1.Establishment of standardized treatment demonstration ward for cancer pain and its influence on the management of narcotic drugs

Objective To explore the effect of setting up standardized treatment demonstration ward for cancer pain on the management of narcotic drugs. Methods The study group (from 2015 to 2016 to create the standardized treatment of cancer pain ward) and control group (from 2013 to 2014 to create a standardized pain treatment demonstration unit ) all kinds of narcotic drugs dosage and medication frequency (DDDs), a monthly 30 prescriptions from tumor anesthesia, analysis of prescription writing quality qualified the rate and the rate of unreasonable use. Results The study group of patients with cancer pain drug usage and DDDs, oxycodone dosage rises fast, DDDs way ahead, dosage of morphine increased gradually, morphine dosage gradually decreased, transdermal fentanyl DDDs decreased, pethidine dosage remains low. The research group for the passing rate of prescription writing quality 99.3%, the control group of prescription writing quality qualified rate is 81.1%, the research group of narcotic drug use rate was only 4%, reasonable utilization rate is as high as 96% is not reasonable, unreasonable use of narcotic drugs control group was as high as 18.5%, reasonable utilization rate is only 81.5% (P<0.05). Conclusion The creation of standardized pain treatment demonstration unit narcotic drugs more reasonable structure, can improve the quality of narcotic drugs prescription writing qualified rate, reduce the rate of unreasonable use, reasonable use of pain medicine has positive significance to ensure patient safety and efficacy of.
2.Prevention and Control after Blood Sampling Infection

OBJECTIVE To take effective disinfection and isolation measures to prevent and control infection after blood sampling and to ensure the safety of the blood donors and the health of the recepients.METHODS Effectively and strictly sterilize and isolate the blood sampling environment,the materials,the donors arms,the medical staff fingers etc;and strictly obey the aseptic technique regulations;and strict sense of disinfection.RESULTS No single case of infection occurred since October,2000.CONCLUSIONS Our measures have prevented and controlled infection after blood sampling;and have ensured the safety of donors.An increasing satisfaction is found from among the donors.More and more citizens are becoming active voluntary blood donors.
3.Study on relationship between polymorphism of the estrogen receptor gene and bone mineral density, and bone metabolism

Objective To investigate the relationship between polymorphism of the estrogen receptor(ER) gene and bone mineral density(BMD), and bone metabolism in Chinese postmenopausal healthy women. Methods In 246 postmenopausal healthy women, aged 44-78 years (average 61 years), BMD of lumbar spine and femoral neck, Ward and trochanteric areas were measured by dual energy X ray absorptiometry. Biochemical indexes(serum Ca, P, ALP, PTH, CT)were measured and PvuⅡ and XbaⅠ restriction fragment length polymorphism(RFLP) of ER were analysed by polymerase chain reaction restriction fragment length polymorphism(PCR RFLP). The relationship between polymorphism of the ER gene and BMD, and bone metabolism were evaluated. The RFLP was represented as Pp(PvuⅡ) and Xx(XbaI). Results In the PPxx genotype, Z score values of BMD were significantly lower than those in other genotypes. Conclusion RFLP of ER gene associated with BMD in postmenopausal healthy women and this might explain the cause of postmenopausal osteoporosis in Chinese women.
4.Effect of angiotensin II on phenotypic transformation in vascular adventi-tial fibroblast subpopulations

AIM:To explore the effect of angiotensin II ( Ang II) on phenotypic transformation in adventitial fibroblast subpopulations isolated from rat thoracic aorta .METHODS: Vascular adventitial fibroblasts were individually expanded by the method of cloning rings .The isolated cells were identified by reverse transcription-polymerase chain reac-tion ( RT-PCR) and immunofluorescence staining .The rat vascular adventitial fibroblasts were cultured in vitro and incuba-ted with or without Ang II.Using the methods of fluorescence quantitative polymerase chain reaction (FQ-PCR), immuno-fluorescence staining and Western blot , the expression of α-smooth muscle actin (α-SMA) was detected .RESULTS:The spindle cells and round cells were isolated by the method of cloning rings .From passage 3 to passage 8,α-SMA expression was decreased in the spindle cells , and increased significantly in the round cells .Induced by Ang II , the expression of α-SMA was increased in the 2 cell subpopulations, and that in the round cells was significantly increased (P<0.01).The results of Western blot indicated that Ang II stimulated the subpopulation of the round cells to transfer to myofibroblasts . CONCLUSION:Ang II promotes the phenotypic transformation of adventitial fibroblast subpopulations , and the 2 subpop-ulations may play an important role in vascular remodeling and reparative process .
5.Surgical Treatment of Ventricular Septal Defect with Severe Pulmonary Hypertension in 37 Patients

