1.Clinical Study of Tuina for Stiff Neck

Objective: To observe the clinical effect of Tuina for stiffneck. Methods: All of 216 cases with stiff neck were randomly allocated into a tuina group (110 cases) and a control group (106 cases). The patients in the tuina group were treated with Three Parts, Four Points,Five Manipulation tuina therapy, while those in the control group were treated with pucturing Wailaogong (Ex-UE 8), Houxi (SI 3), Jianjing (GB 21) and Ashi points in the affected side,Once a day, and 3 times constitute one treatment course. The effect was observed after 2-course treatments. Result: The effective rate in the tuina group and the control group was 93.64% and 83.02%, respectively, and there was statistical difference between the two groups (P < 0.05).Conclusion: The tuina therapy of Three Parts, Four Points, Five Manipulations is better than simple acupuncture therapy in treating stiff neck.
2.Effect of moderate treadmill exercise together with modified hydroxyapatite chitosan composite hydrogel implantation on repair of full-thickness defects of articular cartilage in rats

Objective To investigate the effect of moderate treadmill exercise together with modified hydroxyapatite chitosan composite hydrogel (CS/HA-g-CS) implantation on repair of full-thickness defects of articular cartilage in rats.Methods Full-thickness cartilage defects were drilled in the patellar groove of bilateral femoral condyles in a total of 24 male SD rats before they were randomly assigned into 4 groups.The control group (BC group) was subjected to no exercise or CS/HA-g-CS implantation;the chitosan group (CHI group) to CS/HA-g-CS implantation without exercise;the moderate treadmill exercise group (MIR group) to exercise 4 weeks after modeling without CS/HA-g-CS implantation;the CHI + MIR group to moderate treadmill exercise plus CS/HA-g-CS implantation 4 weeks after modeling.Half of the animals were sacrificed at week 8 and half at week 16 after operation.Femoral condyles were harvested for gross observation and histochemical measurement by O' Driscoll scoring system.mRNA expressions of glycosaminoglycan,collagen type Ⅱ and BMP-2 were detected by RT-PCR.Results Gross observation revealed:at 8 weeks after modeling,the CHI + MIR group was significantly better than the other 3 groups,with the BC group in the poorest (P < 0.05);at 16 weeks after modeling,the BC group was significantly poorer than the other 3 groups (P <0.05) among which there were no significant differences (P > 0.05).O 'Driscoll scoring revealed:at both 8 and 16 weeks after modeling,the CHI + MIR group was significantly better than the other 2 groups and the BC group significantly poorer than the other 3 groups (P < 0.05) but there was no significant difference between the MIR and CHI + MIR groups(P > 0.05).The expressions of collagen type Ⅱ,glycosaminoglycan and BMP-2 were significantly higher in the CHI + MIR group than in the other 3 groups (P < 0.05) and significantly lower in the BC group than in the other 3 groups (P < 0.05).Conclusions Moderate treadmill exercise together with CS/HA-g-CS implantation has significant positive effects on repair of full-thickness defects of articular cartilage in rats than merely moderate treadmill exercise or CS/HA-g-CS implantation alone.The defective cartilage repaired by moderate treadmill exercise together with CS/HA-g-CS implantation contains more collagen type Ⅱ and glycosaminoglycan and shows morphology of nearly normal cartilage.
3.Knowledge,attitude and practice of influenza vaccine and 23-valent pneumonia vaccine among primary healthcare workers

Objective:
To learn the knowledge,attitude and practice(KAP)of primary healthcare workers on influenza vaccine and 23-valent pneumonia vaccine in Xiuzhou District of Jiaxing,and to provide basis for vaccination of influenza vaccine and 23-valent pneumonia vaccine.
Methods:
From November 2018 to January 2019,the primary healthcare workers from eight community health service centers in Xiuzhou District were selected by multi-staged random sampling method. A self-designed questionnaire was used to investigate the KAP of influenza vaccine and 23-valent pneumonia vaccine among primary healthcare workers. Logistic regression analysis was employed to select the influencing factors for vaccination and recommendation behaviors of primary healthcare workers.
Results:
Totally 105 healthcare workers(95.45%)completed the survey. Among them,86(81.90%)and 76(72.38%)workers believed influenza vaccine and 23-valent pneumonia vaccine effective;87(82.85%)and 88(83.81%)workers believed the two vaccines safe;82(78.10%)and 58(55.24%)workers knew the vaccination procedures;58(55.24%)and 34(32.38%)workers knew the key recommended population of vaccines;77(73.33%)workers knew that the vaccine costs could be paid by medical insurance. The vaccination rate of influenza or(and)pneumococcal vaccine among the respondents was 27.62%,and the recommendation rate was 46.67%. The results of multivariate logistic regression analysis showed that healthcare workers who were female(OR=7.250,95%CI:1.388-37.883),believed the two vaccines effective(OR=4.390,95%CI:1.167-16.515),believed the two vaccines safe(OR=11.977,95%CI:2.417-59.343)and knew the payment method of vaccine cost(OR=11.945,95%CI:2.924-48.793)were more likely to vaccinate;healthcare workers who majored in public health(OR=2.641,95%CI:1.338-5.214),believed the two vaccines effective(OR=6.944,95%CI:1.655-29.134),knew the key recommended population of vaccines(OR=4.380,95%CI:1.125-17.053),had vaccinated the two vaccines(OR=7.902,95%CI:1.693-36.877)were more likely to recommend.
Conclusion
Primary healthcare workers in Xiuzhou District have less awareness of influenza vaccine and 23-valent pneumonia vaccine,then have lower vaccination rate(27.62%)and recommendation rate(46.67%).
4.Application of a self-designed handy body position scaffold in fluoroscopy for lower limb surgery

