1.Application value of one haft layer pancreaticojejunostomy with the posterior wall of pancreas reinforced in pancreaticoduodenectomy

Objective To explore the application value and clinical efficacy of one half layer pancreaticojejunostomy with the posterior wall of pancreas reinforced in pancreaticoduodenectomy.Methods The retrospective cross-sectional study was conducted.The clinical data of 17 patients with pancreatic neoplasms and ampullar neoplasms who underwent pancreaticoduodenectomy at the Second Affiliated Hospital of Harbin Medical University from May to September 2015 were collected.One half layer pancreaticojejunostomy with the posterior wall of pancreas reinforced method was applied to the digestive tract reconstruction after pancreaticoduodenectomy in the 17 patients.Observation indicators included:(1)surgical situations:surgical procedures,operation time,time of pancreaticojejunostomy,volume of intraoperative blood loss,tumor sizes,(2) postoperative situations:recovery time of gastrointestinal function,postoperative complications,duration of postoperative hospital stay,(3) postoperative pathological examinations,(4) follow-up.Patients were followed up by outpatient examinations including color Doppler ultrasound or abdominal computed tomography (CT) and telephone interview detecting abdominal pain or distention and general situations (diet,sleep) up to October 2015.Measurement data were represented as average (range).Results (1) Surgical situations:all the 17 patients underwent successful operations without perioperative death,including 16 undergoing radical pancreaticoduodenectomy and 1 undergoing pancreaticoduodenectomy and left liver resection.The average operation time,average pancreaticojejunostomy time,average volume of intraoperative blood loss and average tumor size were 276 minutes (range,230-440 minutes),12 minutes (range,9-16 minutes),310 mL (range,200-950 mL) and 3.25 cm2(range,1.92-5.60 cm2),respectively.(2) Postoperative situations:the average recovery time of gastrointestinal function was 3 days (range,1-7 days).Three patients had postoperative complications,including 1 patient with pancreatic fistula (Grade A) and 2 patients with delayed gastric emptying,and all of them had been healed after symptomatic and supportive treatments.The results of T-tube cholangiography or CT before hospital discharge showed that there was no leakage around the anastomoses.The average duration of postoperative hospital stay was 10 days(range,6-20 days).(3) The postoperative pathological examinations showed 5 patients of pancreatic ductal adenocarcinomas,4 of common bile duct ampulla area adenocarcinomas,3 of duodenal papillary adenocarcinomas,3 of pancreatic intraductal papillary mucinous neoplasms and 2 of duodenal ampullary adenocarcinomas.(4) Followup:all the 17 patients were followed up for 1-4 months and the abdominal color Doppler ultrasound or CT showed that there was no evidence of tumor recurrence or leakage around anastomoses.Conclusion One half layer pancreaticojejunostomy with the posterior wall of pancreas reinforced is safe and feasible,and it can reduce the rate of pancreatic fistula successfully.
2.Advance in studies on anti-inflammatory and immunoregulatory monomers of Tripterygium wilfordii.

Tripterygium wilfordii has complex chemical components. To study and summarize the advance in studies on the anti-inflammatory and immunoregulatory activities and toxicology of known monomers of T. wilfordii, the pertinent literatures related to the studies on the pharmacology, toxicology and pharmacokinetics of T. wilfordii over past 30 years were searched. According to the findings, more than ten anti-inflammatory and immunoregulatory monomers were found in T. wilfordii. The pharmacology and toxicology of wilforidine, triptolidenol, triptonide, demethylzeylasteral shall be further studied.

3.Diagnosis and treatment progress of pancreatic neuroendocrine tumors

Neuroendocrine tumors (NETs) is a rare and heterogeneous group of tumors with widely varying morphologies and behaviors. Due to their rarity and heterogeneity, progress in improving its treatment has been slow. Pancreatic neuroendocrine tumors (pNETs) is a subset of NETs, previously known as islet cell tumors, occupies 3% of the primary pancreatic tumors with the annual incidence rate of (1-2)/100 000. In recent years, it is very necessary to improve the diagnosis and treatment of pNETs.
4.Initial radiographic manifestations in patients with SARS.

