1.Ibandronate for postmenopausal osteoporosis:aMeta-analysis

BACKGROUND:Alendronate has a marked enhancing effect on bone mineral density. Ibandronate is the newest (the third generation) bisphosphonates. Evidence-based medicine data on the therapeutic effects ofboth twodrugs remain lacking because of different administration routes and time.
OBJECTIVE:To compare the therapeutic effects of ibandronate and alendronate on postmenopausal osteoporosis.
METHODS:A computer-based online search was conducted in CNKI, PubMed, CECDB, BMA, VIP, CQVIP, CBM, EMbase, ASP, The Cochrane Library, and EMCC databases from August 1999 toAugust 2015 to screen the relevant randomized controled trials. Meta-analysis was performed using Stata/SE version 12.0 software by extracting data from the relevant articles.
RESULTS AND CONCLUSION:A total of 10 randomized controlled trials involving 725 patients (ibandronate and alendronate for 379 and 346 patients, respectively) were included. The meta-analysis results showed that bone mineral density was significantly increased (P=0.174, I2=29.4%,WMD=0.03, 95%CI (0.01-0.04))at 6 months after treatment and incidence rate of fracture was significantly decreased(P=5.810,I2=0%,OR=0.32, 95%CI:(0.10-1.00))during 1-year treatment with ibandronate compared with alendronate treatment. However, no significant differences were found between two drug treatmentsin lumbar spine bone mineral density at 1 year after treatment and the incidence rate of fracture during 6-month treatment. Six-month treatment ofibandronate can improve bone mineral density and reduce the incidence rate of fracture of patients withpostmenopausal osteoporosis.
2.Value of multi-slice CT in the dassification diagnosis of hilar cholangiocarcinoma

Objective To evaluate the value of multi-slice CT(MSCT)classification in the assessment of the hilar cholangiocarcinoma resectahility.Methods Thirty patients with surgically and histopathologically proved hilar cholangiocarcinomas who underwent preoperative MSCT and were diagnosed correctly were included in present study.Transverse images and reconstructed MPR images were reviewed for Bismuth-Corlette classification and morphological classification of hilar cholangiocarcinoma.Thcn MSCT classification was compared with findings of surgery and histopathology.Curative resectabilty of different types according to Bismuth-Corlette classification and morphological classification were analyzed with chi-square test.Results In 30 cases,the numbers of Type Ⅰ,Ⅱ,Ⅲa,Ⅲb and Ⅳ according to BismuthCorlette classification were 1,3,4,5 and 17.Seventeen patients underwent curative resections,among which 1,2,1,4 and 9 belonged to Type Ⅰ,Ⅱ,Ⅲa,Ⅲb and Ⅳ respectively.However,there was no significant difference in curative resectability among different types of Bismuth-Corlette classification(X2=0.9875.P>0.05).In present study,the accuracy of MSCT in Bismuth-Corlette classification reached 86.7%(26/30).The numbers of periducatal infiltrating,mass forming and intraductal growing type were 13,13 and 4,while 6,8 and 3 cases of each type underwent curative resections.There was no significant difference in curative resectability among different types of morphological classification(X2=1.2583,P>0.05).The accuracy of MSCT in morphological classification was 100%(30/30)in this study group.Conclusion MSCT can make accurate diagnosis of Bismuth-Corlette classification and morphological classification.which is helpful in preoperative respectability assessment of hilar cholangiocarcinoma.
3.Comparative analysis of ursolic acid in Hawthorn leaves by HPLC

