1.Analysis of the clinical effect of ischemic heart valve disease combined coronary heart disease with surgical operation in the same period

Objective To study the clinical effect of ischemic heart valve disease combined coronary heart disease with surgical operation in the same period?Methods The clinical data of 31 cases patients of ischemic heart valve disease combined with coronary heart disease in Xinjiang Uygur Autonomous Region People′s Hospital from July 2013 to January 2015 were retrospective analyzed?According to the degree of cardiac function and symptoms,the patients toke the coronary artery bypass grafting treatment,combined with the actual operation of mitral valve replacement, mitral valve replacement, aortic valve replacement, double valve replacement at the same time?Postoperative follow?up for half year,the cardiac function of patients before and after operation were analyzed, the incidence of postoperative complications after the operation were record?Results All patients were treated by coronary artery bypass grafting, combined with the mitral valve replacement,mitral valve replacement, aortic valve replacement, double valve replacement for treatment during this period, the operations were success?There was statistically significant difference about cardiac functional grading before and after surgery ( P=0?000 )?There were 7 cases patients' left ventricular ejection ejection fraction( LVEF) was more than or equal to 40% of patients,24 cases patients'LVEF<40% before operation;21 cases patients'LVEF was more than or equal to 40%,8 cases patients'LVEF<40% after operation,the difference was statistical significance before and after the operation( P=0?000)?Only 2 patients were died because of low cardiac blood?Conclusion Non ischemic heart valve disease combined with coronary heart disease patients to take simultaneous surgical treatment, the difficulty of its operation has increased, but to strengthen myocardial protection during surgery can effectively enhance the clinical curative effect, reduce mortality, provide good protection for the survival of patients with late life.
2.The relationship between serum pregnancy associated plasma protein A level and pregnancy outcomes in early pregnancy

Objective To explore the relationship between pregnancy associated plasma protein A (PAPP-A) and threatened abortion,whether low level of PAPP-A in early pregnancy can be used as indicators to predict the pregnancy outcomes.Methods The level of serum PAPP-A was measured by double antibody sandwich enzyme-linked immunosorbent assay in 110 cases with threatened abortion (threatened abortion group) and 131 cases with normal intrauterine pregnancy (control group),and track to 20 weeks pregnant.Results The serum PAPP-A multiple of median (MOM) value was 1.02 ± 0.15 in control group,0.98 ± 0.17 in threatened abortion group,and there was no significant difference(P > 0.05).In threatened abortion group,90 cases of spuc success,the serum PAPP-A MOM value was 1.03 ± 0.11,20 patients of spuc failure,the serum PAPP-A MOM value was 0.73±0.21,and there was significant difference (P < 0.01).There was no significant difference in the serum PAPP-A MOM value between control group and the spuc success of threatened abortion group (P > 0.05).Conclusion Pregnant women with PAPP-A levels embryonic development is closely related to the good,as one can predict miscarriage in pregnant women with threatened abortion outcome evaluation.
3.Clinicopathologic analysis of 40 cases of ocular malignant melanoma

Objective To analyze clinical and pathological characteristics of ocular malignant melanoma.Methods The tumor tissue specimens of 40 ocular malignant melanoma patients were given HMB-45,S-100 monoclonal antibody HE and immunohistochemical staining,and the general information of patients,tumor location,histologic type and prognosis were also analyzed.Results In the 40 patients,25 cases occurred in the uvea,ocular surface tissue in 8 cases,orbital secondary tumors in 4 cases,2 cases of eyelid,lacrimal sac in 1 case.Pathological typing:shuttle A cell type in 13 cases,B 12 cases,spindle cell type mixed cell type in 10 cases,epithelial cell type in 2 cases,3 cases of other types.In 40 patients with S-100 showed strong positive.HMB-45 was strongly positive in 20 cases,moderate positive in 18 cases,weakly positive in 2 cases.All patients were followed up for 2 years,the pathological type of diffusion or rewarming rate:shuttle A cell type 7.7%(1/13),shuttle B cell type 0.0%,mixed cell type 10%(1/10),epithelial cells of type 50%(1/2),other types 33.3%(1/3),other types and epithelial type tumor proliferation rate or relapse rate were significantly higher than other pathological types(x2=12.46,P<0.05).<50 years age group 28 cases,aged 50 years in 12 cases,the difference between the two groups was statistically significant(x2=12.80,P<0.05);male 22 cases,female 18 cases,the difference was not statistically significant(x2=0.80,P>0.05).Conclusion ocular malignant melanoma occurred in uveal and ocular surface tissue,spindle cell type is the most common,other types and epithelial cell type melanomas diffusion rate or a high recurrence rate,the disease with high degree of malignancy,should pay attention to its early diagnosis and treatment.
6.Protective effects of 9-(4-ethoxycarboxylyphenoxy)-6,7-dimethoxy-1,2,3,4- tetrahydro acridine on anoxia and ischemic injury in cultured PC12 cells