OBJECTIVETo observe initial radiographic manifestations and evaluate its value for the diagnosis and differential diagnosis in patients with severe acute respiratory syndrome (SARS).
METHODSInitial chest radiographs of 79 patients, 15 to 69 years old (mean age 34.1 years, 40 men and 39 women), were retrospectively reviewed in the study. For all patients, average temperature was 38.8 degrees C and mean time from onset of the fever to the time of initial radiographs taken was 5.8 days. Location, abnormal distribution, appearance of lung abnormalities, effusion of pericadiaum and pleura, and lymphadenopathy were observed. Abnormal distribution and associated findings were compared. Differences of mean age, temperature, and the time were statistically analyzed between patients with and without abnormal findings.
RESULTSInitial chest radiographs were normal in 36 of 79 patients (45.5%). Abnormal radiographs in 43 patients (81.3%) showed exudative changes. No significant difference of mean age and day (form onset of the fever to the time of initial radiographs taken) was found between the patients with and without abnormal initial chest radiographs (P > 0.05). No lymphadenopathy, effusion of pericadiaum and pleura were found on the initial chest radiographs.
CONCLUSIONSThe main abnormal radiographic appearance of lung in patients with SARS is exudative changes. SARS patients with normal initial chest radiographs are not uncommon and abnormal radiographic findings may appear to be late after onset of fever. Therefore, more attention should be paid in the clinical diagnosis of SARS.

5.Epidemiological feature of metabolic syndrome in Shanghai residents aged 15 - 74 years.

OBJECTIVETo assess the prevalence of the metabolic syndrome in Shanghai residents.
METHODSA cross-sectional study was conducted in Shanghai with a representative sample of 14,327 Chinese adults from 15 to 74 years of age. The sample was randomly selected using stratified cluster sampling. The criteria of International Diabetes Federation (IDF), the US National Cholesterol Education Program Adult Treatment Panel III (ATP III) guidelines and the China Diabetes Society (CDS), respectively, were applied for diagnosis.
RESULTSDiagnosed by using the IDF criterion, the crude prevalence of metabolic syndrome was 17.51%, and the age and gender-adjusted prevalence was 12.81% (95% CI: 12.26 - 13.36). The prevalence of the metabolic syndrome was higher in females than in males (14.79% and 10.93%, respectively), and higher in urban residents than in rural ones (13.71% and 10.72%, respectively). The age-specific prevalence of the metabolic syndrome increased in both male and female residents and the prevalence of metabolic syndrome in females 45 years of age or elder was significantly increased. The prevalence of having at least 3 parameters of metabolic syndrome was 22.28%. According to the ATP III guideline, the crude and age-adjusted prevalence of metabolic syndrome was 12.03% and 8.62%, respectively. Based on the criterion of CDS, they were 13.98% and 10.41%.
CONCLUSIONThere is a high prevalence of metabolic syndrome in Shanghai residents. Metabolic syndrome has become a noteworthy public health problem. It suggests that community-integrated control of metabolic syndrome should have its priority.

6.Dynamic changes of T lymphocyte subsets in the long-term follow-up of severe acute respiratory syndrome patients.

OBJECTIVETo study the dynamic changes of T lymphocyte subsets in severe acute respiratory syndrome (SARS) patients.
METHODSSequential anti-coagulated blood samples were collected from 62 seropositive SARS patients during the first week, the second week, the first month, the 2nd-3rd month, and 1 year after disease onset. T-lymphocyte subsets including CD4 + T cells, CD8 + T cells, and naive and memory CD4 + T cells were detected by flow cytometry. Samples from 56 healthy blood donors were also detected as healthy controls.
RESULTSA rapid restoration in T-lymphocyte subsets was observed during a short period after infection. During the 2-3 months after disease onset, CD8 + T cell count returned to the normal level. At the same time point, CD4 + T cell and naive CD4 + T cell counts increased to (534 +/- 197)/mm3 and (175 +/- 75)/mm3 respectively. Total lymphocytes, T lymphocyte, CD4 + T and naive CD4 + T cell were still significantly lower than the normal levels even one year after disease onset.
CONCLUSIONThe periphery T lymphocyte subsets in SARS infection shows transient decrease and then rapid recovery.

7.Change in body compositions in female patients with human immunodeficiency virus related lipodystrophy syndrome.

OBJECTIVETo study the changes of body composition in females patients with human immunodeficiency virus (HIV)-related lipodystrophy (LD) syndrome (HIV-LD).
METHODSTotally 25 female patients who were treated in our hospital from January 2002 to December 2009 were divided into LD group and non-LD group based on the existence of LD. All these patients were receiving highly active antiretroviral therapy (HAART). In addition, 12 healthy women were set as the controls. Total and regional body composition were measured by dual X-ray absorptiometry in all three groups.
RESULTSThe fat mass (FM) was correlated negatively with the duration of HAART (r=-0.431, P=0.029). Multiple linear regression analysis showed that FM had positive correlation with weight and negative correlation with lean mass (LM) (r = - 0. 973, P =0. 000). Total, trunk and leg FM were significantly lower in LD patients than that in controls (P <0.05).Meanwhile, total, trunk and leg bone mineral contents were statistically lower in LD patients than that in controls (P <0. 05). Lumbar bone mineral density of LD patients was lower than that of non-LD patients and controls, and there was significant difference between LD patients and controls (P = 0. 001). LM of LD patients was higher than that of non-LD patients but without statistical difference (P > 0. 05).
CONCLUSIONSThe peripheral and central FM and bone mineral contents remarkably decrease in female patients with HIV-LD. How-ever, HIV-LD patients tend to have higher LM than non-LD patients. .

