1.Percutaneous kyphoplasty for treatment of osteoporotic vertebral fractures and collapse with intravertebral vacuum sign

Objective To investigate the clinical effect of percutaneous kyphoplasty (PKP) in treatment of osteoporotic vertebral fractures and collapse with intravertebral vacuum sign.Methods A retrospective study was conducted on the clinical and radiological data of 31 patients with osteoporotic vertebral fractures and collapse with intravertebral vacuum sign treated by PKP from June 2009 to June 2011.Vertebrae body variation,visual analog scale (VAS) and Oswestry disability index (ODI) before operation,during follow-up at postoperative one week and at postoperative 3-6 months were used as outcome measurements.Results All the patients went through operations successfully and presented significant mitigation of low back pain in 24 hours after operation.The vertebrae body height at postoperative one week [(17.2 ±4.2) mm] and at postoperative 3-6 months [(16.8 ±5.1)mm] were statistically different from that before operation [(11.4 ± 1.7) mm,P <0.01],while there was no statistical difference between the two follow-ups (P > 0.05).VAS and ODI at postoperative one week [(2.8 ± 1.7) points and (31.6 ± 8.4) points] were statistically different from those before operation [(8.6 ± 1.3) points and (78.3 ±8.5) points,P<0.01].VAS and ODI at postoperative 3-6 months [(2.3 ±0.8) points and (23.7 ± 2.3) points] presented statistical differences from those before operation (P < 0.01),but no statistical differences from those at postoperative one week (P > 0.05).Conclusion PKP obtains satisfactory clinical outcomes,for it relieves low back pain and restores vertebral body height.
2.Study of ERPs and Topographic Distribution Mapping on Obse ssive-Compulsive Patients

Objective: To study the characteristics of P300 and topographic dist ribution mapping in obsessive-compulsive patients. Methods: The P300 and topog raphic distribution mapping were recorded in 36 patients, using a Bneuro Galileoinstrument. Results: Compared with normal subjects, the wa ve patterns of obsessi ve-compulsive patients were unstable; the frontal wave variation and dissymmetry between the two sides was 63.9%; the N2 and P3 latency was prolonged; the P2 an d P3 amplitude was decreased; the P3 topographic distribution mapping was uneven ly distributed, the normal high amplitude in the parietal region was absent. For the patient group, energy levels below grade 5 in the left brain area, the fron tal area, and for both were 42.3%, 30.3%, 15.15% respectively. Conclu sion: P300 and topographic distribution might be served as an objective index for reflectin g cognitive activity in obsessive-compulsive patients.
3.The design and clinical application of a new type of disposable heparin free dialysis tube

Objective To investigate the clinical effect of a new type of disposable heparin freedialysis tube in uremic patients without heparin dialysis. Methods A total of 110 patients withoutheparin dialysis were divided into control group and observation group by random digits table method with55 cases each. Based on the same pre dialysis and dialysis for heparin saline flushing of discontinuoussaline on the control group using the traditional dialysis pipe for hemodialysis without heparin, theobservation group used disposable non heparin dialysis pipe for hemodialysis without heparin. The numberof patients with venous catheter blockage, blood loss and completion of dialysis time in two groups wasrecorded. Results The number of patients with venous catheter blockage, blood loss and completion ofdialysis time was 6, 6, 47 cases in control group and 0, 0, 54 cases in observation group. The differences were statistically significant (χ2=4.407, 4.407, 4.356, P<0.05). Conclusions The new disposableheparin free dialysis tube can reduce the loss of blood and prolong the treatment time, and the operation issimple, safe and effective, and has good application value and application prospect.
4.Evaluation of the efficacy of CAD/CAM glass-ceramic onlays in molars with complicated oblique crown fracture