Objective To summarize the operative experience of ventricular septal defect(VSD) with severe pulmonary hypertension(PH).Methods 37 patients with VSD complicated with severe PH,who underwent surgical repair were treated combinatively in perioperative period including resting,oxygen inhalation,and administration of captoprill and prostaglandin E 1(PGE 1).All of the patients were operated by cardiopulmonary bypass with moderate hypothermia.Arterial oxygen saturation(SaO 2),pulmonary arterial pressure and cardiac function were observed before and after operation,and progression of cardiac function and mortality were followed-up after discharged.Results 2 patients died of low cardiac output syndrome(LCOS) and one patient died of respiratory failure,and the operative mortality was 8 1%.one patient was diad after 8 months and the cardiac function of 33 patients markedly improved.Conclusions Reasonable treatment in perioperative period and holding the indications for the operation strictly are the keys to decrease the mortality of the operation,and SaO 2 could be a simple measurement to identify the indication of operation and the prognosis of the patient with VSD complicated with severe PH.
6.The effects of losartan potassium and simvastatin combination therapy on oxidative stress indicators in diabetic patients on peritoneal dialysis

Objective To explore the effects of losartan potassium and simvastatin combination therapy on oxidative stress indicators in diabetic patients on peritoneal dialysis through 12 weeks observation. Methods Diabetic patients with end-stage nephropathy (n=80) who were treated with continuous ambulatory peritoneal dialysis were randomly divided into two groups:control group who received routine treatment (n=40), treatment group who were given losartan potassium 50 mg, once per day and simvastatin 20 mg, once every night (n=40). HbA1C, Insulin dosage, Oxidative stress indicators(SOD, GSH-PX, MDA and Hcy)were compared between two groups before and after peritoneal dialysis. Results There was no significant difference of HbA1C between the 2 groups before and after treatment(P>0.05). The insulin doses increased be?fore dialysis in both groups after CAPD treatment. It is lower in the treatment group than that in the control group ( P<0.05). The expression levels of GSH-PX and SOD in treatment group were higher while the expressions of Hcy and MDA were lower after treatment. The expressions of GSH-PX and SOD were higher while the expressions of Hcy and MDA were lower in treat?ment group than those in control group when comparing the same time point(P<0.05). GSH-PX expression level was lower while the expressions of MDA and Hcy were higher after dialysis than those before dialysis in control group ( P<0.05). Con?clusion Losartan potassium combined with simvastatin treatment can improve curative effect and oxidative stress indicators in diabetic patients on peritoneal dialysis.
7.The effects of serum C-reactive protein, B-natriuretic peptide and cholinesterase on prognosis in elderly patients with pulmonary infection

