1.Effect of respirator resistance on tolerant capacity during graded load exercise.

Respirator breathing resistance impacts performance of wearers during constant work load. However, it is less clear as to how breathing resistance affects the tolerant capacity of users during graded work load. The present study investigated the tolerant capacity of 8 individuals during incremental work load. The 8 subjects were required to wear two matched respirators (respirators I and II which were designed to have different breathing resistances and the same dead space) respectively on separate days and then work to end points. Minute ventilation (V(E)), breathing frequency (BF), oxygen consumption (VO(2)) and heart rate (HR) were recorded during exercise, while tolerant time, response time and breathing discomfort were measured at the end of each test trial. The test variables were compared between the two respirators by using matched-pairs t-test. The results showed that the tolerant time was significantly reduced for the respirator I with higher level of breathing resistance when compared with its counterpart with lower breathing resistance (respirator II) (P<0.05). The same changes occurred for response time. Results also showed a significant increase in V(E) and BF for respirator I wearers when the work load was above 125 W. The O(2) consumption was similar under the two breathing resistance conditions. These findings suggested that the respiratory resistance caused by self-contained breathing apparatus (SCBA) has an impact on the tolerant capacity of users.

3.Present status in studying immunotherapy for acute leukemia and its perspective--Editorial.

One of the important approaches for further prolonging remission duration and eradicating minimal residual disease in acute leukemia is immunotherapy. Four kinds of immunotherapy for acute leukemia are under investigation: (1) monoclonal antibodies, among them, Mylotarg (cytotoxic antibiotic calicheamicin linked to CD33 Mab) is given for the treatment of refractory or relapsed acute myeloid leukemia and molecular relapse in acute promyelocytic leukemia with good results, Campath-1H (antiCD52 Mab) is administered in the treatment of prolymphocytic leukemia and Rituximab (anti-CD20 Mab) in B-PLL with high complete remission rates. Other Mabs under preclinical and clinical trials include anti-IL-2 receptor Mab for the treatment of acute T lymphocytic leukemia, anti-220 kD Mab-6G7 for acute leukemias, recombinant immune toxin BL22 (anti-CD22) for hairy cell leukemia and Mabs labeled with radio-isotopes for different types of acute leukemias; (2) adoptive cellular immunotherapy using cytokine-induced killer cell, alloreactive NK cells, allogeneic or autologous leukemic-specific CD8(+) cytotoxic T lymphocytes, and other immune effector cells; (3) cytokines and other immune modulators comprising IL-2, IL-12, GM-CSF, CD40L, FLT-3L and thalidomide and its derivatives; (4) leukemia vaccines of several different formulations including antigen-specific, leukemia cell-based, leukemia antigen-pulsed dendritic cell (DC) and leukemia-derived DC vaccines, the latter two formulations are more attractive. In conclusion, up to now, the most effective example of immunotherapy in acute leukemia is provided by the administration of Mabs, and the majority of other approaches in immunotherapy for acute leukemia although promising, need further studies.

4.Expression of bcl-2 mRNA and protein suppressed by antisense oligodexyonucleotide with phosphorothiote-modification in human melanoma A375 cells line

Objective To observe the effect of bcl-2 antisense oligodexyonucleotide (ASODN) by phosphorothiote-modification on the expression of bcl-2 mRNA and protein in human melanoma A375 cells line. Methods With the phosphorothiote-modification and liposome-encapsulation of ASODN, A375 cells were divided into ASODN group, nonsense oligodexyonucleotide (SODN) group and control group. Bcl-2 mRNA and protein were detected by RT-PCR and immunohistochemistry, respectively. Results The expression of bcl-2 protein was remarkably decreased in ASODN group than that in SODN group and control group (53.14 ±4.26 vs 138.22 ± 8.45, 53.14 ± 4.26 vs 141.08 ± 7.83, both P < 0.01 ). The level of bcl-2 mRNA was significantly lower than that in SODN group and control group (0.38 ± 0. 11 vs 0.96 ±0.13, 0. 38 ± 0.11 vs 0. 97 ± 0. 14, both P < 0. 01 ). Conclusion Bcl-2 antisense oligodexyonucleotide could down-regulate the bcl-2 level and block its protein expression.
5.Progress and problems of county-level public hospitals reform in 15 provinces

Based on the organization and implementation of the comprehensive reform of countylevel public hospitals in 15 provinces (municipalities)in southern China,the research discussed the progress and problems of such reforr.The progress made and areas of the reform vary among provinces and municipalities.For example,the reform has completely covered all the counties in four provinces such as Zhejiang and Jiangsu province.Medicine markups have been canceled in all the pilot counties by adjusting the service prices to pave the way for hospital compensation.Contemporary hospital management,remuneration system and personnel system reform are also main targets of the reform,while the collaboration between superiors and subordinates,and hospital regulatory system have recently been included.Problems are also found with the current reform:for example,lack of attention for the system reform,no fundamental changes in place in the business orientation of hospitals,as well as problems in collaboration among departments,and hospital debts.

7.Rehabilitation and Exercise for Senior Coronary Heart Disease (review)

Based on the physiological properties of ammonium-resistant N2-fixingbacterium ( Klebsiella oxytoca NG13/pMC73A), the fermentation technology of it was studied. The basic medium of high-density culture was established, with glucose as carbon source coupled with appropriate nitrogen source and inorganic salts. At the middle and late phase of culture, glucose and ammonia were added to supply carbon source and nitrogen source, stabilizing the pHat 6.5 ~ 6.8. Optimal level of dissolved oxygen was kept by controlling aeration and stirring rate. Bacterium number of Klebsiella oxytoca NG13/pMC73A reached 600 ~ 700 x 10s cfu/mL at the end of culture. Compared with previous technology, bacterium number was increased by more than ten-fold with a comparable culture period.
10.The clinical,neuroimaging and electrophysiological presentations in 62 cases with multiple system atrophy

Objective To evaluate the clinical,neuroimaging and electrophysiology features of 62 patients with multiple system atrophy(MSA).Methods Sixty-two cases with diagnosis of probable MSA were recruited in a retrospective studied.Clinical,neuroimage and external anal sphincter electromyography (EAS-EMG)data was retrospectively analyzed.Results In 62 cases(44 male and 18 female),the onset age was between 37 and 76.Among them,29 cases(46.8 % )were MSA-A,with orthostatie hypotension as the main clinical manifestation;24 cases(38.7 % )were MSA-C,with cerebellar ataxia ag the main chnical manifestation;9 cases(14.5 % )were MSA-P,with extrapyramidal symptoms as the main clinical manifestation.MRI showed that main lesion of MSA-A was in the cerebellum:that of MSA-C was in the cerebellum,pons and medulla;and that of MSA-P was in the putamen.Fifty-one cases did EAS-EMG and 46 cases showed neurogenie impairments.Nineteen cases were initially misdiagnosed with other diseases.Conclusions MSA is easy to be omitted or misdiagnosed at early stage.The diagnostie rate of MSA can be increased by the combination of clinical expressions,neuroimage,EAS-EMG and other necessary examinations.
