1.Effect of different body positions on cardio - pulmonary function and sleep in preterm infants
Chinese Journal of Practical Nursing 2011;27(17):1-4
Objective To evaluate the effects of different body position on the heart rate、respiratory rate, average arterial pressure、SpO2 and sleep in preterm infants. Methods 90 preterm infants from August 2009 to August 2010 in our Neonates Division were randomly divided into 0°supine group 、0°prone group 、0°tilt group,30°supine group、30°prone group、30°tilt group. The heart rate、respiratory rate、SpO2、average arterial pressure and sleep time were recorded at 1 h、2 h、3 h、4、 h、5 h、6 h. Results At 3~6h the heart rate、respiratory rate in 0°prone group and 30°prone group were significantly lower than those in 0°supine group、30°supine group ,0°tilt group and 30°tilt group. The SpO2 and sleep time were significantly higher in 0°prone group、30°prone group than those in 0°supine group, 30°supine group、0°tilt group and 30°tilt group. Conclusions Prone position had physiological benefits for infants, which is worthy of promotion.

Objective To analyze the risk factors associated with allergic disease in preterm infants during the first year of life. To investigate whether there was any association between peripheral blood eosinophils and allergic di-sease during the first year of life benefit of hydrolyzed formula feeding in preterm infants. Methods A nested case -control study was conducted in a cohort of artificial feeding preterm infants admitted to Neonatal Intensive Care Unit, Shanghai Children's Medical Center from January 2012 to April 2013. The preterm infants were monitored from birth up to 1 year of age and the findings were related to development of allergic disease. Individuals with allergic disease during the first year of life(cases)were matched with gender,as previously described with two individuals(controls)who re-mained event - free during the study. The risk factors associated with allergic disease in preterm infants during the first year of life were analyzed. And whether there was any association between peripheral blood eosinophils and allergic di-sease during the first year of life benefit of a hydrolyzed formula feeding in preterm infants was investigated. Different variables in control and case individuals were compared with t test for normal distribution measurement date,χ2 tests for categorical variables and Mann - Whitney U test for non - normal distribution measurement date, and multiple conditio-nal Logistic regression were used to investigate the risk factors associated with allergic disease in preterm infants. Results Thirty - four individuals were in cases and 68 individuals were in controls. In a conditional multivariable Logistic model, peripheral blood eosinophils of preterm infants at full enteral feeding(EOS - 2)(OR = 5. 941,95% CI:1. 165 - 41. 375,P ﹤ 0. 05),and family history of allergy(OR = 3. 316,95% CI:1. 201 - 9. 152,P ﹤ 0. 05)were the two independent risk factors for allergic disease in preterm infants during the first year of life. Individuals fed with standard preterm formula after birth, the association between EOS - 2 and allergic disease was significantly enhanced (OR = 21. 459,95% CI:1. 686 - 273. 152,P ﹤ 0. 05). By contrast,in individuals fed with hydrolyzed formula, the risk of EOS - 2 was substantially attenuated(OR = 1. 708,95% CI:0. 148 - 19. 743,P ﹥ 0. 05). Conclusions Peripheral blood eosinophils of preterm infants at full enteral feeding EOS - 2 and family history of allergy were the 2 independent risk factors for allergic disease in preterm infants during the first year of life. Contrast to individuals fed with standard preterm formula after birth, individuals fed with hydrolyzed formula had lower association between factors of peripheral blood eosinophils and family history of allergy, and allergic disease during the first year of life.

Objective To summarize the experience of perioperative management of coronary artery bypass graft(CABG) in high-risk patients with coronary heart disease. Methods The clinical data of 164 patients underwent CABG from March 2004 to November 2005 were analyzed.Of 164 patients,151(92.05%) cases had severe coronary heart disease. Results Totally 144 artery-vessels and 353 venous-vessels were transplanted to patients.Combined operative procedures included 11 cases of ventricular aneurysm resect,5 cases of valve replacement,5 cases of valve repair, and 1 case of surgical repair of perforation of ventricular septum.Post-operative complications included 2 cases of low cardiac output,1 case of respiratory failure,1 case of renal failure, and 1 case of cerbral infarction. Two patients died and the operative mortality was 1.22%.Conclusion CABG procedure is safe in the treatment of high-risk patients with coronary heart disease.Proper preoperative and perioperative treatment can improve the curative effects significantly.

Objective:To review relevant research on the application of self-help intervention model in adult depression patients, analyze the main characteristics and application effects of self-help intervention model, in order to provide reference for related research in this field. Methods:According to the methodological framework of the scope review, searched 12 Chinese and English databases including CNKI, Wanfang, PubMed, Web of Science, etc. from the establishment of the database until April 20, 2021, and sorted and analyzed the obtained documents according to the inclusion and exclusion standards. Results:A total of 45 documents were included, of which only 5 were from China; the included studies were mainly based on the Internet platform, using cognitive behavioral therapy, reading therapy, acceptance and commitment therapy, and problem-solving therapy for intervention; the duration of the intervention was 3 weeks to 4 months, participants complete an intervention module or read a specified number of chapters every 3 to 7 days on average, and receive regular guidance and supervision from the intervention facilitator every 1-2 weeks. The total contact time with the facilitator was 80-160 minutes. Various modes of self-help intervention based on the Internet could effectively improve the negative emotions of patients with depression. Conclusions:The self-help intervention model has shown good results in the treatment of patients with depression, but there is a lack of local intervention research in China, and there is no clear standard for the number of interventions and the length of the intervention guide. Under the general trend of "Internet + medical health", it is necessary to actively construct a self-help intervention model for depression with Chinese cultural characteristics, and form a systematic and standardized intervention strategy to address the many obstacles in time, location, resources and cost of people with depression, and finally achieve the purpose of improving the patients′ depressive symptoms and helping them to restore their social functions.

Objective:To analyze the research status, development trend and frontier hotspots of midwifery education in China in recent 20 years. Methods:Based on the topic of "midwifery education" or "midwifery teaching", this paper searched the periodical literatures from 2001 to 2021 on CNKI database, and used CiteSpace5.7R5 software to analyze them visually and generate knowledge map. Results:A total of 548 Chinese papers were included in this study, and the annual number of published papers showed an overall upward trend. The research field of midwifery education in China formed an obvious core team, and there was few cooperation among core author groups. Health Vocational Education, Chinese Nursing Education and Chinese Higher Medical Education were the top three journals. The six topics with the highest frequency were midwifery specialty, midwifery personnel, midwifery education, practice teaching, delivery mode and teaching mode, forming 10 clusters of midwifery education. In recent three years, the research of midwifery education in China has gradually changed into simulation teaching, flipped classroom, postpartum rehabilitation and so on. Conclusion:The research scope of midwifery education in China is wide and has formed an obvious core team, but the correlation is weak and there is less communication and cooperation among the research teams. The research in the field of high-level midwifery education is insufficient. Midwifery educators and researchers should pay enough attention to carry out in-depth research on relevant aspects.