AIM: To develop a simple HPLC for the determination of ursolic acid in Hawthorn leaves, and to compare ursolic acid content in Hawthorn leaves of different species, locations and growth stages, so as to supply some evidences for the exploitation and utilization of Hawthorn leaves reasonably. METHODS: By high-performance liquid chromatography method. Lichrospher C18 column (250 ×4.6 mm I. D. 5 μm); mobile phase, acetonitrile-water-orthophosphoric acid (85: 14.95: 0.05) with a flow-rate of 1.00 ml/min; column temperature at 30 ℃; injection volume, 5μl; UV detector at 210 nm. RESULTS: The detection limit (S/N=3) was less than 4. 024 μg/ml and the limit of quantification( S/N =10) was less than 12.05 μg/ml. The calibration curve showed good linear regression(r =0. 9999) within measurement ranges( 16.09 - 1030 μg/ml). The intra-day and interday variation were 0.71% and 6. 15%, respectively. The recoveries at low to high concentration were 89%-105%. Under these conditions, the ursolic acid content in different Hawthorn leaves were determined: 1.90%-1.95% in C. scabrifolia (Franch.) Rehd, 1.00%-1.45% in C. cuneata Sieb. & Zucc, 0.45%-0.65% in C.pinnatifida Bge. var. major N. E. Br.; In differnet growth stages of C. pinnatifida Bge. var. major N. E. Br. , the young leaves contain higher content of ursolic acid. CONCLUSION: The method is successfully applied to quantify ursolic acid in Hawthorn leaves. And the ursolic acid contents in Hawthorn leaves of differnent species are very different; C. scabrifolia (Franch.) Rehd contains the highest ursolic acid content in them. However, there is a little difference among different locations and growth stages for same species.
4.STS gene in a pedigree with X-linked ichthyosis.

To investigate the gene mutation in a pedigree with X-linked ichthyosis (XLI) and to explore the relationship between the mutation and its clinical manifestations, genomic DNA of affected members, the normal member of the pedigree and 50 unrelated normal members was extracted with a whole blood genomic DNA extraction kit and the DNA was used as a template for the polymerase chain reaction (PCR)-mediated amplification of exon 1 and exon 10 of the STS gene. hHb6 (human hair basic keratin) gene was used as the internal control. Our results showed that the STS gene was deleted in affected members in the pedigree with X-linked ichthyosis. The normal member of the pedigree and 50 unrelated normal members had no such deletion. The proband and his mother had products in the internal control after PCR amplification. The blank control had no product. It is concluded that deletion of the STS gene existed in this pedigree with X-linked ichthyosis, and it is responsible for the unique skin lesions of X-linked ichthyosis.

5.Clinical study on different diffusion-weighted MR imaging sequences for pancreatic cancer at 3.0 T

Objective To investigate the diagnostic value of different DWI sequences for pancreatic cancer on a 3.0 T MR scanner.Methods Different DWI acquisitions based on SE-EPI sequence were preoperatively performed in 30 patients with pancreatic cancer proven by histopathology on a 3.0 T MR scanner.The patients included 17 males and 13 females.Their age ranged from 39 to 72 years with mean of (57.6 ± 14.9) years.The b values were 0 and 600 s/mm2 and the following sequences were included:breath-hold DWI with MPG pulses in X,Y,Z direction (BH600ALL),breath-hold DWI with MPG pulses in Z direction(BH600SI),respiratory-triggered DWI with MPG pulses in X,Y,Z direction (TRIG600ALL),respiratory-triggered DWI with MPG pulses in X,Y,Z direction and inversion recovery for fat saturation (TRIG600ALL+FS) and free-breathing DWI with MPG pulses in X,Y,Z direction and inversion recovery for fat saturation (FB600ALL+ FS).C,CNR and ADC of pancreatic cancer were caculated and compared among different DWI sequences by statistic mathed.Results CNR (F =11.444,P < 0.001) and C (F =5.447,P < 0.05) of pancreatic cancer displayed noticeably statistical difference among BH600ALL、BH600SI、TRIG600ALL、TRIG600ALL + FS and FB600ALL + FS DWI sequences by ANOVA.Among those five different DWI sequences,the highest CRN of pancreatic cancer was observed in TRIG600ALL and TRIG600ALL + FS(the values were 16.45 ± 10.37 and 13.38 ± 9.10 respectively),while the highest C of pancreatic cancer was noticed in TRIG600ALL + FS (0.39 ±0.15).ADC of pancreatic cancer had no statistical difference among BH600ALL,BH600SI,TRIG600ALL,TRIG600ALL + FS and FB600ALL + FS DWI sequences by Kruskal-Wallis test.For BH600ALL and BH600SI,there were no statistical difference among ADC of pancreatic cancer,adjacent pancreatic tissue and distal pancreatitis by ANOVA.While for all TRIG600ALL,TRIG600ALL + FS and FB600ALL + FS,there were statistical difference among ADC of pancreatic cancer,adjacent pancreatic tissue and distal pancreatitis by ANOVA (F values were 5.353,15.976 and 14.556 respectively,P < 0.05).ADC of pancreatic cancer was statistically lower than that of adjacent pancreatic tissue and distal pancrcatitis on all TRIG600ALL,TRIG600ALL + FS and FB600ALL +FS (P < 0.05).Conelusion Compared to other four DWI sequences,TRIG600ALL + FS had higher C and CNR in pancreatic cancer,and measurement of ADC value was helpful to disclose the histopathological state in pancreatic cancer,adjacent pancreatic tissue and distal pancreatitis.
6.Diagnostic value of adrenal CT scan for screening adrenal hypertension in out-patients with hypertension

