BACKGROUND:Photoelectric navigation-aided percutaneous pedicle screw placement has been developed extensively, but its accuracy, safety and effectiveness have not yet been confirmed by evidence-based medicine.OBJECTIVE:To compare the curative efficacy of photoelectric navigation-aided percutaneous pedicle screw placement and traditional open posterior pedicle screw fixation for thoracolumbar fractures.METHODS:Sixty patients with thoracolumbar fractures were equivalently randomized to treatment and control groups and then underwent photoelectric navigation-aided percutaneous pedicle screw placement and traditional open posterior pedicle screw fixation, respectively. The perioperative indexes, imaging indexes, function recovery and incidence of complications were compared between two groups.RESULTS AND CONCLUSION:(1) The Visual Analogue Scale scores, intraoperative blood loss, radiant times, and hospitalization time in the treatment group were significantly less than those in the control group (P < 0.05).(2) The operation time did not differ significantly between two groups (P > 0.05).(3) The postoperative sagittal Cobb angle, and percentage of anterior height in the vertebral body in the two groups were significantly improved compared with those before surgery (P < 0.05),but all above imaging indexes showed no significant differences between two groups (P > 0.05).The endplate-screw angle in the treatment group was significantly less than that in the control group (P < 0.05).(5) The excellent and good rate of placement in the treatment group was significantly higher than that in the control group (P < 0.05).(6) These results suggest that compared with the traditional open posterior pedicle screw fixation, the photoelectric navigation-aided percutaneous pedicle screw placement exhibits high placement accuracy, less radiant times, less trauma, less blood loss and rapid functional recovery.

Objective To investigate the effect of chronic intermittent hypoxia(CIH)on serum high-density lipoprotein cholesterol(HDL-C)and the changes of HDL-C metabolism-related indicators such as the expressions of peroxisome proliferators-activated receptor a (PPARα)and adenosine triphosphate binding cassette transporterA1(ABCA1)in liver in male SD rats. Methods Obstructive sleep apnea syndrome(OSAHS)-induced CIH rats were randomly allocated into 6 groups:10%CIH-3 weeks,5%CIH-3 weeks,5%CIH-3 weeks +RH(Removal of hypoxia-3 weeks),10%CIH-3 weeks + RH(Removal of hypoxia-3 weeks),control group-3 weeks, and control group-6 weeks.Serum lipids were measured and compared. To observe and compare the liver pathology, the expression levels of PPARα and ABCA1 in liver tissue of CIH rats were detected by immunohistochemical method. Results The levels of serum TC,TG and LDL-C was significantly higher in CIH rats than in control group. The levels of TC,TG and LDL-C were significantly lower in reoxygenation groups than in CIH groups. There was no significant difference between experiment groups and correspondent control groups(all P>0.05).Compared with control group,CIH rats had significantly lower levels of serum HDL-C;Compared with CIH groups, the levels of HDL-C were significantly higher in reoxygenation groups. There was no significant difference between experiment groups and correspondent control groups(all P>0.05).Compared with control group, the expression of PPARα and ABCA1 of CIH group was significantly lower;Compared with CIH groups, the expression of PPARα and ABCA1 was significantly higher in reoxygenation groups; There was no significant difference in the expression of PPARα and ABCA1 between reoxygenation groups and correspondent control groups,in which the oxygen level was recovered to normal. Conclusions The serum HDL-C level was obviously decreased in OSAHS-induced CIH rats, and the decreased serum HDL-C can be effectively improved by reoxygenation intervention.OSAHS-induced CIH may lead to the dyslipidemia through PPARα-ABCA1 pathway, and reoxygenation intervention for three weeks can effectively recover the expression levels of PPARα and ABCA1 to normal levels, which suggests that if CIH is effectively intervented, the change of PPARα and ABCA1 of liver can be reversed, thereby reversing dvslipidemia.

Objective:To investigate the expression of serum galectin-3 in elderly patients with or without atrial fibrillation and to explore its impact on the prognosis of patients with atrial fibrillation. Methods:In this retrospective study, 100 patients aged 60 years or older treated at the Department of Geriatric Cardiovascular Medicine, Second Xiangyang Hospital, Central South University between March 2018 and September 2018 were enrolled.Based on electrocardiogram and previous history of atrial fibrillation, 73 participants were assigned to the atrial fibrillation group and 27 patients with sinus rhythm and no history of atrial fibrillation during the same period served as the control group.Elisa kits were used to measure the expression of galectin-3 in both groups, and echocardiography was used to measure the size of each cardiac chamber in patients. The Mann-Whitney test was used to compare galectin-3 levels between the two groups, and binary logistic regression analysis was used to determine risk factors for atrial fibrillation in the elderly.Kaplan-Meier survival curves and Cox proportional risk regression were used to analyze survival and the relationship between galectin-3 and prognosis. Results:Serum galectin-3 levels of patients in the atrial fibrillation group were higher than in the control group[(395.13±113.24)ng/L vs.(328.53±89.11)ng/L, t=2.626, P<0.01]. The level of galectin 3 in participants aged ≥ 65 years(n=48)was higher than in those aged<65 years(n=52), (414.01±105.03)ng/L vs.(343.11±106.01)ng/L( t=2.626, P<0.01). The galectin-3 level had a positive correlation with age( r=0.40, P<0.01), duration of atrial fibrillation( r=0.224, P<0.05)and C-reactive protein( r=0.305, P<0.01), but no correlation with N-terminal pro-B-type natriuretic peptide, atrial or ventricular size and the score of CHA2DS2-VASc in patients with atrial fibrillation, .Galectin-3 was a risk factor in patients with atrial fibrillation( P<0.01). Galectin-3 levels did not affect survival(log-rank=0.990)or prognosis( P>0.05)in patients with atrial fibrillation. Conclusions:Galectin-3 levels in elderly atrial fibrillation patients are higher than in people without atrial fibrillation and are positively correlated with age, duration of atrial fibrillation, and C-reactive protein.Galectin-3 is a risk factor for atrial fibrillation in elderly patients.

Objective To evaluate the diagnostic efficacy of narrow band imaging (NBI) international colorectal endoscopic (NICE) classification in distinguishing neoplastic from non-neoplastic colorectal polyps during routine clinical practice. Methods A total of 224 lesions detected by white light colonoscopy by non-expert endoscopists were collected in this retrospective study. Each lesion was assessed by NBI and classified by NICE classification. The results were compared with pathological findings from endoscopic or surgical resected specimen. Results Among these 224 polyps, there were 59 of type 1, 159 of type 2 and 6 of type 3 according to NICE classification. There were 58 non-tumorous and 166 tumorous polyps according to pathological diagnosis. The total diagnostic sensitivity, specificity, positive predictive value, negative predictive value and accuracy of NICE classification for colorectal tumor were 91. 6%, 77. 6%, 92. 1%,76. 3%and 87. 9%, respectively.Diagnostic sensitivity and accuracy in big (>10 mm in diameter), small (>5-10 mm in diameter) and mini (≤5 mm in diameter) polyp groups were 100. 0%, 97. 0% and 80. 9%, as well as 95. 7%, 87. 8%, and 83. 3%, respectively. Diagnostic accuracy showed a decreasing tendency on polyp size, without significant difference between the three groups ( P=0. 694). Conclusion NICE classification with non-magnified NBI is effective in distinguishing neoplastic and non-neoplastic colorectal polyps by non-expert endoscopists and is potentially worth popularizing for routine clinical practice.
