1.The current status in treatment of severe acute pancreatitis
Chinese Journal of General Surgery 1997;0(04):-
Objective To introduce the current status of treatment of severe acute pancreatitis (SAP). Methods This summarization paper of SAP was made on literature review. Results During the stage of acute reaction , therapy should be emphasized on non surgical management, such as maintaining the homodynamics ; inhibiting the overexpress of cytokines and pancreatic secretion; administering antibiotics preventively, TPN and improving the function of other damaged organs. The operation timing should be delayed if possible. Thus it could increas the cure rate, decrease complications and the possibility of reopeneration. But in the case of SAP with biliary obstruction, secondary infection with necrosis of pancreas; failure to respond to initial intensive treatment and clinical diagnostic was uncertain, early surgery should be performed. The operation should be easy to be done, ensuring enough drainage, and resolving the obstruction of biliary tract. During the septic stage, besides administerded antibiotics and EN, surgery is primary therapeutic method: necrosectomy and continuous closed lavage were considered as a better treatment of infected necrosis focus, while pancreatic absecess should be drained operatively. During the last stage of residual infection, abdominal abscesses should be drained thoroughly. Conculsions According to the etiology and stage of SAP, comprehesive management guided by "individualized plan"is probably a better strategy of treatment of SAP.

Objective To investigate the clinical symptoms and pathogeny of acute myocardial infarction (AMI) in emergency normal coronary angiography ( CAG). Methods From October 2015 to October 2017,the clinical data of 207 AMI patients withemergency CAG in the First People's Hospital of Huainan were retrospectively analyzed, including 7 patients with normal CAG. The clinical symptoms and pathogeny of these patients were analyzed. Results Among the patients with AMI,the patients with normal CAG accounted for 3. 38 % (7 / 207),the average age was (49. 00 ± 11. 94)years old,4 patients were men,3 patients were smokers and 3 patients had hypertension. The results of laboratory examination were cTnI (7. 81 ± 4. 32)μg/ L,myoglobin (231. 43 ± 136. 03)μg/ L,CK - MB (15. 3 ± 4. 63)μg/ L. The pathogeny of these patients were one case with coronarospasm(14. 29 % ),one case with myocardial bridge(14. 29% ),two cases with intracoronary thrombolysis(28. 57% ),two cases with aortic dissection (28. 57% ),and one case with myocarditis(14. 29% ). In addition to aortic dissection in 1 case of death,1 case was transferred outside, the rest of the patients discharged after hospitalization. Conclusion AMI mainly occurres because of vascular thrombosis caused by the unstable coronary atherosclerosis plaque rupture, but some patients with normal CAG can also occure AMI,it has different clinical characteristics and pathogenys. In practice it should be carefully identified in order to improve the cure rate and prognosis.

Pro-inflammatory cytokines selectively induce specific signals to regulate host responses. Since the immunological imbalance caused by abnormal release of cytokines is involved in the development of many diseases, cytokine-specific detections are of great clinical significance for disease diagnosis and efficacy monitoring. Aptamers are single-stranded nucleic acid sequences which can bind to the target with high selectivity and affinity. It has attracted much attention in the development of novel aptamers-based methods for the detection of specific biomarkers of inflammatory diseases. This paper reviewed recent advances in electrochemical and optical aptasensors for the detection of tumor necrosis factor-α, interleukin-6, transforming growth factor-β, interferon-γ and interleukin-8. The findings offered valuable insights into the clinical application potential and challenges of nucleic acid aptamers in the detection of key inflammatory cytokines, providing a meaningful reference for the development of aptamer-based cytokines sensitive detection point-of-care testing equipment and novel diagnostic strategies for inflammatory disorders.

Depression and diabetes are difficult to cure chronic diseases. Both of them are in the clinical presence of the situation. There is a certain biological basis.Based on the analysis of relevant literatures both at home and abroad, this paper briefly reviewed the relationship on the aspect of biological mechanism between depression and diabetes,as well as its clinical treatment status. It provided new ideas for the treatment of two diseases, comorbidity and drug development.
