1.Influence of Anxiety of Caregiver on Disease Management of Preschool Asthma Children

Objective To describe the anxiety of caregivers and condition management of preschool asthma children ,and analyze the relation?ship between anxiety and disease management. Methods A total of 208 caregivers of asthma children from two hospitals in Shanghai were select?ed using convenient sampling,and surveyed by self?rating anxiety scale(SAS),condition management ability scale and condition management diffi?culty scale. Results The score of SAS were negatively correlated with the condition management ability and positively correlated with the condi?tion management difficulty. Hierarchical regression analysis showed that the model explained 26.6%amount and 35.1%amount of variation respec?tively. Conclusion The anxiety of caregiver of children with asthma may affect the disease management. Future interventions should be strength?ened on the caregivers'psychological problems,so as to improve disease management and control asthma more effectively.
2.Evaluation of Medical Instruments Cleaning Effect of Fluorescence Detection Technique.

OBJECTIVETo compare the cleaning effect of automatic cleaning machine and manual cleaning on coupling type surgical instruments.
METHODSA total of 32 cleaned medical instruments were randomly sampled from medical institutions in Putuo District medical institutions disinfection supply center. Hygiena System SUREII ATP was used to monitor the ATP value, and the cleaning effect was evaluated.
RESULTSThe surface ATP values of the medical instrument of manual cleaning were higher than that of the automatic cleaning machine.
CONCLUSIONCoupling type surgical instruments has better cleaning effect of automatic cleaning machine before disinfection, the application is recommended.

3.Alcoholic liver disease:gut microbiota and therapeutic perspectives

Alcoholic liver disease ( ALD ) , a chronic progres-sive disease, threatens human health seriously. An increasing number of studies have shown that gut flora dysbiosis plays an important role in the development of ALD. Intestinal microbiota maintains a steady state under normal conditions, regulating gut flora normal physiological function. However, chronic alcohol consumption produces intestinal bacteria overgrowth and dysbio-sis, including the alteration of the composition of intestinal mi-croflora, the increment of gut permeability and bacterial translo-cation. Subsequently, the host immune is activated, promoting the production of inflammatory cytokines in liver, which plays a central role in the development of ALD. Notably, the supple-ment of prebiotics or probiotics reverses the intestinal flora disor-der,ameliorating the clinical symptoms effectively in ALD pa-tients. The evidence impies that the modulation of dysbiosis may be effective in the prevention and treatment of ALD. This review summarizes the research progress on the mechanism of the devel-opment of dysbiosis-mediated ALD, to provide a theoretical basis for the research on intestinal flora and ALD.

AIM To investigate the protective effects and mechanism of propofol on cultured rat hippocampal neurons subjected to anoxia injury METHODS Hippocampal neurons of neonatal rats, which had been cultured in vitro for 10 days, were allocated to control groups and propofol treating groups In propofol treated groups,the culture medium were loaded with propofol at the concentrations of 3, 6, 12, 24, 48 mg?L -1 respectively, and then the neurons were exposed to oxygen glucose deprivation for 24 h, to 40 ?mol?L -1 H 2O 2 for 24 h or to 100 ?mol?L -1 glutamate for 50 min The cell survival rate in each group was evaluated by MTT colorimetry. 5. Law and mechanism of incorporating different insoluble drugs into mesoporous silica nanoparticles by dipping centrifugation method

Objective: To investigate the relationship between drug concentration and drug loading efficiency and to explore mechanism of drug loading by incorporating different insoluble drugs into mesoporous silica nanoparticles with dipping centrifugation method. Methods: Mesoporous silica nanoparticles were characterized by transmission electron microscopy and nitrogen adsorption-desorption analysis. Silybin, puerarin, berberine, curcumin, and ferulic acid were used as model drugs and loaded into mesoporous silica nanoparticles using two technologies: The volume of drug solution or drug amount was fixed respectively. The relationship between drug concentration and drug loading was analyzed. Paclitaxel, tanshinone IIA and tetrahydropalmatine were used to validate the law. Nanoparticles loaded silybin, puerarin, and ferulic acid with high drug loading efficiency were characterized by scan electron microscopy and DSC. Results: The average size of mesoporous silica nanoparticles was (220 ± 21) nm, with a specific surface area of 353.53 m2/g, pore size of 1.53 nm and pore volume of about 0.4 cm3/g. In the condition of a fixed mass ratio of drug to mesoporous silica nanoparticles, the drug loading efficiency was always linearly related to the drug concentration at the above preparation technology even if drug loading of different drugs were analyzed together. No obvious drug crystallines were observed in the surface of nanoparticles by scan electron microscopy. Melting peaks of silybin, puerarin and ferulic acid were all observed in DSC curves of mesoporous silica nanoparticles loaded drugs, but their heat absorption amounts per mg were all smaller than physical mixtures. Conclusion: The drug solution concentration was the key factor on drug loading efficiency of mesoporous silica nanoparticles by dipping centrifugation method and drugs existed in pores of mesoporous silica nanoparticles in crystalline and amorphous forms.
6.Inter-rater Reliability of Wisconsin Gait Scale and Gait Abnormality Rating Scale in Hemiplegic Patients after Stroke

Objective To study the inter-rater reliability of Wisconsin Gait Scale (WGS) and Gait Abnormality Rating Scale (GARS) in patients with stroke. Methods 20 hemiplegic patients were required to walk on their comfortable speed and videotaped from frontal, backward and lateral. The video recordings were scored with WGS and GARS by 2 experienced physical therapists. Intraclass correlation coefficient (ICC) was calculated for the scores in each category and the total score. Results ICC for the WGS were 0.372~1, and were 0~0.875 for the GARS. Conclusion WGS is more appropriater to assess the gait of hemiplegic stroke patients than GARS.
7.Genetic characteristics of influenza A/H3N2 virus neuraminidase gene: a survey from 2010 to 2012 in Qinghai Province, China.

