1.Preliminary results of locally recurrent nasopharyngeal carcinoma treated with intensity modulated radiation therapy

Purpose:To evaluate the physical dose distributions and tolerance in locally recurrent nasopharyngeal carcinoma treated with intensity modulated radiation therapy (IMRT).Methods:From June 1999 to August 2000, 10 patients with locally recurrent nasopharyngeal carcinoma proven by histology, without neck lymphoid node recurrence and distant metastasis, were treated by IMRT. The interval from the time of initial radiotherapy to recurrence ranged from 14—50(median 25) months. The initial radiotherapy in all patients were treated with external radiotherapy alone to a median dose of 69Gy/35fractions/52days. IMRT method was performed by means of the MIMiC collimator and inverse treatment planning system (NOMOS company, USA). The prescribed dose of re radiotherapy to target volume was 57Gy/19fractions/4weeks.Results:All patients, except for some patients with mild weight loss and reaction in the oral cavity during radiotherapy, tolerated this IMRT well. IMRT achieved the better immediate responses in 10 patients with complete response in 6 cases and partial response in 4 cases. Recurrence was found in two patients, lung distant metastasis in one patient. The median survival in the 10 patients was 14 months. The median planning target volume (PTV) was 95.8(60—134) cm 3. The dose distributions in the PTV were as follows: median average dose 59.65?2.47Gy, median homogeneity index (HI): 1.18(?0.06); median dose to 95% PTV ≥ 53.2 (?1.36)Gy. The median dose to the organs at risk (OAR) were as follows: spinal cord 9.46?5.23Gy; brain stem 20.24?3.55Gy; left parotid gland 18.53?5.30Gy, right parotid gland 19.68?6.21Gy; left lens 2.11?0.65Gy, right lens 2.94?0.57Gy;Chiasm12.34Gy; left optical nerve13.14 Gy, right optical nerve17.65 Gy.Conclusions:Better dose distributions treated with IMRT was achieved in the patients with locally recurrent nasopharyngeal carcinoma and the patients tolerated this radiotherapy well. 2.Clinical analysis of early allograft dysfunction in patients receiving donation after cardiac death liver Transplantation

Objective To investigate the occurrence and influencing factors of early allograft dysfunction (EAD) in patients receiving donation after cardiac death (DCD) liver Transplantation.Methods Forty-five patients received orthotopic liver transplantation in our center during January 2016 and May 2016 were included in this study.Based on the occurrence of EAD,patients were divided into EAD group and non-EAD group.Perioperative data of donors and recipients were collected to analyze the risk factors of EAD and the effect of EAD on the short-term prognosis of patients.Results In 45 patients,the incidence of EAD was 57.8% (26 cases).During the follow-up period,the mortality of recipients in EAD group and non-EAD group was 15.4% and 15.8% respectively,which showed no significant difference.Donor age,preoperative ALT and AST levels,cold ischemia time (CIT) were significantly increased in EAD group than in non-EAD group (43.7 ±2.5 and36.2±2.5,P=0.0409;64.2 ±13.2 and 31.0 ± 5.9,P=0.0407;87.3±16.2 and48.2±6.2,P=0.047 3;629.5 ± 35.2 and 484.6 ± 30.5,P =0.004 0,respectively).Perioperative CRP level in EAD group was significantly higher than in non-EAD group (22.6 ± 5.9 and 4.7 ± 1.1,P =0.012 1).What's more,postoperative platelet level was significantly lower in EAD group than in nonEAD group (73.7 ± 8.0 and 111.7 ±16.0,P =0.0439).However,pre-and post-operative neutrophil-lymphocyte ratio (NLR) in recipients showed no significant difference between EAD group and non-EAD groups.Conclusion Older donor age,high levels of preoperative ALT and AST,prolonged cold ischemic time,as well as high level of preoperative CRP and decrease of postoperative platelet level in recipients are risk factors for the occurrence of EAD after liver transplantation.

This study examined the effect of GHRP-6, a known GHSs receptor agonist, on the phosphorylation of cAMP-responsive element-binding protein (CREB) and the underly mechanism. GH3 cells were cultured and subjected to different treatments as follows: GHRP-6, GHRP-6 plus GHRH, phorbol ester (PMA), an activator of PKC, alone or in combination with GHRP-6, Gö6983, a general inhibitor of PKCs, in the presence or absence of GHRP-6, rottlerin, an inhibitor of PKCs, alone or plus GHRP-6. The cells were transiently transfected with PKCsigma-specific siRNA and then treated with GHRP-6. GH level was measured by enzyme-linked immunosorbent assay (ELISA). The expression of phosphor-CREB, PKCsigma, PKCtheta and phosphor-PKCsigma was determined by Western blotting. The results showed that GHRP-6 stimulated GH secretion in both time- and dose-dependent manners and enhanced the effect of GHRH on GH secretion. GHRP-6 was also found to induce CREB phosphorylation. Moreover, GH secretion was enhanced by the PKC activator PMA and reduced by the PKC inhibitors (Gö6983, rottlerin) and knockdown of PKCsigma. PKCsigma could be activated by GHRP-6. It is concluded that PKC, especially PKCsigma, mediates CREB phosphorylation and GHRP-6-induced GH secretion.