Objective:
To evaluate the clinical efficacy of CAD/CAM glass-ceramic onlays in molars with complicated oblique crown fracture within 3.0 mm subgingival.
Methods:
Fifty-six molars from 56 patients with complicated oblique crown fracture within 3.0 mm subgingival were recruited after endodontic treatment and divided into 2 groups according to the restorative methods used. The glass-ceramic onlays group was restored with CAD/CAM glass-ceramic onlays, while the all-ceramic crown group received CAD/CAM all-ceramic crowns. The success rates of the restorations were analyzed, and the sulcus bleeding index (SBI), plaque index (PLI), and gingival index (GI) were recorded at the prerestoration and postrestoration stages.
Results :
After one year of follow-up, the success rate of the glass-ceramic onlay group was 96.4%, and the success rate of the all-ceramic crown group was 92.9%. The difference was not statistically significant (P > 0.05). No differences in SBI, PLI, or GI were found between the glass-ceramic onlay group and the all-ceramic crown group (P > 0.05).

OBJECTIVETo evaluate the expression of PTEN, MMP-2 and MMP-9 in thymomas and their correlation with clinical significance.
METHODSImmunohistochemical S-P assay was used to detect the expression of PTEN, MMP-2 and MMP-9 in 45 thymomas and 16 non-neoplastic thymuses.
RESULTSThe positive rate of PTEN, MMP-2 and MMP-9 expression in 45 thymomas were 53.5%, 48.9% and 62.2%, respectively. There were significant differences between non-neoplastic thymus and thymomas (P < 0.05). The expression of PTEN, MMP-2 correlated with histological type (P < 0.05) and different clinical stage of the thymoma (P < 0.01). The expression of MMP-9 was not correlated with histological type and different clinical stages of thymoma (P > 0.05).
CONCLUSIONPTEN, MMP-2 and MMP-9 may play an important role in the occurrence and development of thymoma. The expression of PTEN and MMP-2 correlates with the malignance and the aggression of thymoma.

6.Intracellular CMTM2 negatively regulates human immunodeficiency virus type-1 transcription through targeting the transcription factors AP-1 and CREB.

BACKGROUNDThe CKLF-like MARVEL transmembrane domain-containing family (CMTM) is a novel family of proteins linking chemokines and TM4SF. Different members exhibit diverse biological functions. In this study, the effect of intracellular CMTM2 on regulating human immunodeficiency virus type-1 (HIV-1) transcription was evaluated.
METHODSThe effects of CMTM2 on regulating full-length HIV-1 provirus and the HIV-1 long terminal repeat (LTR)-directed transcription were assessed by luciferase assay. Transcription factor assays, using the luciferase reporter plasmids of AP-1, CRE, and NF-κB were conducted to explore the signaling pathway(s) that may be regulated by CMTM2. The potential relationship between CMTM2 and the transcription factor AP-1 was further analyzed by Western blotting analyses to investigate the effect of CMTM2 on PMA-induced ERK1/2 phosphorylation.
RESULTSThe results from the current study revealed that CMTM2 acts as a negative regulator of HIV-1 transcription. CMTM2 exerted a suppressive action on both full-length HIV-1 provirus and HIV-1 LTR-directed transcription. Transcription factor assays showed that CMTM2 selectively inhibited basal AP-1 and CREB activity. Co-expression of HIV-1 Tat, a potent AP-1 and CREB activator, can not reverse CMTM2-mediated AP-1 and CREB inhibition, suggesting a potent and specific effect of CMTM2 on negatively regulating these two signaling pathways.
CONCLUSIONIntracellular CMTM2 can negatively regulate HIV-1 transcription, at least in part, by targeting the AP-1 and CREB pathways. Exploring the mechanisms further may lead to new ways to control HIV-1 replication.