Objective To investigate the effects of levels of serum C-reactive protein (CRP), B-natriuretic peptide (BNP) and cholinesterase (ChE) on prognosis of elderly patients with pulmonary infection.Methods Sixty cases aged ≥ 65 years old with lung infection admitted to Departments of Infectious Diseases and Respiration of Hebei Provincial People's Hospital from March 2012 to March 2014 were enrolled. According to the diagnostic criteria of pulmonary infection and the severity score (PSI) of pneumonia, the patients were classified into three groups with different grades of infectious severity (PSI Ⅰ-Ⅲ, PSI Ⅳ, PSI Ⅴ groups), 20 cases in each group; in the same period, 20 elderly healthy people having accepted the physical examination in this hospital were designed as the healthy control group. Furthermore, according to the difference in prognosis, the patients were divided into a survival group (52 cases) and a death group (8 cases). The PSI scores, the levels of white blood cell count (WBC), D-Dimer, CRP, BNP and ChE, mortalities, times of stay in hospital were compared between the PSI Ⅰ-Ⅲ, PSI Ⅳ, PSI Ⅴ groups and the healthy control group as well as between the survival and death groups.Results The periods of stay in hospital of patients with PSI Ⅳ and Ⅴ were much longer than that patients with PSI Ⅰ-Ⅲ (days: 14.7±2.1, 18.3±2.6 vs. 9.2±1.7, both P < 0.05), the degree of increase in time in the PSI Ⅴ group being the most significant. Before treatment, compared with the healthy control group, the PSI score, the levels of WBC, D-dimer, CRP and BNP were significantly increased, the level of ChE was markedly decreased in patients with PSI Ⅱ-Ⅲ, Ⅳ and Ⅴ groups, and along with the rise of PSI score the changes were more remarkable [PSI score: 78.9±13.7, 117.4±20.2, 152.1±34.8 vs. 51.3±7.9; WBC (×109/L): 11.4±1.4, 13.6±1.9, 16.7±2.2 vs. 8.9±1.1; D-Dimer (μg/L): 779.6±46.1, 1 068.2±142.6, 1 846.2±215.0 vs.348.3±31.8; CRP (mg/L): 30.1±3.7, 58.2±6.7, 74.7±9.2 vs. 6.8±0.9; BNP (ng/L): 1 057.4±205.1, 1 976.7±333.7, 2 437.7±468.9 vs. 115.8±21.7; ChE (U/L): 6 260±826, 3 390±437, 1 861±268 vs. 7 804±904, allP < 0.05]. The mortalities of groups PSI Ⅳ and PSI Ⅴwere more than that in group PSI Ⅱ - Ⅲ [10.0% (2/20), 30.0% (6/20) vs. 0]; there was no statistically significant difference in age between the survival group and death group (P > 0.05). Compared with the survival group, the PSI score, the levels of WBC, D-dimer, CRP and BNP after treatment in death group were markedly higher [PSI score: 141.5±23.3 vs. 97.6±18.2; WBC (×109/L): 15.9±1.9 vs. 12.1±1.8; D-Dimer (μg/L): 1 373.9±179.4 vs. 627.4±69.6; CRP (mg/L): 69.8±9.8 vs. 42.6±7.3; BNP (ng/L): 2 745.6±374.9 vs. 1 022.2±171.3; allP < 0.05], the level of ChE was significantly lower (U/L: 2 569±309 vs. 5 692±777,P < 0.05).Conclusion The serum levels of CRP, BNP and ChE in aged patients with pulmonary infection are helpful to the early diagnosis of disease severity, so as they have guiding significance in predicting prognosis.
8.Application of Spinal Ultrasound in Children with Congenital Anomalies of Spine

ObjectiveTo assess the value of spinal ultrasound in children with congenital anomalies of spine. MethodsUltrasound findings of 36 children with congenital anomalies of spine were analyzed and compared with the finding of corresponding MRI or operation. ResultsUltrasound finding was the same as MRI in the anatomy in 31/36 children, in addition, it also could show the pulsatile motion and blood flow signals of conus medullaris. ConclusionSpinal ultrasound can present more other informations for diagnosis for congenital anomalies of spine, especially the blood supply of conus medullaris.
9.Application of Color Doppler Ultrasonography for Superior Mesenteric Artery Diseases in the Aged

Objective To evaluate the value of color Doppler ultrasonography for diagnosis of superior mesenteric artery diseases. Methods78 patients with abdominal pain suspected to be superior mesenteric artery diseases underwent color Doppler ultrasonography. The results were compared with other imaging methods or findings in operation. Results4 patients with superior mesenteric artery embolization coincided with the founding in operation. 18 of the 22 patients suggested with superior mesenteric artery stenoses were confirmed by CTA or DSA. ConclusionColor Doppler ultrasonography can be used in diagnosis of superior mesenteric artery diseases.
10.Mental health and the related factors of Zangzu undergraduates in inland

Objective To explore the state of the mental health and related factors of Zangzu undergraduates in inland.Methods Two inland typical universities were chosen,according to random whole-class sampling principle from different grades,and 370 Zangzu undergraduates in inland were tested with Symptom Checklist 90(SCL-90),Self-Esteem Scale(SES),Simplified Coping Style Questionnaire (SCSQ) and social support scales,and compared with norm and Hanzu undergraduates.Results The scores of the factors of compulsion,interpersonal relation sensitiveness,depression,anxiety,hostility of Zangzu undergraduates in inland in SCL-90 were higher than 2.The rate of their mental problems(the total average score was higher than 2) was 45.3%,among them the rate of mild mental problems was 42.0%,the rate of secondary mental problems was 3.3%,without undergraduates of serious mental problems.Scores of the 9 factors were higher than that of the norm,and differences were significant (P< 0.01).Scores of the 9 factors were higher than that of Hanzu undergraduates,and scores of somatization,compulsion,anxiety,hostility,terror were significantly higher than that of Hanzu undergraduates (P <0.01).Rate of their mild mental problems was significantly higher than that of Hanzu undergraduates(P<0.01).Scores of their self-esteem and positive coping styles were lower than that of Hanzu undergraduates,and differences were significant (P =0.000).The negative coping styles and self-esteem could predict the state of mental health of Zangzu undergraduates in a great extent.Conclusion Zangzu undergraduates in inland have more mild mental problems,which are correlated with their self-esteem and coping styles.