5.Reconstructive operation with transpositional colon behind sternum for esophageal stricture after corrosive burns

Objective To summarize clinical experience of reconstructive operation with transpositional colon behind the sternum after corrosive esophageal burns and to explore the treatment for its complications.Methods Clinical data of 65 cases with esophageal scarred stricture after corrosive burns receiving reconstructive operation with transpositional colon behind the sternum were reviewed,56 of them by end-to-end anastomosis between transpositional anterograde peristaltic colon and esophagus,seven by end-to- end anastomosis between transpositional anterograde peristaltic colon and pharyngeal fundus,and two by end- to-end anastomosis between transpositional reversed peristaltic colon and esophagus,to summarize treatment experiences in pre-operation,operation and post-operation.Results Fifty-one of this group of patients recovered and discharged form the hospital smoothly,12 with cervical anastomotic leakage after operation including two cured by re-operation and ten cured by conservative treatment,and two with necrosis of transpositional colon including one died during operation and the other cured.Conclusions Corrosive burns of esophagus can be cured by leaving scarred stricture esophagus open without resection,and the effectiveness of reconstructive operation with transpositional colon behind the sternum is satisfactory with good pre-operative preparation,correct surgical operation,and correct post-operative treatment.
6.The clinical application and follow-up study of f non-invasive prenatal testing

Objective To provide valid data and useful genetic counseling in the clinical application of non‐invasive prenatal test (NIPT) ,fetal chromosomal disorder were screened by massive parallel sequencing and made a follow‐up study .Methods Preg‐nant women with Down screening in high‐risk were screened by NIPT ;NIPT verified high‐risk individuals were suggested for kary‐otyping ;and we follow up on whoever showed low risk by NIPT before and after their deliveries .Results (1)Totally 1 676 cases of pregnant women were tested by NIPT ,25 cases prompted to be abnormal ,with an abnormal rate of 1 .49% ,karyotype analysis re‐sults in 12 cases of abnormalit ,the accuracies of NIPT for T21 ,T18 ,XO ,XXY ,and XYY were 99 .93% ,100 .00% ,99 .66% , 100 .00% ,100 .00% respectively ;the accuracy of NIPT for women with advanced paternal age and twins were both 100 .00% ;kary‐otyping positive individuals underwent abortion ,which gives a prenatal intervention rate of 100 .00% .(2)Out of 1 651 cases of NIPT low risk testers ,1 468 cases were successfully followed up ,with a 88 .91% success rate .We found chromosome abnormality with one case of inversion of chromosome 9 (maternal) .(3)Ultrasound‐detection possessed 98 .17% accuracy and 7 .69% in detec‐tion rate;in high‐risk pregnant woman ,Down screening had an accuracy of 0 .88% and false positive rate of 99 .12% ;98 .71%women were avoided prenatal diagnosis via NIPT .Conclusion Compare to ultrasound and maternal plasma screening ,NIPT is a far more accurate prenatal screening approach .To build effective follow‐up and service systems of NIPT is necessary to reduce birth de‐fects in medical institutions .
7.Meta-analysis of safety of human purified Vero cell rabies vaccine after exposure

Objective To evaluate the safety of human purified Vero cell rabies vaccine(PVRV)after exposure in China by Meta-analysis.Methods With rabies,vaccine and safety as key words,a systematic search was performed in PubMed,EMBASE,Cochrane and China National Knowledge Infrastructure(CNKI),supplemented by manual retrieval.A Meta-analysis was performed to analyze the incidence of adverse events of two immunization regimens Zagreb and Essen using Review Manager 5.4 software after literature screening and data extraction according to the inclusion and exclusion criteria.Results A total of 12 studies were included,of which 7 were prospective studies and 5 were retrospective studies.Most included in the studies showed a low risk of bias.The incidence of adverse events in Zagreb regimen was significantly higher than that in Essen regimen[relative risk(RR)= 1.01,95% CI = 0.90 ~ 1.14;I2= 73.00%,P<0.05],but there was a high degree of heterogeneity.The incidence of fever,pain and induration in Zagreb regimen was significantly higher than that in Essen regimen(RR = 1.14,0.92 and 0.86,95% CI = 0.82 ~ 1.60,0.73 ~ 1.14 and 0.29 ~ 2.51;I2= 73.00%,P<0.05],but there was a high degree of heterogeneity.The incidence of fever,pain and induration in Zagreb regimen was significantly higher than that in Essen regimen(RR = 1.14,0.92 and 0.86,95% CI = 0.82 ~ 1.60,0.73 ~ 1.14 and 0.29 ~ 2.51;I2= 81%,65% and 92%,respectively,P<0.01).Conclusion Two regimens of PVRV vaccination after exposure showed good safety.However,when adopting Zagreb regimen,attention should be paid to the physical conditions of children and the elderly with relatively poor immunity to avoid adverse events.
8.Polybrominated diphenyl ethers load in human serum of Taiyuan city in 2010.

