BACKGROUNDAdvances in minimally invasive surgical techniques and neonatal intensive care for neonates have allowed for repair of the neonatal esophageal atresia with tracheoesophageal fistula (EA/TEF) to be approached endoscopically. However, thoracoscopic surgery in children is still performed in only a few centers throughout the world. The aim of this study was to compare the neonatal tolerance to the thoracoscopic repair (TR) and the open repair (OR) and also to discuss anesthetic management in thoracoscopic procedure.

METHODSWe performed a prospective study enrolling newborns diagnosed with EA with distal TEF (type C) receiving the repair surgery between June 2009 and January 2012 in our institution. Data collected included the newborns' gestational age and weight at the time of the operation, operative time, parameters of intraoperative mechanical ventilation, oxygenation, end-tidal carbon dioxide (ETCO2), and analysis of blood gases. Time to extubation and length of stay were also recorded.

RESULTSIntravenous induction with muscle paralysis followed by pressure-control ventilation and tracheal intubation regardless of the position of the fistula can be performed uneventfully in EA/TEF newborns with no additional airway anomalies and large, pericarinal fistulas in our experiences. The thoracoscopic approach appeared to take longer than the open approach. During the procedure of repair, hypercarbia and acidosis developed immediately 1 hour after pneumothorax in both groups. CO2 insufflation did have additional influence on the respiratory function of the newborns in the TR group; values of PaCO2 and ETCO2 were higher in the TR group but the difference did not reach statistical significance. By the end of the procedure, values of PaCO2 and ETCO2 returned to the baseline levels while pH did not, but all parameters made no difference in the two groups. Besides, time to extubation was shorter in the TR group.

CONCLUSIONSThoracoscopic repair of EA/TEF is comparable to the open repair, and is believed to be safe and tolerable in selected patients. A wider range of neonates may be acceptable for thoracoscopic EA/TEF repair with increasing surgical experience.

OBJECTIVETo investigate the chemical constituents of Saussurea laniceps.

METHODThe ethanol extract of S. laniceps was separated by means of silica gel chromatography. The compounds isolated from the plant were identified by their spectral evidence.

RESULTFifteen compounds were isolated and identified as beta-stiosterol (1), umbelliferone (2), 4-hydroxyacetophenone (3), scopoletin (4), isoscopoletin (5), xuelianlactone (6), methyl 3-(2', 4'-dihydroxyphenyl) propanoate (7), apigenin (8), neoechinulin A (9), daucosterol (10), scopolin (11), xuelianlactone 8-O-beta-D-glcuoside (12), apigenin 7-glcuoside (13), apigenin 7-lutinoside (14) and syringin (15).

CONCLUSIONCompounds 5-15 were isolated from S. laniceps, and among them, 7 and 9 were isolated from genus Saussurea for the first time.

Objective To study the antitumor effect in vivo of Coix leaf′s water extract and alcohol extract of different concentrations on H 22 tumor-bearing mice.Methods Take 120 CL Kunming mice to prepare tumor models of cell transplantation , and divided these into 12 groups, with 10 mice in each group.There are model group and control group ( cy-clophosphamide , 20 mg · kg -1 ) , five difference concentration ( 0.15 , 0.30 ,0.45 ,0.60 ,0.75 g · mL -1 ) groups with Coix leaf′s water extract (CLWE) and alcohol extract(CLAE).The tumor of H22 tumor-bearing mice was weighed.The inhibitory rate, and calculate liver index, spleen index and thymus index were calculated .Results The best antitumor concentration of the extract from Coix leaf′s is 0.45 g· mL-1 .The inhi-bition rate of CLAE is better than the CLWE .Compared with the model group, the 0.45 g· mL-1 group of CLWE and each concentration groups of CLAE can significantly inhibit the growth of tumor ( P <0.05 , P<0.01 ) .The tumor inhibition rate of CLAE of 0.45 g · mL -1 reaches 63.2%.The Coix leaf′s extracts exert antitumor effect , and have a sig-nificant protective effect on thymus at the same time .Conclusion The best antitumor concentration of the extract from Coix leaf′s is 0.45 g· mL-1 .The inhibition rate of CLAE is better than that of the CLWE .

Objective To compare the pharmacokinetics and bioequivalence between domestic hydrochloric trimetazidine capsules and imported hydrochloric trimetazidine tablets in healthy male Chinese volunteers after single oral administration.Methods A single oral dose (test and reference formulations) was given to 24 healthy male Chinese subjects according to an open randomized crossover design.The blood samples were collected before and after administration.Plasma trimetazidine concentration was determined by HPLC-MS/MS.The pharmacokinetic parameters were calculated by WinNonlin Ver 6.2.1 software.Results The main pharmacokinetic parameters of domestic and imported formulation of trimetazidine were similar:Cmax(70.9 ± 15.3),(66.4 ± 13.8) μg/ml; tmax(1.70 ± 0.72),(1.85 ± 0.55) h; t1/2z(4.70 ±1.75),(4.77 ± 1.96)h; AUC0-24h(481 ± 176),(469 ± 171)μg· h·ml-1; AUC0-∞(511 ±189),(500 ± 188) μg · h · ml-1.The estimated 90％ CIs for the ratio of C and AUC0-24 h were also similar:101.9％-112.5％ and 99.4％-104.9％.The relative bioavailability of domestic formulation was (102.2 ± 8.3) ％.Conclusion The results demostrats that the domestic hydrochloric trimetazidine capsules and imported hydrochloric trimetazidine tablets are bioequivalent.

