1.Application of PRVC in laparoscopic surgery

Objective To monitor and compare the breathing mechanics on PC,VC and PRVC during pneumoperitoneum,and to discuss the significance of the clinic use of PRVC.Method Ninety laparoscopic cholecystectomy patients were equally divided into 3 groups (PC,VC,PRVC).Levels of PES,PAWM,PAP,PaCO2,ETCO2,TV MAP and HR were detected before pneumoperitoneum,and at 5,10,15 and 20 minutes postpneumoperitoneum.Results Pneumoperitoneum made three respiratory patterns with different levels of PAWM,PAP,and PES.PES post-pneumoperitoneum in the VC model was obviously higher than that in the PC and PRVC group.At 10 min post-pneumoperitoneum,levels of PaCO2 and ETCO2 increased obviously in the PC and VC group(P < 0.05).Levels of PaCO2 and ETCO2 were increased in the PC group,but TV level post-pneumoperitoneum was significantly lower than that in the other two groups (P < 0.05).Level of PaCO2 and ETCO2 were increased in the PC and VC group post-pneumoperitoneum,along with increases of MAP and HR (P < 0.05).Levels of MAP and HR in the PRVC group post-pneumoperitoneum were significantly lower than those in the PC and VC group (P < 0.05).Conclusion PRVC mode can effectively reduce the increases of pneumoperitoneum-induced PAWM,PAP,PES,without the unusual increase of PaCO2 and ETCO2 during surgeries,contributing to the stability of vital signs of perioperative patients.
2.Pedicle screw fixation combined with artificial bone graft improves bone formation after thoracolumbar fractures

BACKGROUND:Simple pedicle screw fixation for thoracolumbar fractures has better outcomes, but there are some deficiencies, such as poor applicability for severe compression or burst fractures, strong vertebral pain, easy to cause vertebral wound denervation and paraspinal muscle injury, and slow recovery. OBJECTIVE:To investigate the clinical effects of pedicle screw fixation combined with artificial bone graft for the treatment of thoracolumbar fractures. METHODS:A total of 126 patients with thoracolumbar vertebral compression fractures, who had undergone pedicle screw fixation without bone graft (control group,n=62) and with bone graft (test group,n=64) were enroled. The fracture healing, anterior vertebral height ratio, sagittal Cobb angle, and loss rate of vertebral height after 6 months were observed by X-ray in the two groups. RESULTS AND CONCLUSION:Al patients were folowed up, and had complete fracture healing after 12-16 months. The anterior vertebral height and Cobb angle were both improved in the two groups at 1 week after operation (P < 0.01), but there was no difference between the two groups (P > 0.05). New bone formation was observed in the test group at 6 months after operation, and patients were pain-free; but the speed of bone formation was slower in the control group, and patients stil suffered from painful thoracolumbar fractures. There was no difference in the loss of anterior vertebral height and Cobb angle between the two groups (P> 0.05). These findings indicate that pedicle screw fixation combined with artificial bone graft lead to a better recovery in thoracolumbar fractures.
3.Effects of Modified Danshen Decoction on SSAT Activity of Rats with Myocardial Ischemia/Reperfusion Injury

