1.Determination of Main Component in Megestrol Acetate Tablets by RP-HPLC

OBJECTIVE:To establish the RP-HPLC method for the determination of main component in megestrol acetate tablets.METHODS:RP-HPLC was carried out using a SinoChrom OPS-AP column with column temperature kept at room temperature.The detection wavelength was set at 288 nm.The mobile phase was composed of water-acetonitrile(20∶80,containing 2% triethylamine,pH 5.28)at a flow rate of 1.0 mL?min-1 with dixiben served as internal standard.The injection volume was 20 ?L.RESULTS:The standard curve was linear over the range of 7~336 ?g?mL-1 for megestrol acetate(r=0.999 6)and its recovery rate was 99.95%(RSD=0.51%).CONCLUSION:The method is simple,reproducible and accurate,and it is applicable for the determination of the megestrol acetate tablets.
2.Consistency of three measures commonly used in the assessment of balance and functional outcome of patients with post-stroke hemiplegia

BACKGROUND: To assess the balance function of post-stoke patients would be conducive to establish rehabilitative goal and to set up therapeutic plan.OBJECTIVE: To investigate the relationship between parameters of three commonly used measures and those of other functional evaluation for patients with post-stroke hemiplegia.DESIGN: Analysis of the relationship between parameters of balance rating scales.SETTING: At otolaryngology department and rehabilitative medicine department of the general military hospital.PARTICIPANTS: From September 2002 to July 2003, totally 57 patients with post-stroke hemiplegia, 45 males and 12 females, aged from 14 to 78years old, with the course of disease ranged from 0. 3 to 11 months, received rehabilitative treatment at rehabilitative department of the general military hospital, were selected to take part in the study. All were confirmed of internal carotid artery system lateral hemispheric disease.METHODS: Three balance measures, namely Berg balance scale, modified Balance subscale of the Fugl-Meyer test and Postural assessment scale for stroke patients were administered on 57 patients for evaluating their lower limb motor function, daily life capability and walking ability after rehabilitative treatments. The relationship between three measures was analyzed with Pearson-correlative analysis, and the relationship between three measures and lower limb motor function, daily life capability and walking ability were also investigated.MAIN OUTCOME MEASURES: ① relationship between three measures;② relationship between three measures and lower limb motor function, daily life capability and walking ability.RESULTS: The data from 57 stroke hemiplegic patients was analyzed without missing. ① Scores for Berg balance assessment, Fugl-Meyer balance rating scale and posture controlling scale for stroke patients was 46.70 ± 5. 15, 10. 26 ± 1.58 and 32.56 ± 2.06, respectively, and the scores of lower limb motor function in Fugl-Meyer assessment was 22. 35 ± 5.82,and moving and marching item in functional independence assessment 24.95 ± 5.2, 10 m maximum walking speed(49.06 ±24.68) m/s. The three measures were proved of significantly higher consistency( r = 0.613-0. 766,P < 0.001 ). ② The three measures were also found to be obviously associated with the lower limb motor function, daily life capability and walking ability of patients with stroke( r = 0. 479-0. 718, P < 0. 001 ).CONCLUSION: Berg balance assessment, modified Fugl-Meyer balance rating scale and posture-controlling scale have been proved of higher consistency and efficacy for assessing the balance function of post-stroke patients, thereby can be used as predictor for the balance functional outcomes,as well as the recovery of the lower limb motor function, daily life capability and walking ability of post-stroke patients.
3.Berberine preconditioning protects against hepatic cold ischemia reperfusion injury through the modulation of PI3K/Akt/mTOR signaling pathway

