1.Change of T lymphocyte subsets and their different apoptosis in acute pancreatitis

Objective To investigate the dynamic change and different apoptosis of T lymphocyte subsets in patients with acute pancreatitis.Methods Forty-four patients with MAP and 32 with SAP were enrolled in this study.Blood samples were collected on the lst,3rd and 7th d after onset.T lymphocyte subsets and their apoptotic rate were determined by flow cytometry.Results Compared with the control group and MAP group,CD4+ T lymphocytes and the ratio of CD4+/CD8+remarkably decreased while the apoptotic rate of CD4+ T cells significantly increased in patients with SAP.However,there was no marked difference in apoptotic rate of CD8+ T cells among different groups.Conclusion The different apoptosis of T lymphocyte subsets in AP,especially in SAP,results in severe and sustained cellular immune suppression.
3.Electroencephalogram diagnosis and biofeedback treatment for the child with attention deficit hyperactivity disorder

BACKGROUND: The concern over the use of electroencephalogram (EEG)diagnosis and biofeedback training as a viable option for children with attention deficit hyperactivity disorder(ADHD) has grown gradually.OBJECTIVE: To evaluate the diagnostic accuracy and therapeutic efficacy of A620 EEG system to ADHD children.DESIGN: Prospective observation and comparative study on ADHD children.SETTING: Psychological department of a municipal hospital.PARTICIPANTS: From October 2003 to May 2004, 113 outpatient children in Psychology Hyperactivity Department of the Central Hospital of Anshan City were selected. Inclusion criteria: 6 to 14-year-old outpatient children in Hyperactivity Department. Exclusion criteria: nervous system organic diseases, pervasive developmental disorder(PDD), mental retardation, epilepsy, psychotic disorder, acoustical and visual abnormalities,etc. Of all the ADHD children, 88 were male and 25 were female, with the mean age of(10 ± 3) years.METHODS: A620 EEG system was applied to examine the 113 participants. According to the Diagnostic Criteria for ADHD in Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition (DSM-Ⅳ), published by American Psychiatry Association, ADHD children were diagnosed and then the EEG diagnostic accuracy and specificity were calculated. The System was also used for biofeedback treatment on 27 ADHD children. Fluctuation of EEG θ/β value was measured both before and after treatment, and the change of hyperactivity index was investigated by Conners Parent Symptom Questionnaire as well.MAIN OUTCOME MEASURES: EEG θ/β value, Conners index of hyperactivity (CIH).RESULTS: The diagnostic sensitivity of EEG on ADHD was 83.58%, and the specificity of it was 82.61%; EEB θ/β value of post-treatment was 8.26 ±4.05, which decreased significantly compared with pre-treatment value (12.38 ± 7.96) ( t = 2. 39, P < 0.05); CIH was also dramatically declined from(24. 73 ± 10. 96) (pre-treatment) to( 12.05 ± 8.63)(post-treatment) ( t= 4.72, P < 0. 01 ).CONCLUSION: EEG can be used as an accessory method in ADHD diagnosis due to its high sensitivity and specificity. Abnormalities of brainwave and hyperactivities of ADHD patients can be greatly ameliorated by EEGbiofeedback treatment.
4.Inhibition of caspase-1-dependent apoptosis suppresses peste des petits ruminants virus replication

Background:
Peste des petits ruminants (PPR), caused by the PPR virus (PPRV), is an acute and fatal contagious disease that mainly infects goats, sheep, and other artiodactyls.Peripheral blood mononuclear cells (PBMCs) are considered the primary innate immune cells.
Objectives:
PBMCs derived from goats were infected with PPRV and analyzed to detect the relationship between PPRV replication and apoptosis or the inflammatory response.
Methods:
Quantitative real-time polymerase chain reaction was used to identify PPRV replication and cytokines expression. Flow cytometry was conducted to detect apoptosis and the differentiation of CD4+ and CD8+T cells after PPRV infection.
Results:
PPRV stimulated the differentiation of CD4+ and CD8+ T cells. In addition, PPRV induced apoptosis in goat PBMCs. Furthermore, apoptosis and the inflammatory response induced by PPRV could be suppressed by Z-VAD-FMK and Z-YVAD-FMK, respectively.Moreover, the virus titer of PPRV was attenuated by inhibiting caspase-1-dependent apoptosis and inflammation.
Conclusions
This study showed that apoptosis and the inflammatory response play an essential role in PPR viral replication in vitro, providing a new mechanism related to the cell host response.
5.Combined high dose leucovorin and 5-fluorouracil continuous infusion for head-neck and digestive tract cancers.