This study aims to perform a survey of genetic variation in neuraminidase (NA) gene of influenza A/H3N2 virus, as well as related resistance to NA inhibitors, in Qinghai Province of China, 2010 to 2012. Strains of influenza A/H3N2 isolated during an influenza survey from 2010 to 2012 in Qinghai were enrolled by random sampling. Viral RNA was extracted and amplified by RT-PCR. Purified PCR products were sequenced thereafter. Genetic analysis of nucleic acid and the derived amino acid sequences was performed by MEGA 4.0. Phylogenetic trees were also constructed. Strains isolated during 2010-2011 in this study clustered closely with World Health Organization (WHO) 2010-2012 reference vaccine strain A/Perth/16/2009 and 2008-2010 reference vaccine strain A/Brisbane/10/2007 on the phylogenetic tree, while the 2012 isolates were located on another branch. In analysis of derived amino acid sequences, the 2010 isolates mutated at K81T, the 2011 isolates mutated at I26V and D127N, while the 2012 isolates mutated at E41K, P46A, I58V, T71N, L81P, D93G, D127N, D151N, and I307M. The D151N mutation added a glycosylation site to the activity center of NA. No significant variation was discovered in H3N2 NA gene of 2010-2011 isolates in Qinghai, China. Isolates of 2012 were found with significant mutation, which has the potential of inducing minor resistance to NA inhibitors like zanamivir and oseltamivir.

8.Study on thermoanalysis-based calamina processing.

Thermogravimetry (TG), TG-MS, Fourier transform infrared spectroscopy (FTIR), scanning electron microscope (SEM)-energy dispersive spectrometer(EDS) were adopted to investigate the pyrolysis characteristics of calamina. According to the findings of the qualitative and quantitative studies on the changes in the content of relevant elements, the whole shape, the functional groups, and the volatile components of calamina before and after being pyrolyzed, the 200-360, 580-750 degrees C were two sensitive temperature ranges related to the changes in effective component during calamina processing. Thermal weight loss was observed for ZnCO3, Zn(OH)2 and ZnCO3-2Zn(OH)2-H2O under 200-360 degrees C and for CaCO3 under 580-750 degrees C. The results of studies on chemical reaction kinetics showed good linear relations. This experiment integrated relevant methods and theories of physical chemistry and science of traditional Chinese medicine processing, and interpretes calamina processing techniques and mechanism, in order to provide a good example for modem studies on other traditional Chinese medicine processing.

9.Clinical research of the treatment on cervical esophagus fistula with low negative pressure suction by double caping pipe

Objective to investigate the clinical effect of negative pressure suction by double caping pipe on cervical esophagus fistula after esophageal carcinoma surgery,and search for the effective treatment of cervical anastomotic fistula after esophageal carcinoma surgery.Methods The clinical data of 140 patients with cervical esophagus fistula after esophageal carcinoma surgery in Nanjing General Hospital of Nanjing Command from September 2004 to September 2015 were retrospective analyzed.Among them,85 cases were treated with low negative pressure suction by double caping pipe as experimental group,55 cases were treated with conventional drainage and dressing as the control group.The length of hospital stay,healing time,dressing frequency,neck bleeding risk,anastomotic stenosis and mortality rate between two groups were analyzed and compared.Results The length of hospital stay,the time of wound healing,the frequency of wound change dressing,the rate of neck bleeding in experimental group were (15.94± 1.57)d,(8.00± 1.55)d,(6.22± 1.52)times,1.18% respectively,significantly lower than that of control group ((23.64 ± 2.36) d,(15.64 ± 2.08) d,(27.56±3.58) times,12.24%;P=0.000,0.000,0.000,0.029).While the rate of anastomotic stenosis after half a year in experimental group was 17.65%,lower than that of the control group (23.64%),the difference was no significant(P=0.387).Conclusion The treatment on cervical esophagus fistula with low negative pressure suction by double caping pipe has superiorities on hospitalization and healing time,dressing frequency,neck bleeding risk,mortality,and does not increase the incidence of anastomotic stenosis,and it can achieve a better therapeutic effect compare with the conventional drainage and dressing.
10.The influence of proton pump inhibitors on incidence of bacteremia after endoscopic ultrasonography guided fine needle aspiration

Objective To investigate whether preoperative proton pump inhibitor (PPI) would increase the risk of bacteremia after endoscopic ultrasonography guided fine needle aspiration (EUS-FNA).Methods Dogs were randomly divided into experimental group ( n = 8) to take 20mg of omeprazole orally twice a day for 3 days before EUS-FNA, and control group (n = 10) to take placebo.EUS-FNA was performed to puncture the pancreas, and blood was collected before and after the procedure for culture.Bacteremia incidence of the two groups were compared.Results Bacteria culture was positive in 11 edogse.After excluding contamination, 1 dog in experimental group and 3 in control group were authentical bacteremia.There was no significant difference on the incidence of bacteremia between 2 groups based on Fisher test ( P = 0.383).Conclusion Aplication of PPI before EUS-FNA is not likely to increase the incidence of bacteremia.
