2.Therapeutic effect of adipose derived mesenchymal stem cell conditioned medium thermosensitive hydrogel on skin scald

BACKGROUND: Studies have shown that adipose derived mesenchymal stem cells are involved in the skin repair after scald, but the hydrogel made of the excreta by adipose derived mesenchymal stem cells is rarely reported in the treatment of skin scald. OBJECTIVE: To analyze the therapeutic effect of human adipose derived mesenchymal stem cell conditioned medium hydrogel in a mouse model of skin scald. METHODS: Adipose derived mesenchymal stem cells were obtained from adipose tissues by enzyme digestion combined with adherent culture method. Morphological and flow cytometry were used to identify phenotype and induce differentiation. Secondly, the stable proliferative phase cells were harvested to obtain the conditioned medium, and chitosan, mannitol, beta-glycerol phosphate sodium and hyaluronic acid were added to prepare the thermosensitive hydrogel. Then the 95 ℃ aluminum block was used to rapidly establish a model of degree III skin scald on the left (experimental group) and right (control group) sides of the back of 24 C57BL/6 mice. In the experimental group, adipose derived stem cell conditioned medium hydrogel was applied twice a day on the right side of the mouse back, and in the control group, fresh medium hydrogel was applied twice a day on the left side of the mouse back. The treatment period lasted for 7 days. Healing time and healing process were observed to calculate the healing rate. Histopathological changes were observed by hematoxylin-eosin staining at paraffin sections at 4, 14, 28 days after skin scald. RESULTS AND CONCLUSION: (1) The human adipose derived mesenchymal stem cells had fibroblast-like morphology and proliferated vigorously, and the average doubling time was 55 hours. These cells could be induced to differentiate into osteoblasts, chondrocytes and adipocytes. High expression of CD29, CD44, CD90 and CD105 were observed on these cells with low expression of CD31 and CD34, which met the standard of mesenchymal stem cells. (2) Thethermosensitive hydrogel prepared by the conditioned medium was cool and transparent viscous liquid at 4-20 ℃, and was changed into semi-solid gel at( 37 ℃ after 15 minutes. (3) The normal structure) subcutaneous fat and muscle tissue (of 95 ℃ aluminum block scalded mice wer) standards of degree III burns. The wound area was roughly 3 cm2. (4) In the repair process, shorter wound healing time, less scar and better dermis structure were observed in the experimental group compared with the control group. (5) Inflammatory infiltration, thickness of granulation tissue, epidermal thickness, fibroblasts and vascular density were significantly improved in the experimental group as compared with the control group (P < 0.05). To conclude, human adipose derived mesenchymal stem cells conditioned medium hydrogel can promote the wound healing and promote the quality of regenerated skin after skin scald.

Objective To investigate the effect of arsenic trioxide(As_2 O_3)nanoparticles on rabbit vascular smooth muscle cells in vitro in comparison with normal form As_2 03.Methods The rabbit vascular smooth muscle cells were cultured in vitro.Nano and normal forms of As_2O_3 with drug concentrations of 3?mol/L were added into the cells.Cell proliferation curve was drawn according to the light absorption values of MTT test.Flow cytometry was applied to observe the apoptosis.DNA was extracted and underwent electrophoresis.Results Cell proliferation treated with the 3?mol/L concentration of As_2O_3 was inhibited. 4.Cardiovascular circulation feedback control treatment instrument.

The cardiovascular circulation feedback control treatment instrument (CFCTI) is an automatic feedback control treatment system, which has the function of monitoring, alarming, trouble self-diagnosis and testing on the line in the closed loop. The instrument is designed based on the successful clinical experiences and the data are inputted into the computer in real-time through a pressure sensor and A/D card. User interface window is set up for the doctor's choosing different medicine. The orders are outputted to control the dose of medicine through the transfusion system. The response to medicine is updated continually. CFCTI can avoid the man-made errors and the long interval of sampling. Its reliability and accuracy in rescuing the critical patients are much higher than the traditional methods.

5.Study on HIV related stigma and discrimination among people living with HIV/AIDS and their family members

Objective To understand the perceived stigma and discrimination and received stigma and discrimination as well as the differences and reasons of them among people living with HIV/AIDS and their family members. Methods 307 people were investigated and 117 of which were HIV/AIDS and the others were their family members. Face to face interview was carried out. Results HIV/AIDS and their family were living in the social environment rounded by stigma and discrimination. Stigma and discrimination could be divided into perceived stigma and discrimination and received stigma and discrimination, and these two kinds of stigma and discrimination were statistically significant different between HIV/AIDS and their family members (t=-12.540, P=0.000) , and the perceived stigma and discrimination was more serious than received stigma and discrimination. The perceived stigma and discrimination were related to their self-efficacy (OR=0.558, P=0.041 ), family APGAR (OR= 0.650, P=0.027), infected with HIV or not (OR= 2.116,P=0.004). The received stigma and discrimination were related to their self-efficacy (OR=0.468,P=0.028), family APGAR (OR=0.427, P=0.000) whether infected by HIV (OR=3.412, P=0.001 ). Conclusion Stigma and discrimination did exist in the environment where HIV/AIDS and their family lived, suggesting that it was necessary to develop a series of policies and measures to fight against stigma and discrimination.