OBJECTIVETo evaluate the feasibility of combined high dose leucovorin plus 5-fluorouracil infusion for head-neck and digestive tract cancers.
METHODSFifty-six patients with head-neck and digestive tract cancer were treated by combined high dose leucovorin (HD-CF) plus 5-fluorouracil (5-Fu) 48 hour continuous infusion with each patient receiving an average of 3.8 cycles (2-6 cycles). Twenty-five of these 56 patients were untreated and 31 recurrent. Their clinical stages were II 4, III 13 and IV 39.
RESULTSThe over all response rate (CR + PR) was 35.7% and the clinical beneficial response rate was 80.4%. The main side effects were peripheral phlebitis, suppression of bone marrow, oral ulcer, nausea and vomiting of grade I to II.
CONCLUSIONHigh dose leucovorin plus 5-fluorouracil 48 hour continuous infusion is useful with favorable cost/utility ratio for head-neck and digestive tract cancer patients. Further studies are warranted.

6.Chaput tuberosity osteotomy and lateral malleolus sagittal osteotomy in treatment of talus osteochondral lesions with autologous osteochondral transplantation

Objective:To evaluate the clinical outcomes of Chaput tuberosity osteotomy and lateral malleolus sagittal osteotomy in the treatment of talus osteochondral lesions with autologous osteochondral transplantation (AOT).Methods:From January 2017 to December 2019, Chaput tuberosity osteotomy and lateral malleolus sagittal osteotomy were performed in the AOT treatment of 11 patients with talus osteochondral lesions. They were 9 men 2 women, with a mean age of 32. 6 years (range, from 22 to 45 years). Their lesions were all Hepple type V. Their American Orthopaedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS), International Knee Documentation Committee Knee Evaluation Form (IKDC) score, and imaging data were measured pre- and post-operatively for assessment of clinical outcomes.Results:All the patients were followed up for 13 to 24 months (average, 15.6 months). Their AOFAS (94.3±2.9) and VAS (1.2±0.4) scores at the final follow-up were significantly improved compared with the pre-operative values (49.5±6.6 and 5.7±1.2) ( P<0.05). There was no significant difference between their pre- and post-operative IKDC scores for the ipsilateral knee joint ( P>0.05). The imaging showed that the talus cystic change disappeared and the grafted bone was fully fused with the talus with no abnormal change in the joint space. Conclusion:In the AOT treatment of talus osteochondral lesions, Chaput tuberosity osteotomy and lateral malleolus sagittal osteotomy can obviously relieve ankle pain and improve ankle function.
7. Articular compression molding techniques for acetabular posterior wall fracture

