2.Analysis of a survey result on the state of Kashin-Beck disease in Tibet

ObjectiveTo investigate the distribution of Kashin-Beck disease(KBD) in Tibet, and assess the disease status. Methods Between 2007 and 2008, a survey was done on KBDepidemiology which was carried out in four prefectures of 26 counties according to the east, south, west, north and center in Nakchu,Lhoca, Nyingtri and Shigatse districts of Tibet, with towns and villages as baseline survey points. According to the KBD e survey scheme, KBD clinical examination for adults was also carried out and at the same time clinical and right hand anteroposterior X-ray examinations were given to children aged 4 - 13. The partition of endemic area was based on the criteria of national standards for Kashin-Beck disease diagnoses《GB 16395-1996》. Slight KBD area:clinical prevalence of Kashin-Beck disease grade Ⅰ and above was less than 10% or X-ray detection rate < 10% of children; the moderate prevalent KBD area: clinical prevalence of Kashin-Beck disease grade Ⅰ and above was between 10% and 20% or X-ray detection rate was between 10% and 30% of children; severe KBD area: clinical prevalence of KBD grade Ⅰ and above was more than 20% or X-ray detection rate was higher than 30% of children.ResultsA total of 108 townships of 26 counties were surveyed, 14 686 adults were clinically examined, cases detection of grade Ⅰ and above were 637 people, the prevalence was 4.34%, and no case of grade Ⅲ was detected.Of 5769 children's right anteroposterior X-ray film, 102 were detected positive; the prevalence rate was 1.77%.Metaphysis was affected in most of the child cases, which accounting for 89.2% (91/102). Amongst all the counties, there were 10 counties, clinical detection rate of adult KBD was 0, and children's X-ray detection rate of KBD was also 0. In 1 county the clinical prevalence rate for adults KBD was 0 and X-ray detection rate for children was 3.66(7/191 ). In 12 counties the clinical prevalence rate for adults KBD was between 1.03% and 7.54%, X-ray detection rate for children was between 0 and 7.76%, amongst all these counties surveyed there were 5 counties,the detection rate for children was 0. In 3 counties the clinical prevalence rate for adult KBD was between 10.69%and 13.88%, the X-ray detection rate for children was between 5.31% and 7.76%. Conclusions According to the criteria for diagnoses of KBD, within the 26 counties surveyed, 10 counties are non-endemic areas, 13 counties are slight endemic areas, 3 counties are medium endemic areas. So far, KBD is prevalent in 52 counties of 7 prefectures (cities) in Tibet, the disease is widely distributed, the situation is still severe, and there is a need to continue to strengthen KBD surveillance.
3.Effect of continuous blood purification for management of multiple organ failure with acute renal failure.

OBJECTIVETo evaluate the effect of continuous blood purification (CBP) for treatment of multiple organ failure (MOF) with acute renal failure (ARF).
METHODSForty-seven patients with MOF underwent CBP for an average of 3.1-/+0.5 days averagely. Continuous veno-venous hemofiltration was performed at daytime, and the substitute fluids were infused with pre-dilution at the rate of 2000-4000 ml/h. The general conditions, clinical symptoms, and serum biochemical indexes of the patients were observed and MODS score was calculated.
RESULTSAfter CBP, the MODS score of the patients decreased significantly from 9.1-/+3.5 to 5.4-/+2.7 (P<0.01) and serum creatinine decreased from 451.3-/+134.5 to 223.7-/+100.2 micromol/L (P<0.05). Twenty-nine patients survived with the survival rate of 61.7%.
CONCLUSIONCBP is effective for treatment of MOF and may help reduce the mortality rate of MOF complicated by ARF.