Objective To evaluate the application of a self-designed handy body position scaffold in the fluoroscopy for lower limb surgery.Methods We reviewed the intraoperative X-ray films of 161 patients with lower limb fracture who had undergone intraoperative fluoroscopy using our self-made handy body position scaffold during surgery from October 2013 to May 2017.They were 105 males and 56 females,aged from 10 to 91 years (mean,39.7 years).There were 42 femoral fractures of middle or lower shaft,12 fractures of medial or lateral condyle or tibial plateau,96 tibiofibular fractures,7 ankle fractures and 4 foot fractures.The operation time,intraoperative fluoroscopy time,intraoperative bleeding,rate and time of fracture union,and complications were recorded.Results All the 161 patients completed intraoperative fluoroscopy and radiography uneventfully,indicating that the handy body position scaffold effectively maintained the body position of the affected ipsilateral lower limb.The body position scaffold was slightly deviated due to one overweight (121 kg) patient when the knee joint was flexed 90°,but the fluoroscopy and radiography was completed after adjustment of the C-arm roentgenographic machine.The operation time averaged 71.5 minutes (from 28 to 119 minutes),the X-ray exposure time 9.8 seconds (from 6 to 31 seconds),and the intraoperative bleeding 157.2 mL (from 80 to 500 mL).The 161 patients obtained follow-up for 5 to 22 months (average,14.1 months).The rate of primary union was 97.5% (157/161).The time for fracture union averaged 8.8 weeks (from 4 to 17 weeks).Fracture nonunion occurred in one case,and delayed union in 3 cases.The final follow-ups revealed normal functional recovery of the affected knees and ankles,but no limb shortening,or no rotational,lateral or anteroposterior angulation deformity.Conclusion In the lower limb surgery,the intraoperative fluoroscopy can be completed better with the aid of our self-designed handy body position scaffold that prevents movement of the affected lower limb and lowers the risk of radiation exposure.
5.Reliability and validity of Chinese Pediatric Quality of Life Inventory in influenza cases

Objective:
To explore the reliability and validity of the Chinese Pediatric Quality of Life Inventory(PedsQL)in influenza children.
Methods:
From January 2017 to February 2018,we selected laboratory-diagnosed influenza cases and healthy children according to age and gender ratio by stratified random sampling and systematic sampling method. We employed Chinese version of PedsQL 4.0 to investigate their quality of life,used Cronbach's α to evaluate the reliability,and used Pearson correlation coefficient, t-test,confirmatory factor analysis(CFA)and ROC curve to evaluate the validity.
Results:
Totally 300 influenza cases and 300 healthy children were surveyed,with 294(98.00%)and 295(98.33%)valid questionnaires recovered. The general Cronbach's α was 0.89,and the Cronbach's α of each dimension ranged from 0.79 to 0.84. The Pearson correlation coefficients between the items and their belonged dimension ranged from 0.537 to 0.755,between the items and other dimension ranged from 0.203 to 0.384. The CFA resulted in RMSEA of 0.06,GFI of 0.88,AGFI of 0.90,CFI of 0.91 and NFI of 0.89. The scores in total and in all the dimensions in influenza cases were significantly different with those in healthy children(P<0.05). Taking the scores in influenza cases as a golden standard,the area under the ROC curve was 0. 985(P<0.05),the sensitivity was 0.92,and the specificity was 0.95. When the limit score was 82.18,the Youden index was the largest.
Conclusion
The Chinese version of PedsQL4.0 has good reliability and validity, which can be applied to quality of life assessment in children.
6.Effect comparison between PFNA and DHS in treatment of senile patients with intertrochanteric fracture