Objective To observe the effects of modifiedDanshen Decoction on spermidine/spermine acetyltransferase (SSAT) /polyamine pathways of SD rats with IRI; To investigate its protective mechanism. Methods The model of IRI was established by ligating left anterior descending coronary artery for 30 min followed by reperfusion for 90 min. The SD rats were randomly divided into the control group, sham-operation group, model group and modifiedDanshen Decoction group, with 10 rats in each group. The myocardial infarction size was measured by using TTC staining. The contents of SSAT were measured by ELISA. The SSAT mRNA and SSAT protein expression level were detected with real-time fluorescent quantitative PCR method and Western blot, respectively. The contents of polyamines (putrescine, spermidine, spermine) in cardiac tissue were detected by HPLC. Results Compared with sham-operation group, the myocardial infarction size, the SSAT content, the SSAT mRNA and SSAT protein expression levels of model group increased significantly, the contents of polyamines decreased significantly, with statistical significance (P<0.01); Compared with model group, the myocardial infarction size of modifiedDanshen Decoction group was significantly reduced, while the SSAT content and SSAT mRNA and protein expression level decreased significantly, the contents of polyamines increased, with statistical significance (P<0.05, P<0.01).ConclusionModifiedDanshen Decoction can adjust the SSAT polyamine pathways and increase polyamine content in cardiomyocytes, and thus play a role of protection of myocardial ischemia-reperfusion injury.
4.Value of endorectal ultrasonography and MRI examination in the preoperative evaluation of T staging and circumferential resection margin of rectal cancer

Objective To investigate the value of endorectal ultrasonography (ERUS) and MRI examination in the preoperative evaluation of T staging and circumferential resection margin (CRM) of rectal cancer.Methods The retrospective cross-sectional study was conducted.The clinicopathological data of 193 patients [122 males and 71 females,age (60± 12)years with the range of 26-90 years] who underwent radical resection of rectal cancer at the First Hospital of Jilin University from May 2016 to January 2018 were collected.All patients underwent ERUS and MRI examination before surgery,total mesorectal excision during surgery and postoperative pathological examination.Postoperative pathological results as the gold standard,the sensitivity,specificity in T staging and the CRM diagnostic coincidence rate of rectal carcinoma by ERUS and MRI examination are evaluated.Observation indicators:(1) evaluation of T staging of rectal cancer by ERUS and MRI examination;(2) evaluation of CRM in rectal cancer by ERUS and MRI examination.Measurement data with normal distribution were represented as Mean±SD.Sensitivity,specificity and coincidence rate were calculated by chi-square test of paired fourfold table.McNemar test was used to compare the coincidence rate of T staging between ERUS and MRI examination.Consistency between CRM measurement by ERUS examination and pathological examination of rectal cancer was conducted by Kappa analysis.Fisher exact probability test was used to compare the coincidence rate of positive CRM between ERUS and MRI examination.Results (1) Evaluation of T staging of rectal cancer by ERUS and MRI examination.The overall coincidence rate of T staging of rectal cancer and coincidence rates of T1,T2,T3,T4 staging by ERUS examination were 74.61% (144/193),93.78% (181/193),80.83% (156/193),79.79% (154/193) and 94.82% (183/193),respectively.The sensitivity ofT1,T2,T3 and T4 staging was 55.56% (10/18),77.50% (31/40),78.46% (102/130),20.00% (1/5),and the specificity was 97.71% (171/175),81.70% (125/153),82.54% (52/63),96.81% (182/188),respectively.The overall coincidence rate of T staging of rectal cancer and coincidence rates of T1,T2,T3,T4 staging by MRI examination were 50.78% (98/193),90.67% (175/193),74.09% (143/193),58.55% (113/193) and 78.24% (151/193),respectively.The sensitivity of T1,T2,T3 and T4 staging was 0 (0/18),17.50% (7/40),68.46% (89/130),40.00% (2/5),and the specificity was 100.00% (175/175),88.89% (136/153),38.10% (24/63),79.26% (149/188),respectively.There was statistically significant difference between the overall coincidence rate of ERUS and MRI examination for T staging of rectal cancer (x2 =8.631,P<0.05).(2) Evaluation of CRM in rectal cancer by ERUS and MRI examination.The sensitivity and specificity of positive CRM evaluation of rectal cancer by ERUS examination were 100.00% (5/5) and 97.34% (183/188) respectively,and the coincidence rate with results of pathological examination was 97.41% (188/193),showing a high consistency between positive CRM evaluation of rectal cancer by ERUS examination and pathological examination (Kappa value =0.655,P < 0.05).The sensitivity and specificity of positive CRM evaluation of rectal cancer by MRI examination were 40.00% (2/5) and 92.02% (173/188),and the coincidence rate with pathological examination was 90.67% (175/193),respectively,showing a high consistency between positive CRM evaluation of rectal cancer by MRI and pathological examination (Kappa value =0.206,P<0.05).There were statistically significant differences in the diagnostic coincidence rate and specificity of CRM positive evaluation for rectal cancer between ERUS and MRI examination (x2 =5.896,P<0.05).Conclusion ERUS examination has a high coincidence rate in the preoperative T staging of rectal cancer and a high consistency between positive CRM evaluation of rectal cancer with pathological examination,which are superior to MRI examination in the two aspects.
5.The use of aspirin for primary prophylaxis in patients with non-variceal upper gastrointestinal bleeding may be more benefit

