1.Effects of Compound Musk Injection Combined with Edaravone on Coagulation and Neurological Func-tion of Patients with Traumatic Brain Injury

OBJECTIVE:To investigate the effects and safety of Compound musk injection combined with edaravone on coag-ulation and neurological function of patients with traumatic brain injury. METHODS:A total of 84 patients with traumatic brain in-jury in our hospital during Apr. 2015 to Apr. 2016 were divided into observation group and control group,with 42 cases in each group. Control group was given Edaravone injection 30 mg+0.9% sodium chloride injection 100 mL,bid,ivgtt;observation group was additionally given Compound musk injection 20 mL+0.9% sodium chloride injection 250 mL,qd,ivgtt. Both groups were treated for 4 weeks. NIHSS score,GCS score,the levels of NSE,CRP,PT,APTT and D-dimer,GOS score,the occurrence of ADR were compared between 2 groups before and after treatment. RESULTS:After treatment,NIHSS score of 2 groups decreased significantly,compared to before treatment;the observation group was significantly lower than the control group,with statistical significance (P<0.05). GCS score of 2 groups increased significantly,compared to before treatment;and the observation group was significantly higher than the control group,with statistical significance(P<0.05). The serum levels of NSE,CRP,PT,APTT and D-dimer in 2 groups decreased significantly,compared to before treatment;and the observation group was significantly lower than the control group,with statistical significance(P<0.05). GOS score of observation group was significantly higher than that of control group,with statistical significance(P<0.05). No obvious adverse reaction was found in 2 groups during treatment. CON-CLUSIONS:Compound musk injection combined with edaravone can improve coagulation and neurological function of patients with traumatic brain injury,relieve traumatic brain injury and improve prognosis.
2.Expression and prognostic significance of survivin in the progression of bladder transitional cell cancer.

The expression of survivin, a member of inhibitor of apoptosis (IAP) family, was examined in bladder transitional cell cancer (BTCC) tissue and adjacent normal tissues to examine its clinical implication in the development of BTCC. Thirty specimens of bladder cancer were detected for the expression of survivin by using immunohistochemistry and real-time quantitative reverse transcription polymerase chain reaction (RT-QPCR) in BTCC tissue and adjacent normal tissues. Our results showed that the positive rate of survivin immunostaining specimen were 0 and 60% (18/30) in the adjacent normal tissues, bladder cancer, respectively. The-DeltaDeltaCT value of survivin in bladder cancer tissue was 10.2829 (9.0034-11.5624) times that in the adjacent normal tissues. The expressions of survivin were correlated with the pathological grades of tumor and clinical stages. It is concluded that there was only weak expression of survivin mRNA in the adjacent normal tissues, but the expression of survivin mRNA in bladder cancer tissue was much higher than that in the adjacent normal tissues and the expression of survivin was correlated with pathological grades and clinical stages of tumor.

3.Application of gallbladder plate approach using Laennec membrane based on APR triangle region in laparoscopic anaphylactic right lobe hepatectomy

Objective To investigate the safety and clinical efficacy of the combined gallbladder plate approach using Laennec membrane based on APR triangle in laparoscopic anaphylactic right lobe hepatectomy.Methods Clinical data of 27 patients underwent laparoscopic anaphylactic of right lobe hepatectomy based on the APR triangle combined with the gallbladder plate approach using Laennec membrane were collected from January 2021 to December 2023.The relevant data of patients were statistically analyzed,including operation time,intraoperative blood loss,postoperative complications,and postoperative hospital stay.Results All the 27 patients underwent laparoscopic anatomic hepatectomy,and no perioperative death occurred.Among them,segment Ⅴ was resected in 4 cases,segment Ⅵ in 3 cases,segment Ⅴ+Ⅷ in 6 cases,segment Ⅴ+Ⅵ in 4 cases,segment Ⅵ+Ⅶin 5 cases,segment Ⅴ+Ⅵ+Ⅶ in 3 cases,and segment Ⅴ+Ⅵ+Ⅷ in 2 cases.All cases belonged to the China Liver Cancer(CNLC)clinical staging,stage Ⅰa in 15 cases,stage Ⅰb in 8 cases and stage Ⅱa in 4 cases.The operative time was(258.3±62.3)min,the intraoperative blood loss was(168.8±48.1)mL,and there was no transfusion.The patients were given liquid diet on the first day after surgery,and were active in bed on the second day after surgery.The mean postoperative length of hospital stay was(6.5±1.7)d after removal of abdominal drainage tube.There were 5 cases of Clavien-Dindo Ⅰ and Ⅱ complications(3 cases of abdominal effusion,1 case of pleural effusion,1 case of pulmonary infection),and no complications such as bile leakage and abdominal hemorrhage occurred.Postoperative pathology:21 cases of hepatocellular carcinoma,4 cases of hepatociliary duct carcinoma,2 cases of liver metastasis.Patients with liver malignant tumor were followed up for 2 months to 2 years,and all patients survived during the follow-up period without tumor recurrence.Conclusion The gallbladder plate approach based on APR triangle combined with Laennec membrane is a safe and effective surgical method for laparoscopic anaphylactic right lobe hepatectomy,which is worthy of clinical application.

5.Roles of multidisciplinary team in diagnosis and treatment of suspected cases of COVID-19.

OBJECTIVE:
To analyze the roles of multidisciplinary team (MDT) in the diagnosis and treatment of suspected cases of coronavirus disease 2019 (COVID-19).
METHODS:
The clinical data of 48 patients with suspected COVID-19 admitted in Jinhua Municipal Central Hospital from January 21, 2020 to March 20, 2020 were retrospectively analyzed.
RESULTS:
In the 48 suspected cases, 18 were diagnosed with COVID-19, and 30 were excluded. Each of the confirmed cases were discussed among MDT for 2 to 12 times with an average of (4.7±3.2) times; while for non-COVID-19 patients were discussed for 2 to 4 times with an average of (2.3±0.6) times. With the guidance of MDT, one COVID-19 patient was transferred to designated provincial hospital after effective treatment; one patient complicated with acute cholecystitis underwent gallbladder puncture and drainage; and COVID-19 was excluded in a highly suspected patient after alveolar lavage fluid examination. Except one transferred patient, all 17 confirmed COVID-19 patients were cured and discharged. There was no cross-infection occurred in suspected patients during the hospitalization. There were no deaths and no medical staff infections.
CONCLUSIONS
The efficiency of diagnosis and treatment for suspected COVID-19 patients can be improved with MDT, particularly for complicated cases.