Objective:
To evaluate the efficacy of articular compression molding techniques in the surgery for acetabular posterior wall fracture.
Methods:
A retrospective study was conducted of the 28 patients (28 hips) with fracture of acetabular posterior wall plus comminuted compression of articular surface who had been treated using the articular compression molding techniques at Department of Orthopedics, The Hospital of 81 Group Army of PLA from January 2014 to January 2018. They were 25 males and 3 females, aged from 26 to 63 years (average, 49.3 years). The time from injury to surgery ranged from 4 to 12 days (average, 7.8 days). According to the Letournel-Judet classification, all were single element (simple) posterior wall fractures combined with posterior dislocation of the femoral head and articular compression of different degrees; transient sciatic nerve injury was complicated in 12 cases. The posterior hip dislocations were timely and successful reset at emergency treatment. The compression fractures of the posterior wall and hip joint were treated by open reduction and internal fixation via the acetabular posterior (K-L) approach. The articular compression was treated by 4 stabilization techniques: absorbable screwing and blocking technique in 10 cases, indwelling screwing in 12 cases, indwelling Kirschner wiring in 2 cases and die push and squeeze tamping in 4 cases. The operation time, intraoperative bleeding and complications were recorded. The therapeutic efficacy was evaluated by the modified Merle d’Aubigne & Postel scoring criteria at the final follow-up.
Results:
The operation time lasted from 76 to 118 minutes (average, 94.2 minutes); the intraoperative bleeding ranged from 100 to 320 mL (average,220.8 mL). No incision liquefaction, infection or other complications occurred. The 28 patients were followed up for 10 to 36 months (average, 17.6 months). According to the Matta scoring criteria, 24 cases achieved anatomical reduction but 4 dissatisfactory reduction. By the modified Merle d’Aubigne & Postel scoring criteria at the final follow-up, the efficacy was evaluated as excellent in 26 cases and as good in 2. Heterotopic ossification was observed in 3 cases and microscopic free dense shadow in the joint cavity (about 2 mm in the round ligament) in 3 cases. There were no cases of obvious traumatic arthritis or osteoarthritis, femoral head necrosis, walking pain, lameness or hip abduction weakness. The 12 patients complicated with preoperative transient sciatic nerve injury recovered within 3 months after surgery.
Conclusion
The articular compression molding techniques can effectively treat severely comminuted articular compression in the acetabular posterior wall fracture and improve reduction of articular surface and hip joint matching, leading to stability and good joint function.
8.Construction and validation of rectal cancer prognostic model by LASSO-Cox regression

Objective To construct and appraise a new model for predicting the prognosis of rectal cancer patients using the Lasso-Cox strategy.Methods The clinical pathological data of 599 rectal cancer patients who underwent radical resection were analyzed.Comparison between groups,Lasso and Cox regression were used to select varia-bles and construct a model,and its discrimination,consistency,and clinical benefits were appraised by the receiv-er operating characteristic(ROC),calibration curve,and decision curve analysis.Results Comparison between groups showed that age,body mass index(BMI),preoperational nutrition status,carbohydrate antigen199(CA199),preoperative chemotherapy,intraoperative blood transfusion,vascular or nerve invasion,cancer nod-ules,pathologic T,N,and TNM stages,tumor recurrence or metastasis,radiotherapy and postoperative survival time were associated with grouping of death or survival in rectal cancer patients.Among them,8 variables were se-lected by lasso and contained into the Cox regression model.Age(HR=1.04,P<0.05),BMI(HR=0.89,P<0.05),blood transfusion(HR=2.29,P<0.05),postoperative chemotherapy(HR=0.16,P<0.01),recur-rence(HR=43.67,P<0.01),and metastasis(HR=2.75,P<0.05)were identified as independent prognostic factors,which were used to construct a nomogram model.The area under the curve(AUC)and the 95%confi-dence interval of the receiver operating characteristic(ROC)curve of the predictive model was 0.95(0.91-0.99),P<0.01.The predicted probability of 1-year and 3-year survival was close to the actual probability.The DCA curve of the model was far away from a decision line parallel to the X-axis and another line with a negative slope.Conclusion The newly established nomogram has good discrimination,consistency and clinical benefits,which help predict the prognosis of rectal cancer after surgery.
9.Open reduction and internal fixation via the para-Achilles tendon approach for the treatment of posterior malleolus sandwich fractures

