Objective To analyze the situation and trend of the application of antibacterial drugs in the case of typeⅠincision operation in our hospital. Methods A total of 1 964 patients in our hospital from 2010 to 2014 were randomly selected, and the changes of antimicrobi-al drugs during the treatment period were analyzed. The surgical site infection probability and antimicrobial application rules in typeⅠincision surgery in 3 years, including probability, category, first time and duration of drug use,and the relationship between antibacterial drugs and typeⅠincision infection were analyzed. Results Application of antibacterial drugs in typeⅠincision operation probability decreased significant-ly,and the surgical infection probability decreased year by year, with statistically significant difference(P<0.05).The first application of an-tibacterial drug time was more reasonable, the choice of antimicrobial drugs were mainly one generation or two generation of Cephalosporin. There was a significant increase in the probability of stopping medication within 2 days after surgery, and the difference was statistically signif-icant(P<0.05).Conclusion The application of antibiotics should be strictly applied to the application of the indications for the use of anti-biotics.To understand the scope of application of antimicrobial drugs, and should try to shorten the use of time, control of infection during peri-operative period, reduce drug resistance.

Objective To evaluate the clinical, neuroimaging and electrophysiology features of 62 patients with multiple system atrophy(MSA).Methods Sixty-two cases with diagnosis of probable MSA were recruited in a retrospective studied.Clinical, neuroimage and external anal sphincter electromyography (EAS-EMG)data was retrospectively analyzed. Results In 62 cases(44 male and 18 female),the onset age was between 37 and 76.Among them,29 cases(46.8 % )were MSA-A,with orthostatie hypotension as the main clinical manifestation;24 cases(38.7 % )were MSA-C,with cerebellar ataxia ag the main chnical manifestation;9 cases(14.5 % )were MSA-P,with extrapyramidal symptoms as the main clinical manifestation.MRI showed that main lesion of MSA-A was in the cerebellum:that of MSA-C was in the cerebellum, pons and medulla;and that of MSA-P was in the putamen.Fifty-one cases did EAS-EMG and 46 cases showed neurogenie impairments.Nineteen cases were initially misdiagnosed with other diseases. Conclusions MSA is easy to be omitted or misdiagnosed at early stage. The diagnostie rate of MSA can be increased by the combination of clinical expressions,neuroimage,EAS-EMG and other necessary examinations.

Journal of Applied Clinical Pediatrics 2006;0(19):-
Objective To investigate the different expressions of adiponectin receptor 2(AdipoR2) mRNA in liver between diet-induced obese rats and controls. Methods The level of AdipoR2 mRNA expression was detected by reverse transcriptase polymerase chain reaction(RT-PCR).Insulin sensitivity was evaluated by insulin sensitivity index(ISI).Results Diet-induced obese rats showed higher expression of AdipoR2 mRNA compare to controls. The ISI of diet-induced obese rats was lower than that of controls. Conclusion High energy diet induceds AdipoR2 expression in liver.

Objective:To evaluate the molecular diagnosis marker of papillary thyroid carcinoma (PTC),the relationship between lymphatic metastasis of central neck compartment PTC,and the opera-tion indication of prophylactic central neck dissection. Methods:We conducted a retrospective study, including 275 PTC patients and detected their BRAF mutation rates during 201 2 and 201 4 and explored the risk factors of the central node lymphatic metastasis by Logistic regression model. Results:Of the 275 PTC patients,224 (81 .5%)were female and 51 (1 8.5%)were male.BRAF mutational rates were 53.8% (1 48 /275)and lymphatic metastasis 57.8% (1 59 /275).Multivariate analysis showed calcifica-tion (ORadjusted =1 .47,95%CI:1 .1 0 -1 .98,P =0.01 ),tumor diameter (ORadjusted =1 .48,95%CI:1 .04 -2.30,P =0.048)and age (ORadjusted =1 .48,95%CI:1 .04 -2.30,P =0.048)were associa-ted with lymphatic metastasis. In stratified analysis,BRAF mutation (ORadjusted =3.1 9,95%CI:1 .1 8 -9.43,P =0.023 )in clear boarder group and BRAF mutation (ORadjusted =4.84,95% CI:1 .68 -1 3.84,P =0.003)in calcification group were more likely to have lymphatic metastases. Conclusion:Central neck metastasis takes up a high ratio in papillary thyroid cancer patients,BRAF mutation in pa-pillary thyroid carcinoma is a characteristic molecular event.Furthermore, patients with calcification un-der ultrasound detection, lower age group and longer tumor diameter are more susceptible to suffer central neck metastasis.Especially for stratified analysis,non-calcified BRAF mutation or BRAF mutation with clear border under ultrasound detection are more susceptible to suffer central neck metastasis, and radical prophylactic central neck dissection should be carried on for these patients.

Objective To report the result of analysis of 72 cases of facial cutaneous sporotrichosis. Methods Seventy-two patients were included in this study. They were diagnosed as facial cutaneous sporotrichosis according to clinical mainfestations and fungal culture. Histopathological examinations were done in some atypical cases. The patients were treated with combined antifungal agents. Results The lesion forms were varied, in which lymphangitic cutaneous form accounted for 9.72% and fixed cutaneous form accounted for 90.28% respectively. Nineteen cases (26.4%) had a history of skin trauma before the cutaneous lesions. All of the patients were successfully treated with potassium iodide and/or itraconazole, terbinafine for 6-8 weeks. Conclusions The skin lesions and mycological examination are the important evidences for the diagnosis of sporotrichosis. The main form of the facial cutaneous sporotrichosis is the fixed cutaneous form. The misdiagnosis rate is 16% in primary level clinics. Potassium iodide is an effective and inexpensive medicine in the treatment of cutaneous sporotrichosis.

Objective Intranuclear forkhead/winged helix transcription factor p3(Foxp3) plays a key role in T cell-mediated immunosuppression. The present study was performed to investigate the effects of high mobility group box-1 protein(HMGB1) on Foxp3 gene as well as protein expressions in splenic regulatory T cells(Tregs) and their potential regulating mechanisms in mice. Methods CD4+CD25+Tregs isolated from the spleens of male BABL/c mice by magnetic beads were seeded on 96-well(1?105 cells/well) cell culture plates coated with anti-CD3(1 ?g/ml) and soluble anti-CD28(1 ?g/ml),and the cells were stimulated with HMGB1 at various intervals or at different concentrations. After being stimulated, the Foxp3 mRNA/protein expressions in the Tregs were determined. The time-dependent and dose-dependent responses between HMGB1 and intranuclear Foxp3 expression were analyzed by flow cytometry, and the expressions of Foxp3 mRNA of Tregs were analyzed by quantitative PCR of SYBR GREEN.Results After stimulation with HMGB1,the intranuclear Foxp3 protein and mRNA expressions of splenic Tregs in mice were markedly down-regulated in 24 h to 72 h(P