Objective To compare the effect of PFNA and DHS on the treatment of senile patients with femoral intertrochanteric fracture.Methods 89 senile patients with femoral in-tertrochanteric fracture were divided into two groups.42 patients were treated with PFNA and 47 patients were treated with DHS.Incision length,amount of bleeding,operation time,postoperative functional recovery and intraoperative complications were compared between two groups.Results The incision length,intraoperative bleeding volume,the time of fracture healing and surgical time as well as postoperative functional recovery in the PFNA group were significantly better than those in the DHS group.Conclusion Application of PFNA in the treatment of senile patients with femoral intertrochanteric fracture is simple,and it has advantages such as fewer traumas and bleed-ing,shorter operation time and desired effect,which is especially helpful for senile patients with in-tertrochanteric fracture.
7.Disease burden of influenza in schools and child care settings in rural areas of Hangzhou, 2016-2018

Objective The objective of this study was to analyze the disease burden of influenza in schools and child care settings in rural areas of Hangzhou. Methods Hospital visit influenza cases aged 3-17 years in hospitals that reported based on influenza surveillance system from 2016 to 2018 in Chun′an county, Hangzhou city were selected as study subjects and a total of 294 confirmed cases of influenza were selected using system sampling method. Questionnaires were designed to investigate the basic information and data on inpatients and outpatients among, health care and life quality, etc.. Direct and indirect economic burden and disability adjusted life year (DALY) were analyzed and compared among different age groups. Results The mean age of investigated subjects was (8.88±3.92) years. A total of 143 (48.64%) investigated cases were male. In total of 283 (96.26%) cases were outpatients. The total economic burden was 124 743.95 CNY. The mean economic burden was 424.30 CNY per person. The mean direct and indirect economic burden was 361.33 and 62.97 CNY per person respectively. The difference of the mean direct, indirect and total economic burden per person between different age group was statistically significant (P<0.001). The 3-5 years age group showed the highest economic burden with the median value of direct, indirect and total economic burden per person being 276.24, 50.98 and 321.26 CNY, respectively, while the 12-17 years age group showed the lowest values with 175.30, 26.54, 201.79 CNY, respectively. The DALY of 294 influenza cases was 1.18, and the median of burden strength was 3.21 DALY/thousand. The difference of the burden of strength between different age group influenza case was statistically significant (P<0.001), of which the 12-17 years age group showed the highest value with 4.25 DALYs/thousand while the 3-5 years age group showed the lowest value with 2.60 DALY/thousand. Conclusion The disease burden of influenza was heavy in schools and kindergartens in rural areas of Hangzhou city, with the cases aged from 3 to 5 years showing higher economic burden and cases aged from 12 to 17 years showing greater burden strength.

10.Analysis of the status and related factors of vaccination against respiratory diseases among elderly people aged ≥60 in Zhejiang Province

The objective of this study is to assess the current status of vaccination against respiratory disease among the elderly aged ≥60 and analyze the factors influencing vaccination rates at both service provider and recipient levels in Zhejiang Province. Using a stratified random sampling method, a questionnaire survey was conducted from September 2022 to January 2023 among elderly people aged ≥60 in 30 townships/streets in Zhejiang Province, as well as immunization planning staff at the provincial, municipal, county/district, and township/street levels. Logistic regression models were used to analyze the factors related to vaccination among elderly people in Zhejiang Province. Based on the Zhejiang Provincial Comprehensive Management Information System for Vaccine and Vaccination, the systematic coverage rates of influenza vaccine and pneumonia vaccine for the elderly were 21.76% and 4.57%, respectively. Multivariate logistic regression analysis indicated that advanced age ( OR=1.74, 95% CI: 1.51-1.99), knowing that influenza is more severe than the common cold ( OR=1.67, 95% CI: 1.37-2.04) and having heard of the influenza vaccine ( OR=9.78, 95% CI: 7.03-13.59) were motivating factors for elderly to receive influenza vaccines. Advanced age ( OR=1.71, 95% CI: 1.43-2.06), knowing the serious consequences of pneumonia in the elderly ( OR=1.93, 95% CI: 1.47-2.55) and knowing that pneumonia vaccines can prevent pneumonia ( OR=6.36, 95% CI: 4.84-8.36) were motivating factors for elderly to receive pneumonia vaccines. Zhejiang Immunization Program staff believed that the main reasons why the elderly aged ≥60 would not be vaccinated against influenza or pneumonia were that they felt they would not get sick (55.52% and 56.35% respectively), it would not be serious if get sick (47.73% and 37.46% respectively), lacking trust in vaccine efficacy and safety (38.31% and 43.69% respectively). Vaccination rates for influenza and pneumonia vaccines among the elderly aged ≥60 in Zhejiang Province are suboptimal. Advanced age, awareness of the severity of respiratory diseases and awareness of vaccines against such diseases are related factors for elderly individuals to receive influenza and pneumonia vaccines.
