Objective To compare the differences and consistency of three criteria of metabolic syndrome(MS)including the third Report of the Adult Treatment Panel Ⅲ of National Cholesterol Education Program(NCEP-ATPⅢ),Chinese Diabetes Society(CDS)and International Diabetes Federation(IDF)used for evaluating in children and adolescents.Methods Four hundred and thirty-two overweight or obesity children and adolescents from Chuansha district of Pudong New Area of Shanghai were selected.Data of investigation of the risk factors of cardiovascular disease were reviewed.Anthropometric measurements and fast plasma glucose(FPG),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)and dynamic blood pressure monitoring were performed.MS were identified according to NCEP-ATPⅢ criteria modified by Cook,CDS criteria modified by Liang,and IDF criteria,respectively.The consistency of the 3 criteria were studied.Results The number of subjects who met the criteria of MS were 31.0%,9.5% and 25.7% using Cook,Liang and IDF criteria,respectively.The conformity rate between Cook and Liang,Cook and IDF,Liang and IDF were 78.5%,89.1% and 83.8%,and the Kappa value were 0.378 20,0.719 18 and 0.465 37,respectively.Conclusions There are differences in consistency among the 3 criteria of MS.The agreement in the diagnosis of MS using Cook and IDF definition is higher than using Liang and other 2 definitions.The number of subjects who met the Cook criteria of MS is the most.The Cook definition may be more suitable for MS diagnosis in children and adolescents.A high conformity rate between Cook and and IDF criterion is proved.It is necessary to work out a universally accepted criterion adapted to children and adolescents.

Objective To identify novel proteins of human adipocytes by proteomic study,and analyze their strueture and function.Methods The expressed proteins of human adipocytes were identified by 2-D electrophoresis and mass spectrometry,and the structure and function of the novel proteins were predicated via bioinforrnatics.Results My027 is a novel protein of human adipocytes identified by 2-D electrophoresis and mass spectrometry.This protein was confirmed by RT-PCR,sequencing,and prokaryotic expression.My027 protein is located in the cytoplasm and has highly conserved structure,indicating that it is an important functional protein. This protein is highly homologous to glyoxalase superfamily with a whole domain of glyoxalase.It shares 96.7% homologous to glycoxalase between 141-253 amino acids and 95.7% homologous to glycoxalaseⅠbetween 144-244 amino acids.Conclusion The results show that protein My027 appears to have the same function as glyoxalaseⅠ.

Objective:To investigate the m echanism of D- amino acid oxidase/D- Alanine system in killing K5 6 2 e cells in vitro.Methods:The killing effects of D- Ala on K5 6 2 e cells stably expressing DAAO and GFP were observed.H2 O2 production by DAAO+ cells were m easured by the phenol red oxidation assay.L owry method was used to determ ine the protein quantities of cells and fluorescent intensities of GFP+ cells were assayed by flow cytom eter.Results:KDf Gd cells were killed completely after treated with 2 5 mm ol/L D- Ala for 2 4 h.The effect of D - Ala at 2 0 m mol/L on KDf Gd cells increased apparently within 4 8h,but the same effect was not observed if D - Ala was below 15 m mol/L .The cytotoxicity of D- Ala in KDf Gd cells was more sensitive than in parental K 5 6 2 e cells.The H2 O2 levels in the medium were consistent with the killing effects of D- Ala.Conclusion:The killing effects of DAAO/D- Ala system is m ediated by H2 O2 . Objective To explore the role of MyD88 in heterotopic tracheal transplantation in mice and its relationship with histopathological changes.Methods The mouse model of hetemtopictracheal transplantation was used.The mice were divided into three groups:(1) tracheal isograft of C57BL/6 to C57BL/6 mice;(2) tracheal allograft of BALB/c to C57BL/6 mice;(3) tracheal graft of BALB/c to C57BL/6 mice with MyD88 inhibitor treatment.The tracheal grafts were collected at indicated time points.Histological sections were stained with hematoxylin and eosin.The pathological changes were observed and their semi-quantitative measurement was done with Image J software.Results (1) Pathological results showed that the structure of the trachea with MyD88 inhibitor treatment was clear and the loss of epithelial cells was significantly reduced as compared with the positive control group at the time of 7 and 14 days.(2) The results of semi-quantitative measurement showed that luminal occlusion rate of MyD88 inhibitor treatment group was significantly reduced as compared with the positive control group (P＜0.01).However,the loss of epithelial cells was not improved 7 days after transplantation.Both of lumen occlusion rate (P＜0.05) and epithelial cells loss (P＜0.01) in MyD88 inhibitor treatment group were significantly reduced.Conclusion Inhibition of MyD88 molecule could significantly alleviate pathological changes of the transplanted trachea.Both of luminal occlusion rate and loss of epithelial cells were significantly ameliorated.

OBJECTIVETo investigate the correct method of bone resection and posterior capsular soft tissue releasing in total knee arthroplasty (TKA) for the patients with rheumatoid arthritis with stiff knee in flexion.

METHODSFrom November 2009 to January 2012,15 patients with rheumatoid arthritis with stiff knee in flexion underwent primary TKA and releasing of the posterior soft tissues. There were 7 males and 8 females,aged 22 to 75 years old (58.7 years old on average). The preoperative range of movement(ROM) was (3.2 ± 1.7)°. According to Knee Society score (KSS) criterion, the preoperative clinical score was 23.3 ± 12.5 and functional score was 35.2 ± 9.8. Based on the correct osteotomy, effective releasing of posterior structures was used for different degrees of flexion contracture during the TKA procedure.

RESULTSAll the patients were followed up, and the average duration was 2.3 years (1.6 to 3 years). At the latest follow-up,the KSS clinical score was 81.7 ± 6.5 and functional score was 82.8 ± 9.3. The flexion and extension ROM of the knee joint was (103.5 ± 13.1). Three knees remained 50 flexion contracture deformity, but the function of the affect knees was good.

CONCLUSIONThe effective releasing of the soft tissue of posterior capsule is a major management for correction of the flexion contracture in TKA. The correct releasing of posterior structure can not only achieve fundamental gap of TKA but also effectively avoid bone over-resection.

OBJECTIVETo compare therapeutic effects between arthroscopic medial retinaculum plication and plaster external fixation for the treatment of acute patellar dislocation.

METHODSFrom February 2006 to October 2012,29 patients with acute patellar dislocation were divided into two groups: operation group and non-operation group. The patellar dislocation duration was 2 weeks. In operation group, there were 7 males and 10 females, with an average age of (16.2 ± 6.2) years old, and the patients were treated with arthroscopic medial retinaculum plication. In non-operation group, there were 5 males and 7 females,with an average age of (16.3 ± 5.0) years old,and the patients were treated with plaster external fixation. The Kujala scores, patellar tilt angle measured on CT film, apprehension test and recurrence rate of patellar instability were observed before and 1 year after treatment.

RESULTSIn operation group, the pre-treatment and post-treatment patellar tilt angles had no statistical difference, but the post-treatment Kujala score was lower than that of pre-treatment; while in non-operation group, the post-treatment patellar tilt angle was larger than that of pre-treatment, and the post-treatment Kujala score was lower than that of pre-treatment. At 1 year after treatment, the patellar tilt angle (21.2 ± 5.3) of patients in non-operation group was larger than (13.5 ± 3.5) of operation group, and the Kujala score 73.3 ± 10.5 of patient in non-operation group was lower than 84.1 ± 5.6 of operation group.

CONCLUSIONDuring 1 year after operation, arthroscopic medical retinaculum plication is a more effective treatment for acute patellar dislocation compared with plaster external fixation.
