1.Systematic review of dynamic hip plate and proximal femoral intramedullary nail fixation for intertrochanteric fracture in adults
Chinese Journal of Tissue Engineering Research 2015;(22):3595-3603
BACKGROUND:Intertrochanteric fractures are often treated with extramedulary fixation and intramedulary fixation. Which internal fixation can better repair intertrochanteric fractures remains controversial. OBJECTIVE: To compare the efficacy and safety of dynamic hip plate and proximal femoral intramedulary nail fixation in treatment of intertrochanteric fracture in adults. METHODS: Al randomized clinical trials relevant to comparing proximal femoral intramedulary nail fixation with dynamic hip plate for intertrochanteric fracture in adults from 1966 to 2013 were identified. The quality of the trials was assessed. The outcomes included length of surgery, operative blood loss, postoperative femoral shaft fracture, cut-out of the implant from the femoral head, non-union of the fracture, reoperation rate, wound infection, postoperative hip pain (hip joint pain and thigh pain), which were extracted and reviewed systematicaly. RESULTS AND CONCLUSION:4 757 trial participants with 4 748 fractures were included in the 31 published trials. There was no significant difference in the length of surgery, reoperation rate, cut-out of the implant from the femoral head, non-union of the fracture, wound infection and postoperative hip pain between the two groups (P > 0.05) between dynamic hip plate and proximal femoral intramedulary nail fixation. Compared with dynamic hip plate fixation, proximal femoral intramedulary nail fixation had less blood loss and significantly higher rate of femoral shaft fractures (P=0.003). Experimental findings show that, in treatment of intertrochanteric fracture, intramedulary nail fixation is associated with significantly lower blood loss. However, the risk of femoral shaft fracture is increased compared with dynamic plate fixation. There were no differences in length of surgery, cut-out of the implant from the femoral head, non-union of the fracture, reoperation rate, wound infection and postoperative hip pain. The results of this systematic review should be explained prudently because of some limitations of included trials. To obtain more reliable conclusions, large-scale randomized controled trials of strict design, uniform index and long-term folow-up are needed.
2.Effects of Early Enteral Feeding Containing MCT/LCT on Improving Protein Catabolism of Burned Patients
Journal of Chinese Physician 2000;0(12):-
Objectives To compare the effects of different early enteral nutrition formula on improving metabolic status of burned patients. Methods Thirty cases of burned patients with total burn surface area (TBSA) more than 30% were randomly divided into two groups:⑴Group F, enteral feeding with Fresubin 750MCT containing MCT/LCT as fat source, ⑵Group N, enteral feeding with Nutrison containing only LCT as fat source. All of the thirty patients received isocaloric, isonitrogenous and isolipidic enteral feeding with intragstric tube and enteral feeding pump, within 24h of postburn. Plasma insulin, glucagons, tumor necrosis factor (TNF) and 3-methylhistidine in urine were measured on day 1,4,7and 10 days of postburn. Results The results showed that the patients of group F had an increased blood concentration of insulin and the ratio of insulin/ glucagons, a decreased plasma concentration of TNF, and a reduced excretion of 3-methylhistidine in urine as compared with the patients of group N. Conclusions An enteral compound diet containing MCT/LCT in the early enteral feeding in burned patients can improve the metabolic status of burned patients more effectively.

Transient receptor potential vanilloid 1 (TRPV1) is a non-selective positive ion channel that is mainly expressed in sensory neurons and a member of transient receptor potential (TRP) family. The receptor could be activated by mechanical irritation, chemical irritation or endogenous ligand to mediate pains and cause injury to body functions. Traditional Chinese medicine believes that the mechanism of pain is that "stagnation leads to pain". Specifically, both of the contracture and tautness caused by cold and the blood stasis could result in blood impassability and pain. Most of traditional Chinese medicines for clearing heat and removing toxicity have the anti-inflammatory effect, while those for warming interior, and promoting blood circulation to remove blood stasis have the effect in smoothening blood vessels. Therefore, either with the anti-inflammatory effect or the effect in smoothening blood vessels, traditional Chinese medicines for clearing heat and removing toxicity, warming interior, and promoting blood circulation have the analgesic effect In this paper, the authors summarize the analgesic effect of the above three traditional Chinese medicines, with TRPV1 as the target.

OBJECTIVETo study the effect and its mechanism of antihypertensive of drug combination of Jimaitong tablt and nifedipine on spontaneously hypertensive rats.
METHODThe spontaneously hypertensive rats (SHR) were treated by intragastric administration (ig) with Jimaitong (450 mg x kg(-1)), Jimaitong (300 mg x kg(-1)) combined with the nifedipine (0.5 mg x kg(-1)), nifedipine (5 mg x kg(-1)) for 8 weeks. Blood pressure and heart rate was measured. The level of oxidation indicators, lipid indicators, as well as hormones related to blood pressure was detected.
RESULTSCompared with that in the control group, systolic blood pressure, diastolic blood pressure, mean arterial pressure of SHR decreased significantly in all the three administration groups (P < 0.01). Heart rates of nifedipine group was faster than other groups (P < 0.05). In combination group, contents of URE, MDA, ET and Ang II were decreased significantly (P < 0.05, P < 0.05, P < 0.05, P < 0.01), the contents of NO and CAMP, SOD and NOS were significantly enhanced (P < 0.05).
CONCLUSIONCombination use of Jimaitong and nifedipine has synergistic effects on anti-hypertension in spontaneously hypertensive rats. It indicates the potential applications of combination of traditional Chinese medicine and western medicine for the hypertension treatment.

Objective To evaluate the relationship between Multi-slices spiral(MSCT) perfusion and microvessel density (MVD) in maxillofacial tumors. Methods Thirty-one cases of maxillofacial tumors were studied with MSCT perfusion imaging before operation. The time-density curve, perfusion, time to peak(TTP), and Peak enhancement imaging (PEI) of tumors were calculated. MVD of the tumors was measured with immuno-histochemical method by means of detecting factor Ⅷ in all the histologic specimens. Relativity analysis was carried between MSCT perfusion imaging parameters, perfusion curve types and MVD. Results MVD of maxillofacial tumors were higher than normal tissue. MVD remarkably correlated with malignancy of the tumors. Perfusion and time to peak (TTP) correlated well with MVD(t=7.09,4.10, P0.05). Significant difference of MVD in three types of perfusion curve was found(F=8.09,P

To optimize the Scutellaria baicalensis extraction process, the filter paper method and the bacteriostatic ratio method were adopted to determine the in vitro bacteriostatic efficacy of water extracts and 60% alcohol extracts from S. baicalensis. The quantitative analysis of multi-components by single-marker (QAMS) was used to determined the contents of four active components, baicalin, wogonoside, baicalein and wogonin. In addition, with the bacteriostatic ratio and the overall desirability of the contents of four active components as indexes, the orthogonal experiment was adopted to detect the effect of water addition, extraction frequency and extraction time. The optimal extraction process was to add 12 times of water for the first time, 10 times of water for the second time, extract for 2 time, 2 h for each time. This optimization process is stable and feasible, with a higher bacteriostatic ratio in extracts.
