, P=0.0007), male subgroup (U+03C7 2=8.767, P=0.003) and serotonin noradrenaline reuptake inhibitors (SNRIs) subgroup (U+03C7 2=10.51, P=0.001).
CONCLUSIONParticular haplotype of MTHFR gene may be related with antidepressant effect, in which the haplotype (rs1801131, rs1801133) A-C type may be associated with better antidepressant efficacy, particularly in males and patients receiving SNRIs drugs.

10.Studies on production, purification and partial characteristics of the extracellular laccase from Armilliria mellea.

The production conditions of extracellular laccase from Armilliria mellea and the characteristics of the enzyme were studied. The experiment proved that initial pH5.5 of the culture medium and temperature at 25 degrees C were favorable for laccase synthesis. As carbon resources, cellobiose and raffinose were better in terms of productivity than maltose, sorbose and galactose. Organic nitrogen source was more suitable for Armilliria mellea to synthesize laccase than inorganic nitrogen source. Peat extract (PE) enhanced notably the yield of laccase; the maximal yield was 7 times as much as that of the control when PE concentration was 50%. Three isozymes were detected in culture supernatant named A, B and C respectively after their mobility on PAGE. After concentrated by (NH4)2SO4 precipitation, laccase A was further purified to homogeneity by preparative native PAGE and anion exchange column chromatography. The native enzyme was a single polypeptide with a molecular mass of approximately 59 kD estimated by SDS-PAGE, while 58 kD by gel filtration chromatography under non-denaturing conditions. Determined by IEF its isoelectric point was 4.0. The optimal pH value and temperature were 5.6 and 60 degrees C respectively in catalytic reaction of oxidizing guaiacol. At 60 degrees C and 65 degrees C, half-lives of laccase A were 45 min and 36.8 min, respectively. Enzyme activity was inhibited with 100 mmol/L Cl-, but was activated with 1 mmol/L SO4(2-). However, if the concentration of NaN3 was only 1 mmol/L, laccase A lost its activity completely. 10 mmol/L EDTA had no effect on laccase A, while 1 mmol/L Cu2+ could enhance its activity. Laccase A showed a good stability when the pH of the buffer varied from 5.2 to 7.2. Using guaiacol as the substrate, the Km was 1.026 mmol/L and the Vmax was 5 mumol/(min.mg); using ABTS instead, the Km was 0.22 mmol/L and Vmax was 69 mumol/(min.mg).