Objective To confirm the protective effect of berberine (BBR) on cold ischemia reperfusion (I/R)-induced liver injury and to show whether the hepatic protection conferred by BBR involves the activation of phosphatidylinositol 3 kinase (PI3K) / protein kinase B (Akt)/mammalian target of rapamycin(mTOR) signal pathway.Method Adult male Sprague-Dawley rats were assigned randomly to four groups:BBR group (BBR was intragastrically administered at a dose of 100 mg·kg-1 · d-1 2 weeks before hepatic cold I/R treatment),dimethyl sulfoxide (DMSO) group (BBR was replaced by DMSO,and others were the same as BBR group),I/R group (BBR was replaced by normal saline,and others were the same as BBR group) and sham group (normal saline was administered 2 weeks before opening and closing abdomen treatment).Then the rats were sacrificed at 3,6,and 24 h after reperfusion.The liver function,oxidative stress level,apoptosis rate,and the expression of PI3K/Akt/mTOR related pathway proteins were assayed.Result As compared with sham group,the I/R-induced liver tissue displayed severe lobular distortion with widespread necrosis,high level of oxidative stress and apoptosis rate.As compared with I/R group,BBR dramatically attenuated the histopathologic damage,restored the liver function and decreased the oxidative stress level.Simultaneously,BBR significantly ameliorated the apoptosis by decreasing the apoptosis rate,increasing the Bcl-2/Bax ratio and inhibiting caspase-3 activity in rats subjected to hepatic I/R.The expression of p-Akt was effectively upregulated with the inhibited expression of p-mTOR.Conclusion Our result provides robust in vivo evidence that BBR can prevent I/R-induced oxidative stress and apoptosis.The mechanisms involved can be attributed to the activation of P]3K/Akt/mTOR signal pathway.
4.Magnetic resonance imaging characteristics of visuospatial location defect in patients with mild cognitive impairment

BACKGROUND: Patients with Alzheimer disease (AD) usually have visuospatial dysfunction at an early stage and most patients with AD were evolved from mild cognitive impairment (MCI).OBJECTIVE: To detect whether a patient with MCI has a visuospatial dysfunction similar to AD. To compare the cerebral activation with ferromagnetic resonance between the healthy people and the patients with mild cognitive disorder.DESIGN: A case controlled observationSETTING: The Department of Neurology of the First Hospital of Peking University.PARTICIPANTS: Nine inpatients with mild cognitive disorder hospitalized at the Department of Neurology of the First Hospital of Peking University from April 2000 to March 2003 were involved and other 9 healthy people matching in age, gender and the educational level were also involved.METHODS: The subjects had the task of discriminating the different finger positions on the clockface. They could see the mission clearly showed on the cloth through refractor, and they were supposed to give the corresponding reaction to different cognitive tasks with left and right hands pressing the keys according to visual sign generated by the computer.Block design was adopted in the cognitive task. Stimulus block and baseline block appeared in turn. Baseline block displayed "+" mapped in the center of the image and was kept for 21s. Stimulus block displayed 10 pictures successively. The finger position of the two clockfaces among three clockfaces was the same in one picture. The absolute value of the rotated angles of the different clockfaces in each image on the same stimulus block was the same. The angles formed by six stimulus block fingers arranged randomly were 300, 450, 1800. The two clockfaces had the same finger positions, and the subjects were requested to discriminate the finger position of these two clockfaces. It was carried out on a 1.5 T GEMRTWIN magnetic resonance scanning apparatus. Then t test was performed to work out the right reaction time and proper rate with Excel software between the patients with mild Alzheimer's disease and the healthy persons. Pretreatment and statistical analyses were performed with analysis of functional neuroimages software used internationally.MAIN OUTCOME MEASURES: Comparison of the reaction time and proper rate between the two groups.parison of the reaction time between the two groups: The reaction time for distinguishing the change of 300, 450, 1800 was longer than that in the control group, and there was significant difference in distinguishing the change of 300, 450, 1800 for the patients [(1 776.7±570.2), (1 646.3±432.7)ms;of the proper rate between the two groups: The proper rate for distinguishing the change of 300, 450, 1800 was significantly lower than that in the control group (62.8±21.4)%, (82.8±15.8)% ;(76.7±17.0)%, (95.0±8.7)%; (69.4vation: the activation density at the bilateral parietal lobe, bilateral temporooccipital borderline and bilateral lateral striate cortex in patients was weaker than that in the control group (P < 0.05).CONCLUSION: Neural network activation related with visual space in patients with MCI was weaker than that in the healthy people indicating that visuospatial impairment exists in the patients with mild cognitive impairment.
6.MAPK mediates cardiac myocyte survival promoted by CT-1 in rats