ObjectiveTo evaluate the priority application of thin adrenal CT scanning in out-patients with hypertension for early diagnosis of adrenal hypertension.MethodsThe patients with hypertension were collected from January to August 2009,in whom the secondary hypertension ( including endocrine and renal hypertension) and malignant cancers were excluded.All patients accepted the adrenal CT scan firstly,and then the patients with the positive result of morphological abnormalities in the adrenal CT( including adenoma and hyperplasia)were evaluated for endocrine function of adrenal glands.Results A total 362 cases of hypertension were studied,in.cluding 162 males and 200 females.The average age was (60.9 ±9.3 ) year-old,the average duration of hypertension was ( 13.8 ±5.3 ) year.After adrenal thin layer scanner CT scan,35 patients ( 9.6% ) with adrenal abnormalities were found.Endocrine functional assessments of adrenal glands were carried out among these patients,17 cases (4.7%) with functional lesions were diagnosed,including 14 cases(3.9% )with primary hyperaldosteronism,2 cases(0.6% )with Cushing's syndrome,and 1 case ( 0.3% ) with pheochromocytoma.Conclusion This study revealed that the percentage of adrenal lesions in outpatients with hypertension was 9.6%.The percentage of adrenal hypertension was 4.7%.Thin layer CT scan seems to effectively screen the adrenal hypertension with adrenal lesions,and would be helpful to the diagnosis of endocrine hypertension.
7.FECH gene mutation in a Chinese family with erythropoietic protoporphyria

Objective To investigate the FECH gene mutation in a Chinese family with erythropoietic protoporphyria, to explore the relationship between gene mutation and clinical manifestations so as to estab-lish a basis for the genetic diagnosis and treatment of erythropoietic protoporphyria. Methods Clinical data on a Chinese family with typical EPP was collected. Peripheral blood was obtained from patients, unaffected individuals in the family and 50 unrelated human controls. Genomic DNA was extracted and PCR was per-formed to amplify the whole coding regions (exons 1 to 11) of FECH gene and their flanking intron sequences followed by direct sequencing to detect possible mutations. Results Based on clinical symptom and por-phyrin levels, a diagnosis of erythropoietic protoporphyria was made in 3 family members. DNA fragments of expected size were amplified by PCR. Gene sequencing revealed a heterozygons mutation (IVS1 + 1G >C) in intron 1 of FECH gene in the proband, his sister and father, but not in unaffected family members or unrelated human controls. Also, an IVS1-23C/T polymorphism associated with low expression alleles was observed in intron 1 of FECH gene of the proband, his sister and mother. Conclusions A novel mutation in the donor splice site of intron 1 of FECH gene is first reported in a Chinese family with EPP; this muta-tion may lead to a deficiency of FECH gene and serve as a molecular basis of development of erythropoietic protoporphyria.
8.Non-ablative skin rejuvenation using Nd: YAG laser and intense pulsed light