4.Detection of interleukin-8, matrix metalloproteinase-9 and interferon gamma levels in the cerebrospinal fluid of patients with tuberculous meningitis and viral meningitis

Objective To investigate the diagnostic values of interleukin-8 (IL-8), matrix metalloproteinase-9 (MMP-9) and interferon gamma (INF-γ) levels in patients with tuberculous meningitis and viral meningitis by detecting the contents of these biomarkers in the cerebrospinal fluid (CSF). Methods Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of IL-8,MMP-9 and INF-γ in the CSF of patients with tuberculous meningitis (n=20),viral meningitis (n=15) and noninfectious neurologic diseases (n=20) who admitted to our hospital from August 2010 to November 2011. Results The IL-8,MMP-9 and INF-γlevels in the samples from the tuberculous meningitis patients were significantly higher than those from either viral meningitis or noninfectious neurologic diseases (P<0.05).The contents of IL-8 and MMP-9 in patients with viral meningitis were significantly higher than those in patients with noninfectious neurologic diseases (P<0.05),while that of INF-γ showed no significant difference between the 2 groups (P>0.05). Conclusion Detection of IL-8,MMP-9 and INF-γ in CSF is useful in the diagnosis of tuberculous meningitis; IL-8 and MMP-9 participate in the progress of viral meningitis; the detective value of high concentration of INF-γ in CSF is more significant in diagnosis of tuberculous meningitis than that of IL-8 and MMP-9.
5.Myocardial revascularization combined with concomitant valve operations: report of 45 cases.

OBJECTIVETo introduce the experience in coronary artery bypass grafting (CABG) combined with concomitant valve operation.
METHODSFrom 1998 to 2002, forty-five patients (38 men and 7 women) with coronary artery lesion and valve diseases underwent myocardial revascularization combined with concomitant valve operation. The mean age of the patients was 59 years (42 to 75 years). Heart function was Class II (NYHA) in 7 patients, Class III in 30, Class IV in 8. The value of ejection fraction was 0.32 to 0.50. Thirty patients had mitral valve lesion, 7 aortic lesion, and 8 both lesions. Seventeen patients had single-vessel diseases, 20 double-vessel diseases, 8 triple -vessel diseases, and 8 left main artery lesion. After cardiac arrest by cold blood cardioplegia under moderate cardiopulmonary bypass (CPB), distal anastomosis of the saphenous vein (SV) to the target vessels were firstly performed and followed by valve replacement (32 patients) or plasty (2). The left internal mammary artery (LIMA) was grafted to the left anterior descending artery (LAD) before aortic unclamping. Proximal anastomosis of the SV to the aorta was finally finished on beating heart. The mean bypass time was 173.5 min and the mean duration of aortic cross-clamping time was 112.6 min.
RESULTSNo mortality occurred during hospitalization and all patients were discharged 9.2 days after operation. Cardiac function of all patients was improved to Class I-II and no patient died during follow-up.
CONCLUSIONSCABG combined with valve surgery can be safely performed with good results.

6.Survey on situation and cognition of influenza and pneumonia vaccination in patients with pneumoconiosis and pneumoconiosis comorbided with COPD in Hunan Province

Objective:To understand the current status and influencing factors of influenza and pneumonia vaccination in patients with pneumoconiosis and pneumoconiosis comorbided with Chronic obstructive pulmonary disease (COPD) in Hunan Province, and provide a basis of vaccination planning strategies for the patients.Methods:Inpatients pneumoconiosis and completed 1-year follow-up in the Hunan Prevention and Treatment Institute for Occupational Disease in May 2019 were selected as the research objects, and the data of their age, gender, education level, medical insurance type, number of hospitalization, smoking status, and comorbidity of COPD were collected retrospectively. They were followed up for 1-year of the vaccination status of influenza and pneumonia, and the main factors influencing vaccination decision. Unconditional logistic regression analysis was used to analyze the factors influencing vaccination.Results:A total of 474 hospitalized pneumoconiosis patients were included, of which 96 were comorbided with COPD; the follow-up results showed that the awareness rate of pneumoconiosis patients with influenza and pneumonia vaccine was 13.1%, and the vaccination rate within one year was 7.6%. The vaccination rate of pneumoconiosis patients with COPD within one year was 8.3%. The positive correlation factors of the vaccination were multiple hospitalization, residence of >3 people and comorbided with COPD. The route of vaccination is mainly by doctors' recommendation.Conclusions:The vaccination rate of influenza and pneumonia in pneumoconiosis and pneumoconiosis patients with COPD in Hunan Province is low; the main way for patients to learn is doctor education, so it is necessary to strengthen the health education of pneumo-coniosis doctors and patients to improve the vaccination rate.
7.Clinical analysis of 1018 cases of coronary artery bypass grafting.