Objective Cardiac muscle cells play a critical role in maintaining normal function of the heart.Cardiotrophin-1(CT-1),a potent cardiac survival factor,is capable of inhibiting apoptosis or promoting survival in cardiomyocytes.To elucidate the mechanism of CT-1 promoting cardiac myocyte survival in cultured neonatal rat cardiomyocytes.To explore the potential signaling pathway that might be responsible for this effect.Methods We examined the cardiac myocyte survival effect of CT-1 in cultured neonatal rat cardiomyocytes.The cardiomyocytes were stained [3-(4,5-dimethyl-thiaziazol-2-yl)-2-5-diphenyltetrazolium bromide,MTT] and the counted.Results The survival rate of cardiac myocytes was increased by CT-1 in a dose-dependent manner(10-10~10-7 mol/L) and in a time-dependent manner(1~4 d,10-8 mol/L) in cultured neonatal rat cardiomyocytes.Pretreatment of PD098059(5?10-5mol/L),a MAPK blocker,decreased significantly survival rate of cardiac myocytes by promoted CT-1.The phorbol 12-myristate 13-acetate(PMA)(10-5mol/L),a PKC activator,increased significantly this effect of CT-1,but inhibited significantly by MAPK blocker PD098059.Conclusion CT-1 is a potent factor of promoting cardiac myocyte survival,and increase significantly survival rate of cardiac myocytes in a dose-dependentand a time-dependent manner in cultured neonatal rat cardiomyocytes.The MAPK signaling pathway mediates CT-1 induced cardiac myocyte survival.PKC signaling molecule may be a upstream signaling transduction pathway which cascades of MAPK in CT-1 induced cardiac myocyte survival.
7.Suction drainage replacement or not in 50 patients undergoing total knee arthroplasty:Randomized controlled comparison

8.Effect of dexmedetomidine on myocardial injury in pediatric patients undergoing living-related liver transplantation

Objective To evaluate the effect of dexmedetomidine on myocardial injury in pediatric patients undergoing living-related liver transplantation. Methods Fifty-eight pediatric patients of both sexes,aged 5-20 months,weighing 4.5-15.0kg,of American Society of Anesthesiologists physical status Ⅲ or Ⅳ,scheduled for elective living-related liver transplantation,were divided into 2 groups(n=29 each)using a random number table:control group(group C)and dexmedetomidine group(group D).In group D,dexmedetomidine was intravenously infused at a dose of 0.5 μg/kg over 10min starting from the time point immediately before skin incision,followed by an infusion of 0.8 μg·kg-1·h-1 until the end of surgery. The equal volume of normal saline was given instead in group C. Immediately before skin incision(baseline,T0),at 10min of anhepatic phase(T1),at 30min of neohepatic phase(T2)and at the end of surgery(T3),blood samples were obtained from the central vein for determination of serum cardiac troponin I(cTnI),lactate dehydrogenase(LDH),alpha-hydroxybutyrate dehydrogenase(α-HBDH),interleukin-6(IL-6)and IL-10 concentrations. The changing rate of serum cTnI concentrations were calculated at T2. The occurrence of myocardial ischemia and ventricular premature beat and requirement for dopamine were recorded during surgery. Results Compared with the baseline at T0,the serum concentrations of cTnI,LDH and α-HBDH were significantly increased at T2,3,and the serum concentrations of IL-6 and IL-10 were increased at T1-3 in both groups(P<0.05).Compared with group C,the serum concentrations of cTnI,LDH,α-HBDH and IL-6 were significantly decreased at T2,3,the serum concentration of IL-10 was increased at T1-3,the changing rate of serum cTnI concentrations was decreased(P<0.05),and no significant change was found in the incidence of myocardial ischemia and ventricular premature beat and requirement for dopamine in group D(P>0.05).Conclusion Dexmedetomidine can attenuate the myocardial injury to some extent in pediatric patients undergoing living-related liver transplantation.
9.Clinical observation of the tension-free vaginal tape-obturator treatment for 130 cases of female stress urinary incontinence

