Although premature ejaculation (PE) is a common type of male sexual dysfunction, to date we lack a unified definition of PE. The multidimensional definition of PE has been accepted by more and more clinicians. Intravaginal ejaculatory latency time (IELT) is one of the three important dimensions (time to ejaculation, inability to control or delay ejaculation, and negative consequences) for defining PE. Rapid ejaculation is one of the core symptoms of PE and IELT is an objective measurement as well as an important tool for the evaluation of PE. This article reviews estimated IELT, stopwatch-measured IELT, the correlation between estimated and stopwatch-measured IELT, and the factors affecting IELT in the general male population, PE patients, and those complaining of PE.

Objective To study the changes of pulmonary function in patients with active rheumatoid arthritis(RA).Methods Sixty patients with active RA were divided into low lung function group and normal lung function group by detecting their pulmonary function.The clinical symptoms and signs,laboratory indicators,health index(HAQ) score,disease activity score(DAS-28) and regulate T-cells(Treg) were observed.Results In 60 patients with active RA,42 cases(70%) were lower lung function and 18 cases(30%) were normal lung function.Compared with normal lung function group,Hb of low lung function group significantly lowered(P

OBJECTIVE:To compare therapeutic efficacy and safety of lobaplatin or cisplatin combined with tegafur,gimeracil and oteracil potassium in the treatment of advanced metastatic breast cancer.METHODS:A total of 160 patients with advanced metastatic breast cancer were randomly divided into observation group and control group,with 80 cases in each group.Control group was given Cisplatin injection 30 mg/m2 intravenously,every 3 weeks Tegafur,Gimeracil and Oteracil Potassium capsules 50 mg orally after meal,twice a day,for consecutive 14 days.Observation group was given Lobaplatin for injection 30 mg/m2 intravenously,every 3 weeks+Tegafur,Gimeracil and Oteracil Potassium capsules (same usage and dosage as control group),every 3 weeks.A treatment course lasted for 3 weeks,and both groups received 2 courses.Short-term efficacies (ORR、DCR),chemotherapy effects of lymph node,lung,bone and liver,ADR and long-term efficacy were compared between 2 groups.RESULTS:After treatment,ORR(67.50％ vs.46.25％),DCR(85.00％ vs.66.25％),ORR of lymph node metastasis (71.43％ vs.47.83％),ORR of lung metastasis (60.71％ vs.40.00％),DCR of lung metastasis (78.57％ vs.56.00％),ORR of bone metastasis (28.57％ vs.16.67％),1-year survival rates (75.00％ vs.52.50％) and 2-years survival rates (42.50％ vs.17.50％) of observation group were significantly higher than control group;the incidence of chemotherapy ADR in observation group was significantly lower than control group (43.75％ vs.70.00％),with statistical significance (P＜0.05).There was no statistical significance in lymph node metastasis DCR,bone tissue metastasis DCR,liver metastasis ORR and DCR,or half year sarvival rate between 2 groups (P＞0.05).CONCLUSIONS:Compared to cisplatin combined with tegafur,gimeracil and oteracil potassium,lobaplatin combined with tegafur,gimeracil and oteracil potassium show better short-term therapeutic efficacy,therapeutic efficacy of lymph node metastasis,bone metastasis and lung metastasis,more than 1-year long-term therapeutic efficacy and safety in the treatment of advanced metastatic breast cancer.

ObjectiveTo investigate the effect of nickel-titanium(Ni-Ti) shape memory alloy embracing internal fixer on oblique or spiral fracture.Methods23 cases with spiral or oblique fracture accepted internal fixation with Ni-Ti shape memory alloy embracing.The time of bone healing,articular function,the appearance and function of the wound extremity were evaluated after 9～34 months follow-up.ResultsAll fractures healed in 4～16 weeks after operation.None infection,nonunion,embracing emersion or break had been found. The fine rate was 91.3%.ConclusionThe effect of Ni-Ti shape memory alloy embracing internal fixer on long spiral or long oblique fracture is satisfactory.

Objective To detect the level of nitric oxide (NO) and amino acids in cerebrospinal fluid and serum of epilepsy children in different time. Methods The level of NO and amino acids in cerebrospinal fluid and serum of epilepsy children in different time were determined with nitrate reductase and high performance liquid chromatography (HPLC) with fluorimetric detection. Results The level of NO2 - increased 30 min, 60 min, 120 min after seizures (P<0.01), but dropped to normal 180 min after seizures (P>0.05). The level of glutamate (Glu) significantly increased in all epilepsy groups (P<0.05). The level of glycine (Gly) dropped gradually in all epilepsy groups (P<0.05).Conclusion NO may participate in the generation of seizures and the injury process to brain. The imbalance between excitatory and inhibitory amino acids may play an important role in the pathogenesis of epilepsy. The toxicity of excitatory amino acids may be the main injury to the brain.