Objective To investigate the effect of plateau hypoxia on the blood-brain barrier after subarachnoid hemorrhage (SAH) in rats. Methods Adult male SD rats (n = 78) were randomly divided into 4 groups: sham group (sham), SAH model group (SAH), plateau hypoxia sham group (Hp sham) and plateau hypoxia SAH model group (Hp SAH). The rat model of plateau hypoxia was established through low-pressure simulation chamber (altitude 5000 m), and the SAH model was established by endovascular perforation method. At 24 hours after SAH, neurobehavior score and SAH grade were assessed. The morphological changes of neurons and apoptosis of nerve cells in the CA1 region of hippocampal were observed by the staining of Nissl and TUNEL. The expression of phosphorylated PI3K (p-PI3K), PI3K, phosphorylated Akt (p-Akt), Akt, phosphorylated nuclear factor κB (p-NF-κB), NF-κB, matrix metalloproteinase-9 (MMP-9), occludin and claudin-5 in hippocampal were detected by the method of Western blotting. The expression of occludin and claudin-5 proteins in the CA1 region of hippocampal were observed by immunofluorescent staining. Results At 24 hours after SAH, the neurobehavior score decreased significantly and SAH grade increased significantly in the SAH and Hp SAH group (P< 0.05). Neurobehavior score decreased significantly in the Hp SAH group compared with the SAH group (P < 0.05). In the SAH group, neurons in the CA1 region of hippocampus were atrophied and deformed, the arrangement were disordered, the number of neurons decreased significantly, and the apoptosis of nerve cells increased significantly(P< 0.05). Plateau hypoxia could aggravate the morphological damage of neurons and apoptosis of nerve cells. The expression of p-PI3K/PI3K, p-Akt/Akt, occludin and claudin-5 proteins decreased significantly, while the expression of p-NF-κB/NF-κB and MMP-9 proteins increased significantly in the SAH and Hp SAH group (P< 0.05). The expression of p-PI3K/PI3K and MMP-9 proteins increased significantly in Hp SAH group compared with the SAH group. The expression of claudin-5 protein increased significantly in Hp sham group compared with the sham group (P < 0.05). Immunofluorescent staining showed that the expression of occludin and claudin-5 proteins in the CA1 region of hippocampus decreased in the SAH group. Plateau hypoxia could further decreased the expression of occludin and claudin-5 proteins. Conclusion Plateau hypoxia aggravates blood-brain barrier disruption after subarachnoid hemorrhage in rats through inhibiting PI3K/Akt/NF-κB pathway.

This study was to investigate the protective effect of paeoniflorin (PF) on hydrogen peroxide-induced injury. Firstly, "SMILES" of PF was searched in Pubchem and further was used for reverse molecular docking in Swiss Target Prediction database to obtain potential targets. Injury-related molecules were obtained from GeenCards database, and the predicted targets of PF for injury treatment were selected by Wayne diagram. Inductively coupled plasma mass spectrometry (ICP-MS) was applied to determine the concentrations of lead (Pb), cadmium (Cd) and arsenic (As) in Lindera aggregata (Sims) Kosterm. The physiologically based extraction test (PBET) digestion in vitro/Caco-2 cell model was established to investigate the bioaccessible contents of Pb, Cd and As in decoction of Lindera aggregata (Sims) Kosterm. The target-organ toxicity dose modification of HI method (TTD) was used to evaluate the cumulative risk caused by the combined exposure of the total levels of Pb, Cd and As in Lindera aggregata (Sims) Kosterm. and the bioaccessible contents in the decoction. The results showed that the total contents of Pb, Cd and As in 4 batches of samples were in the range of 2.901-3.872, 1.299-1.800 and 0.062-0.216 mg·kg^-1, respectively. After transportation by Cacco-2 cells, the bioaccessible contents of Pb, Cd, and As in the decoction were in the range of 0.045-0.080, 0.070-0.112 and 0.004-0.018 mg·kg^-1. The results of risk assessment showed that calculated by the total amounts of heavy metals in the Lindera aggregata (Sims) Kosterm., for the end points of nervous system, the cumulative risks of co-exposure of heavy metals in 3 batches of samples were of concern. After decoction and transportation by Caco-2 cells, for the end points of cardiovascular system, blood, nervous system, kidney and testis, the TTD modification of HI values of all batches of samples were less than 1, and the health risks were acceptable. The study provided methodology basis for a more objective assessment of the health risks of heavy metals and harmful elements in traditional Chinese medicine and for a more scientific limit standard of heavy metals and harmful elements.