OBJECTIVEThis study was conducted to characterize polybrominated diphenyl ethers (PBDEs) load in human serum of Taiyuan city in 2010.
METHODSA total of 42 blood samples were collected in Taiyuan city, 2010. Solid-phase extraction (SPE)-gas chromatography-negative chemical ionization-mass spectrometry method was used to measure the concentration of brominated diphenyl ethers (BDE)-17, BDE-28, BDE-47, BDE-66, BDE-99, BDE-100, BDE-153, BDE-154, BDE-183 and BDE-209 and ΣPBDEs was calculated.
RESULTSOf the tested serum samples, ΣPBDEs ranged from 2.09 to 66.74 ng/g of lipid weight (lw), the median and mean was 6.32 ng/g lw and 9.84 ng/g lw, respectively. BDE-209 was the main source of ΣPBDEs and its concentration ranged from no detection to 64.99 ng/g lw, the median was 4.11 ng/g lw, accounting for 67.6% of ΣPBDEs. Second source was BDE-153 and its concentration ranged from 0.29 to 4.92 ng/g lw, the median was 0.62 ng/g lw accounting for 10.1% of ΣPBDEs.
CONCLUSIONThe overall load of PBDEs in the serum of local residents was low in 2010.

9.Recent progress in nanotechnology for cancer therapy.

The application of nanotechnology significantly benefits clinical practice in cancer diagnosis, treatment, and management. Especially, nanotechnology offers a promise for the targeted delivery of drugs, genes, and proteins to tumor tissues and therefore alleviating the toxicity of anticancer agents in healthy tissues. This article reviews current nanotechnology platforms for anticancer drug delivery, including polymeric nanoparticles, liposomes, dendrimers, nanoshells, carbon nanotubes, superparamagnetic nanoparticles, and nucleic acid-based nanoparticles [DNA, RNA interference (RNAi), and antisense oligonucleotide (ASO)] as well as nanotechnologies for combination therapeutic strategies, for example, nanotechnologies combined with multidrug-resistance modulator, ultrasound, hyperthermia, or photodynamic therapy. This review raises awareness of the advantages and challenges for the application of these therapeutic nanotechnologies, in light of some recent advances in nanotechnologic drug delivery and cancer therapy.

10.Study on the relationship between organochlorine pesticides and level of residues in serum and daily foodstuff.

OBJECTIVETo study the contents of organochlorine pesticides in human bodies and residues in serum of populations with non-occupational exposure as well as to study the relationship between organochlorine pesticides in foodstuff and residues levels in serum.
METHODSA cross-section study was conducted. 107 men and 142 women who were all healthy and living in the communities were investigated from Mar. 2004 to Jul. 2004. Level of daily food exposure was estimated through questionnaires while DDTs and HCHs serum levels were detected by EC-ECD. The relationship between organochlorine pesticides contents in foods and residues in serum were analyzed by ridge regression.
RESULTSFresh fish was positively correlated to men's serum level of beta-HCH and p,p'-DDT (beta = 0.1266 and beta = 0.0595) while vegetables and fruits were negatively correlated to women's serum level of beta-HCH (beta = -0.1066). Soybean was negatively correlated to women's serum level of p,p'-DDE and p,p'-DDT (beta = -0.0965 and 3 = -0.0581). Alcohol consumption was negatively correlated to men's serum level of beta-HCH and p,p'-DDE and women's serum level of p,p'-DDE (beta = -0.1315, beta = -0.1599 and P = -0.1128).Salted meat was negatively correlated to men's serum level of beta-HCH and p, p'-DDT (P = -0. 066 and P = - 0.0569).
CONCLUSIONIn this study, fresh fish might increase the body burden of organochlorine pesticides and residues while alcohol might promote the excretion of organochlorine pesticides. Pickled meat and vegetal foodstuff might contain low-level of organochlorine pesticides and residues.