With the kernel of efficacy, "Xiaohe Silian" was a pattern and method for new drug discovery which was constituted with "metabolism-efficacy, toxicity-efficacy, quality-efficacy and structure-efficacy". Its connotation was in keeping with traditional Chinese medicine (TCM) clinical pharmacy. This paper systematically summarized the research method of new drug discovery practice process for TCM. To avoid western drug like in TCM new drug discovery, we carried out combination analysis with TCM clinical pharmacy. The correlation analysis between basic elements of "Xiaohe Silian(n) and TCM clinical pharmacy was studied to guarantee this method could integrate closely with TCM clinic from all angles. Hence, this method aimed to provide a new method for TCM new drug discovery on the basis of TCM clinical pharmacy with insisting on holistic view of multicomponent study, kinetic view of metabolic process when the curative effect occurred and molecular material view of quality control and structure-activity exposition.

7.Analysis of application effect of cardiac remote real -time monitoring system

Objective:To explore clinical application effect of cardiac remote real -time monitoring system.Methods:Cardiac remote real -time monitoring warning instrument(iHolter)was applied in 210 patients.The iHolter gave a-lerts automatically in case of ECG abnormalities for corresponding treatment in time,and it offered corresponding reports on monitoring results.According to age,patients were divided into youth group(<45 years,n=59),mid-dle-aged group(45160 years,n=56)and aged group(≥60 years,n=95).Characteristics of arrhythmias among different age and sex were analyzed.Results:ECG abnormalities were found in 191 cases(90.95%)out of 210 pa-tients,including eight cases with malignant arrhythmias,in which five cases were treated and survived after timely intervention via alerts.Compared with youth group and middle -aged group,there were significant rise in percenta-ges of paroxysmal atrial tachycardia(16.9%,26.8% vs.54.7%)and atrial fibrillation(0%,0% vs.8.4%)in aged group,P< 0.05 or <0.01;abnormal percentages of heart rate variability of middle -aged group and aged group were significantly higher than that of youth group(25.0%,36.8% vs.5.1%,P<0.01 both).Male percent-age of paroxysmal ventricular tachycardia was significantly higher than that of female(81.3% vs.18.8%,P=0.013).Conclusion:Cardiac remote real-time monitoring system can offer timely alerts for cardiovascular events such as arrhythmia and myocardial ischemia etc.,fight for more time for rescuing patients,maximally protect cardi-ac function and improve accuracy of clinical diagnosis.

Oral anticoagulants has become the cornerstone preventing thromboembolic complications in patients with atrial fibrillation (AF ) who are at high risk for stroke .Advantages of new oral anticoagulants (NOAC ) include greater pharmacokinetic predictability ,less risk of fatal or intracranial hemorrhage compared to vitamin K antago-nists ,fewer drug-food interactions and no long time monitoring of international standardized ratios etc .The present article focused on recent studies of NOAC in AF complicated coronary diseases ,for secondary prevention of stroke and its specific antagonists .
9.A predictive analysis of the association between clinical phenotypes and genotypes in children with Becker muscular dystrophy/Duchenne muscular dystrophy.

OBJECTIVE:
To study the association between clinical phenotypes and genotypes in children with Becker muscular dystrophy (BMD)/Duchenne muscular dystrophy (DMD) so as to provide a theoretical basis for disease management, gene therapy, and prenatal diagnosis.
METHODS:
A retrospective analysis was performed for the clinical data and gene detection results of 52 children with BMD/DMD. Multiplex ligation-dependent probe amplification (MLPA) was used to detect the DMD gene. The children with negative results of MLPA were further screened by exon chip capture combined with next-generation sequencing (NGS). The mothers of 20 probands were validated by sequencing.
RESULTS:
The pathogenic genes for BMD/DMD were detected in 50 children by MLPA and NGS, with a detection rate of 96%. Among the 52 children, 36 (69%) had gene deletion, 7 (13%) had duplication, and 7 (13%) had micromutation. Among the 43 children with deletion/duplication, 32 had DMD and 11 had BMD; 37 children (86%) met the reading frame rule, among whom 27 (96%) had DMD and 10 (67%) had BMD. All 7 children with micromutation had DMD.
CONCLUSIONS
The reading frame rule has an extremely high predictive value for DMD but a limited predictive value for BMD.

10.Measures for brain protection in carotid angioplasty and stenting in patients with carotid artery stenosis and contralateral occlusion

Objective To assess the value of brain protection measures in carotid angioplasty and stenting(CAS)for carotid stenosis and contralateral occlusion in reducing the perioperative cerebral isehemic events.Methods Twelve patients undergoing CAS in our department were included in this analysis.All the patients received balloon occlusion test(BOT)of the carotid artery preoperatively.Two patients who were intolerant to BOT received extracranial-intracranial vascular anastomosis,and another 2 patients relatively intolerant to BOT received CAS under general anesthesia.The brain protection measures including reduction of the cerebral blood flow occlusion time and prophylactic management of thrombosis were administered to prevent the ischemic events.The efficacy of the brain protection measures was evaluated by diffusion-weighted imaging(DWI)and observation of the clinical ischemic neurological symptoms.Results Three patients suffered transient ischemic attacks(TIAs),1 patient had temporary vision loss,and 1 had permanent ischemic neurological deficits after the operation.DWI yielded positive results in 50% of the patients after the operation,characterized mostly by small local lesions of 1-2 ram.Eight patients presented with carotid artery dilation by over 90%,and 4 by 70% to 90% after the operation.Conclusion The brain protection measures including precise preoperative evaluation of the ischemic tolerance and blood flow and reduction of cerebral blood flow occlusion time and prevention of thrombosis during the operation can effectively reduce the incidence of perioperative ischemic events in high-risk patients with carotid stenosis and contralateral occlusion.