Objective To compare the outcomes of patients with non-variceal upper gastrointestinal bleeding (NVUGIB) taking aspirin for primary prophylaxis to those not taking it.Methods Patients without any vascular disease (coronary artery or cerebrovascular disease) who were admitted to the 89th Hospital of PLA between 2006 and 2016 with non-variceal upper gastrointestinal bleeding were included.The frequencies of in-hospital mortality,re-bleeding,severe bleeding,need for surgery or embolization,and of a composite outcome of the 4 bleeding related adverse outcomes were compared between patients receiving aspirin and those on no antithrombotics.We also compared the frequency of in hospital complications and length of hospital stay between the two groups.Results Of the 179 eligible patients,47 were on aspirin and 132 patients were on no antithrombotics (control group).Patients in the aspirin group (58.4 years) were elder than that in controls (66.8 years) (P <0.01).Patients in the aspirin group had significantly more co-morbidities,including diabetes mellitus [25.5 % (12/47) vs 12.1% (16/132)] and hypertension [46.8% (22/47) vs 28.0% (37/132)],respectively,as well as dyslipidemia [21.3% (10/47) vs 6.1% (8/132)] (P < 0.01).Smoking was more frequent in the aspirin group [36.2% (17/47) vs 22.7% (30/132),P < 0.05].The frequencies of endoscopic therapy and surgery were similar in both groups,while the frequencies of in-hospital complications and re-bleeding were similar in the two groups,too.However,the patients who were on aspirin had lower in hospital mortality rates (2.1% vs 13.6%,P <0.01),shorter hospital stay (4.9 d vs 7.0 d,P <0.05),and fewer composite outcomes (10.6% vs 24.2%,P < 0.05).Conclusions Patients who present with NVUGIB while receiving aspirin for primary prophylaxis had fewer adverse outcomes.Thus aspirin may have a protective effect beyond its cardiovascular benefits.
6.Value of application of combined ultrasound and spinal CT/MRI-assisted lumbar puncture in spinal muscular atrophy patients with spinal deformities

Objective:To investigate the value of application of combined ultrasound and spinal CT/MRI-assisted lumbar puncture in spinal muscular atrophy (SMA) patients with spinal deformities.Methods:Twelve SMA patients with spinal deformities were evaluated for combined ultrasound and spinal CT/MRI imaging-assisted lumbar intrathecal administration of nusinersen between November 2021 and September 2022.The degree of difficulty of lumbar puncture in SMA patients was graded according to the spinal imaging, and then different puncture methods were designed according to different puncture difficulty grades.The success rate, preparation time, operation time, the number of attempts, and complications of lumbar puncture were counted.Results:In this study, 48 lumbar punctures were performed in 12 patients with SMA, with a success rate of 100%.The average preparation time for puncture was 13.60 min, the average puncture time was 4.96 min, and the average number of attempts was 1.33. No complications such as injury to the spinal cord, nerve and viscera were found in all patients.Conclusions:Combined ultrasound and spinal CT/MRI-assisted lumbar puncture has high value when applied for SMA patients with spinal deformities.