8.Clinical research of vestibular autorotation test for patients with vertebrobasilar insufficiency.

OBJECTIVETo explore the diagnostic values of vestibular autorotation test (VAT) for patients with vertebrobasilar insufficiency (VBI).
METHODSVAT and videonystagmography ( VNG) were performed on 73 patients with VBI and 48 patients with peripheral vestibular lesions (contrast group). Parameters analyzed included Gain, phase and asymmetry of VAT, as well as the canal paresis (CP) of caloric test and results of optokinetic-pursuit tests in VNG. Positive result of the test could be defined if anyone of the parameters was abnormal.
RESULTSFor VAT test, Gain was enhanced in VBI group and was reduced in contrast group. In VBI group and contrast group, Gain enhanced showed in 47 (64.4%) cases and 5 (10.4%) cases, respectively (chi2 = 31.19, P < 0.01). Simultaneity, Gain reduced in 11 cases (15.5%) and 22 cases (45.8%), respectively (chi2 = 13.82, P < 0.01). But there was no statistics significant for results of the parameters of phase, asymmetry and integration between two groups. For VNG test, results with optokinetic-pursuit tests were more abnormal in VBI group than that in contrast group, which showed central lesions characteristics. Forty-four cases (60.3%) in VBI group and 10 cases (20.8%) in control group showed central lesions results with optokinetic-pursuit tests and visual fixation test (chi2 = 15.89, P < 0.01). Unilateral or bilateral CP showed in 33 cases (68.6%) in control group and 51 cases (69.9%) in VBI group with caloric test.
CONCLUSIONSGain of VAT is mostly enhanced in VBI group, and Gain as a main characteristic is reduced in patients with peripheral vestibular lesions. The Gain parameter is availability for assessing characteristics of vestibular lesions. Phase and asymmetry can be used to assess the vestibular function but can not indicate the characteristics of vestibular lesions.

9.Calpain I inhibition prevents pacing-induced structural remodeling for atrial fibrillation in canine.

OBJECTIVETo study the relation of the structural remodeling processes and activation of calpain I.
METHODSFifteen dogs were randomly divided into three groups. The dogs in pacing group (n=5) and inhibitor group (n=5) were subjected to 3 weeks of rapid atrial pacing at 600 beats/min, control dogs (n=5) were in sham-operated group. The dogs in inhibitor group were administered intravenous N-Acetyl-Leu-Leu-Met (ALLM), a calpain inhibitor, and in pacing group and sham-operated group were administered intravenous DMSO. The activity of calpain I was measured by hydrolyzing Suc-Leu-Leu-Val-Tyr-7-amino-4-methyl-coumarin. The ultrastructure of atrium was examined by light and electron microscopy. TnT expression was assessed by Western blot. Echocardiography examination was performed in all the three groups.
RESULTSCalpain I activity was significantly increased in pacing group (2.3-fold, P<0.01), and decreased in inhibitor group (1.1-fold, P>0.05), compared to sham-operated group respectively. The percentages of myolysis were (76.7 +/- 5.9)% and (20.8 +/- 8.1)% in pacing group and inhibitor group respectively (P<0.01). TnT expression decreased in the rapid pacing-induced persistent atrial fibrillation, and these effects were inhibited by calpain I inhibitor ALLM. The area and volume of left atrium tended to increase after 3 weeks ALLM treatment in inhibitor group, but the change was not as prominent as in pacing group (P<0.05).
CONCLUSIONSALLM can decrease calpain I activity, and prevent canine atrial cardiomyocyte structural remodeling during atrial fibrillation. This study provided a capacity of atrial cardiomyocyte protection.

10.A new chromone glycoside from Cassia siamea lam.

AIMTo study the chemical constituents of the stem of Cassia siamea.
METHODSThe compounds were isolated by chromatography on silica gel, and identified on the basis of spectral analysis.
RESULTSFive compounds were isolated and identified as: beta-sitosterol (I), sucrose (II), n-octacosanol (III), 2-methyl-5-(2'-hydroxypropyl)-7-hydroxy-chromone-2'-O-beta-D-glucopyranoside (IV) and piceatannol (V).
CONCLUSIONCompound IV is a new compound. Compounds II, III and V were obtained from this plant for the first time.
