1.The influence of different postures on the efficacy of mechanical ventilation in critically ill patients with 2009 H1N1 influenza

28 patients with acute respiratory distress syndrome caused by 2009 H1N1 influenza were mechanically ventilated for 1 hour in the supine, lateral, semi-supine, lateral prone, and prone position respectively. Changes of heart rate, respiratory parameters and arterial partial pressure of oxygen ( PaO2 )were measured. There were no significant differences ( P > 0. 05 ) of heart rate and mean arterial pressure among these postures. Lateral prone ( P < 0. 05 ) and prone posture ( P < 0. 01 ) , however, can both significantly improve tidal volume, mean airway pressure, PaO2, static lung compliance and oxygenation index. We concluded that mechanical ventilation delivered in the prone position can remarkably improve oxygenation in patients with H1N1 influenza-associated acute lung injury.

Objective To investigate the effect of cyclooxygenase-2(cox-2)on prostaglandin I2(PGI2)/thromboxane A2(TXA2)system in ischemia reperfusion(I/R)injury to small bowel in rat.Methods Forty-eight male SD rats were randomly divided into three groups:control group(n=8),I/R group(n=20)and intervention group(n=20).Small intestine I/R model was established with the animals of I/R group:after anesthesia,rats were split along the middle abdominal incision,the mesenteric artery was then bluntly separated and clipped with a blood vessel forceps at its root for 45 minutes to block the blood flow,then releasing the forceps for reperfusion.Animals of intervention group were lavaged with Nimeshulide 1h before operation,the remaining steps were the same as did in I/R group.The samples of blood and intestinal tissue were collected at 3,6,12 and 24 hours after operation.The content of plasma D-lactate was detected by ultraviolet spectrophotometric method.Radioimmunoassay was used to detect 6-keto-PGF1?(the stable metabolite of PGI2,reflected the level of PGI2)and thromboxane B2(TXB2,stable metabolite of TXA2,reflected the level of TXA2)in serum and tissues of small bowel.The expression of cox-2 mRNA was detected by RT-PCR.Results Compared with the control group,the levels of plasma D-lactate,cox-2,6-keto-PGF1? and TXB2 were significantly increased in I/R group,with more obvious increase of TXB2,and the value of 6-keto-PGF1?/TXB2 decreased significantly(P

7.Effect of extracorporeal shock wave on tendon-bone healing after reconstruction of the anterior cruciate ligament:an experimental study in rabbits

[Objective]To explore the effects of extracorporeal shock waves(ESW) therapy on tendon-bone healing after reconstruction of the anterior cruciate ligament(ACL).[Method]Animal model of reconstruction of the anterior cruciate ligament in 18 healthy F/M New Zealand white rabbits(2~3 months old) were established using digital long extensor tendon and randomly classified into 2 groups.The ESW group was given extracorporeal shock waves therapy on 24 hours after reconstruction of the anterior cruciate ligament while the control group with only reconstruction of the anterior cruciate ligament but no more therapy.All animals were sacrificed at 2、4、8 weeks after operation for histological examinations.The specimens were stained with hematoxylin-eosin and Masson trichrome to investigate pathology change of the tendon-bone healing tissue.The ink intravascular injection was used to observe the neo-vascularization of tendon-bone healing tissue.[Result]The histological examination revealed that at 4 weeks after operation,the ESW group,tendon-bone interface was filled with proliferated fibroblasts and chondroblasts,the collagen fibers were obviously increased and regularly arranged.At 8 weeks after operation,the healing tissue at the bone-tendon interface had developed into dense connective tissue,the collagen fibers were formed in abundance and the perpendicular collagen fibers were regularly and longitudinally arranged.The transition zones of collagen fibers,fibrocartilage and bone(the structure alike a direct ligamentous insertion) occurred in the local region of tendon-bone interface.The histological observations showed that the tissue healing at tendon-bone interface of the ESW group were quicker than those of the control group.The proportion of neo-vessels at the tendon-bone interface of the ESW group were significantly higher than those of the control group at 4 and 8 weeks after operation(respectively P=0.028,P=0.008).[Conclusion]Extracorporeal shock wave treatment can significantly improve the tendon-bone early healing after reconstruction of the anterior cruciate ligament in rabbits.

9.A Study on Extraction Technology of Qingyan Xiehuo Granule

Orthogonal design was used to study the extraction technology of Qingyan Xiehuo Granule. The solvent volumn, extracting time, extracting times and smashing degree (only for Fructus Canarii) were treated as the factors and examined at various levels. The extraction technology for Folium Polygoni tinctorii and Radix Scutellariae was determined. The methonal extract absorption (at 310nm) of the dried water extract of Folium Polygoni tinctorii and the content of Baicalin were used as the criteria.
10. Mechanism of autophagy in osteosarcoma

BACKGROUND: Autophagy may be a tumor-inhibiting factor at the early stage of carcinogenesis, while at late stage, it may support or promote the tumor growth and spread, and may contribute to drug resistance. OBJECTIVE: To review the related literature at home and abroad, summarize the relationship between autophagy and osteosarcoma, and explore the mechanism of autophagy in osteosarcoma and its therapeutic prospect. METHODS: CNKI, Wanfang, VIP and PubMed databases were retrieved with the keywords of “autophagy, chemotherapy, autophagy related factors, osteosarcoma, apoptosis, drug resistance” in Chinese and English, respectively. The articles related to the mechanism of autophagy in bone tumors were included. The literature was screened according to the selection criteria, and the repetitive and the long-standing literature were excluded. Fifty-two articles were included for review and analysis. RESULTS AND CONCLUSION: (1) Autophagy may be an important factor in promoting and preventing cancer, and its effect varies with the occurrence of development of tumor. (2) The full understanding and regulation of autophagy pathway can significantly improve the potential of tumor therapy. (3) Considering the importance of autophagy in tumor biology, research on autophagy-regulated miRNA will help us to better understand malignant tumors and may discover new disease markers and treatment strategies. (4) The above conclusion still needs a lot of scientific research and reasonable experiments to confirm.