8.Correlation between pathology and clinical indexes in IgA nephropathy patients

Objective · To analyze pathology and clinical indexes of the patients diagnosed as IgA nephropathy by renal biopsy so as to determine the correlation between the clinical indexes and the extent of renal pathological damage. Methods · The pathological files of 73 patients with IgA nephropathy confirmed by renal biopsy in East Hospital affiliated to Tongji University from 2012 to 2016 were retrospectively analyzed. Clinical indexes, such as blood pressure, serum uric acid, serum creatinine, blood urea nitrogen, 24 h urine protein quantitation, evaluated glomerular filtration rate (eGFR), triacylglycerol,total cholesterol and complement C3, were compared among the patients with different grades, types and extents of IgA nephropathy. The correlations were also conducted. Results · The 73 patients with IgA nephropathy had an average onset age of (37.07±12.58) years old, whose male to female ratio was 1.21:1. The results showed that there was statistic difference in the indexes of serum uric acid, serum creatinine, blood urea nitrogen and eGFR among the different Hass grades and Oxford pathological classification (P<0.05). Besides, serum complement C3 level and partial immunofluorescence sediment intensity of kidney were correlated with pathological types. The lower serum complement C3 level was, the stronger immunofluorescence sediment intensity was, the severer pathological grading was, and the worse renal function was. Conclusion · The clinical parameters of IgA nephropathy patients can be used to predict the extent of renal pathological damage. Serum uric acid, serum creatinine, blood urea nitrogen, eGFR and serum complement C3 level can be used as the indexes to judge the degree of pathologic damage and the progression of the disease at early stage.

Objective To evaluate the application of western blot in treponema pallidum antibody screening among blood donors.Methods Through standard blood liquidation of syphilis,the sensitivity and specificity of two kinds of TP -ELISA regents and one TPPA regent were evaluated.TP -WB was used to test 279 positive specimens with TP -ELISA,and the correlation between test results of the two methods and the distribution characteristics of WB bands were analyzed. Results The sensitivity of the two kinds of TP -ELISA reagent was 1 00.00%,while the specificity were 92.86% and 85.71 % respectively.At the same,the sensitivity of TPPA reagent was 88.46%,while the specificity was 1 00.00%.In 279 positive specimens with TP -ELISA method,WB confirmed positive was 21 6,positive rate was 77.42%;Including S /Co value >5 and two kinds of ELISA reagent testing both positive specimens were 205,the WB confirmation test positive rate was 1 00.00%,accounted for 99.91 % in 21 6 WB positive samples.Orderly Logistic regression analysis,the method of ELISA S /Co value >5 and double reagent is positive,had statistical correlation with test positive for WB,respectively(P <0.01 );21 6 TP -WB positive specimens WB banding distribution analysis,TP1 7 belt with syphilis antibody IgM,TP1 5 belts and IgG +IgMand IgG exist statistical correlation respectively(P <0.01 ).Conclusion ELISA method of double reagent positive and S /Co value >5 specimens of the basic can be diagnosed with syphilis,WB test with positive results for blood donation member state judgment has guiding significance.
10.Preliminary application of 3.0 T intraoperative magnetic resonance imaging neuronavigation system in China.

OBJECTIVETo report the preliminary experience in clinical application of 3.0 T intraoperative magnetic resonance imaging (iMRI) neuronavigation system in China.
METHODSFrom September 2010 to March 2011, a consecutive series of 122 patients with intracranial lesions underwent operations in guidance with 3.0 T iMRI. A retrospective analysis was conducted regarding clinical efficiency.
RESULTSAmong 122 procedures, the numbers of intraoperative scanning were 2 - 4 times with an average of 2.6. The qualities of images were excellent. Due to the discovery and further possibility of resection of residual tumors, the ratio of gross total resection was increased from 71.7% to 90.0% in cerebral gliomas (n = 60), while from 75.9% to 93.1% in macroadenomas (n = 29). There were 6.7% of all patients occurred postoperative paralysis, but only 3.3% of patients had persistent paralysis at 1 - 2 months follow-up. There was no iMRI-related adverse event occurred. During the same period, more than 2500 patients underwent diagnostic MRI scanning.
CONCLUSIONS3.0 T iMRI neuronavigation system provides high-quality intraoperative structural, functional and metabolic images for real time tumor resection control and accurate functional preservation, resulting in an improvement in maximal safe brain surgery. The system is cost-effective.
