3.In vitro study on Coxsackie virus B3 infection on islet cells

Objective To study the effect of Coxsackie virus B3 (CVB3) infection on islet cells in vitro, and to explore the mechanism of islet cells caused by CVB3. Methods Bone marrow mesenchymal stem cells( BMSCs) were separated from the bone marrow and cultured. Then they were induced to differentiate into islet-like cells using nicotinamide and mercaptoethanol. Differentiated cells were detected by morphology , special staining and RT-PCR. Observe CVB3 infection on islet cells under inverse microscope and detect the specific gene fragment by RT-PCR. Results BMSCs showed half suspended shape and gathered to form a cluster after induction. Cells became red brown by dithizone specific staining. RT-PCR also proved the existence of mRNA expressing insulin. Infected islet cells appeared typical pathological changes like shrinks, refraction decreases. RT-PCR detected the desired specific gene fragment of 299 bp in infected islet cells. Conclusion CVB3 can directly injury islet cells, and damage the function of islet cells of secreting insulin.
4.Diagnostic test of screening depressive disorders in general hospital with the Patient Health Questionnaire

Objective:Using a diagnostic test research,to explore the reliability and validity of the Patient Health Questionnaire (PHQ-9)on screening of depressive disorders in general hospital,and to determine the cut points for depressions with different severities.Methods:Three hundred and nineteen help-seekers (n =319)con-sulting the outpatient department of psychiatry in a general hospital were collected continuously.Among them,80 were people without any psychiatric diagnosis,239 were patients with mental disorders.All of subjects were asked to fulfill PHQ-9,and were assessed by psychiatrist with Mini-International Neuropsychiatric Interview (M.I.N.I.) and Hamilton rating scale for depression.M.I.N.I.was gold standard to diagnose depressive disorder,HRSD was used to assess the illness severity.Result:Factor analysis showed good structure validity of PHQ-9.46 patients were diagnosed depressive disorders.The area under the receiver operator characteristic analysis (ROC)of PHQ-9 was 0.94 (95%CI:0.91 -0.97)for depressive disorders.With 15 as the critical value for depression screening,the sen-sitivity was 0.83 and specificity was 0.90,respectively.By the result of HRSD as the standard to rank different lev-els of depression,the cut points of PHQ-9 for mild,moderate,and severe depression was 6,12,and 15.The consis-tency between results of PHQ-9 (with cut points of 6,12,15)and HRSD was moderate (with kappa =0.49).The kappa between results of PHQ-9 (with recommended cut points of 5,10,and 15)and HRSD was 0.46.Cronbach αcoefficient of PHQ-9 was 0.892.Conclusion:It suggests that PHQ-9 has satisfying reliability and validity,and is applicable for screening depression and assessment of its severity of outpatients in general hospital.The recommen-ded cut points of PHQ-9 are applicable to depressive outpatients in general hospital in China.
7.Optimal timing of laparoscopic cholecystectomy in mild acute gallstone pancreatitis

Objective To assess the optimal timing of laparoscopic cholecystectomy ( LC ) in mild acute gallstone pancreatitis ( mAGP) .Methods From May 1, 2012 to August 30, 2015, consecutive patients with mAGP were prospectively assessed.Each patient underwent abdominal computed tomography scan within 48 h after mAGP onset to assess the presence of peripancreatitc fluid collection, and Marshall score was used to assess if there was organ failure.Patients with neither peripancreatic fluid collection by CT ( classified as grade A, B or C based on the Balthazar CT grading system) nor organ failure by clinical data (Marshall score <2) were randomized according to simple randomization into early laparoscopic cholecystectomy ( ELC; LC performed within 7 days after a pancreatitis attack, without waiting for symptom resolution) or late laparoscopic cholecystectomy ( LLC; LC performed ≥7 days following an attack, with complete remission of AGP symptoms) group.The mean LC operation time, bleeding during LC, post-LC complications and lengths of hospital stay between the ELC group and LLC group were compered.Results The study enrolled 102 patients with mAGP.A total of 49 and 53 patients were assigned to ELC and LLC group, respectively.The mean LC operation time and lengths of hospital stay were significantly shorter in the ELC group than in the LLC group [(19.9 ±5.3) vs(31.1 ±8.4) min;(7.9 ±1.8) vs (16.8 ±5.3) d, P<0.05], while there were no significant difference on bleeding during LC and post-LC complications.Conclusions LC for patients with mAGP who had neither peripancreatic fluid collection nor organ failure within 7 days after the onset was safe and feasible.
8.Histological changes of domestic e-PTFE implanted into subcutenous tissue of rat

Objective To investigate the relative histological change of transplanted domestic expanded polytetrafluoroethylene (e-PTFE), which were treated with different methods, in order to offer the referential data for clinical application. Methods e-PTFE treated with different methods was transplanted into subcutaneous tussue of rat. The samples harvested according to time sequence were examined by using histological and histochemical methods. Dynamic change of the structure between the e-PTFE and it's surrounded tissue was investigated based on the examination. Results Cell and tissue were observed on the inside of all the e-PTFE including the control group and the experiment groups. Heavy cell infiltration on the 3rd day was the most significant in control group, and the quantity of tissue ingrowth was also the most until the 28th day. The next was trimming group. The quantity of both cell infiltration and the tissue ingrowth in high pressure steamed group and forceps squeezed group were less than that in other groups. Conclusion Cell infiltration into domestic e-PTFE is significantly achieved since 72 hrs and fibrovascular ingrowth since the 7days after implanted e-PTFE under subcutaneous tissue of the rat. Different treatment methods of e-PTFE can affect the speed of tissue ingrowth into the e-PTFE, which could be a reference for clinic application of e-PTFE.