Objective:To observe the changes of lung index,lung function,regulatory T cells and Foxp3,TGF-β1,Smad3,Smad7 protein expression in adjuvant arthritis(AA) rats,and to investigate the possible mechanism of Foxp3 and TGF-β/Smads signal transduction pathway in reducing lung function.Methods: 24 Wistar rats were randomly divided into normal control group and model group,12 rats in each group.The rats were injected with 0.1 ml Freunds'complete adjuvant to induce inflammation at the part of their rightback foot in model group.On the 48~(th) day post-inflammatory,the swelling degree of toe and arthritis index(AI) were observed.Lung index of the rats was calculated,and pulmonary function of the rats was detected.The percentage of regulatory T cells was determined,and pathological changes of the lungs was observed by HE staining.Foxp3,TGF-β1,Smad3,Smad7 protein expression in the lungs of the two groups of rats was examined by immunohistochemistry.Results:①Compared with those of the normal,the swelling degree of toes,AI,and the lung index were increased.The average peak expiratory flow in 1 second (FEV1/FVC),alveolitis points,TGF-β1 and Smad3 protein expression was significantly higher in the AA model group rats(P＜0.01).Forced vital capacity (FVC),25% of vital capacity of the peak expiratory flow (FEF25),50% of vital capacity of the peak expiratory flow (FEF50),75% of vital capacity of the peak expiratory flow (FEF75),maximum mid-expiratory flow (MMF),forced maximal expiratory flow (PEF),pulmonary dynamic compliance (Cldyn),the percentage of CD4~+ T cell,CD25~+ T cell,CD4~+ CD25+~ T cell,Foxp3 and Smad7 protein expression was obviously lower in the AA model group rats(P＜0.01).②FEF50,Cldyn and MMF were negative correlated with the swelling degree of toes,pulmonary coefficient and TGF-β1.FEF50,FEF75,MMF and FEV1/FVC were positiyely correlated with the arthritis index,pulmonary coefficient and Foxp3(P＜0.05 or P＜0.01).Conclusion:The decline in lung function in AA rats.Tip-induced may be sustained after the inflammation,and the development of cause inflammation,to chronic inflammatory response leading to lung injury.Simultaneously,the decline in lung function and Foxp3.Smad3,Smad7,TGF-β1 protein expression was associated.Description transcription factor Foxp3 and TGF-β1/Smads signal transducation pathway may participate in the mechanism.

Objective To investigate the employment status of higher vocational medical col-lege graduates in Hainan province, analyze the existing employment problems, and put forward some effective suggestions. Methods A questionnaire was applied to survey the employment problems of 1 048 medical graduate in 11 medical related field of Grade 2012. A total of 989 valid questionnaires were collected. The questionnaires covered six aspects including graduates' basic information, their employment intention, access to employment information channel, service and satisfaction about school employment agencies, and employment psychology etc. SPSS 13.0 was used to analyze the statistical numbers in each option and the percentage was calculated. Results Only 22.9%(227/989) of the graduates were willing to accept the provincial employment, 49.1%(486/989) of the graduates obtained employment information channels through the school employment sector; 10.6%(105/989) of gradu-ates expressed dissatisfaction with employment services; graduates' entrepreneurial intention reached 26.9%(267/989); only 19.8% of graduates signed employment agreement, and 25.1%(248/989) of gradu-ates face the employment with anxiety and confusion. Conclusion The higher vocational medi-cal graduates in Hainan provinces need to change their employment concept and school employment guidance needs strengthening to link up the employer position requirements and expectations of grad-uates employment. Besides, the school enrollment professional settings and employment market demand also need to be linked up, to guarantee the system interface between career guidance, internship and training base.

Objective: The modulatory effects of nociceptin/orphanin FQ ( OFQ) on acupuncture ( EA, a odern version of acupuncture)-induced analgesia are still controversial. Transgenic OFQ knock-out mice rovide us a useful tool to investigate the role of endogenous OFQ in EA analgesia. The present tudy aims to investigate the role of OFQ in the EA-induced analgesia with OFQ knock-out mice. ethods; Acupoints were selected as "Zusanli" (ST36) and "Sanyinjiao" (SP6) , EA parameters were as ollows; constant current output, rectangular (square) wave pulses, increased intensities of 0. 5, 0. 7,0. 9 mA with stepwise fashion, 10 min for each intensity, frequency of 100 Hz (with 0. 2 ms as ulse width) or 2 Hz ( with 0. 6 ms as pulse width). Tail flick latency (TFL) evoked by radiant heat was used to evaluate the change of pain threshold before, during and after EA application. Thenock-out mice were divided randomly into 3 groups; needling control, EA at 100 Hz and 2 Hz. ild-type mice of litter-mates of were used as Control. Results: It was found that OFQ knock-out mice had a longer basal ther-mal threshold; EA had enhanced analgesic effect in the knock-out mice than in wild-type Control mice. Conclusion: Endogenous OFQ might be algesic in basal condition and antagonize EA analgesia.