Aim To investigate the effects of EDT on anoxia and ischemic injury in cultured PC12 cells. Methods Cultured PC12 cells were treated with 1 mmol?L -1 Na 2S 2O 4 and 20 mmol?L -1 NaCN in combination with glucose deprivation. The protective effects of EDT on these two models were evaluated by lactate dehydrogenate (LDH) efflux assay and colormetric MTT assay.ResultsEDT, within the range of 10 -8~ 10 -6 mol?L -1, significantly antagonized LDH efflux induced by two models and increased the optical density at 570 nm tested by colorimetric MTT assay in concentration-dependent manner. 10 -6 mol?L -1 EDT might time-dependently inhibit two injuries and reach maximal level at 48 h. Conclusion EDT can protect PC12 cells from anoxia and ischemic injury.
7.One-staged release and reduction by posterior approach to treated basilar invagination with irreducible atlantoaxial dislocation

Objective To evaluate the safety and effectiveness of one stage surgery of release and bone reduction by posterior approach to treat basilar invagination (BI) with irreducible atlantoaxial dislocation (IAAD),and to explore the indications and crucial techniques of posterior approach.Methods All of 17 Consecutive patients (8 males and 9 females) with BI and IAAD who underwent release and reduction by posterior approach from July 2000 to June 2015 were enrolled in the present study,the mean age was 35.2±13.8 years with a range of 12-56 years.The clinical symptoms and signs was recorded,and preoperative imaging examination,including anteroposterior,lateral,dynamic films,MRI and CT of cervical spine,were performed to identify the series.There were 14 cases with atlanto-occipital fusion,7 cases with C2,3 fusion,6 cases with Chiari malformation,6 cases with Syringomyelia,and 8 cases with myelomalacia.The clinic symptoms include occiput/neck pain in 15 cases,cervical movement limitation in 13 cases,short neck in 9 cases,torticollis in 12 cases,Paresthesia in 14 cases,weakness in 13 cases,tendon reflexes hyperfunction in 16 cases and ataxia in 13 cases.The postoperative X-rays,MRI or CT were used to observed the results of decompression,fixation and fusion.Neurological function was assessed by JOA scale and Ranawat's score before,after surgery and at final follow-up.Pre-and post-operative Chamberlain (CL),Wackenheim (WL),McGae (ML),atlantodental interval (ADI) and cervico-medullary angle (CMA) were analyzed by student t-test.Results The average operation time was 145 mins (90-210 mins) and blood loss was 175 ml (150-350 ml).The average follow-up was 44.47 months (9-94 months).JOA score was increased from 8.06 preoperatively to 15.20 postoperatively,the improvement rate was 77.2%.Preoperative Ranawat's score was Ⅱ in 1 case,Ⅲla in 12 cases,ⅢB in4 cases.Postoperative score was Ⅰ in 13 cases,Ⅱ in 4 cases.The preoperative CL,WL,ML,ADI and CMA were (12.52±5.17) mm,(6.59±3.04) mm,(6.96±4.32) mm,(9.88± 1.93) mm,115.35°± 12.40°,respectively.and the postoperative CL,WL,ML,ADI and CMA were (2.0±3.67) mm,(-3.06±1.85) mm,(-1.76±2.88) mm,(1.17± 1.18) mm,136.76°±11.44°,respectively.The perioperative complications were discovered in 2 cases,including 1 case of infection and1 case of cerebrospinal fluid(CSF) leakage.Conclusion Primary surgery of nerve release and bone reduction by posterior approach may be safe and efficient for the treatment of BI and IAAD.Preoperative evaluation,proper surgical indications and advanced surgical techniques are important for treatment results.
8.Curative effect analysis on proximal frmoral nail antirotation for the treatment of femoral intertrochanteric fracture and integrity of lateral trochanteric wall.

OBJECTIVETo explore clinical efficacy and key matters for the treatment of femoral intertrochanteric fracture and integrity of lateral trochanteric wall by proximal frmoral nail antirotation (PFNA).
METHODSFrom June 2010 to December 2012,210 femoral intertrochanteric fracture patients treated with PFNA were retrospectively analyzed, including 76 males and 134 females aged from 46 to 96 years old with an average of 71 years old. All fracture were caused by injury and classified to type I (5 cases) type II (16 cases), type III (73 cases) and type IV (116 cases) according to Evans classification. The time of getting out of bed, postoperative complications and displacement of screw blade and fracture healing were observed, Baumgaertner criteria were used to evaluate quality of fracture reduction, Harris criteria were used to evaulate hip joint function.
RESULTSAll incisions were healed at stage I, no complications occurred except incomplete of lateral trochanteric wall patients without reconstruction, other patients could get out of bed with crutches at one week and all patients discharged from hospital at 10 days after operation. One hundred and seventy-eight patients were followed up from 3 to 17 months with an average of 10 months. One case occurred unhealed fracture displacement caused by screw blade cutting, 2 cases occurred screw blade transfomed to proximal and out femoral head, other patients obtained fracture healing at 12 to 16 weeks after operation. According to Baumgaertner criteria, 130 cases obtained good results, 45 cases acceptable, and 3 poor; while 107 cases obtained excellent results, 65 good, 3 good and 3 poor according to Harris score.
CONCLUSIONPFNA with mechanical advantage of intramedullary fixation has advantsges of stable fixation, shorter operation time, minimally invasive. Satisfied clinical effects could obtained by grasping fixation principle, dealing with negative factors in operation. Intraoperative reconstruction for integrity of lateral trochanteric wall could assure stable fixation and earlier get out of bed.