BACKGROUND:Accurate reduction of ankle fractures under direct vision has become a common understanding among foot and ankle surgeons.How to fully expose free or compressed fragments of the posterior ankle remains one of the most challenging problems. OBJECTIVE:To explore the clinical efficiency of the para-Achilles tendon approach in exposing and repositioning the posterior malleolus sandwich fracture. METHODS:A retrospective study was made for 26 patients with posterior malleolus sandwich fracture treated with open reduction and internal fixation via para-Achilles tendon approach from January to December 2020 in The Second People's Hospital of Dalian.21 of 26 cases were managed with the lateral approach of Achilles tendon and 5 cases were managed with the medial approach of Achilles tendon.There were 19 males and 7 females,with the age of 24-69 years,averagely 38.6 years.The operation time and postoperative complications were recorded.The fracture reduction condition was evaluated by Burwell-Charnley imaging standard.Before operation,3 months after operation and last follow-up,visual analog scale score,ankle plantar flexion and dorsiflexion and American Orthopedic Foot&Ankle Society Ankle Hind Score were used to evaluate the treatment effect. RESULTS AND CONCLUSION:(1)All 26 patients were followed up for an average of 14.6 months(range 13-18 months).The operation time was 52-85 minutes(average 64.2 minutes).(2)Part of the skin edge of the lateral malleolus incision was necrotic in one patient and healed completely after a dressing change.Incision healing of the other patients was in one stage.(3)Through Burwell-Charnley imaging standard,anatomical reductions were obtained in 24 of 26,and good reductions were in 2 of 26,with an excellent and good rate of 100%.(4)At the last follow-up,the visual analog scale score(1.19±0.40)was significantly lower than the preoperative score(6.81±0.80)(P<0.01).Ankle plantar flexion(33.5±5.02)° and ankle plantar back stretch(17.23±0.99)° were significantly increased compared with preoperative data(14.58±2.50)° and(5.81±1.02)°(P<0.01).American Orthopedic Foot&Ankle Society Ankle Hind Score was improved to(89.31±3.62)points compared to preoperative(46.00±5.45)points(P<0.01).The results were excellent in 23 patients and good in 3 patients,with an excellent and good rate of 100%.(5)It is concluded that the open reduction and internal fixation via the para-Achilles tendon approach for the treatment of posterior malleolus sandwich fracture were efficient with decreased operation time,satisfying fracture reduction,fewer complications,satisfactory motion range,and functional recovery.
10.An online survey on iodine deficient disorders knowledge and its control in urban doctors and nurses

Objective To understand the awareness level of iodine deficiency (ID) impairments and the attitude on edible iodized salt,and its consumption among doctors and nurses in cities,in order to provide scientific evidence for health education on iodine deficient disorders (IDD) in the future.Methods The questionnaire was self-designed,and spread through the web page of Wenjuanxing,an online server company,from May 6 to June 6,2017.At the end of the survey,a total of 481 valid questionnaires were reclaimed,in which ratio of gender was female 63.8% (307/481),male 36.2% (174/481).The data were analyzed by SPSS 22.0,including logistic regression analysis with backward according to the statistical significant level of P < 0.05.Results Among doctors and nurses,90.4% (435/481) knew ID impairments;72.8% (350/481) answered intelligent disability as the most serious problem of ID;55.5% (267/481)misunderstood that the areas in which they lived were not ID areas,although all cities surveyed were ID areas;41.0% (197/481) of doctors and nurses misunderstood that coastal residents did not need to consume iodized salt.About the evaluation of iodine nutrition status of current population,15.6% (75/481) of respondents judged as iodine excess.About consuming edible salt,76.3% (367/481) selected iodized,9.6% (46/481)non-iodized,and 14.1% (68/481) both iodized and non-iodized.The results of logistic regression analysis showed the factors that prevents health care workers from choosing iodized salt were:"living in coastal areas","think him or her as iodine adequate","know that iodine deficiency can affect the development of children but still adhere to the consumption of non-iodized salt";the factors that promoted the choice of iodized salt for medical staff were "insist on buying iodized salt,and do not choose non-iodized salt",and 75.8% (238/314)of them knew that intelligent disability as the most serious problem of ID.Conclusions Most doctors and nurses have high level of knowledge on ID and its control.But lack of information,as well as misunderstanding of "coastal areas iodine adequate" and worry about "excess iodine causes thyroid diseases and cancer",which would hinder the active consumption of iodized salt.Health education for them should be conducted through professional ways,stressing on the threaten of ID environment,sharing the information about national and local progress on control of IDD and iodized salt safety,and clarifying the relationship between iodine salt or iodine and thyroid cancer and nodules.