OBJECTIVETo analyze retrospectively 1018 patients who underwent coronary artery bypass grafting surgery (CABG) in order to summarize surgical techniques and clinical outcome.
METHODSFrom 1997 through 2004, data of same surgeon for 508 patients who underwent conventional coronary artery bypass surgery on pump (CCABG) and 510 patients who underwent off-pump CABG (OPCAB) were collected and analyzed retrospectively. Eight hundred and fifty-two patients had unstable angina, 582 patients were over 60 years old (57.2%) and 784 patients had concomitant diseases including valve lesion, hypertension, diabetes, myocardial infarction, left ventricular aneurysm with septal defect, stroke, chronic obstructive pulmonary diseases, renal failure and cancer. A hundred and fifty-six patients had left main stem (LIMS) stenosis and 671 patients, triple-vessel disease.
RESULTSTotal mortality was 0.39% (4-case death) and morbidity, 1.6% (sternal dehiscence, stroke and mediastinitis). The grafts per patient with CCABG and OPCAB were 3.3 +/- 0.6 vs. 2.5 +/- 0.4. Left internal mammary artery use was 93.8% of the patients, 29 patients were implanted intra-aortic balloon pump intraoperatively. Follow-up was 4 months to 7 years.
CONCLUSIONSScientific surgical strategies, excellent surgical techniques and improvement of cardiac anesthesia and cardiopulmonary bypass make the mortality and morbidity decrease significantly, CABG surgery is safe and effective in patients with coronary artery disease.

9. Clinical analysis of 100 adult patients with pure red cell aplasia.

OBJECTIVE: To evaluate clinical characteristics and treatment response of 100 patients with pure red cell aplasia(PRCA).METHODS: We retrospectively analyzed the clinical data of 100 adult patients with acquired PRCA from October2009 to July 2019, and compared the difference in efficacy between idiopathic and secondary patients.RESULTS: 100 patients were evaluated, including 60 idiopathic patients and 40 secondary patients.The most common reasons for secondary PRCA were large granular lymphocytic leukemia(LGLL)(28 cases,70.0%)and thymoma(6 cases, 15.0%). The remission induced regimens included corticosteroids(CS), cyclosporine A(CsA), or other agents, and the response rate were 66.7%,71.4% and 50%, respectively(P=0.336). Secondary PRCA was less effective than idiopathic PRCA(52.5%,78.3%,P=0.007). PRCA related to large granular lymphocytic leukemia was also less effective compared to idiopathic PRCA(46.4%,79.3%,P=0.003). When treated by CsA, idiopathic PRCA was more effective than secondary PRCA and LGLL related PRCA(P=0.001, P=0.000). Logistic regression analysis showed that lower response rate was related to secondary PRCA and LGLL related PRCA.CONCLUSION: The response rate were similar by different induced regimens. Idiopathic PRCA could acquired better response to CsA than secondary, LGLL related PRCA was less effective to treatment.
10.A new macrocyclic phenolic glycoside from Sorghum vulgare root.

Eleven compounds were isolated and purified from Sorghum vulgare root extract, through column chromatography over silica gel, MCI gel, and preparative HPLC. Their structures were established by MS, 1 D NMR and 2 D NMR data as sorgholide A(1), β-sitosterol(2), stigmastero(3), daucosterol(4), 4-methoxycinnamic acid(5), taxiphyllin(6), chlorogenic acid(7), p-hydroxybenzaldehyde(8), succini acid(9), trans-p-hydroxycinnamic acid(10), obtusalin(11). Compounds 4,5 and 9-11 were reported from this species for the first time, and compound 1 is the first 24 ring dimeric double lactonol glycoside formed by reverse polymerization of p-hydroxyphenylacetate glucoside, named sorgholide A.
