Objective To study the feasibility of endoscopic thyroidectomy with bipolar electrocoagulation. Methods Endoscopic partial thyroidectomy via precordial approach, mainly by using bipolar electrocoagulation and scissors, was accomplished in 21 patients with thyroid tumors with a diameter of 0.7~5.5 cm. Results The operation was accomplished endoscopically in 20 patients, whereas a conversion to open thyroidectomy was conducted in 1 patient. The mean incision length was 22 mm, the operation time was 69?47 min, and the blood loss was 55?41 ml. Postoperative analgesic treatment was required in 2 patients and no serious complications happened. Conclusions Endoscopic thyroidectomy with bipolar electrocoagulation is feasible in the absence of ultrasonic scalpel.

Objective To evaluate the feasibility of hand-assisted laparoscopic colorectal resection. Methods Clinical data of 21 cases of hand-assisted laparoscopic colorectal resection in this hospital from October 2003 to August 2004 were retrospectively reviewed. Results The hand-assisted laparoscopic resection was accomplished in 20 cases. The mean operation time was 144 min, the mean blood loss was 120 ml, the mean number of dissected lymph nodes was 8.5, and the mean time to intestinal function recovery, 69 h. Postoperative abdominal bleeding was found in 1 case, which was cured with conservative therapy. A conversion to open surgery was needed in 1 case. Conclusions Hand-assisted laparoscopic colorectal resection safe, feasible, and radical, with advantages of fewer complications and simplicity of performance.

Objective To detect of clostridium perfringens by qPCR in mouse models and a clinical case in order to offer early diagnosis. Methods 40 Kunming mice were randomly grouped and intramuscular injected clostridium perfringens type A in leg 0.1 ml(3.5 × 109cfu/ml or 3.5 × 108cfu/ml or 3.5× 107cfu/ml,diluted with saline),while control group was injected with 9% sodium chloride 0.1ml.The mouse models and a clinical case were detected by qPCR.Results The death rate of 3.5 × 109,3.5 × 108,3.5 × 107cfu/ml and the blank group were 90%,70%,10% and 0% after intramuscular injection for 72 h spectively. The mean Ct values among these groups were 21.21 ±2.69,28.45 ±2.74,32.49 ±2.87 and 0.00 ± 0.00(P < 0.05).The Ct values of the patient were 30.67 and 30.44.Conclusions Cclostridium perfringens could be successful identified with qPCR in mouse models when the mice still did not show any symptoms.

Objective To evaluate the effect of deep and moderate neuromuscular blockade on surgical conditions during minor laparoscopic gynecologic surgery. Methods Sixty-five patients, with expected surgery time < 3 h,aged 18-60 yr,with body mass index<30 kg/m2,of American Society of Anesthesiologists physical status Ⅰ orⅡ,scheduled for elective laparoscopic gynecological surgery, were allocated into deep neuromuscular blockade group (group D,n =33) and moderate neuromuscular blockade group (group M,n=32) using a random number table. After induction of anesthesia, the patients were tracheally intubated and mechanically ventilated.Cisatracurium was continuously infused to maintain the degree of neuromuscular blockade in both groups to achieve the target degree post-tetanic count of 1 or 2 in group D and train-of-four (TOF) count of 1 or 2 in group M.Surgical conditions were assessed and scored after surgery. The recovery index, time for TOF ratio returning to 0.7 and 0.9,surgery time, mean intra-abdominal pressure, extubation time and TOF ratio at extubation were recorded. Results Compared with group M,the mean intra-abdominal pressure was significantly decreased, and the extubation time and time for TOF ratio returning to 0.7 and 0.9 were prolonged in group D (P<0.05).There was no significant difference in the other parameters between the two groups (P>0.05).Conclusion Moderate neuromuscular blockade can provide better surgical conditions for minor laparoscopic gynecological surgery with shorter recovery time.

Objective To study the relationship between ultrasonography of arteria dorsalis pedis and Hemorheology in patient with diabetic foot. Methods According to with or without frank anabrosis,51 patients were devided into two sub-groups:anabrosis group (23 patients) and no-anabrosis group (28 patients). The correlativity was analysed between the parameters of ultrasonography in arteria dorsalis pedis and hemorheological indices on patients with diabetic foot. Results When compared to patients without ulcers, vascular inner diameter (D),peak systolic velocity (PSV),resistent index (RI) and pulsatility index (PI) on patients with frank anabrosis were decreased significantly, but whole blood viscosity at high shear rate (BVH),whole blood viscosity at low shear rate (BVL),plasma viscosity (PV),fibrinogen (Fib) were increased.Multiple regression analyses indicated that the relationship between BVH,BVL and D was presence, partial regression coefficient (r) was -0.004 and -0.009 ; the relationship between PSV and BVH,PV was intimate,r was -0.957 and -25.655 ; the relationship between RI and BVH,PI and BVH,PV,Fib were relatively intimate,r was -0.009 , -0.141 , -2.666 , -0.383 ,respectively. Conclusions The changes of hemorheology can aggravate the abnormality of hemodynamics and the degree of vascular lesion in small vessels of lower limb,so it is one of influencing factor leading to lower limb arterial ischaemia and diabetic foot deterioration.

AIM: To study the effects of dietary factor on liver gene expression profile in apoE~(-/-)/LDLR~(-/-)/Lepr~(db/db) mice and atherogenesis. METHODS: The gene expression profile was determined by using cDNA microarray. Total plasma TC and TG level were measured with COD-PAP and GPO-PAP methods respectively. The morphological characters of the aorta were also observed. RESULTS: Among the 4000 target genes, 78 and 114 genes were up-and down-regulated, respectively, in treble genes mutants fed with high fat high cholesterol diet compared with that of regular chow littermates, including lipid metabolism, carbohydrate, cell skeleton and immune related genes. Atherogenic diet induced severe plasma hyperlipidemia even in the early stage and also accelerated atherogenesis in the treble genes mutant mice. CONCLUSION: The data suggest that atherogenic diet aggravates abnormal plasma lipid metabolism and induces severe hyperlipidemia, and atherogenesis in the treble genes mutant mice as well.

Chinese Journal of General Surgery 2000;0(12):-
Objective To investigate the value of ultrasound examination in the diagnosis of acute appendicitis. Methods Emergency abdominal ultrasound examination was performed on 152 children who had clinically suspected acute appendicitis. Results Sixty-three children were diagnosed as acute appendicitis by ultrasound . Among them, fifty-nine children had emergency appendectomy; the other 4 had delayed appendectomy six months after conservative therapy for appendiceal mass. All of the diagnoses were confirmed to be correct by pathologis examination . Among the other 89 children, the diagnosis was luteinic rupture in 4, choledochal cyst in 2, omental cyst in 1, hydronephrosis in 1, and nephrolithiasis in 1. Eighty of them were diagnosed as mesenteric lymphadenitis and treated conservatively. Seventy-six of these eighty children recovered , and appendectomy was performed on four patients. In two of them the diagnosis was suppurative appendicitis, and in two, the diagnosis was simple appendicitis. Conclusions Combined with consideration of the symptoms and signs, ultrasound is valuable in the diagnosis of appendicitis.