Objective To observe the histological change of different waves in treating SD rats of the long-pulse 1064nm Nd:YAG laser and the 560~1200 nm intense pulse light,in order to provide the theory bases of non-ablative rejuvenation.Methods Two waves were used on experimental mice.The dermic thickness and the expression of collagen typesⅠand Ⅲwere detected by HE stain and immunohistochemical methods. Semiquantitative analysis was used to determine the mean of absorbance.Results Thedermal thicknesses and the mean of absorbance of collagen typesⅠandⅢin two different waves were higher than those in common control groups(P<0.05).The effect of Nd:YAG laser groups were higher than IPL groups(P<0.05).The expression of collagen typeⅠwas higher than that of collagen type Ⅲ(P<0.001).Conclusion After Nd:YAG laser or IPL irradiation,the dermal thickness and collagen typesⅠandⅢof SD rats are increased.The effects of Nd:YAG laser are better than those of 560~1 200 nm IPL.The expression of collagen type Ⅲ is obviously more than that of collagen typeⅠin the early,whereas the expression of collagen typeⅠis obviously more than that of collagen type Ⅲin the later.It proves that the mechanism of dermal remodeling of non-ablative skin rejuvenation is mainly correlation with raising range and time of collagen typeⅠ.
9.A correlation study on depression and life events of patients with primary sjogren syndrome

Objective To explore the relationship between life events and depression of patients with primary sjgren's syndrome (PSS).Methods 80 patients with PSS complicating depressive disorder were tested by self-rating depression scale(SDS) and life event scale (LES),and compared with the control group including 80 PSS patients with non-depressive disorder matched with age and sex.Then the correlation between SDS and LES were analyzed.Results The total score of life events (32.72 ± 13.93),the score of negative life events (24.36 ±11.24),the score of SDS(58.60 ± 16.78) and the frequency of life events(91.3%) in the research group were higher than those in control group(P < 0.05),but the positive life events score (7.15 ± 5.84) was lower than that in control group (9.76 ± 7.29),and there was statistically significant difference between the two groups (P < 0.05),there were more negative life events,especially on the aspects of family,work and study in the research group.There was positive correlation among the negative life events score,total score of life events and SDS score in the research group(P< 0.01),meanwhile,negative correlation was revealed between the score of positive life events and SDS score in the research group(P < 0.05).Conclusion Patients with PSS complicating depressive disorder experienced more negative life events which play an important role in depression of patients with PSS.
10.Rare and atypical imaging manifestations of abdominal lesions associated with IgG4-related disease:CT and MRI findings

Objective To investigate rare and atypical imaging manifestations of abdominal lesions associated with IgG4-related disease (IgG4-RD).Methods Forty-one patients with IgG4-RD proved histopathologically or clinically were investigated retrospectively.All the patients underwent precontrast and dynamic enhancement scan of the abdomen.CT was performed in 32 patients and MR imaging in 27 patients (including both of CT and MRI in 18 patients).Whether tissue and organs of the abdomen were involved was observed,especially rare and atypical imaging manifestations including focal pancreatic lesion,pseudocyst and (or) abscess as well as calcification of pancreas,peripheral blood vessels of pancreas involvement and mass-like lesions were existed.Involvement of the organs including liver,gallbladder,spleen,gastrointestinal tract,mesentery and their manifestations were also observed.Results One to four abdominal organs were involved,the numbers of involved organs were as follows:one organ in 13 patients (31.7%),two organs in 12 patients (29.3%),three organs in 13 patients (31.7%),and four organs in 4 patients (7.3%).The most commonly involved organ of the abdomen was the pancreas which was found in 35 patients.Abdominal extrapancreatic lesions were found in 33 patients.Focal involvement of pancreas as a rare manifestation showed in 11 cases (11/35,31.4%).The patterns were focal and multifocal.Rare cases showed pseudocysts (3/35,8.6%) and abscess was found in 1 case.Capsule-like rim was present around the pancreas lesions in 25 cases.Ten cases showed thick and wide rim with unclear boundaries with adjacent blood vessels,and 7 of them showed the peripheral blood vessels surrounded.Bile duct involvement was observed in 26 patients,and gallbladder involvement was observed in only 2 of them.Rare renal involvement patient has mass-like lesion,with hypointensity on T2WI,then showed decreased enhancement and gradually delayed enhancement.One retroperitoneal fibrosis patient also showed mass-like lesion as an atypical and rare manifestation.Three IgG4-related liver inflammatory pseudotumors were found in 2 patients.Lesions showed iso or hypointensity on T2WI,continuously enhancement,with abnormal edge ring.Mesentery involvement was found in 1 patient.Conclusion Several abdominal lesions associated with IgG4-RD have rare and atypical imaging manifestations.There are specific imaging characteristics,which are helpful for most accurate diagnosis of IgG4-RD.