Objective To observe the clinical short-and long-term curative effect of the surgical treatment of female stress urinary incontinence (SUI) with tension-free vaginal tape-obturator (TVT-O) treatment,and discuss the safety of the operation and postoperative quality of life.Methods The data were collected from 130 patients with SUI who were underwent TVT-O treatment.The patients'perioperative period,follow-up of postoperative complications,and comparison of the quality of life before and after surgery were analyzed retrospectively.Results A total of 130 patients was successfully completed their surgeries with a mean operative time (47.01 ± 18.82)min,average blood loss (64.38 ±99.62)ml,mean catheterization (2.67 ±0.90)d,and the average length of stay (4.73 ±2.14)d.A total of 101 cases was completed the postoperative follow-up with (a) preoperative quality of life in patients with ⅡQ-7 score 6 to 21 points and an average of (15.74 ± 3.87) min,(b) symptoms of lower urinary tract UDI-6 score of 3 to 22 minutes and the average (10.51 ± 3.70) min,(c) postoperative quality of life improved significantly ⅡQ-7 score from 0 to 21 points and an average of (1.59 ±4.37)points,and (e) UDI-6 score 0 to 14 points and an average of (1.63 ± 2.66)points.A total of 73 patients (72.3%) had the postoperative urinary incontinence,which subjective symptom were completely cured,significant improvement was 11 cases (10.9%),ineffectiveness was 6 cases (5.9%),pad using was 13 patients (12.9%),vaginal mesh exposure and erosion was 2 patients(2.0%),the sexual life postoperatively was affected in 8 patients (7.9%).Conclusions TVT-O treatment of SUI is not only easy to operate but also has clinically high security,few complications,low recurrence rate,and significantly improved patients' living quantities.However,patch compli cations can not be ignored and need further discussion.
10.Analysis of carbapenemase genotypes of carbapenem-resistant Proteus mirabilis strains clinically isolated from 2009 to 2012

Objective To investigate the clinical distribution and the drug resistance of carbapenemresistant Proteus mirabilis strains isolated from 2009 to 2012 ; and to study carbapenemase genotypes of the isolates.Methods A total of 15 non-repetitive carbapenem-resistant Proteus mirabilis strains were selected from 422 Proteus mirabilis strains isolated from Ningbo First Hospital during January 2009 to December 2012.The minimal inhibitory concentrations (MIC) of 6 antibacterial agents,including imipenem (IPM),meropenem (MEM),ertapenem (ETP),ciprofloxacin (CIP),amikacin (AK) and minocycline (MIN),against 15 isolates were determined by E-test.The modified Hodge test (MHT) was used to detect the carbapenemase production in isolates.The PCR assay was performed to detect drug resistance genes of blaKPC,blaNDM-1,blaGES,blaSME,blaIMI-1/NmcA and blaSHV-38.Plasmids were extracted from the blaKPC-positive strains and then transformed into Escherichia coli J53 strains by electroporation.The transformed and untransformed Escherichia coli J53 strains were tested for MIC values and blaKPC gene by E-test and PCR respectively.Results The resistance rates of the 15 carbapenem-resistant Proteus mirabilis strains to IPM,MEM,ETP,CIP,AK and MIN were 100%,100%,100%,86.7% (13/15),33.3% (5/15) and 80% (12/15),respectively.7 out of 15 strains were Hodge test positive,and 11 strains were blaKPC-2 positive.Results of PCR amplification showed that the transformed Escherichia coli J53 strains,whose MIC values to IPM,MEM,and ETP were increased by 2 to 64 times,were blaKPC-2 gene positive.Conclusion The carbapenem-resistant Proteus mirabilis strains in this study were resistant to many commonly used antibiotics,however,the resistance rates to AK were relatively low.The dominant carbapenemase genotype was blaKPC-2 carried by the plasmid.Attention should be paid to its easily transmissible feature among the strains in clinic.
